Description
The utility model relates to medicine, namely to orthopedic dentistry, and can be used to stabilize pathological tooth mobility resulting from periodontal tissue diseases.
М. 2021].Periodontal pathology is still one of the most common and pressing problems in modern dentistry. This is due to the variety of etiological and predisposing factors, clinical manifestations, difficulties in prevention, diagnosis and treatment. According to the World Health Organization (WHO), periodontal diseases occur in different regions of the globe: from 55.0 to 99.0% in the age group from 15 to 20 years, and in the group aged from 35 to 44 years this figure varies from 65.0 to 98.0% [Navruzova U.O., 2019; Kovalevsky A.M. et al., 2021].
The frontal group of teeth is most susceptible to the development of inflammation in periodontal tissues with the appearance of their mobility in the vestibulo-oral and medio-distal directions to varying degrees, depending on the severity of the chronic inflammatory process - periodontitis [Nedelskaya L.A., 2019; Nazarova N.Sh. et al., 2020; Zaitsev L.A. et al., 2019]. One of the ways to stabilize mobile teeth is to splint them, which allows to distribute and maintain the functional integrity of the patient’s dentofacial system.
There is a known method of splinting teeth (RF patent No. 2340306), which consists in the fact that in the manufacture of the splint continuous interdental splinting elements are used, which can be made in the form of a fishing line or tape, a chain or a rope, a thread, or in the form of continuous, evenly repeating links of various shapes and length. Installed on the splinted teeth in a single fragment. On the splinted teeth, a single cavity is formed, located on the contact and occlusal, and (or) contact and buccal (vestibular), and (or) contact and lingual (palatal) surfaces of the splinted teeth with and (or) without capturing the cutting edge, and ( or) masticatory hillocks, and (or) simultaneously on all surfaces and anatomical formations of the splinted teeth, including the roots of the teeth. The cavities are formed in such a way that their shape and size allow splinting elements to be placed in them. This method of splinting has the following disadvantages: the need to completely replace the structure if it is fractured; significant preparation of hard dental tissues when forming additional cavities for an elastic splinting element; the presence of an excess amount of composite material on the lingual side of the lower anterior teeth increases the time of adaptation to the design; the presence of composite material in excess on the palatal side of the upper frontal teeth disrupts phonetics and occlusal contacts with antagonist teeth. The materials used to make the tire have a number of disadvantages, such as fragility; the presence in their composition of monomers, plasticizers, dyes and pigments; shrinkage, elasticity, swelling, thermal expansion and thermal conductivity.
Technical result: increasing the efficiency of orthopedic treatment of patients with periodontal tissue diseases, due to gentle preparation of hard dental tissues, improved strength, bioinert and aesthetic-functional characteristics of the splinting structure with the possibility of its correction within a separate element without complete replacement.
The proposed utility model is shown in the drawing, where figure 1 shows a general view of the device; in fig. 2 – side view. In the drawings: 1 – element of a splinting structure of a trapezoidal shape; 2 – base of the splinting structure 3 – hard tooth tissues; 4 – pulp chamber of the tooth.
10-150. The splinting structure for fixing mobile teeth consists of individual trapezoidal elements (1), having a base (2) facing the hard tissues of the teeth (3) and a smaller pulp chamber (4) with a divergence angle of 10-15 0 .
The splinting structure is made as follows: after professional oral hygiene, hard dental tissues with degree I-II mobility are prepared to be combined into a splinting structure to form trapezoidal cavities with a divergence angle of 10-15 o on the contact surfaces with a transition to the oral (lingual) or palatal) surface. Working (A-silicone impression material) and auxiliary (alginate impression material) impressions are obtained, the color of the teeth is determined using a special dental shade, and a working (type 3 or 4 super plaster) and auxiliary (plaster) models are cast. The working model is coated with anti-reflective powder and scanned using a laboratory scanner (for example, D-series - “3SHAPE” (Denmark), after which information about the appearance of the model is converted into a 3D model on a computer using specialized software (for example, Exocad (Exocad DentalCAD , Germany)). After obtaining a digital model using software (CAM module), future elements of the trapezoidal shape of the splinting structure (1) are modeled, taking into account interocclusal relationships. Next, the digital file with the structure is sent to the control unit of the milling machine, in which milling occurs. standard blanks - zirconium dioxide blocks, for example, Coritec Zr Disc (imes-icore GmbH, Germany), of the selected color.
Ceramics based on zirconium dioxide Coritec Zr Disc (imes-icore GmbH, Germany) are currently used for the manufacture of fixed denture structures (microprostheses, crowns, bridges). This material has not previously been used for splinting teeth.
The splinting structure is used as follows: after manufacturing the splint elements (1), they are checked and fitted to the prepared hard tissues of the tooth (3) in the oral cavity. Fixation of splint elements is carried out according to the traditional protocol for adhesive fixation of permanent metal-free milled structures [CAD/CAM systems in dentistry: textbook / S.I. Abakarov, A.S. Balandina, D.V. Sorokin, K.S. Adzhiev, S.S. Abakarova, D.S. Arutyunov; FGBOU DPO RMANPO, 2016. – 96 p. ISBN 978-5-7249-2616-4]. Next, the place of contact of the splinting elements with the hard tissues of the tooth is polished. The patient is given recommendations on hygienic care of the structure and an appointment for a follow-up examination.
- №5. – С. 10-14]. Positive effect: the splinting structure for fixing mobile teeth is made of zirconium dioxide, which has the necessary strength characteristics and parameters of bioinertness to the tissues and organs of the oral cavity; the high aesthetic characteristics of ceramics based on zirconium dioxide make it possible to use it in the aesthetically significant zone of the dentofacial system in the frontal region. The presence of a trapezoidal cross-section of the splint elements with a small base facing the hard tissues of the teeth allows for gentle preparation of the hard tissues of the teeth, taking into account safety zones (vital teeth) and the location of the enamel prisms, preventing chipping of the hard tissues of the teeth, and the use of individual elements makes it possible to replace the splint link when necessary. The use of CAD/CAM technology makes it possible to produce a precision fit of the splinting structure to the prepared hard tissues of the teeth (10-20 microns compared to 50-70 microns for casting). Previous studies in vitro showed the inhibitory ability of zirconium dioxide on the physiology of biofilms, which inevitably form on the surfaces of hard dental tissues, filling materials and dentures, thereby preventing the development of possible inflammatory phenomena of soft tissue periodontium, which is especially important when splinting patients with tissue diseases periodontal [Rogozhnikov A.G. et al. Microbial films of Staphylococcus epidermidis ATCC 29887 on the surface of zirconium dioxide / Russian Dental Journal // 2014. - No. 5. – P. 10-14].
