Операционный эндоскоп с инструментальным каналом через видеокамеру подсоединяется к монитору. Под визуальным контролем инструмент продвигают в дистальном направлении, по ходу ахиллова сухожилия. В проекции последнего подкожно-жировая клетчатка рыхло спаяна с фасцией и редко вовлекается в рубцово-индуративный процесс. Субфасциально выделяется перфорантный пучок, его элементы обнажаются в зоне наиболее стабильной морфологии (на уровне входа в субфасциальное пространство). Полностью дифференцировать все элементы пучка нет необходимости. Префорантную вену отделяют от окружающих тканей с помощью препаровочного крючка и диссектора. Затем ствол перфоранта захватывают би- или нонополярными щипцами. После облитерации вены ее пересекают тем же крючком или эндоскопическими ножницами, применяя дополнительно монополярную коагуляцию, предупреждающую возможное кровотечение из культи перфорантной вены. Глубокие вены проходимы. Выполнена операция. Разрезом по медиальной поверхности голени в с\3 на 2 см выше зоны трофических расстройств произведен разрез длиной 2 см. Субфасциально введен троакар, операционный эндоскоп с инструментальным каналом, под фасцию нагнетался углекислый газ под давлением 100 мм рт.ст. Визуализировано 5 недостаточных перфорантных вен от 4 до 6 мм в диаметре, перфорантные вены отделены от остальных элементов перфорантного пучка с помощью диссектора и препаровочного крючка, а затем коагулированы (в режиме "коагуляция - 2-2,5" и пересечены эндоскопическими ножницами. В послеоперационном периоде получала антибиотики, флеботоники, дезагреганты, лазеро-, магнитотерапию на трофическую язву. Рана зажила первичным натяжением. Эпителизация язвы наступила на 15 сутки. Уменьшился отек конечности, нет болевого синдрома. Контрольная флебография перед выпиской продемонстрировала отсутствие низкого вено-венозного сброса. На 16 сутки в удовлетворительном состоянии выписана домой. Отдаленные результаты оценены через 1,5 годы. Рецидивов язв не было. Трофические расстройства кожи регрессировали. Нами прооперированно 30 больных, страдающих ХВН в стадии декомпенсации с трофическими расстройствами (размеры трофических язв варьировали от 1 до 5-6 см, длительность существования от 1 года до 3 лет). Из них - 23 больным произведена традиционная эндоскопическая газовая диссекция перфорантных вен, 7 больным выполнена раздельная перевязка перфорантных вен голени с сохранением перфорантных артерий. Сроки заживления трофических язв в 1 группе больных составили в среднем 21-22 сут, во второй группе 16-17 сут. Example. B.S., 65 l.
