Abstract
The invention relates to medicine, specifically to neurosurgery, neuroorthopedics, can be used to treat spinal cord injuries. The aim of the invention is to prevent the secondary formation of paraossal ossifications and to improve trophic limbs. To do this, during a surgical procedure, heterotopic ossification is removed, then an overt osteotomy is performed so that the fragments of the femur resemble a hip joint. Using a microsurgical technique, a full-fledged muscle flap on the neurovascular leg is formed from the muscle and interposed between the fragments of the femur. On the basis of the flap, an electrode thread from a graphitized tissue is fixed and withdrawn away from the surgical incision area. The next day, electrostimulation is carried out by biphasic pulses with a duration of 0.5-1.0 ms, a frequency of 300-500 Hz, an amplitude of 15-25 V for 20-40 minutes. Manipulation is carried out daily for 14-16 days. with C
Description
The invention relates to medicine, specifically to neurosurgery, neuroorthopedics, can be used to treat the effects of damage to the spinal cord
A known method for the treatment of heterotopic ossifications is Paraarticulosis and paraossal ossification in patients with spinal cord and spinal cord injuries (Al-Al Din Mohamed Abdel Gani Salam, 1989). The disadvantage of this method is that avoiding repeated heterotopic ossification is not always possible, since the main cause of such a pathology is not eliminated, namely: perverse local tissue reactions due to
нарушений вегетативной и соматической иннервации в пораженных конечност х.disorders of the autonomic and somatic innervation in the affected limbs.
The purpose of the invention is to prevent re-ossification and to improve trophic limbs.
The goal is achieved in that the prevention of re-ossification and improvement of trophic limb is achieved by transverticular thigh osteotomy with the simultaneous formation of the roof in the proximal fragment of the bone. A full muscle flap on the feeding neurovascular leg is interposed, an electrode is fixed on the basis of the flap, and the electrical stimulus is connected
The operation is carried out on the day after the operation by pulses with a duration of 0.5-1 0 ms. with a frequency of 300-500 Hz, an amplitude of 15-25 V for 20-40 minutes, Manipulation is performed daily for 14-16 days.
The method is carried out as follows: During the surgical procedure, the maximum possible amount of heterotopic ossificate is removed, then an alternate osteomy is performed so that a roof resembling the roof of the acetabulum of the hip joint is formed in the proximal fragment of the femur. From muscle m. tensor fasciae latae form using a microsurgical technique a full-fledged muscle flap on the neurovascular leg, which is placed between fragments of the femur. On the base of the flap, an electrodes made of graphitized tissue are applied for electrical stimulation and removed away from the surgical incision area. Careful hemostasis and layered sutures on dissected tissues are distinguished. Stimulation is carried out by biphasic pulses with a duration of 0.5-1.0 ms, a frequency of 300-500 Hz, an amplitude of 15-25 V for 20-40 minutes every day for 14-16 days
An example of a specific implementation is the case history 373151, patient K., 45 years old. Diagnosis: consequences of a compression fracture of Ce-7 vertebrae with the syndrome of complete transverse lesion of the spinal cord, paraossal ossificates in the area of the hip joints. An operative incision along the lateral surface of the grave in the region of the greater trodic skewer. Removed ossification. Percutaneous osteotomy was performed, a roof resembling a roof was formed in the proximal fragment of the femur
