Abstract
Способ ранней диагностики портальной гипертензии при хронических диффузных заболеваниях печени путем измерения показателей гемодинамики в воротной и селезеночной венах методом ультразвуковой допплерографии, отличающийся тем, что в результате проведения анализов крови и биопсии ткани печени определяют лабораторные показатели биохимической активности, а также морфологической активности заболевания печени по результатам биопсии печени, затем дополнительно рассчитывают индексы застоя в воротной и селезеночной венах, характеризующие степень выраженности портальной гипертензии по формулам: ИЗ BB=S ВВ/ЛСК ВВ, где S ВВ - площадь поперечного сечения ВВ, см; ЛСК ВВ - линейная скорость кровотока в воротной вене см/с; ИЗ СВ=S СВ/ЛСК СВ, где S СВ - площадь поперечного сечения СВ, см, ЛСК СВ - линейная скорость кровотока в селезеночной вене см/с; а затем сопоставляют значения индексов застоя в воротной и селезеночной венах с показателями биохимической и морфологической активности заболевания печени, проводят анализ зависимости ИЗ СВ и ВВ от указанных показателей, на основании чего выявляют более тесную корреляционную зависимость между ИЗ СВ и показателями лабораторной (АЛАТ, АСАТ, билирубин) и морфологической (ИГА, фиброз) активности, и на основе приоритетного значения ИЗ СВ устанавливают ранние признаки портальной гипертензии на доцирротической стадии заболевания при отсутствии клинических симптомов портальной гипертензии.A method for the early diagnosis of portal hypertension in chronic diffuse liver diseases by measuring hemodynamic parameters in the portal and splenic veins by ultrasound dopplerography, characterized in that as a result of blood tests and biopsy of the liver tissue, laboratory indicators of biochemical activity and morphological activity of the liver disease are determined by the results liver biopsy, then additionally calculate the stagnation indices in the portal and splenic veins, characterizing e degree of severity of portal hypertension according to the formulas: FROM BB = S BB / LSC BB, where S BB is the cross-sectional area of BB, cm; LSK VV - linear blood flow velocity in the portal vein cm / s; FROM SV = S SV / LSK SV, where S SV - cross-sectional area of SV, cm, LSK SV - linear velocity of blood flow in the splenic vein cm / s; and then compare the values of stagnation indices in the portal and splenic veins with indicators of the biochemical and morphological activity of liver disease, analyze the dependence of FROM and BB on these indicators, on the basis of which a closer correlation between FROM and laboratory (ALAT, ASAT, bilirubin) and morphological (IGA, fibrosis) activity, and on the basis of the priority value of IU SV, early signs of portal hypertension are established at the docirrotic stage of the disease in the absence of vii of clinical symptoms of portal hypertension.
