Abstract
Description
The invention relates to medicine, in particular to ophthalmic plastic and reconstructive surgery, can be used to restore various forms of contraction of the conjunctival cavity, shortening the arches, deformations of the conjunctival lining, prolapse of the lower arch, transverse commissures in the arches, with inversion of the eyelids, accompanied by lagophthalmus.
Currently, the most common methods for reconstructing the conjunctival membrane during anophthalmos are methods involving transplantation of an autologous skin flap.
А. Г. В. 1991. So, the best known method of autologous transplantation of epithelial layers with underlying tissue, taken from the surface of the patient’s lips or vulva (Surgical treatment of simblepharons, Methodical recommendations, R. A. Gundorova, M. G. Kataev, E. V. Chentsova, M., 1991 .
Известен также современный метод использования выращенных аутологичных клеток конъюнктивы для восстановления конъюнктивальной полости (Scuder G.L., Mancino R., Nucci С., Alfano С., Scuderi N., Clical transplantation of cultured conjuncdvain oculoplastic surgery, XXVIII International Congress of Ophthalmology, Book of Abstracts, Amsterdam, 1998, P. 171). There is also a modern method for using grown autologous conjunctival cells to restore the conjunctival cavity (Scuder GL, Mancino R., Nucci C., Alfano C., Scuderi N., Clical transplantation of cultured conjuncdvain oculoplastic surgery, XXVIII International Congress of Ophthalmology, Book of Abstracts Amsterdam, 1998, P. 171).
Common disadvantages of the methods is the additional trauma of the patient associated with the creation of an additional surgical field and an increase in the complexity and duration of the operation.
To avoid additional injury to patients, transplantation of allogeneic epidermis is used.
For example, a method is known for reconstructing a conjunctiva by transplanting a conjunctiva or peritoneum from stillborn fetuses (Zaykova MV Plastic ophthalmic surgery. M., 1980. S. 58.). This method is the closest analogue.
However, the disadvantage of this method is the extremely low efficiency of restoration of the conjunctival cavity due to the low viability of the tissues of the dead fetus and the formation of scars. In addition, the use of material from stillborn fetuses causes a number of ethical problems, and is also associated with the risk of transmission of vector-borne diseases. These reasons probably caused an extremely rare use of this approach.
The objective of the invention is to remedy these disadvantages.
The problem is solved in that in the method of surgical restoration of the conjunctival membrane using an allogeneic transplant, the epithelium isolated in its pure form or epithelial cells grown outside the body are used as the latter.
Moreover, the epithelium is isolated from biopsies of the skin of healthy donors (for example, after plastic surgery) by methods known and widely used in medicine and cosmetology: either by the enzymatic method, or by vacuum peeling, or by the method of vacuum lemon peel.
Methods for culturing epithelial cells outside the body are also widely known in biology and medicine.
The graft is transferred to a conjunctival cavity formed by surgery or otherwise, maintaining a polar orientation, and fixed to the wound surface, for example, using dressings or otherwise.
Transplantation is carried out once or repeatedly until the conjunctiva is completely restored. Repeated transplants are performed at intervals of one day to one week.
1 - полностью отказаться от использования собственных тканей и избежать необходимости нанесения дополнительных травм пациенту;
2 - повысить эффективность восстановления конъюнктивальной полости за счет трансплантации живых клеток;
Using the proposed method allows you to:
1 - completely abandon the use of their own tissues and avoid the need for additional injuries to the patient;
2 - increase the efficiency of restoration of the conjunctival cavity due to transplantation of living cells;
3 - to avoid the effect of scarring.
The method is illustrated by the following example.
The method is carried out when restoring the conjunctival cavity in patient A, 34 years old. Patient A., condition after orbital ectentration due to a lacrimal gland tumor followed by plasty of the soft tissue contents of the orbit with omentum and reconstruction of the outer wall of the orbit with a rib graft, double-operated with ankyloblephar (complete fusion of the eyelids), with pronounced shortening of the palpebral fissure. Two months before surgery, the patient underwent a course of chemo- and radiotherapy.
The first transplantation is carried out on the next day after the surgical formation of the conjunctival cavity. When examining the wound bed, a fresh wound is determined, with pronounced signs of inflammation, slight capillary bleeding. The epithelium isolated enzymatically is placed on the wound surface with a layer of basal cells down. The wound is closed so that the entire conjunctival cavity is uniformly covered by fragments of allogeneic epithelium. Good adhesion of the epithelium to the wound surface is observed.
After 8 days, when examining the cavity on the inner surface of the lower eyelid at the junction in the bottom of the conjunctival cavity, an epidermal transplant is tightly connected to the underlying tissue.
After wound sanitation, a transplantation of allogeneic epithelium is performed - after 12 days, the transplanted epithelium was not found on the wound surface (lysis occurred, i.e., dissolution of the graft cells). The upper and lower arches are saved.
By the time of the third transplantation (after 15 days), pronounced marginal epithelization is observed on the wound surface. In addition, on the part of the wound surface, in the region of the upper and to a lesser extent of the lower conjunctival pocket, transplants are tightly connected to the granulation tissue.
The third transplantation is carried out to places unprotected by the epithelium, as well as where lysis of previously transplanted pieces of the epithelium is observed.
A histological examination of biopsy material taken 17 days after the last transplant reveals a connective tissue covered with a multilayer non-keratinizing squamous epithelium of various thicknesses.
Thus, the reconstructed epithelium is its own epithelium, restored due to stimulation by transplanted allogeneic epithelium.
The expected effect from the introduction of the proposed method into practice seems very significant and is associated with an increase in the effectiveness of treatment, a decrease in the volume and morbidity of the surgical intervention.
