Abstract
Description
Изобретение относится к медицине, а именно к офтальмологии, и может быть использовано при осуществлении пластики сквозного дефекта века небольшого размера различной этиологии: после удаления опухоли или иного патологического очага века, врожденной или посттравматической колобомы. Способ может быть использован при размерах сквозного дефекта верхнего века не более 3 мм по высоте и 11 мм по длине и нижнего века - не более 2 мм по высоте и 6 мм по длине. Известен способ пластики сквозного дефекта века путем замещения дефекта трансплантатом, представляющим собой участок интактного (здорового) века во всю его толщу - трансмуральным трансплантатом (Basic and Clinical Science Course, Section 7, Orbit, Eyelids, and Lacrimal System, 1997-1998, pp. 187-192. American Academy of Ophthalmology.). Способ предназначен для пластики сквозных дефектов верхнего века не более 7 мм по длине, а нижнего - не более 4 мм. К недостаткам способа следует отнести травму интактного века в ходе взятия трансплантата, что в ряде случаев является источником дополнительных психоэмоциональных реакций пациента как при получении согласия пациента на операцию, так и в послеоперационном периоде в ходе заживления раны. Морфофункциональный итог приживления свободного трансплантата всегда связан с той или иной степенью дистрофии пересаженной ткани, что приводит к истончению ткани, изменению ее цвета и, в конечном итоге, к низкому косметическому эффекту. В случае нагноения послеоперационных ран пораженными оказываются либо оба века одного глаза, либо ранее интактное веко парного глаза. Использование известного способа ставит вопрос об оптимальной пластике века-донора. The morphofunctional outcome of free graft engraftment is always associated with a varying degree of dystrophy of the transplanted tissue, which leads to thinning of the tissue, a change in its color and, ultimately, to a low cosmetic effect. In the case of suppuration of postoperative wounds, either both eyelids of one eye or the previously intact eyelid of the paired eye are affected. Using the known method raises the question of the optimal plasticity of the donor eyelid.

Образованный хряще-конъюнктивальный лоскут мобилизуют с сохранением питающей сосудистой ножки из тканей круговой мышцы глаза и перемещают вплоть до совпадения с контуром глазной щели. Лоскут фиксируют узловыми сквозными швами 8-0 (викрил) к боковым стенкам сквозного дефекта и в интермаргинальном пространстве века. После выворота верхнего века в продолжение боковых стенок сквозного дефекта выполнены сквозные разрезы хряще-конъюнктивальной пластинки - 3 мм по высоте и 11 мм по длине (горизонтальный разрез, соединяющий концы сквозных вертикальных разрезов) и вновь образованный хряще-конъюнктивальный лоскут мобилизован с образованием питающей сосудистой ножки из тканей круговой мышцы глаза. На следующем этапе хряще-конъюнктивальный лоскут смещен вертикально вниз вплоть до оптимального совпадения нижнего края лоскута с контуром глазной щели. Углы хряще-конъюнктивального лоскута фиксированы сквозными узловыми швами 8-0 к боковым стенкам хряще-конъюнктивального сквозного дефекта, наложены 2 узловых фиксирующих шва во вновь формируемом интермаргинальном пространстве. Образованный хряще-конъюнктивальный лоскут мобилизован с сохранением питающей сосудистой ножки из тканей круговой мышцы глаза и смещен вертикально вверх вплоть до совмещения с контуром глазной щели по нижнему веку. Смещенный лоскут фиксирован 4 узловыми сквозными швами 8-0 (викрил) и 2 узловыми швами (8-0) в интермаргинальном пространстве века. Кожный дефект раны устранен путем пластики полнослойным кожным лоскутом, взятым с верхнего века того же глаза. Example 2. A skin wound defect was removed by plastic surgery with a full-layer skin flap taken from the upper eyelid of the same eye.
