Надежность анастомоза, накладываемого в условиях выраженного натяжения анастомозируемых фрагментов трахеи, обеспечивается тем, что формирование анастомоза начинается с одиночных узловых швов на наиболее прочной фибразно-хрящевой части трахеи. И лишь вслед за завершением этого этапа такими же швами, но уже при значительно меньшем натяжении, ушивают мембранозную стенку анастомоза, которая при задне-торакотомном доступе является передней по отношению к хирургу. Таким образом, формирование наиболее опасной, в плане несостоятельности, стенки анастомоза происходит в технически благоприятных условиях. Способ избран в качестве прототипа. Такого рода способ формирования анастомоза оправдал себя при стенозах и доброкачественных опухолях трахеи с заранее известной распространенностью при отсутствии необходимости уточнения ее в ходе операции. Недостатком способа-прототипа, как показывает практика, является техническая сложность его воспроизведения при боковых торакотомиях, стернотомных и комбинированных доступах наиболее удобных, как показал опыт, в онкологической практике подходах, позволяющих хирургу сразу сориентироваться в резектабельности процесса путем ревизии анатомических структур переднего средостения. При этих доступах вслед за преодолением натяжения путем формирования фибразно-хрящевой стенки анастомоза завершающий и наиболее ответственный этап формирование мембранозной стенки выполняется в предельно сложных в техническом отношении условиях (задняя от хирурга стенка становится ему доступной лишь при перекручивании передней стенки вокруг продольной оси; сформированная в условиях натяжения фиброзно-хрящевая стенка испытывает при этом дополнительную нагрузку). В итоге страдает как передняя (фиброзно-хрящевая), так и задняя (мембранозная) стенки анастомоза, что, как известно, чревато несостоятельностью. Вслед за верхней медиастинотомией и мобилизацией верхней полой веной прослежена и выделена трахея практически на всем протяжении. Трахея и левый главный бронх взяты на труникеты, после чего резецирована бифуркация трахеи проксимальный уровень резекции 4 полукольца трахеи над кариной, дистальный уровень 2 полукольца от устья левого главного бронха. На этом этапе использовали режим высокочастотной вентиляции левого легкого. Трахео-бронхиальный анастомоз формировали следующим образом накладывали одиночные направляющие провизорные швы на края фиброзно-хрящевой части трахеи и бронха, после чего потягивая за провизорные швы накладывали сосудистым атравматическим шовным материалом, проленом, непрерывный провизорный обвивной шов на всю мембранозную стенку анастомозируемых фрагментов, затем завязывают первый провизорный шов и аналогичными швами формируют всю фиброзно-хрящевую часть анастомоза.
