В кардиохирургии, наряду с широко используемой срединной стернотомией, применяется чрездвухплевральный доступ к сердцу и магистральным сосудам. В случае возникновения инфекционного осложнения после чрездвухплевральной торакотомии развивается передний медиастинит в сочетании с двусторонней эмпиемой плевры. Лечение этого тяжелого осложнения осуществляется "открытым" способом и заключается в разведении краев послеоперационной раны, грудины, удаления с поверхности раны гнойного отделяемого, частых перевязок с гипертоническим раствором, с последующим наложением масляно-бальзамических повязок (1). Заживление больших раневых дефектов протекает крайне длительно, с деформацией грудной клетки, формированием грубых келоидных рубцов, развитием костнохондральной инфекции, септических осложнений в виде эррозивных кровотечений, тромбоэндокардита. The healing of large wound defects takes an extremely long time, with chest deformation, the formation of gross keloid scars, the development of bone chondral infection, septic complications in the form of erosive bleeding, thromboendocarditis.

Способ поясняется чертежом и осуществляется следующим способом: в день установки диагноза гнойного медиастинита и двусторонней эмпиемы плевры больному в операционной под интубационным наркозом выполняют хирургическую обработку: широко разводят грудную клетку, удаляют гнойные массы, сгустки крови, некротические ткани. Затем плевральные полости, сердце, магистральные сосуды промывают большим количеством антисептической жидкости (3% раствор перекиси водорода, фурациллина, "Микроцида") с целью уменьшения микробного обсеменения тканей. Острые края грудины обрабатывают кусачками Листона. После этого проколом VI-VII межреберья по среднеаксилярной линии в плевральную полость вводят двухпросветный дренаж с перфорационными отверстиями и располагают его S-образно во фронтальной плоскости с подведением концов дренажей к грудине. При этом перфорационное отверстие в дренаже, который эвакуирует отделяемое наружу, должно находиться вблизи стенки плевральной полости во избежание образования "слепого пространства". Дренажи подшивают к коже. Края грудины сшивают отдельными проволочными швами (из нержавеющей стали). After this, a double-lumen drainage with perforations is inserted into the pleural cavity by puncture of the VI-VII intercostal space along the midaxillary line and placed in an S-shape in the frontal plane with the drainage ends leading to the sternum. In this case, the perforation hole in the drainage, which evacuates the discharge to the outside, should be near the wall of the pleural cavity in order to avoid the formation of "blind space". Drainages are sutured to the skin. The edges of the sternum are sutured with separate wire sutures (stainless steel).
В послеоперационном периоде, наряду с проводимой дезинтоксикационной, антибактериальной терапией, осуществляется проточно-аспирационное дренирование плевральных полостей. Для промывания обычно используют стандартные емкости по 400 мл физиологического раствора с добавлением 20 мл 1%-го раствора диоксидина, либо физиологический раствор с антибиотиком, к которому чувствительна данная микрофлора. Осмолярность раствора должна быть не ниже 300 м/осмоль. Раствор подается из флакона через разовую капельницу, устанавливаемую сбоку от больного. Промывные воды удаляются активной аспирацией вакуумным отсосом с разряжением 1-2 атм. Темп промывания первая порция должна подаваться струйно (для профилактики тромбирования дренажей фибрином), затем капельно 30-60 капель в минуту. Перед удалением дренажей выполняют цитологический контроль промывных вод и, если количество лейкоцитов в поле зрения не превышает 10-25, то дренажи удаляют. Швы с кожи снимают на 12-13 сутки. В ходе клинического обследования установлен диагноз: врожденный порок сердца дефект межпредсердной перегородки вторичного типа, частичный аномальный дренаж правых легочных вен в устье верхней полой вены. Операция 2.08.93 г. в условиях углубленной гипотермической защиты, из чрездвухплеврального доступа выполнено перемещение аномально дренирующих правых легочных вен в левое предсердие путем закрытия межпредсердного дефекта, располагающегося в области устья верхней полой вены, заплаткой из аутоперикарда. Послеоперационный период протекал тяжело с явлениями сердечно-легочной недостаточности, повышенным накоплением в плевральных полостях жидкости, по поводу чего выполнялись плевральные пункции. Характер полученного пунктата имел признаки инфицирования (количество лейкоцитов в поле зрения больше 45). На этом фоне появилась гипотермия до 38oC, озноб, симптомы интоксикации. Из плеврального пунктата получен рост синегнойной палочки. В этой связи заподозрено инфекционное осложнение - развитие двусторонней эмпиемы плевры, медиастинита. 9.06.93 г. в условиях интубационного наркоза в операционной выполнена хирургическая санация плевральных полостей, переднего средостения: удалена жидкость желто-мутного цвета, фибрин, некротические ткани. Раневые полости промыты большим количеством 3% раствора перекиси водорода, фурациллина, раствором "Микроцида", затем в плевральные полости проколом в VII м/р по средне-аксиллярной линии установлены двухпросветные дренажи, которые располагают S -образно с подведением концов дренажей к грудине. Дренажи подшивают к коже отдельными швами. Края поперечного распила фиксируют проволочными швами. Мягкие ткани торакотомной раны ушивают в один слой. В послеоперационном периоде проводят проточно-аспирационное дренажирование плевральных полостей и переднего средостения. Дренажи удалены на 15 сутки, после цитологического контроля швы с кожи сняты раньше на 13 сутки. Послеоперационный шов зажил по типу первичного натяжения. Wound cavities are washed with a large amount of a 3% solution of hydrogen peroxide, furatsillina, Microcide solution, then double-lumen drainages are installed in the pleural cavity with a puncture in the VII m / r along the mid-axillary line, which have an S-shape with the drainage ends leading to the sternum. Drainages are sutured to the skin in separate sutures. The edges of the transverse cut are fixed with wire seams. Soft tissue of the thoracotomy wound is sutured in one layer. In the postoperative period, flow-aspiration drainage of the pleural cavities and the anterior mediastinum is performed. Drainages were removed on the 15th day, after cytological control, the sutures from the skin were removed earlier on the 13th day. The postoperative suture healed by type of primary tension. The child was discharged in satisfactory condition.
