Abstract
Description
Предлагаемое техническое решение относится к медицине, в частности к сосудистой хирургии, и предназначено для лечения хронической ишемии, возникшей при облитерирующем эндартериите или атеросклерозе, посттромбоэмболической недостаточности кровообращения. The proposed technical solution relates to medicine, in particular to vascular surgery, and is intended for the treatment of chronic ischemia that occurs with obliterating endarteritis or atherosclerosis, post-thromboembolic circulatory failure.
Известен способ лечения хронической ишемии конечности путем образования новых сосудистых связей при дистракции костных фрагментов в проксимальном сегменте с темпом 1-1,5 мм/сут и в дистальном сегменте с темпом 0,25-0,5 мм/сут (1). Поставленная цель достигается тем, что дистракцию производят с последовательным растяжением тазобедренного, коленного и голеностопного суставов пораженных сегментов с темпом 2 мм в сутки, чередуя каждые 5 суток дистракцию и расслабление ее до исходного состояния суставных поверхностей. До госпитализации всем больным производилось в обязательном порядке исследование температуры кожных покровов, капилляроскопия, сегментарная реовазография, а также ультразвуковое исследование кровотока в конечности. Проведенное исследование полученных реовазограмм также свидетельствует о снижении кровотока в пораженной конечности. Больной В 48 лет. Ист. бол. 1226, поступил на лечение в хирургическое отделение 3.04 с диагнозом: Облитерирующий эндартериит нижних конечностей, гангрена 1-2 пальцев правой стопы. На реограмме (фиг. 1) резкое снижение зубцов на стопе вплоть до прямой линии. Patient At 48 years old. Начиная с первых дней лечения больной отказался от обезболивающих средств, ранее длительное время применяемых для купирования болевого синдрома. После курса дистракционного лечения состояние больного улучшилось. Он приступил к своей прежней работе электрика. Пример 2. Больной С., 47 лет Ист. бол. 1567, поступил на лечение в хирургическое отделением с диагнозом: Облитерирующий атеросклероз сосудов нижних конечностей IVa стадии. Гангрена 1-4 пальцев правой стопы. На вторые сутки произведена операция экзартикуляция 1-4 пальцев правой стопы и с ортопедических соображений 5 пальца. Наложен дистракционный аппарат на бедро, голень стопы. На реограмме выявлено резкое снижение зубцов на уровне сосудов голени (фиг. 5). Ультразвуковым исследованием выявлено резкое сужение даже бедренных артерий с обеих сторон (фиг. 6). По нашей методике проведено дальнейшее лечение. Через 4 недели аппарат снят, лечение закончено. Больной приступил к своей прежней работе - токарем. Наблюдение в течение 1,4 года свидетельствует об эффективности лечения. (фиг. 7 - реовазограмма правой голени через 1 год после лечения; фиг. 8 - на ангиограммах резкое усиление коллатерального кровообращения в исследуемой конечности). Example 2.
