Известно, что проблема несостоятельности колоректального анастомоза является всегда актуальной и далекой от решения, поскольку многочисленные предлагаемые способы анастомозирования как ручным, так и аппаратным способами не дают абсолютной уверенности в надежности анастомоза и по литературным данным от 1,3% до 44% регистрируется несостоятельность колоректального анастомоза [Fontanili М., Caforio M., Asteria С.Le complicanze perioperatorie nella chirurgia tradizionale del retto // Minerva Chir. - 1997. - Vol.52. - No 3. - P.217-224; Remzi FH, Fasio VW, Gorgun E, Zutshi M et al. Известна защита колоректального анастомоза, которую осуществляют местными воздействиями - погружением его под тазовую брюшину [Балтайтис Ю.В. Корсуновский А.И.\ Способ восстановления непрерывности кишечного тракта при резекции толстой кишки, А.с. №1007655], созданием защиты за счет избытка брыжейки, жировых привесков, большого сальника и капсулы прямой кишки [РЦФХГ, Оноприев В.И., Павленко С.Г., Яргунин С.А. Способ защиты колоректального анастомоза. За ближайший аналог нами принят способ [Желобцова П.М. и Фомичева А.Л. авт. свид. SU №1158169], в котором к линии анастомоза для его защиты подшивают с трех сторон приготовленные лоскуты на ножках, выкроенные из большого сальника. С этой целью авторы использовали лоскуты шириной 1,5-2,0 см как наиболее оптимальные и подшивали их на расстоянии не более 1,2 см друг от друга.For the closest analogue, we adopted the method [P. Технической новизной изобретения является то, что после создания однорядного колоректального анастомоза последний окутывают верхней частью задней стенки влагалища, а с боков ниже и выше соустья на 0,5-1,0 см фиксируют широкие связки матки, которые также фиксируют к краям тазовой брюшины. Техническим результатом предложения является использование широких связок матки, обеспечивающих значительно более надежный и герметичный анастомоз в сравнении с его формированием с помощью лоскутов, выкроенных из большого сальника, за счет высокой прочности тканевых структур связок матки.The technical novelty of the invention is that after the creation of a single-row colorectal anastomosis, the latter is wrapped in the upper part of the posterior wall of the vagina, and from the sides below and above the anastomosis 0.5-1.0 cm wide ligaments of the uterus are fixed, which are also fixed to the edges of the pelvic peritoneum. Способ исполняют следующим образом. После наложения (см. чертеж) колоректального анастомоза 1 его защиту осуществляют укрытием спереди верхней частью задней стенки влагалища 2 (на чертеже покрыта брюшиной) и с боков широкими связками матки 3. Связки фиксируют отдельными швами ниже соустья на 0,5-1,0 см по окружности и выше соустья также на 0,5-1,0 см по окружности. Фиксацию широких связок матки выполняют также и к краям мобилизованной тазовой брюшины по обеим сторонам. Клинический пример. Больная Л., 65 лет, находилась на стационарном лечении в хирургическом отделении Краснодарского клинического госпиталя для ветеранов войн с 26.04.05 г. по 17.05.05 г., ист. болезни №3278. Диагноз: болезнь Крона, концевая колостома после обструктивной резекции сигмовидной кишки по поводу перфорации кишки и перитонита, желчно-каменная болезнь, калькулезный холецистит. 05.05.05 г. выполнена операция: холецистэктомия, реконструкция толстой кишки. При формировании прямого однорядного колоректального анастомоза произведена оментизация анастомоза избытком брыжейки толстой кишки, сальниковыми привесками. Дополнительно область анастомоза укрыта спереди верхней частью задней стенки влагалища, а с боков - широкими связками матки, которые фиксировали по окружности на 0,5-1,0 см выше и ниже линии колоректального соустья, дополнительно края широких связок матки сшиты с краями тазовой брюшины у подвздошных сосудов. Послеоперационный период протекал без осложнений, больная выписана на 11-е сутки после операции.Clinical example. The postoperative period was uneventful, the patient was discharged on the 11th day after the operation.
Данным способом было прооперировано 11 пожилых женщин: 1 пациентка с болезнью Крона, 6 больных раком сигмовидной кишки и 1 больная раком прямой кишки, 3 больных подверглись реконструкции толстой кишки с наложением колоректального анастомоза. Всем больным область анастомоза дополнительно укрывалась спереди верхней частью задней стенки влагалища (на чертеже покрыта брюшиной) и с боков - широкими связками матки. В послеоперационном периоде осложнений не было.
