Description

 The invention relates to medicine, in particular to obstetrics and gynecology, and may find application in the treatment of miscarriage, especially with a low location of the chorion.

 There is a method of non-drug therapy for miscarriage, by percutaneous electrical stimulation at the point of BM-147 TA, using a microcurrent of 5-10 μA, for 2-3 sessions (Orlov V.I. Clinical and experimental justification for the diagnosis and non-drug therapy of some forms of miscarriage. Diss.d.s., Rostov-on-Don, 1987).

 The disadvantage of this method is the limited use of the second half of pregnancy. In addition, with the right choice of acupuncture point, the phenomenon of “pain migration” appears, which contributes to the occurrence of neurogenic reactions and psycho-emotional discomfort in patients. The use of special equipment, the need for daily sessions of electrical stimulation and control allows assistance only in stationary conditions.

 The closest in technical essence is the method of acupuncture for the treatment of threatening abortion of the second half, which consists in conducting acupuncture at points F2, F3, RP-6 for 30 minutes from 5 to 7 times (L.A. The main disadvantages of this method are:
1. The inability to prevent placenta previa due to the lack of effect on the branching chorion migration (As the authors note, all women who were diagnosed with low chorion attachment to 38 weeks of pregnancy had placenta previa, that is, the method does not promote chorion migration).

 2. The possibility of using the method only in the second half of pregnancy (For the treatment of threatening preterm birth).

 3. Mandatory combination with tocolytic therapy (B-mimetics).

 4. The need for daily acupuncture (5-7 sessions of 30 minutes) to achieve a clinical effect.

 The objective of the invention is a method of treating miscarriage in the first and second trimester, especially with a low location of the chorion, which allows to reduce the frequency of the threat of interruption and drug therapy, to simplify it by conducting on an outpatient basis.

 The problem is solved by acupuncture exposure to corporate biologically active points GI1O-DS, V26-DS, V25-DS, MC6-DS, V24-DS, V23-DS with microneedles for 14 days, while receiving Wobenzym, with 2-3 times repetition of the procedure.

The neurohumoral reflex mechanism, which includes:
1. Mechanical long-term (from several hours to 7-14-21 days) needle irritation of the skin receptor apparatus, which activates the formation of histokines at the site of exposure.

 2. Creation of chronic aseptic inflammation in certain points of the skin with a focus of prolonged impulse, which leads to the activation and release of proteases and enzymes involved in the formation of kinins, which through neurohumoral pathways lead to stimulation of diencephalic structures.

 3. The emergence of a single reaction of the whole organism in the form of three simultaneously interconnected reactions - local, general and segmental - due to a change in the functioning of functional elements and the functional systems themselves, which leads to normalization of the placental system, treatment of the threat of termination of pregnancy and creates conditions for migration branching chorion.

In practice, the method is carried out in a room specially equipped for acupuncture, in a sitting or lying position, in a position convenient for the patient and always relaxed. The skin at the injection site with a microneedle and around 1 cm ^{2 is} thoroughly disinfected with 75 ^o ethyl alcohol. In order to avoid unpleasant sensations, the introduction of needles is done after the alcohol has dried at the injection site. Using a sterile micro-forceps, the doctor grabs the needle by the handle and, with a quick, light rotational movement, inserts it at an angle of 3-5 ^o to the selected point at the desired depth.

This technique causes rapid subthreshold irritation of the superficial nerve formations of the skin and does not cause any pain. Fixation to the skin is carried out with a round-shaped medical adhesive plaster of 1 cm ² size. After the expiry of the exposure with tweezers, remove the patch and remove the needle.  According to ultrasound, the pregnancy is 11 weeks, the chorion is formed along the back wall, the lower edge reaches the area of the internal pharynx (low location of the chorion).

 Microiglotherapy was carried out at points GI1O-DS, V26-DS, V25-DS, MC6-DS, V24-DS, V23-DS for 14 days, while taking Wobenzim 1 tablet 6 times a day.

 After 14 days, according to ultrasound, the chorion forms along the back wall and in the bottom of the uterus.

 Clinical signs of the threat of interruption ceased, the needles were removed with a satisfactory condition of the pregnant woman.

 Example 2: Patient Yakimova E.V., 20 years old.

 Was admitted to the clinic of obstetrics and gynecology of the Sechenov MMA with complaints of drawing pains in the lower abdomen.

 Diagnosis at admission: Pregnancy 11-12 weeks. Threatening early miscarriage. Low placentation. Hirsutism.

 From the anamnesis: menstruation from 13 years, after 30 days, 5-7 days.

 Pregnancy-1.

 According to the data of ultrasound upon admission: the chorion is formed along the front wall, the lower edge reaches the region of the internal pharynx.

 Against the background of taking Wobenzym 1 tablet 6 times a day, microigloreflexotherapy was carried out at points GI10-DS, V26-DS, V25-DS, V24-DS, MC6-DS, V23-DS for 14 days.

 14 days after ultrasound therapy, the placenta is located on the front wall 2 mm above the area of the internal pharynx.

 Taking into account the positive effect of the course of microigloreflexotherapy, it was decided to extend the exposure for another 14 days.

 After 28 days by ultrasound - the placenta is located on the front wall, closer to the bottom. The needles are removed and the pregnant woman was discharged in satisfactory condition in a period of 14-15 weeks.

 Example 3: Patient Kozlova A.M., 23 years old.

 I went to the clinic of the obstetrics and gynecology clinic of the Sechenov MMA with complaints of pain in the lower abdomen and lower back.

 Diagnosed with Pregnancy 20-21 weeks.

 Threatening late miscarriage. Regional presentation of the placenta. Carriage of HSV and CMV.

 From the anamnesis: primary infertility within 3 years, pregnancy occurred on the background of stimulation of ovulation with clostelbigitis and dexamethasone.

 According to ultrasound data Pregnancy is 21-22 weeks. The tone is "0". Regional presentation of the placenta.

 Microneedles for 14 days were delivered to the points GI1O-DS, V26-DS, V25-DS, MC6-DS, V24-DS, V23-DS against the background of taking Wobenzim 1 tablet 6 times a day.

 Ultrasound in 14 days. Pregnancy is 24-25 weeks. The tone is "0". The placenta is located on the back wall.

 Microneedles were removed with a satisfactory condition of the pregnant woman and with a progressing pregnancy.

 The proposed method treated 20 patients with miscarriage and presentation of the branched chorion and placenta in the period from 4 to 24 weeks of gestation.

 Using the proposed method led to a decrease or complete disappearance of clinical signs of the threat of termination of pregnancy in all 20 patients. While in the treatment in a known manner, the effectiveness of the treatment of the threat of interruption was 80%.

 Using the proposed method led to the creation of conditions for the migration of chorion and prevented placenta previa in all 20 women, which is confirmed by the data of the ultrasound study. Whereas when applying the known method there is no effect on the branching chorion migration (as the authors note, all women who were diagnosed with low chorion attachment to 38 weeks of pregnancy had placenta previa).

 Using the proposed method allows to achieve a clinical effect with a single injection of microneedles for prolonged exposure for 14 (28, 32) days. In the known method, daily sessions are necessary (5-7 times) for 30 minutes.

 The proposed method allows to reduce or completely cancel the intake of drugs, the known method must be combined with tocolytic agents.

 Using the proposed method is not limited to the duration of pregnancy, but the greatest efficiency is achieved when using it in a period of 4 to 24 weeks.

 The application of the known method is limited to the second half of pregnancy.

 The proposed technique allows for treatment on an outpatient basis, and also reduces the time spent by patients in the hospital.
