Abstract
Description
The invention relates to medicine and can be used in surgery, dermatology, dermatocosmetology.
Hypotrophic scars in the form of impressions that occur on the site of deep pustular elements during acne, cicatricial changes in the face and body after traumatic effects, animal bites, surgical measures, as well as atrophy of the scars against the background of their diligent drug treatment are a serious cosmetic defect and cause a lot of problems to patients.
If hypertrophic scars can be quite successfully eliminated by physiotherapeutic, surgical or medication, hypotrophic scars require special attention. The use of traditional agents that are used in the treatment of hypertrophic and keloid scars is not always effective in this case.
[1, 2].In a typical wound, 6–8 weeks after injury, a balance is established between anabolic and catabolic processes. At this stage, the strength of the wound is approximately 30-40% of the strength of healthy skin. With the development of the scar, its ultimate strength increases as a result of progressive bonds of collagen fibers. When there is a mismatch between anabolic and catabolic processes, more collagen degrades than it is produced, and the scar tends to contract in all directions, located below the edges of healthy skin, then it is classified as a hypotrophic scar. Moreover, the nutrition of the scar can not only not be disturbed, according to the traditional theory of collagenogenesis, but even enhanced due to the developed microcirculation system [1, 2].
There are several methods for correcting atrophic scars: surgical, laser resurfacing, various chemical peels. In recent years, specialists most often use a combination of microsurgical procedures with peeling or dermabrasion. Of the microsurgical techniques, the most popular among specialists are panchexcision (excision of the scar), panchelevation (raising the day of the scar), subcision (cutting the bottom of the scar from the underlying tissues), as well as the dermal philling method using absorbable hyaluronic acid preparations or biopolymers.
However, the use of an integrated approach in the treatment of hypotrophic scars not only requires a constant complication of methods, but also indicates their low effectiveness in the fight against cicatricial deformity of the skin.
Существует способ лечения атрофических рубцов, заключающийся в том, что отделяют рубцово-измененный кожный лоскут окаймляющим разрезом, деэпителизируют его, отсепарованные края кожной раны подтягивают кверху, накладывают на деэпителизированный лоскут и адаптируют двумя видами хирургических швов: П-образными внутрикожными из рассасывающего материала (викрил 6-0) и узловыми из нерассасывающегося материала (пролен 8-0) (Патент РФ №2254071, МПК7 А61В 17/00 от 26.11.2003).There is a method of treating atrophic scars, which consists in separating a scar-changed skin flap with a bordering incision, deepithelializing it, pulling off the separated edges of the skin wound, applying it to a de-epithelial flap and adapting it with two types of surgical sutures: U-shaped intradermal from resorbable material ( 6-0) and nodular from non-absorbable material (Prolene 8-0) (RF Patent No. 2254071, IPC 7 A61B 17/00 of 11/26/2003).
However, this method manages to achieve good results only in the early stages after the formation of scars and, relatively, with their small sizes (up to 3-5 cm 2 ). In addition, surgical removal of the scar provokes collagenogenesis, which is poorly controlled by additional drug and radiation exposure.
The objective of the invention: improving the treatment of hypotrophic scars, preventing atrophy of scar tissue against the background of drug treatment of scars, obtaining a cosmetic and functional effect.
The problem is solved due to the fact that scar tissue is exposed to laser radiation with a wavelength of 540 nm, pulse energy of 50-150 mJ, pulse repetition rate of 1-4 Hz, pulse duration of 1-2 ns.
Multiline. The method is as follows. Once the vessels of the bottom of the hypotrophic scar are irradiated with an infrared laser beam with a wavelength of 540 nm, a pulse duration of 1-2 ns (Q-sw generation mode), with pulse energy varying depending on the clinical situation (vessel size, skin color, skin thickness) (50 -150 mJ) and pulse repetition rate (1-4 Hz) obtained using the active medium Nd: YAP (Q - sw) / KTP of the medical multiline laser apparatus.
