В последние годы чаще стали регистрироваться атипичные формы псориаза, в том числе с экссудативными проявлениями типа "экзема-псориаз", протекающие как аллергический кожный процесс. У больных псориазом выявлены изменения в коагулирующей и иммунной системах, липидном и холестериновом обменах, психоэмоциональной и других сферах, поэтому обострения при таком течении дерматоза могут возникать от различных причин (психических и физических перенапряжений, травм, обострения хронических заболеваний и др.) без влияния сезонного фактора и характерных рецидивов псориаза преимущественно зимой - в 60-75% случаев. Атипичные формы псориаза требуют и нестандартного подхода к их лечению, т.к. обычные методы в таких случаях бывают мало эффективны, поскольку они оказывают одностороннее действие на какое-то одно нарушенное звено в патогенезе псориаза. Цель достигается тем, что вводят эпиталамин вначале внутримышечно по 1 мл ежедневно, в течение 10-15 дней, в островоспалительную прогрессирующую стадию, а затем в стационарно-регрессивную стадию по 10 мл в электрофорезе биназально с положительного полюса (второй электрод помещается на затылочную область или шейно-грудной отдел позвоночника) в количестве 10-15 процедур. Одновременно наружно проводят внекурортное лечение азотно-кремнистой минеральной водой в виде повязок в течение 1-1,5 часов 2 раза в день (в островоспалительную, прогрессирующую стадию) и по 1,5-2 часа 1 раз в день в последующие дни лечения при разрешении экссудативно-воспалительных явлений. Общий курс продолжается в течение 20-25 дней. Назначение эпиталамина биназально в электрофорезе проводилось с целью непосредственного введения препарата в нервные структуры регулирующего гипоталамического отдела головного мозга больных, кроме того, учитывалась нетравматичность введения препарата, т.к. у больных псориазом выражен симптом Кебнера (когда на месте повреждения от уколов или раздражения кожи появляются новые псориатические элементы). После активной терапии, проведенной больному в течение нескольких лет, ежегодно (ПУВА-терапия, на следующий год - гемосорбция, цитостатики при возникновении эритродермии, гормонотерапия) у данного больного возникло атипичное течение дерматоза с наличием экссудативных проявлений и внесезонных обострений, возникающих от различных стрессовых факторов. Продолжено лечение введением эпиталамина биназально по 10 мг с положительного полюса и наложением второго электрода на затылочную область в течение 10 дней. Наружно продолжалось безмазевое лечение повязками с азотно-кремнистой водой по 1,5 часа, также в течение 10 дней. Общий курс лечения составил 24 дня.
