The disadvantage of this method is that the present invention relates to a system and method for the treatment of diseases of the paranasal sinuses and is not suitable for the treatment and passage into the maxillary sinuses.
Курс лечения включает в себя обычно от 3 до 5 процедур (интернет-издание сайт СКЖД Ростов на Дону dor-clinicrostov.ru).A known technique for drainage of the paranasal sinuses using a SEMI-catheter NMIC. The JAMIC sinus catheter is a soft polymer-silicone device equipped with inflatable balloons. The design of the device allows you to: drain the sinuses, pump out the contents from all the paranasal sinuses (maxillary, frontal, ethmoid) without puncture (puncture) of the maxillary sinuses, effectively inject drugs into the sinuses, stop nosebleeds, treat relapse of the sinusitis (sinusitis) after endoscopic microsurgery and microsurgery sinuses. The JAMIC sinus catheter is an original domestic development. The YAMIK procedure is performed under local anesthesia. The course of treatment usually includes from 3 to 5 procedures (online edition of the SKJD website Rostov on Don dor-clinicrostov.ru).
The disadvantage of this method is the temporary opening of the natural anastomosis of the maxillary sinus due to a change in positive and negative pressure in the nasal cavity and a reflex decrease in the volume of the inflamed mucous membrane; in no way affects the structural options of the anatomical structures in the anastomosis, which predispose to narrowing or block of the anastomosis and the recurrent nature of the course of the disease.
Known balloon sinusoplasty is a less traumatic method for the treatment of chronic and acute sinusitis. The method is based on the expansion of the natural anastomosis of the frontal, maxillary and (or) sphenoid sinuses using a special balloon catheter. Performed under endoscopic observation. It is used to expand the natural mouth of the sinuses using a balloon catheter. The anastomosis is accessed using a guide catheter and a special guide. It is a minimally invasive instrument for FESS (functional endoscopic sinus surgery of the paranasal sinuses). A modern and effective method for inflammation of the paranasal sinuses.
Наиболее близкой к заявленному техническому решению по совокупности признаков является баллонная синусопластика, применяемая для увеличения просвета естественного выводного соустья гайморовой пазухи с помощью специального катетера, по форме напоминающего баллон (интернет-ресурс gaimoritus.ru/bekararstva-i-procaduri/ballonnaya-sinusoplastika.html).The closest to the claimed technical solution for the totality of signs is balloon sinusoplasty, used to increase the lumen of the natural excretory anastomosis of the maxillary sinus using a special catheter shaped like a balloon (internet resource gaimoritus.ru/bekararstva-i-procaduri/ballonnaya-sinusoplastika.html )
Разрыв соустья ведет к отрицательному результату в виде грубого рубцевания, закрытию естественного соустья пазухи и возобновлению синусита (рецидиву).The disadvantage of this method is that this system is one-time and, therefore, very expensive. The main drawback is that the natural sinus fistula is dilated, that is, expanded, however, very high pressure in the balloon up to 8-12 atmospheres and difficulties in choosing the correct diameter of the inflated balloon leads to compression of other adjacent sinuses and may cause inflammation or rupture the anastomosis and even the hook-shaped process itself, since pressure is exerted around the anastomosis. Anastomosis rupture leads to a negative result in the form of rough scarring, closure of the natural sinus anastomosis and the resumption of sinusitis (relapse).
The task is to create a reusable instrument, carefully only displacing the hook-shaped process and making the natural fistula passable, privately under the control of a reverse vision endoscope, which inevitably leads to the recovery of patients suffering from sinusitis with a block of natural fistula.
The technical result is achieved by the fact that the tool for mobilization of the hook-shaped process in the region of the ostiomeatal complex, comprising a handle made in the form of a scissor handle connected by a pull rod with a removable working part, made with the possibility of turning the tool equipped with an extension mechanism in the form of a folding tongue from the base, mechanically pushes the hook-shaped process, mobilizing anteriorly and medially for the purpose of stable restoration of patency of the natural anastomosis of the maxillary sinus due to con truktivnyh features of the present tool in the form poluokrugloy forms its working parts and less traumatic impact on the fabric hooklike process. The ability to rotate the working part through 360 ° allows you to choose the most convenient location for the expansion mechanism to work in the natural anastomosis of the maxillary sinus, in the maxillary sinus in private, which allows you to use the tool with both right and left hands. The disclosure from 0 to 80 ° of the reclining tongue in the expansion mechanism allows you to gently push the hook-shaped process in a specific direction without injuring neighboring organs. Traction lock allows you to lock the selected rotation and restricts movement.
