Способ осуществляется следующим образом. В положении больного на правом боку с установленным валиком на уровне XI-XII грудного позвонка выполняется разрез по IX межреберью слева от задней подмышечной линии до парарентальной на уровне пупка. Послойно рассекается кожа, подкожная клетчатка, мышцы. Вскрывается плевральная полость. Диафрагма рассекается по периметру. Брюшинный мешок тупым и острым путем широко отслаивается кпереди от поясничных мышц, почки, диафрагмы. В рану устанавливается ранорасширитель, после чего становится доступной задняя поверхность хвоста и тела поджелудочной железы с расположенной на ней селезеночной веной, передняя поверхность почки и почечная ножка. Вскрывают преренальную фасцию и выделяют почечную вену на протяжении 4-6 см. Затем на протяжении 5-6 см выделяют селезеночную вену без мобилизации поджелудочной железы и дуоденоеюнального изгиба в новых анатомических соотношениях, при которых в ране селезеночная (1, фиг. 3) и поджелудочная вены (2, фиг. 3) располагаются кпереди от поджелудочной железы (3, фиг. 3). Для этого вены поджелудочной железы выделяют, перевязывают и пересекают над поджелудочной железой, ориентируясь на свободную заднюю стенку селезеночной вены, которая в ране при данном доступе является передней. Это значительно облегчает и ускоряет на 1-1,5 ч выполнение данного этапа операции, а также позволяет произвести выделение селезеночной вены без травмы поджелудочной железы и дополнительной кровопотери. Мобилизованную селезеночную вену в проксимальном ее отделе у места соединения с верхней брыжеечной веной, пересекают, прошивают и перевязывают, а на дистальный конец накладывают сосудистый зажим. На почечную вену накладывают зажим Сатинского, в стенке ее высекают полуовальное отверстие, соответствующее диаметру селезеночной вены. После этого формируют дистальный силеноренальный анастомоз по типу конец в бок (фиг. 3). Для этого вначале на заднюю губу анастомоза, а затем на переднюю губу анастомоза под углом наклона оси операционного действия 82-87o (фиг. 1) накладывают непрерывный шов атравматичной нитью снаружи во внутрь. При таком наложении анастомоза края вен выворачивают наружу шовной нитью без травмы нитями, после чего интима вен тесно прилежит друг к другу, а шовная нить со стороны просвета анастомоза становится не видна. Плееральную полость и забрюшинное пространство после наложения анастомоза дренируют, а рану послойно ушивают.The method is as follows. Асцит в течение первых 10 дней несколько уменьшился, однако в дальнейшем без всякой динамики. Учитывая наличие диуретокорезистентного асцита, высокую вероятность рецидива кровотечения, больному показано оперативное лечение: дистальный спленоренальный анастомоз. Послойно рассечены ткани, вскрыта плевральная полость, диафрагма рассечена по периметру. Брюшинный мешок отделен и сдвинут кнаружи и в рану установлен ранорасширитель. После этого стала доступной задняя поверхность хвоста и тела поджелудочной железы с расположенной на ней селезеночной веной, передняя поверхность почки и почечная ножка. Вскрыли преранальную фасцию и выделили почечную вену на протяжении 4-6 см. Затем на протяжении 5-6 см выделили селезеночную вену без мобилизации поджелудочной железы и дуоденоеюнального изгиба в новых анатомических соотношениях, при которых в ране селезеночная и поджелудочные вены расположены кпереди от поджелудочной железы. Для этого вены поджелудочной железы выделили, перевязали и пересекли над поджелудочной железой, ориентируясь на свободную стенку селезеночной вены, которая в ране при доступе является передней. Селезеночная вена выделена без травмы поджелудочной железы и дополнительной кровопотери. Мобилизованная селезеночная вена в проксимальном ее отделе у места соединения с верхней брыжеечной веной пересечена, прошита и перевязана, а на дистальный конец наложен сосудистый зажим. На почечную вену наложен зажим Сатынского, в стенке ее высекли полуовальное отверстие, соответствующее диаметру селезеночной вены. После этого сформирован силеноренальный анастомоз по типу конец в бок. Для этого вначале на заднюю губу анастомоза, а затем на переднюю губу анастомоза под углом наклона оси операционного действия 82-87o наложен непрерывный шов атравматичной нитью снаружи во внутрь. При таком наложении анастомоза край вен выворачивается наружу шовной нитью без травмы интимы, после чего интима вен тесно прилегла друг к другу, а шовная нить со стороны просвета анастомоза стала не видна. Плевральная полость и заброшенное пространство после наложения анастомоза дренированы, а рана послойно ушита. В послеоперационном периоде отмечались кратковременная гипербилирубинэмия (40 ммоль/л) и анемия (Эр. 3,0•1012, гемоглобин 90 г/л). Больному продолжена консервативная терапия, включающая переливание белковых растворов, глюкозы, витаминов, препаратов, улучшающих метаболические процессы в печени, антибиотики, диуретики. Дренажи удалены на 2-е стуки. Лабораторные показатели нормализовались к 7-м суткам послеоперационного периода. Кожные швы сняты на 14-е стуки и на 17-й день больной выписан в удовлетворительном состоянии из клиники. На момент выписки при объективном исследовании живота асцит не определился.April 5, 1996, the patient was operated on.
