Эта цель достигается тем, что до стимуляции выполняют запись электронейромиограммы поврежденной и контралатеральной конечностей, а также запись электронейромиограммы неповрежденной конечности на магнитограф; проводят электростимуляцию плечевого сплетения и магистральных нервов поврежденной верхней конечности в режиме: частота 1-9 Гц, сигнал длительностью 2-4 мс, напряжение от 50 до 500 В, сила тока 20 мА, продолжительностью сеанса 7-10 мин; после чего выполняют стимуляцию мышц поврежденной конечности: при одностороннем повреждении - с помощью электронейромиограмм неповрежденной конечности, записанных на магнитограф; в случае двухстороннего повреждения осуществляют "прямое" управление стимуляцией от донора или управление стимуляцией от встроенного синтезатора программ в режиме: импульсный биполярный прямоугольной формы ток с частотой модуляции от 40 до 240 Гц, длительностью импульсов от 200 до 500 мс ±10%; продолжительность сеанса - 15-20 мин, 5 каналов одновременно, количество сеансов - от 10 до 15, количество лечебных циклов - 2-3, с интервалом между ними - 1,5-2 месяца, причем проводят стимуляцию плечелучевой мышцы, лучевого сгибателя запястья, короткой отводящей большой палец, локтевого разгибателя запястья, общего разгибателя пальцев, первой тыльной межкостной, поверхностного сгибателя пальцев (как п.2 формулы).This goal is achieved by the fact that prior to stimulation, the electroneuromyogram of the damaged and contralateral limbs is recorded, as well as the recording of the electroneuromyogram of the intact limb on a magnetograph; conduct electrical stimulation of the brachial plexus and the main nerves of the damaged upper limb in the following mode: frequency 1–9 Hz, signal duration 2–4 ms, voltage 50–500 V, current strength 20 mA, session duration 7–10 min; after which they perform stimulation of the muscles of the damaged limb: in case of unilateral damage - with the help of electroneuromyograms of the intact limb recorded on a magnetograph; in the case of bilateral damage, “direct” control of stimulation from the donor or control of stimulation from the built-in program synthesizer is carried out in the mode: pulse bipolar rectangular current with a modulation frequency of 40 to 240 Hz, pulse duration from 200 to 500 ms ± 10%; the duration of the session is 15-20 minutes, 5 channels at a time, the number of sessions is from 10 to 15, the number of treatment cycles is 2-3, with an interval between them of 1.5-2 months, and the brachioradial muscle, radial flexor of the wrist are stimulated, short abduction of the thumb, ulnar extensor of the wrist, common extensor of the fingers, the first dorsal interosseous, superficial flexor of the fingers (as in claim 2 of the formula).

После этого выполняют электростимуляцию плечевого сплетения и магистральных нервов поврежденной верхней конечности в режиме: частота 1-9 Гц, длительность сигнала 2-4 мс, напряжение от 50 до 500 В, сила тока 20 мА, продолжительность сеанса 7-10 мин.The electroneuromyogram of the injured and contralateral limbs is recorded on the Nicolet / Viking Quest myograph, as well as the electroneuromyogram of the muscles of the intact limb on the magnetograph. На аппарате "Миотон-604", с накожными электродами площадью 4 см2 проводят стимуляцию следующих мышц поврежденной конечности: плечелучевая, лучевой сгибатель запястья, короткая отводящая большой палец, локтевой разгибатель запястья, общий разгибатель пальцев, первая тыльная межкостная, поверхностный сгибатель пальцев. При одностороннем повреждении управляют электростимуляцией с помощью электронейромиограмм неповрежденной конечности, записанных на магнитограф. Клинический пример. Больная Л., 1955 г.р., история болезни №25029041, поступила в отделение ортопедии-2 СОКБ им. М.И.Калинина 12.09.2004 г. с диагнозом: неправильно срастающийся разгибательный перелом дистального метаэпифиза правой лучевой кости. Травма получена 27.07.2004. С момента травмы прошло 7 недель. 13.09.04 выполнена операция: наложение аппарата внешней фиксации на правое предплечье и кисть. В течение 7 дней проводили дистракцию костных отломков (10-11 мм). 20.09.2004 выполнен демонтаж аппарата внешней фиксации с одномоментной открытой репозицией и остеосинтезом лучевой кости тыльной LCP-пластиной (АО-титан). В послеоперационном периоде проводили электростимуляцию нервно-мышечного аппарата поврежденной конечности по предлагаемому способу. На контрольном осмотре через 6 недель средняя амплитуда М-ответа мышц поврежденной конечности увеличилась на 51% по сравнению с предоперационными показателями, а через 6 месяцев - в 3 раза. Восстановление биоэлектрической активности мышц поврежденной конечности было более полным, чем у пациентов с аналогичной травмой, не получавших электростимуляцию. Общий объем движений в кистевом суставе составил 71,5% от контралатеральной конечности, а сила грубого захвата - 59%, что превышало показатели у больных контрольной группы - на 15-20%. По результатам объективных и субъективных критериев оценки результат лечения признан "хорошим". Через 1 год после операции констатирован "отличный" результат лечения.Clinical example. In the postoperative period, electrical stimulation of the neuromuscular apparatus of the damaged limb was performed according to the proposed method. At the follow-up examination after 6 weeks, the average amplitude of the M-response of the muscles of the damaged limb increased by 51% compared with preoperative parameters, and after 6 months - 3 times. The restoration of the bioelectric activity of the muscles of the damaged limb was more complete than in patients with a similar injury who did not receive electrical stimulation. The total range of movements in the wrist joint was 71.5% of the contralateral limb, and gross grip strength was 59%, which was 15-20% higher than in patients in the control group. According to the results of objective and subjective assessment criteria, the result of treatment is recognized as "good." 1 year after the operation, an “excellent” treatment result was observed.
Предлагаемый способ возможно и целесообразно использовать при лечении больных с переломами дистального отдела костей предплечья в стационарах травматологоортопедического профиля, а также в амбулаторных условиях. Его применение позволит улучшить функциональное состояние периферических нервов верхней конечности, увеличить силу мышц и амплитуду движений в суставах, ускорить восстановление функции кисти и улучшить результаты лечения.The proposed method can and should be used in the treatment of patients with fractures of the distal forearm bones in hospitals with an orthopedic profile, as well as in outpatient settings. Its use will improve the functional state of the peripheral nerves of the upper limb, increase muscle strength and range of motion in joints, accelerate restoration of hand function and improve treatment results.
