Abstract
Description
The invention relates to medicine, namely to abdominal surgery, and can be used in the surgical treatment of portal hypertension syndrome.
The aim of the invention is the prevention of complications, namely the prevention of cicatricial narrowing and thrombosis of omphalo-caval anastomosis.
This method is implemented as follows. Under general anesthesia, an upper-median laparotomy is performed. After the audit of internal organs and measurement of portal pressure, determine the total volume of the operation. The umbilical vein along with the round ligament of the liver is cut off from the umbilical ring and mobilize them in a single block all the way to the place where the umbilical vein flows into the portal vein, while maintaining the vessels that feed the round ligament of the liver. The umbilical vein is recanalized by bugging it to a diameter of 1.0 to 1.2 cm according to the generally accepted technique.
The subhepatic segment of the inferior vena cava is isolated by mobilization of the duodenum according to Kocher.
Using vascular instrumentation, a vascular anastomosis of the end-to-side type is applied, as it is simpler in technical design, between the umbilical end of the recanalized umbilical vein and the subhepatic segment of the inferior vena cava.
After that, the suture region of the omphalocaval anastomosis is peritonized with serous leaves of the round ligament of the liver, while checking the patency of the anastomosis.
The operation is completed by drainage of the abdominal cavity and suturing of the laparotomy wound.
Example. Patient K., 43 years old, was admitted urgently with a clinic of gastrointestinal bleeding. According to the FEGDS, the source of bleeding was tearing of phlebectasia of the lower third of the esophagus and cardia of the stomach. By a conservative method, bleeding is stopped, blood loss is replenished. A history of prolonged alcohol abuse. After preoperative preparation, the patient was operated on as planned.
During the operation, small-node cirrhosis of the liver, dilatation of the veins of the lesser omentum of the bottom and cardia of the stomach were found. Splenoportomanometry revealed a moderate increase in portal pressure to 320 mm of water. Art. The umbilical vein with a round ligament of the liver was mobilized along its entire length and had a length of 28 cm.
The inferior vena cava is exposed by mobilization of the duodenum according to Kocher. An omphalo-caval anastomosis of the end-to-side type was formed with an internal diameter of anastomosis of 1.0 cm, and the anastomosis was covered outside with serous leaves of the round ligament of the liver. Additionally produced: electrocoagulation of the diaphragmatic surface of the liver and ligation of the splenic artery. The abdominal cavity is drained in the right hypochondrium, the surgical wound is sutured tightly. 6 months after the operation, the patient was examined, his condition is satisfactory, the bleeding did not resume. Control angiography of the portal vein system vessels confirmed the functioning of the omphalo-caval anastomosis.