Under our supervision, there were 24 patients with hypotrophic scars of the skin of various parts of the body. The topography, size, and age of the hypotrophic scars were not fundamental, therefore, they were not subjected to clinical classification. The age of the patients ranged from 19 to 46 years.
As a result of therapeutic measures in patients with hypotrophic skin scars, the following indicators were studied:
глубина рубца, площадь рубца, консистенция, цвет, состояние окружающей кожи, микроциркуляция рубца.scar depth, scar area, consistency, color, condition of the surrounding skin, scar microcirculation.
Через 3 недели после воздействия лазерного излучение на гипотрофический рубец отмечали уменьшение глубины рубца в среднем на 2 мм (Р<0,05).Against the background of using laser radiation in the Q-sw generation mode with a pulse duration of 1-2, obliteration of the vessels of the scar bottom does not occur without affecting the scar tissue itself and the surrounding tissues. 3 weeks after exposure to laser radiation on the hypotrophic scar, a decrease in the depth of the scar by an average of 2 mm was noted (P <0.05).
When measuring the scar area in the first 3 weeks, there were no significant changes in this. It should be noted that hypotrophic scars after laser exposure became softer, more mobile, changes in the color of the scar occurred in the direction of the color of the surrounding tissues. The skin surrounding the scar tissue remained unchanged throughout the treatment period.
BLF-21.The study of the state of microcirculation of the hypotrophic scar was carried out by laser Doppler flowmetry (LDF) in the red emission spectrum using a BLF-21 laser analyzer of capillary blood flow.
The LDF-gram was recorded before laser treatment, immediately after exposure and for the next 3 weeks. Microcirculation was measured in the area of the hypotrophic scar and on the symmetrical points of healthy skin.
После лазерной обработки гипотрофического рубца отмечали снижение показателя микроциркуляции на 44,8% по сравнению с исходными значениями (Р<0,05) (таблица).After laser treatment of the hypotrophic scar, a decrease in the microcirculation index by 44.8% was noted compared with the initial values (P <0.05) (table).
Table.
Changes in tissue perfusion in the area of hypotrophic scar
During the 3-week monitoring of the microcirculation index in patients with hypotrophic scars, there was no significant change in perfusion of the pathological focus, which indicates the effectiveness of a single exposure to laser radiation on the scar tissue.
1.Clinical example 1.
Patient A., 45 years old. I was diagnosed with postoperative hypotrophic scar of the skin of the middle third of the sternum.
В течение 1 года - лечение гипертрофического рубца посредством инъекций Дипроспана в рубцовую ткань (до 6 инъекций).From the anamnesis - 2 years ago after the surgical removal of atheroma in the sternum, a hypertrophic scar was formed. Within 1 year - treatment of hypertrophic scar by injection of Diprospan into scar tissue (up to 6 injections).
At the time of the start of treatment, objectively: on the skin of the middle third of the sternum, a rounded atrophic scar with clear boundaries, smooth edges, up to 3 cm in diameter, with a recess in the center of the scar up to 2 mm. The bottom of the scar is represented by a thinned tissue with a network of surface-located vessels of bright pink through which it is visible through it, up to 1 mm in diameter. The skin around the usual color, the scar is not fused with the underlying tissues, mobile, painless.
A single irradiation of the vessels of the scar bottom was performed with a laser beam with a wavelength of 540 nm, a pulse duration of 1-2 ns (Q - sw generation mode), with an energy of 80 mJ and a pulse repetition rate of 2 Hz.
A follow-up examination 24 hours after irradiation showed a partial disappearance of blood vessels, with local areas of darkening and local inflammation (hyperemia, swelling) of the tissues. The surrounding skin is intact. During a clinical examination of the patient after 3 weeks, the following was noted: a decrease in the diameter of the scar by 5 mm with the disappearance of the deepening in the center of the scar and the disappearance of the vascular network. During the follow-up clinical examination after 1 year, a persistent clinical result was noted, which was achieved 3 weeks after laser irradiation.
2.Clinical example 2.
