Изобретение относится к медицине, а именно к офтальмологии, и может быть использовано для предотвращения развития осложнений со стороны сетчатки при микрохирургических операциях по поводу катаракты у пациентов с сопутствующей патологией сетчатки макулярной области.The invention relates to medicine, namely to ophthalmology, and can be used to prevent the development of complications from the retina during microsurgical operations for cataracts in patients with concomitant retinal pathology of the macular region.
Обычно всем пациентам, независимо от толщины сетчатки макулярной области, проводится традиционная ретробульбарная анестезия (РБА) или получившая в последнее время широкое распространение эпибульбарно-внутрикамерная анестезия (ЭВА). При РБА инъекция анестетика осуществляется в ретробульбарное пространство, при ЭВА - анестетик капельно (аппликационно) наносится на роговицу и добавляется к раствору вискоэластика, вводимого в переднюю камеру глаза (внутрикамерная анестезия).Typically, all patients, regardless of the thickness of the retina of the macular region, undergo traditional retrobulbar anesthesia (RBA) or recently received widespread epibulbar-intracameral anesthesia (EVA). Задачей изобретения является создание эффективного способа определения показаний к выбору метода анестезии при микрохирургических операциях по поводу катаракты при наличии сопутствующей патологи сетчатки макулярной области.The objective of the invention is to provide an effective method for determining indications for the choice of anesthesia method during microsurgical operations for cataracts in the presence of concomitant retinal pathology of the macular region.
Задача решается тем, что в способе определения показаний к выбору метода анестезии при микрохирургических операциях по поводу катаракты при наличии сопутствующей патологии сетчатки макулярной области производят определение толщины сетчатки макуляной области методом оптической когерентной томографии оперируемого или парного глаза при выраженном нарушении прозрачности оптических сред оперируемого глаза, и если толщина сетчатки превышает 300 микрометров, то показана местная анестезия переднего отрезка глазного яблока, состоящая из эпибульбарных инстилляций анестетика и внутрикамерного введения анестетика в сочетании с вискоэластиком.The problem is solved in that in the method for determining indications for the choice of anesthesia method during microsurgical operations for cataracts in the presence of concomitant retinal pathology of the macular region, the thickness of the macular region of the retina is determined by optical coherence tomography of the operated or paired eye with a pronounced violation of the transparency of the optical media of the operated eye, and if the thickness of the retina exceeds 300 micrometers, then local anesthesia of the anterior segment of the eyeball, consisting of e ibulbarnyh instillations intracameral anesthetic and anesthetic in combination with viscoelastic.
Авторами проведена большая научно-клиническая работа по изучению воздействия анестетиков на внутренние, наиболее реактивные структуры глаза, в том числе на сетчатку области макулы. Установлено, что минимально используемые концентрации анестетиков для аппликационной или внутрикамерной анестезии не вызывают изменения со стороны основных морфофункциональных систем глаза (роговица, цилиарное тело, радужка, дренажные пути, сетчатка). В то же время концентрация и количество используемого анестетика для РБА многократно превышает таковые параметры при ЭВА. Продолжительное депонирование анестетика у заднего полюса детерминирует ятрогенный эффект на чувствительную нейроглиальную ткань сетчатки и зрительного нерва.The authors carried out a large scientific and clinical work on the effects of anesthetics on the internal, most reactive structures of the eye, including on the retina of the macular region. В литературе описаны случаи развития слепоты в результате прямого контакта анестетика с диском зрительного нерва. В этом же контексте следует рассматривать негативные изменения сетчатки в результате экстраокулярного воздействия анестетика. На большом клиническом материале, включающим до- и послеоперационное обследование больных после факоэмульсификации с использованием РБА (70 глаз) и ЭВА (60 глаз) с исходными изменениями макулярной области сосудистого генеза, нами получены данные, свидетельствующие о том, что при прочих равных условиях, а именно при однородном поло-возрастном составе пациентов, идентичном характере изменений сетчатки по данным ОКТ, одинаковой плотности катаракты, одинаковых значениях ультравуковой факоэмульсификации - послеоперационный отек макулы развивается у пациентов, которым проводилась РБА (64 глаза, 92%); в группе пациентов, которым выполнялась ЭВА значимых изменений со стороны сетчатки макулярной области выявлено не было. Было выявлено, что если толщина сетчатки находится в диапазоне 250-300 мкм, то возможно проведение как РБА, так и ЭВА. Но если сетчатка исходно изменена и ее толщина в макулярной области превышает 300 мкм, то прогнозируется отрицательное воздействие анестетика, введенного в область заднего полюса глаза на сетчатку макулярной области, и показанием в этой ситуации является ЭВА.In the literature, cases of the development of blindness as a result of direct contact of the anesthetic with the optic disc are described. It was found that if the thickness of the retina is in the range of 250-300 μm, then it is possible to conduct both RBA and EVA. But if the retina is initially changed and its thickness in the macular region exceeds 300 μm, then the negative effect of the anesthetic introduced into the area of the posterior pole of the eye on the retina of the macular region is predicted, and the indication in this situation is EVA.

Для определения толщины сетчатки макулярной области проведена ОКТ, показавщая толщину сетчатки 320 мкм на левом глазу. Для определения толщины сетчатки макулярной области проведена ОКТ, показавшая толщину сетчатки маулярной области на правом глазу 336 мкм. Предлагаемый способ позволяет с высокой эффективностью предупреждать прогрессирование отека макулы при исходном ее проявлении на субклиническом или клиническом уровне (по данным ОКТ) благодаря исключению локального токсического действия анестетика, вводимого перед операцией в ретробульбарное пространство непосредственно вблизи макулярной области. В этих случаях анестезиологическое обеспечение операции осуществляется за счет местного применения анестетиков в переднем сегменте глазного яблока, а именно эпибульбарно в виде инстилляций и внутрикамерно - в сочетании с вискоэластиком.The proposed method allows with high efficiency to prevent the progression of macular edema with its initial manifestation at a subclinical or clinical level (according to OCT) due to the elimination of the local toxic effect of the anesthetic introduced before the operation into the retrobulbar space directly near the macular region. In these cases, the anesthetic management of the operation is carried out due to the local application of anesthetics in the anterior segment of the eyeball, namely, epibulbar in the form of instillations and intracamerally - in combination with viscoelastic.
