Abstract
Description
The invention relates to medical equipment and can be used in traumatology and orthopedics to obtain devices that optimize the processes of osteoreportation in the treatment of injuries and diseases of the musculoskeletal system.
Currently used for surgical treatment implants coated with a dielectric in an electret state create a normal biopotential in the field of osteosynthesis, which prevents the development of atrophy and a decrease in bone strength, as well as necrosis and deformation of the surfaces of large joints, while reducing treatment time and minimizing postoperative complications.
To create electret coatings, it is necessary to ensure high purity and a given stoichiometric composition of dielectric films in the electret state, which determine the functionality of implantable structures for osteosynthesis. The most important factor is the optimal mode of their manufacture.
A known method of manufacturing an implant with electret properties for osteosynthesis (1) by sequentially depositing a multilayer coating on an implant that is placed in the chamber of the vacuum evaporation unit, a titanium implant is used as the initial product, onto which tantalum is subsequently coated as a sublayer and then oxide as an electret layer. This method is implemented on an electron beam installation with a removable target of tantalum and its oxide.
(2). The specified method is very effective for the manufacture of flat implants having a length of not more than 10 cm.For implant designs of complex shape (L-shaped and other forms), which are starting to be used in modern medicine, a more promising method of manufacturing an implant in an electric arc installation (2).
The essence of this method is that the implant is placed in a vacuum evaporation chamber, cleaned in a glow discharge, then a tantalum layer is coated with an electric arc, the implant is cooled in a vacuum, then the implant is removed from the vacuum evaporation chamber and placed in an electrolytic bath, where the tantalum layer is oxidized. After electrolytic oxidation, the electret properties of the tantalum oxide film in the corona discharge are ensured.
- нанесение тантала на имплантат в электродуговой установке;
- окисление тантала в электролитической ванне;
The disadvantages of this method is the multi-cycle manufacturing of the implant:
- application of tantalum to the implant in an electric arc installation;
- oxidation of tantalum in an electrolytic bath;
- obtaining electret properties in the corona discharge.
In addition, electric arc installations use massive tantalum cathodes weighing about 1 kg and costing over $ 10,000 at world prices.
It is not possible to fully utilize the entire mass of the cathode in the process of its electric arc evaporation due to the irreversible failure of the cathode assembly of the technological unit due to changes in the interelectrode distances that occur when the cathode geometry changes during its evaporation.
Therefore, in practice, to obtain a high-quality coating, the tantalum target is used on average only by 35-40%, which determines the high cost of finished products.
The main objective of the invention is to increase the production efficiency for the manufacture of implants within a single technological cycle.
The goal is achieved by the fact that when creating an implant for osteosynthesis with electret properties by depositing a coating on an implant made of titanium, which is placed in the chamber of a technological installation for vacuum spraying, the implant is heated in a glow discharge in the range of 250-400 o C in a vacuum no worse than 10 -2 Pa, after which a tantalum oxide film Ta 2 O 5 is sprayed by magnetron sputtering of a tantalum target in a mixture of argon and oxygen, and the sequence of these operations due to ion bombardment of the tantalum oxide film in the process of its application provides its electret properties.
Ta2O5.In addition, the tantalum target is sputtered in an atmosphere of a mixture of argon with a partial pressure of 10 -1 - 1 Pa and oxygen with a partial pressure necessary for the formation of stoichiometric oxide Ta 2 O 5 .
The application of electret coating on implants for osteosynthesis is carried out on the installation, the scheme of which is shown in the drawing.
The installation (see drawing) consists of a vacuum chamber 1 equipped with a magnetron with a tantalum target 2 with a power source 3, a vacuum inlet of rotation 4, a drive for rotating the implants (substrates) 5, on which the substrates are placed 6. In the vacuum chamber 1, a radiation implant heater is mounted (substrates) 7. The inlet of working gases of argon and oxygen is carried out using metering valves (leakage) 8. Measurement of the degree of vacuum in the vacuum chamber 1 is carried out using thermoelectric 9 and ionization 10 gauges. The direction of pumping to create a vacuum is indicated by an arrow. The drawing conventionally does not show a standard device for ion cleaning of implants in a glow discharge.
The method is as follows. The titanium implants are placed in a vacuum chamber 1, cleaned in a glow discharge, heated with a radiation heater to a temperature of 250-400 o C in vacuum no worse than 10 -2 Pa, then the mixture of argon with oxygen and magnetron sputtering of the tantalum target in a mixture of argon with a tantalum oxide film Ta 2 O 5 is sprayed with oxygen. The degree of vacuum in the vacuum chamber and the partial pressures of argon and oxygen are monitored using thermoelectric and ionization manometers. In the process of applying a tantalum oxide film of Ta 2 O 5, it is subjected to intense ion bombardment, which creates electret properties of the tantalum oxide coating. Due to this, there is no need for further processing of the applied coating (for example, in a corona discharge) in order to ensure electret properties, and the production of implants with electret properties is carried out within the framework of one technological process.
The argon and oxygen working gases are inflated as follows: the partial pressure of argon in the range of 10 -1 - 1 Pa ensures stable combustion of the magnetron discharge, i.e. a stable sputtering speed of the tantalum target, and the partial pressure of oxygen is selected from the condition for the formation of stoichiometric oxide Ta 2 O 5 (five oxygen atoms should fall into two tantalum atoms).
Thus, the proposed method of manufacturing an implant with electret properties for osteosynthesis allows to obtain a finished implant with electret properties in the framework of one technological process.
During the development and improvement of the proposed method for manufacturing an implant with electret properties for osteosynthesis, various ranges of process parameters were studied and prototypes of implants, including complex ones, were obtained. In addition, due to the peculiarities of the magnetron sputtering method, the tantalum target is sprayed in layers and uniformly over the entire surface, which made it possible to increase the degree of use of the tantalum of the target to 90-95%. Implant prototypes have been clinically tested with positive results.
The economic effect of the introduction of the proposed method consists of saving the target material (tantalum), reducing the number of technological processes, the manufacturing time of a unit of production, as well as the duration of treatment of injuries and diseases of the musculoskeletal system by reducing the cost of social insurance, sick leave and keeping injured and patients in medical institutions.
