Больного кладут на операционный стол, под спину на уровне мечевидного отростка подкладывают валик. Oпeрационное поле обрабатывают по Гроссиху, выполняют верхне-срединную лапаротомию с обходом пупка влево и вниз на 30-40 мм. Рану расширяют расширителями Сигала, мобилизуют желудок с большим и малым сальником. 12-перстную кишку прошивают аппаратом УО-40 с пересечением и перевязкой сосудов желудка и сальников, отсекают от желудка. Культю погружают в два полукисета и 3-образные швы. На пищевод накладывают зажим, желудок отсекают от пищевода и удаляют. Для формирования пищеводно-кишечного анастомоза мобилизуют начальный отдел тощей кишки на расстоянии 100 мм от Трейцевой связки с пересечением 1-2 радиальных сосудов и рассечением брюшных листков брыжейки. На протяжении 40 мм стенку тощей кишки отделяют от брыжейки, проксимально накладывают аппарат УО-40, оставляя 30 мм скелетизированной тощей кишки. Прошивают два раза аппаратом УО-40 и пересекают между швами. Получают дистальную культю 30 мм без брыжейки с интрамуральным кровообращением, на нее накладывают две нити-держалки. Подготовленную таким образом кишку проводят через окно, сделанное в брыжейке поперечно-ободочной кишки и располагают так, чтобы брыжейка тонкой кишки была справа. При помощи зажима на пищеводе и ниток-держалок на тонкой кишке сближают задние поверхности анастомозируемых органов. На расстоянии 20,50-30 мм от зажима на пищеводе и в месте, где начинается культя брыжейки тонкой кишки (фиг.1) формируют задний ряд серозно-мышечных швов. Двумя швами фиксируют края пищевода и кишки и еще двумя швами между ними на одинаковом расстоянии укрепляют заднюю стенку анастомоза. После этого на края стенки пищевода и кишки накладывают три серозно-мышечных шва (фиг.2) на протяжении 20-25 мм в проксимальном направлении. Формируют ребра анастомоза, исключая тем самым неравномерность погружения внутреннего и среднего цилиндров, укрепляя анастомоз и разгружая внутренний и наружный швы. Краевые нити берут на зажимы. вскрывают просвет пищевода ниже зажима и на этом же уровне вскрывают заднюю стенку тонкой кишки /длина от вскрытого просвета тонкой кишки до культи брыжейки тонкой кишки составляет 20-25 мм, что соответствует длине культи пищевода/. Накладывают редкие узловые швы через все слои анастомоза задней стенки. Передняя стенка кишки имеет избыток, который иссекают овальным разрезом выше задней стенки на 15 мм (фиг.3). Формируют первый ряд передней стенки (фиг.4) редкими узловыми швами через все слои. Вторым рядом передней стенки инвагинируют культю пищевода в кишку, создавая инвагинационный анастомоз. Начинают его с левого края: вкол иглы делают на уровне культи брыжейки тонкой кишки в поперечном направлении с захватом серозно-мышечного слоя, второй вкол - в 20 мм от культи пищевода в поперечном направлении с захватом серозно-мышечного слоя. При завязывании узла наружный край анастомоза вворачивают в просвет тонкой кишки. Линия, образованная на передней стенке тонкой кишки, показывает, где надо делать вкол иглы на кишке. По этой линии проводят вкол иглой, второй вкол на пищеводе - аналагично предыдущему. Таких швов накладывают 5-6. Избыток передней стенки тонкой кишки позволяет погрузить пищевод в кишку на глубину 20-25 мм без натяжения наружных швов. Создается инвагинационный анастомоз, его проходимость проверяется проведением через него толстого желудочного зонда. При его завершении видно (фиг. 5), что отводящая часть тонкой кишки идет не вертикально вниз до анастомоза по Ру, а располагается влево на расстоянии 100 мм, затем уходит вниз и вправо к месту формирования анастомоза по Ру за счет брыжейки тонкой кишки. На фиг.6 проиллюстрирован конечный этап операции - вид на разрез сформированного эзофагоеюноанастомоза.
