Abstract
Description
The present invention relates to medicine, namely to medical instruments, and can be used in experimental modeling of peritonitis diseases in dogs, emergency operations, in acute pathology of the abdominal organs in pediatric surgery.
A known method of suction-washing treatment of purulent cavities, in which a manual or automatic pulsar is connected to the suction drainage channel using a T-shaped adapter to create a forced reciprocating fluid movement (patent RU 2063772, 1996). By blocking the channels of the inflow and outflow, a closed counterpulsar and a drained purulent cavity are created. A pulsar is actuated, which gives a fluid located in a closed cavity, reciprocating motion due to a short-term increase in pressure and its discharge to its original or negative value. This is ensured by short-term alternating forced delivery and evacuation in a closed circuit of an additional volume of washing liquid. The resulting multiple pulsating contractions and expansion of the walls of the purulent cavity and turbulent movements of the wash solution facilitate the penetration of the wash liquid into the slept or clogged mouths of pockets and spurs and wash off detritus from the walls of the cavity. After the necessary rehabilitation time, the outflow hose is opened and the contents of the purulent cavity are evacuated. This method does not provide a complete evacuation of the contents of the purulent cavity, at high pressure of the return fluid there is a chance of increased uncontrolled reflux of return fluid into non-draining zones and generalization of the purulent process.
A known method of sanitation of the abdominal cavity with generalized peritonitis and a device for its implementation, containing a protective frame in the form of a plate with a hole (patent RU 2242170, 2004). The protective frame is made for suturing along the contour of the laparotomy wound, has openings for suture thread. The device contains a grid placed along the contour of the hole side with a protrusion and congruent side cover. The side is made for laying the net. The mesh is made to press the cover in the lower part. In the lower part of the lid there is a recess that can interact with the protrusion on the side and fix. Suture holes are located along the edge. However, the use of this device requires separate cuts for drains that drain fluid from the abdominal cavity, and secondly, the protective frame itself, fixed in the area of surgical access, does not have a high tightness, which leads to the outflow of fluid around the frame and infection of the soft tissues of the access itself .
A known method of treating spilled purulent peritonitis by sanitizing the abdominal cavity with a device containing two separate petals of an elastic perforated hydrophobic film with fan-shaped soldered drainage elastic perforated tubes (application RU 96110063). The device is inserted into the abdominal cavity, the upper lobe containing the sealed working ends of the movable part of the drainage elastic perforated tubes is placed on top of the intestine, and the lower lobe containing open working ends is laid below the intestines, the outlet ends of the tubes are assembled in separate tubes of a larger diameter, out, the upper tube in the left hypochondrium, the lower tube in the right iliac region, while dialysis solutions are injected through the upper tube, the contents are discharged through the lower tube, with At the same time, before the resolution of peritonitis, the intestinal area is electrically stimulated using electrodes soldered in parallel to the elastic drainage perforated tubes inside the hydrophobic film. The disadvantages of the method are the use of additional incisions for drainage, the complexity of laying drainage, which increases the time and invasiveness of the operation.
A device for sanitation of the abdominal cavity containing a container with an inextensible middle part and an elastically extensible peripheral part and two tubes combined into a common collector, the inextensible middle part is made in the form of an inextensible hydrophobic film with holes connected to two silicone inner tubes, and the elastic-extensible part is made in the form of two longer silicone outer tubes and elastic-tensile containers that are located between the outer and inner tubes, with yazany with its manifold perforated tube at one end and provided with elastically stretchable film on the other end (patent RU 2216360, 2003). All four silicone tubes are connected by a collector, equipped with plugs at the peripheral ends and are made with valve holes increasing in size from the collector to the periphery, located on both sides of the outer long tubes and only on the back surface of the short inner tubes. The peripheral ends of the long outer tubes are rounded inward. However, this device does not make it possible to determine the pressure in the closed circuit and the pressure on the abdominal organs, which can lead to disruption of the microcirculation of the intestinal loops, is complex and cumbersome.
The prototype is a drainage device, used, in particular, in the treatment of spilled peritonitis (AS USSR 1694148, 1991). The device contains a perforated container and a tube for outflow, a container with a nozzle for washing liquid, the middle part of which is made inextensible, and the peripheral part is elastic-expandable, while the liquid container is located above the perforated container, connected to it by elastic jumpers located between the perforations. An elastic tube with exits above the perforations of the container for supplying an antiseptic solution is placed on top of the drainage device. After surgery through a cut in the anterior abdominal wall, a drainage device is placed in the abdominal cavity. The working fluid is poured through the nozzle into the tank, when the tank is filled, its peripheral part is stretched, the working area of the device increases, and it assumes a working position. The pipe is closed with a stopper, an antiseptic wash solution is fed into the abdominal cavity through an elastic tube. Removal of pus and exudate, the spent solution of an antiseptic from the abdominal cavity, which enter the tank through perforation holes, takes place through a tube for outflow by gravity or using an electric suction pump. The use of the device limits the possibility of active management of the patient, because involves the patient's forced position on the back due to the possibility of displacement of the device when changing the position of the body. Using the device does not exclude the possibility of sagging into separate pockets due to the fact that the drains are not completely brought into the purulent foci, and it cannot be used with spilled purulent peritonitis.
The technical result is the provision of adequate sanitation of the abdominal cavity, reducing the risk of trauma to the soft tissues of the anterior abdominal wall, prevention of adhesions in the abdominal cavity.

The proposed device is used as follows: perform median laparotomy or relaparotomy, sanitize the abdominal cavity.
