Существует способ фиксации при переломе хирургической шейки плеча V-образными спицами (А.Ф.Лазарев, Э.И.Солод “Вестник травматологии и ортопедии имени Н.Н.Приорова, №3, 2003, М., Медицина). Способ отличает малая травматичность в сравнении с накостным остеосинтезом и более стабильная фиксация по сравнению с изолированным спицевым или винтовым остеосинтезом. Задачей изобретения является повышение эффективности синтеза хирургической шейки плечевой кости путем равномерного распределения компрессирующего эффекта по плоскости перелома и усиления жесткости фиксации путем оптимального расположения минификсаторов и спиц.The objective of the invention is to increase the efficiency of the synthesis of the surgical neck of the humerus by uniformly distributing the compressive effect along the plane of the fracture and enhancing the rigidity of fixation by the optimal arrangement of mini-fixators and knitting needles.
Поставленная задача достигается следующим образом:The task is achieved as follows:
Под общим обезболиванием в асептических условиях производится разрез кожи и подкожной клетчатки по передне-наружной поверхности дельтовидной области длиной 7-9 см. Послойно обнажается место перелома хирургической шейки плечевой кости. После репозиции отломков, практически без отслойки надкостницы, производится фиксация отломков путем проведения изогнутых под острым углом 2.0 мм спиц с наружной поверхности головки плечевой кости в метадиафизарную часть дистального фрагмента плечевой кости, при этом спицы выходят на внутренней поверхности плечевой кости. С последующей фиксацией загнутых концов спиц “корончатой” шайбой и кортикальным винтом на дистальном фрагменте плечевой кости (фиг. 2а, фиг. 2б).Under general anesthesia under aseptic conditions, an incision is made in the skin and subcutaneous tissue along the anteroposterior surface of the deltoid region 7–9 cm long. При наличии осколков дистальнее хирургической шейки, концы спиц, после проведения через головку плеча, вводятся в костномозговой канал дистального фрагмента с упором в кортикальную часть на противоположной стороне от введения спиц, а загнутые концы укрепляются “корончатыми” шайбами как над областью осколков, так и на кортикальном слое дистального фрагмента. При этом обязательно проводится фиксация спиц “корончатой” шайбой и винтом в области головки плечевой кости (фиг.3а, фиг.3б). Для достижения оптимальной жесткости фиксации отломков при оскольчатых переломах количество спиц может быть различным, в зависимости от расположения и числа фрагментов. Далее рана послойно зашивается. Диагноз: закрытый оскольчатый перелом хирургической шейки и головки левой плечевой кости со смещением. После проведенного общего клинического и рентгенологического обследования 18.09.03 произведена открытая репозиция отломков с фиксацией спице-винтовой системой с применением “корончатой” шайбы по описанному методу. В послеоперационном периоде применялась косыночная иммобилизация в течение 2 нед. После снятия швов через 12 дней, бльная приступила к постепенной разработке движений в плечевом суставе.Patient Yu.
