Abstract
Description
The invention relates to medicine, in particular to traumatology-orthopedics, and is intended for the treatment of post-traumatic contractures of the joints of the limbs.
Currently, various methods of manual impact on human limbs are known and used to eliminate joint contractures.
The most famous way to manually affect the joints and soft tissues of the limbs is closed redress - forcible bloodless correction of deformation by stretching the tissues in one direction and compression in the other [1]. This method has a number of significant disadvantages. The strength of the manual action used by the doctor during the reduction is determined empirically during the production of the manipulation. Complications can occur during redress: damage to large vessels and nerves, fracture of healthy extremities, rupture of muscles, hemorrhages in soft tissues, in the joint cavity with subsequent development of scars and adhesions, fat embolism, reflex cramps. To maintain the achieved effect, limb fixation with a plaster cast is required for 2-3 weeks. Reduction is accompanied by severe pain in the patient, therefore, for its implementation, general or regional anesthesia is necessary.
[2].The rhythmic mobilization method, which is used mainly for the treatment of arthrogenic contractures of joints caused by infringement of the meniscoid, through a series of rhythmically repeated oscillatory movements in the joint, the volume of which does not exceed its possible passive mobility, is quite well known [2].
The advantages of this method are safety, ease of execution and painlessness of the procedure, the negative side is the inability to affect the muscles and correct the altered dynamic stereotype of local and regional muscles, insufficient effectiveness in the treatment of post-traumatic joint contractures, caused, as a rule, by damage not only to the joints, but also paraarticular tissues and muscles of the limb as a whole.
The method of postisometric muscle relaxation [3] adopted by us for the prototype is widely known in various schools of manual medicine and can be used to treat contractures of joints of the extremities of predominantly myogenic origin. Its essence lies in the combination of short-term (within 5-10 seconds) isometric work of minimal intensity and passive muscle stretching in the next 5-10 seconds as well. The achieved therapeutic effect is relaxation and muscle analgesia and (as a result) an increase in the range of motion in the joints of the limbs. This technique, if performed correctly, does not lead to any complications and has practically no contraindications. The negative side is the lack of effectiveness in the treatment of joint contractures caused by other (non-myogenic) causes.
The essence of the claimed invention consists in the combination of distinctive features sufficient to achieve the desired result, namely, optimal manual exposure to the limbs, increasing the effectiveness of the treatment of post-traumatic joint contractures.
This essence consists in the fact that after isometric work of the muscles of the affected and contralateral limb, which move in the direction opposite to the direction of limitation of movements in the affected joint, of the maximum possible intensity using respiratory and oculomotor synkinesia, the patient relaxes the muscles of the affected limb, activates the antagonist muscles of the contralateral limbs and performs forced movement in the direction coinciding with the direction of restriction of movements in the lesion In this joint, the doctor at this time fixes the proximal with one hand, grabs the distal segment of the limb with the other hand as far as possible from the affected joint and, acting as a lever, through the pressure of the muscles of the body performs a smooth movement in the direction of restriction under continuous control of the degree of compliance of the tissues with their own muscle sensation, and through contraction of the muscles of the hand, performs rhythmic oscillatory movements in the affected joint of the patient in a direction perpendicular to the long axis of the limb tees with an amplitude of 3-5 mm.
The proposed method of therapy combines the advantages of rhythmic mobilization of joints and post-isometric relaxation of limb muscles, which ensures its high efficiency in the treatment of post-traumatic joint contractures.
The advantages of the proposed method are the safety, ease of implementation and painlessness of the procedure, which allows it to be used both in inpatient and outpatient settings, and in home treatment of patients.
The combination of the isometric work of the muscles of the affected and contralateral limbs with subsequent manual exposure allows to achieve relaxation and analgesia of the muscles of the limb, to correct the altered dynamic stereotype of the local and regional muscles, to provide a therapeutic effect on the affected joint and paraarticular tissues.
The use of respiratory, oculomotor and muscle (with a contralateral limb) synkinesia increases the efficiency of isometric muscle work and subsequent relaxation of the muscles of the affected limb.
The rhythmic oscillatory effect on the affected joint allows you to eliminate the infringement of the meniscoid or folds of the joint capsule, helps to rupture intra-articular adhesions.
This technique, if performed correctly, does not lead to any complications and has practically no contraindications.
6.Carry out the proposed method of therapy as follows. The patient is placed on the massage table 1 so that the affected
2.The doctor with one
2.After fatigue of the contracted muscles of the affected
13.At this time, the doctor with his
