Description
Foreliggende oppfinnelse angår en ny terapeutisk anvendelse av 2-amino-6-trifluor-metoksybenzotiazol, kjent under den generelle internasjonale betegnelsen "riluzol", eller et farmasøytisk akseptabelt salt av denne forbindelse.
Riluzol er kommersialisert for behandling av amyotroflsk lateral sklerose (se for eksempel Neurology, 1996, vol. 47, no.4, side 233-241). Forbindelsen er likeledes anvendelig som anti-konvulsivum, anxiolytikum og hypnotikum (EP50551), ved behandling av schizofreni (EP305276), ved behandling av søvnproblemer og depresjon (EP305277), ved behandling av cerebrovaskulære mangler og som anestetikum (EP28291), ved behandling av spinale, kraniale eller kraniospinale traumatismer (W094/13288), som radiorestaurator (WO94/15600), ved behandling av Parkinsons sykdom (WO94/15601), ved behandling av neuroaids (WO94/20103) samt ved behandling av mitokondriale sykdommer (WO95/19170).
Glutamat er en av de mest utbredte og en av de viktigste neurotransmitterne for nervesystemet. Virkningene på neuronene moduleres ved transport-proteiner som transporterer glutamat til det indre av cellene. Den molekylære struktur for fire glutamat-transportører er velkjente (Gegelashvili, G. et Schousboe, A., J. Pharmacol. Exp. Ther., 52: 6-15,1997; Takahashi, M. et al., J. Exp. Biol., 200: 401-409,1997) og en femte transportør er identifisert i den senere tid (Arriza, J.L., et al., Proe. Nati. Acad. Sei., USA, 94: 4155-4160). Metodene for histologisk lokalisering indikerer at disse transportører ikke i det hele tatt oppviser noen felles oppførsel i de forskjellige typer celler de støter på i nervesystemet. To av de første identifiserte transportører, kalt GLAST (eller EAAT-1) og GLT-1 (eller EAAT-2), er lokalisert på overveiende måte i de gliale celler. Transportøren EAAC-1 (EAAT-3) uttrykkes av neuroner gjennom hele hjernen. Transportøren EAAT-4, identifisert ganske nylig, uttrykkes hovedsakelig av en spesiell type cerebellum neuroner kalt Purkinje celler (Nagao, S., et al., Neurosciences, 78: 929-933,1997). Denne transportør som ganske nylig er identifisert kalles EAAT-5 og gjenfinnes i øyets retina.
Purkinje-cellene benytter y-aminosmørsyre (GABA) som neurotransmitter. Da glutamat er en metabolsk forløper av GABA (Biochemistry and the Central Nervous System, Mcllwain, H.M., et Bachelard, H.S. (eds.), Churchill Livingston, Londres, 1971, ppl93) er en av rollene for transportøren EAAT-4 å delta i forsyningen av glutamatet til cerebellum Purkinje-cellene for å sikre syntesen av deres neurotransmitter. Dette stemmer overens med nylige studier som viser at tap av transportøren EAAT-4 i motsetning til tap av transportørene EAAT-1, EAAT-2 og EAAT-3, resulterer i dysfunksjon i cerebellum som i sin tur medfører atferdssymptomer av typen ataxie hos gnagere (Maragakis, N., et al., Soc. Neurosci. Abstr. 23: 1484,1997).
Det er nu på overraskende måte funnet at riluzol stimulerer transporten av glutamat i cerebellum preparater fra rotter. Således kan riluzol anvendes for prevensjon eller terapi av cerebellum dysfunksjoner og særlig de som skyldes en dårlig forsyning av glutamat til cerebellum cellen. Blant disse dysfunksjoner kan nevnes cerebelløse ataxier.
Denne virkning eller effekt er spesifikk for cerebellum fordi den ikke er observert for tilsvarende preparater fremstilt fra andre hjernestrukturer som den cerebrale cortex og striatum eller fra ryggmargen. Økningen i glutamat opptak i cerebellum forårsaket av riluzol øker med rottens alder fra den postnatale periode til voksen alder og stabiliserer seg ved rundt 10 uker, det vil si ved en vekt over 350 g.
Økningen i glutamat opptak evalueres i en suspensjon av synaptosomer fremstilt fra cerebellum celler fra rotter. Denne type preparater er kjent å tillate in vitro studier av glutamat transport gjennom celleveggen (Kanai et al., Trends Neurosci., 16: 365-370, 1993).
