Description

The invention relates to medical equipment and is intended for suturing wounds of an aponeurosis, for example, after laparoscopy.

Known surgical stapler, containing a connected drive and articulated curved branches with annular handles, a stopper and working parts with bracket grooves.

A disadvantage of the known apparatus is that when using the device, it is difficult to protect the internal organs protruding into the wound with U-shaped brackets after removing the laparoscope tube.

Due to the proximity of the design concept, the device is taken as the base object.

The purpose of the invention to facilitate the closure of wounds of the aponeurosis after laparoscopy.

The goal is achieved by the fact that the working parts are made in the form of rollers placed on the axes with longitudinal external protrusions for the drive in the form of a laparoscope tube with reciprocal transverse protrusions on the outer surface, while the jaws are bent in steps, the bracket grooves are located on the rollers and made under a ring-shaped bracket having needle teeth and an inner surface under the tube of the laparoscope, and the stopper is made in the form of a spring-loaded ratchet connected by a flexible rod with an additional handle, and the ratchet mouth updated with the ability to interact with longitudinal protrusions and is located in guide ledges on the side surfaces of the branches; the ring-shaped bracket is provided with mounting protruding elements and is made with opposite thinned side sections, and the needle teeth are oriented perpendicular to its working surface.

26. The surgical stapler consists of two stepwise curved and articulated jaws 1 and 2 (Fig. 1, 2 and 3) with annular handles 3 and 4 at one end and with mounted axes 5 and 6 at the other, on which rollers 7 and 8 are mounted, on the surface of which longitudinal protrusions 9 and grooves 10 are selected, into which protrusions 11 (Figs. 4 and 5) of the annular bracket 12 are inserted. On the opposite surface of the annular bracket 12 are acicular teeth 13, and its side parts are thinned in sections 14 and 15. On the outer surface of the tube laparoscope 16 (f Ig. 6 and 7) protrusions 17 are also formed, but in the transverse direction with respect to its longitudinal axis. The surgical stapler is equipped with a locking device (Fig. 8 and 9), consisting of a ratchet 18 with a rod 19, equipped with a rear 20. The core 19 passes through the opening of the protrusion 21, and a flexible rod 22 is attached to the rear 20, which is passed through the openings of the protrusions 23 and 24 and attached to additional handles 25. Between the back of the 20 and the protrusion 21 is placed a return spring 26.

7). The operation of the apparatus. An annular bracket 12 is put on the tube of the laparoscopy 16, after which the end of the tube is inserted into the abdominal cavity in a known manner (Fig. 6). After laparoscopy or laposcopic surgery is completed, branches 1 and 2 are pulled apart and the protrusions 11 of the ring-shaped bracket 12 are inserted into the grooves 10 of the rollers 7 and 8, and the bracket 12 is lowered by means of a stapler so that the surfaces of the rollers 7 and 8 do not come into contact with the tube 16 , with its serrated protrusions 17. The needle teeth 13 of the ring-shaped bracket 12 pass through the aponeurosis 27 (Fig. 7).

Damage to internal organs is thus excluded, since air in the abdominal cavity keeps the abdominal wall at a distance from the internal organs. Squeezing the annular handles 3 and 4 with the first and fourth fingers of the right hand, bring the rollers 7 and 8 together until they touch the protrusions 17 of the laparoscope tube 16, and with the second and third fingers pull the additional handles 25, as a result of which the rollers 7 and 8 are released. pulling the laparoscope tube 16 upwards, the rollers 7 and 8 begin to rotate (Fig. 10), as a result of which the tips of the needle teeth 13 of the ring-shaped bracket 12 abut against the surface with the protrusions 17 of the laparoscope tube 16 and are bent. After removal of the tube of laparoscopy 16 (Figs. 11 and 12), a gaping wound 28 remains in the aponeurosis 27. When the jaws 1 and 2 are squeezed by the ring-shaped handles 3 and 4, the ring-shaped bracket 12 bends in its thinned sections 14 and 15 and collapses, as a result of which the edges wounds 28 are closed (Fig. 13). After releasing the annular bracket 12 from the rollers 7 and 8, the apparatus is removed from the wound. The ring-shaped bracket 12 compresses the edges of the wound 28 due to the stiffness of the ring 12, and the needle teeth 13 not only hold the ring 12 in a predetermined position, but partially penetrate into the opposite edges of the wound, additionally bend (Fig. 14) and strengthen the seam.
