Известен способ оперативного лечения больных с врожденной плоско-вальгусной стопой, заключающийся в разрезах на стопе, в выделении и Z-образном удлинении ахиллова сухожилия и сухожилий малоберцовых мышц, вскрытии верхнего, нижнего скакательных суставов и таранно-ладьевидного сустава, репозиции таранной и ладьевидной костей с их фиксацией спицами Киршнера, отсечении сухожилия задней большеберцовой мышцы от места его прикрепления с последующим подшиванием к ладьевидной и головке таранной кости в положении эквино-варусной установки стопы, фиксации пяточной, первой и пятой плюсневых костей аллосухожилием с целью формирования нормального продольного и поперечного свода стопы (см. патент № 1739980, МПК А61В 17/56, 1992 г.).There is a method of surgical treatment of patients with congenital flat-valgus foot, which consists of cuts in the foot, in the selection and Z-shaped lengthening of the Achilles tendon and tendons of the fibula, opening of the upper, lower hock joints and the scaphoid-navicular joint, reposition of the talus and navicular bones their fixation with Kirschner’s spokes, cutting off the tendon of the posterior tibial muscle from the place of its attachment, followed by hemming to the scaphoid and the head of the talus in the position of the equino-varus installation py, fixation of the calcaneus, first and fifth metatarsal bones by allotendinum in order to form a normal longitudinal and transverse arch of the foot (see patent No. 1739980, IPC АВВ 17/56, 1992).

Проведение сухожилия передней большеберцовой мышцы через шейку таранной кости не позволяет решить основную задачу оперативного вмешательства - стабильной фиксации таранной и ладьевидной костей в правильном положении. Возникает необходимость дополнительной фиксации таранной и ладьевидной костей спицей Киршнера, что значительно увеличивает риск возникновения инфекционных осложнений в послеоперационном периоде. Предлагаемый способ осуществляется следующим образом. Выполняют разрез кожи по Цинциннати. Из этого доступа выделяют и Z-образно рассекают ахиллово сухожилие во фронтальной плоскости. Производят артротомию скакательных суставов. Выделяют и пересекают сухожилие 1 передней большеберцовой мышцы у места прикрепления к медиальной клиновидной кости на 1/2 его диаметра, рассекая в сагиттальной плоскости на 4-5 см в проксимальном направлении с образованием сухожильного лоскута 2 (фиг.1). Вскрывают таранно-ладьевидный и ладьевидно-клиновидный сустав. Переводят таранную кость 3 в горизонтальное положение, устраняют смещение ладьевидной кости 4 и формируют костный канал 5 диаметром 3 мм в теле ладьевидной кости 4 и головке таранной кости 3 в косопоперечном направлении снизу вверх под углом 40-45° по отношению к подошвенной поверхности стопы, проходящий через медиальный отдел нижней поверхности ладьевидной кости, центр вогнутой суставной таранной поверхности ладьевидной кости, медиальный отдел сферической суставной ладьевидной поверхности таранной кости с выходом на ее верхнемедиальную поверхность в зоне перехода головки в шейку таранной кости (фиг.2). Проводят сухожильный лоскут 2 через сформированный костный канал 5 в таранной и ладьевидной костях 3 и 4, фиксируют таранно-ладьевидный сустав в правильном положении и подшивают сухожильный лоскут 2 к бугристости ладьевидной кости в экскавировано-варусном положении стопы. Ахиллово сухожилие сшивают в удлиненном положении. Рану послойно зашивают наглухо. Стопу и голень фиксируют гипсовой повязкой типа ”сапожок”.The proposed method is as follows. Клинический пример. Больной В. 6 лет, история болезни № 228490, поступил в отделение ортопедии детей ФГУ «ННИИТО Росмедтехнологий» с диагнозом: врожденная плоско-вальгусная деформация правой стопы 2 степени. При осмотре правая стопа находится в положении отведения до 20°, пятка в положении наружного отведения до 20°, по внутренней поверхности стопы пальпируется низко опущенная головка таранной кости и ядро окостенения ладьевидной кости. На рентгенограммах правой стопы определяется вертикальное положение таранной кости, ось которой проходит между пяточной и кубовидной костями, ядро ладьевидной кости еще не выражено, угол наклона пяточной кости равен 0 градусов. Больной оперирован по предложенной методике. Иммобилизация стопы в экскавировано-варусном положении осуществлялась гипсовыми повязками в течение 4 месяцев, со сменой один раз в 3 недели. На контрольном осмотре через 1 год после операции ребенок ходит в обычной обуви с супинатором для подъема внутреннего продольного свода, походка правильная. Эстетическое состояние оперированной стопы хорошее, форма правильная, восстановлен внутренний продольный свод и опущена пятка. На рентгенограммах стопы правильные взаимоотношения в суставах предплюсны.Clinical example. The patient was operated on by the proposed technique. The immobilization of the foot in the excavated-varus position was carried out by plaster casts for 4 months, with a change once every 3 weeks. At the control examination, 1 year after the operation, the child walks in ordinary shoes with an instep for lifting the internal longitudinal arch, the gait is correct. The aesthetic condition of the operated foot is good, the shape is correct, the internal longitudinal arch is restored and the heel is lowered. On radiographs of the foot, the correct relationship in the joints of the tarsus is.
Способ оперативного лечения врожденной плоско-вальгусной стопы у детей отличается малой травматичностью и позволяет одномоментно устранить все компоненты деформации. Предлагаемый способ учитывает анатомическую особенность прикрепления сухожилия передней большеберцовой мышцы к основанию первой плюсневой кости и телу медиальной клиновидной кости. Пересечение сухожилия передней большеберцовой мышцы у места прикрепления к медиальной клиновидной кости на 1/2 его диаметра и формирование сухожильного лоскута исключает нарушение функции мышцы, сохраняет равновесие между мышцами голени и силу разгибания в голеностопном суставе. Проведение сухожильного лоскута передней большеберцовой мышцы через сформированный костный канал в теле ладьевидной и головке таранной кости позволяет достичь стабильной фиксации этих костей в правильном положении за счет образования тенодеза таранно-ладьевиного сустава. Сухожильный лоскут передней большеберцовой мышцы удерживает таранную кость в горизонтальном положении, стабилизирует таранно-ладьевидный сустав, предотвращая смещение переднего отдела стопы в положение тыльной флексии и аддукции. Способ прост по технике исполнения, позволяет получить стойкую коррекцию деформации и улучшить функциональный результат оперативного лечения.The method of surgical treatment of congenital flat-valgus foot in children is less traumatic and can simultaneously eliminate all components of the deformation. A tendon flap of the anterior tibial muscle holds the talus in a horizontal position, stabilizes the talar-navicular joint, preventing the forefoot from shifting to the back flexion and adduction position. The method is simple in execution technique, allows to obtain a stable correction of deformation and improve the functional result of surgical treatment.