Patient A. 36 years old. She was diagnosed with postoperative hypotrophic skin scar in the jugular notch.
From the anamnesis - 2 years ago, as a result of a household injury in the form of a cut wound in the jugular notch region, she sought medical help. The primary surgical treatment of the wound in the jugular notch, wound closure was performed. Postoperative course without features. Postoperative sutures were removed on the 7th day. According to the words, the formed scar was periodically bothered by itching, a slight burning sensation. 6 months after the injury, she noted a depression in the center of the scar, at the bottom of which vessels were visible. I was not treated on my own.
At the time of treatment initiation, objectively: on the skin in the area of the jugular notch, an atrophic scar with clear boundaries, smooth edges, up to 4 cm in length and up to 1.5 cm in width, with a recess in the center of the scar up to 1.5 mm. The bottom of the scar is represented by a thinned tissue with a network of surface-mounted vessels of bright pink color visible through it up to 1 mm in diameter. The skin around the usual color, the scar is not welded to the underlying tissues, sedentary, painless.
A single irradiation of the vessels of the scar bottom was performed with a laser beam with a wavelength of 540 nm, a pulse duration of 1-2 ns (Q-sw generation mode), with an energy of 50 mJ and a pulse repetition rate of 3 Hz.
A follow-up examination 24 hours after irradiation showed a partial disappearance of blood vessels, with local areas of darkening. Reactions of local inflammation (hyperemia, swelling) of tissues were not noted. The surrounding skin is intact. During a clinical examination of the patient after 3 weeks, the following was noted: a decrease in the size of the scar by an average of 3 mm with the disappearance of the indentation in the center of the scar and the disappearance of the vascular network. During the follow-up clinical examination after 1 year, a persistent clinical result was noted, which was achieved 3 weeks after laser irradiation.
3.Clinical example 3.
Patient B. 48 years. He was diagnosed with a post-burn hypotrophic scar of the skin of the neck on the right.
From the anamnesis - 5 years ago after a chemical burn of the neck on the right, he received appropriate treatment on an outpatient basis by a surgeon. After healing of the burn wound, a scar formed up to 5 cm in diameter, of irregular shape, with a depression.
At the time of treatment initiation, objectively: on the skin in the neck area on the right, the hypotrophic scar is irregular in shape, with smooth edges, up to 6 cm in diameter, with a recess in the center of the scar up to 2 mm. The bottom of the scar is represented by a thinned tissue with a network of pink-colored surface vessels translucent through it, up to 1 mm in diameter. The skin around the usual color, the scar is not fused with the underlying tissues, mobile, painless.
A single irradiation of the vessels of the scar bottom was performed with a laser beam with a wavelength of 540 nm, a pulse duration of 1-2 ns (Q-sw generation mode), with an energy of 70 mJ and a pulse repetition rate of 3 Hz.
A follow-up examination 24 hours after irradiation showed a partial disappearance of blood vessels, with local areas of darkening and local inflammation (hyperemia, swelling) of the tissues. The surrounding skin is intact. During a clinical examination of the patient after 3 weeks noted: a decrease in the diameter of the scar by 6 mm with the disappearance of the deepening in the center of the scar and the disappearance of the vascular network. The scar tissue thus became more elastic and in color corresponded to the surrounding skin. During the follow-up clinical examination after 1 year, a persistent clinical result was noted, which was achieved 3 weeks after laser irradiation.
Thus, the use of a laser with a wavelength of 540 nm, a pulse duration of 1-2 ns (Q-sw generation mode), an energy of 50-150 mJ and a pulse repetition rate of 1-4 Hz in the treatment of hypotrophic scars allows one to obtain a stable the clinical result is already 3 weeks after exposure, which consists in reducing the depth of the scar (on average by 2 mm), improving the consistency and changing the color characteristic of the surrounding skin. Due to the ultra-short time period of laser exposure (1-2 ns), thanks to Q-sw technology, the method is painless and safe and does not affect the surrounding tissues, which is confirmed by the stable clinical result observed in patients 1 year after treatment.