I synaptosomene fra cerebellum fra rotter er opptak av radioaktiv glutamat øket på en konsentrasjonsavhengig måte ved tilsetning av riluzol til disse preparater (figur 1).
Synaptosomene prepareres i henhold til en modifisering av den metode som er beskrevet av M.B. Robinson et al. (Brain Res., 544: 196-202,1991). Vev fra cerebellum eller andre områder av hjernen fra hannrotter av Sprague-Dawley-rasen homogeniseres i en 0,32 M sakkarose-oppløsning ved 4°C og sentrifugeres ved 800 g i 10 minutter. Supernatanten blir så sentrifugert igjen ved 20 000 g i 20 minutter. Sentrifugeklumpen som oppnås bringes i suspensjon i en identisk sakkarose-oppløsning og sentrifugeres nok en gang ved 20 000 g i 20 minutter. Synaptosomene inneholdes i denne andre sentrifugeklump. Disse bringes i suspensjon og benyttes umiddelbart for måling av glutamat opptak. Dettee følges ved hjelp av tritium-merket glutamat ([<3>H]glutamat) som er fortynnet i en ikke-radioaktiv glutamat-oppløsning. Etter 3 minutters innkubering ved 37 °C separeres synaptosomene fra innkuberingsmediet ved filtrering. Effekten av produktet evalueres ved å sammenligne den bibeholdte radioaktivitet på filtrene i fravær eller i nærvær av økende konsentrasjoner av riluzol ved pH 7,3. Denne radioaktivitet er proporsjonal med fanging av glutamat. Resultatene uttrykkes som spesifikt opptak, det vil si det opptak som avhenger av nærværet av natriumioner i innkuberingsmediet. Den skiller seg fra det ikke-spesifikke opptak som tilsvarer radioaktiviteten som måles etter ekvimolare utbytting av natriumklorid i innkuberingsmediet med kolinklorid. Det spesifikke opptak tilsvarer den totale radioaktivitet minus radioaktiviteten som måles i nærvær av kolinklorid.
Summarisk beskrivelse av figur 1 (Riluzol-betinget økning av glutamat-transporten i cerebellum): grafen viser opptak av radiomerket glutamat i synaptosomene av rotte-cerebellum (ordinat-aksen) som funksjon av økende Riluzol-konsentrasjoner (abscisse-aksen). Verdiene er gjennomsnittene ± ESM for n = 8 uavhengige observasjoner. Den maksimale opptaksøkning som observeres i nærvær av riluzol er nær 40 % av kontrollverdiene. Den demi-maksimale effektivitetskonsentrasjon EC50 av riluzol i denne forsøksserie, beregnet ved sigmoidal regresjon, ligger ved 24,5 um.
Som farmasøytisk akseptable salter for riluzol kan særlig nevnes syreaddisjonssalter med mineralsyrer, for eksempel hydroklorid, sulfat, nitrat eller fosfat, eller med organiske syrer, for eksempel acetat, propionat, succinat, oxalat, benzoat, fumarat, maleat, metansulfonat, isetionat, theofilinacetat, salisylat, fenolftalinat, metylen-bis-P-oksynaftoat eller substitusjonsderivater av disse derivater.
Medikamentene består av i det minste riluzol i fri form eller i form av et syreaddisjons-salt med en farmasøytisk akseptabel syre, i ren tilstand eller i form av et preparat i hvilket det er forbundet med et hvilket som helst annet farmasøytisk kompatibelt produkt som kan være enten inert eller fysiologisk aktivt. Medikamentene ifølge oppfinnelsen kan benyttes oralt, parenterealt eller rektalt.
Som faste preparater for oral administrering kan man benytte tabletter, piller, pulvere (gelatinkapsler, puter) eller granuler. I disse preparater er den aktive bestanddel ifølge oppfinnelsen blandet med et eller flere inerte fortynningsmidler som stivelse, cellulose, sakkarose, lactose eller silika, under argon. Preparatene kan likeledes omfatte andre stoffer enn fortynningsmidler, for eksempel et eller flere smøremidler som magnesium-stearat eller talkum, et farvestoff, et omhyllingsmiddel (drageer) eller en ferniss.
Som flytende preparater for oral administrering kan man benytte oppløsninger, suspensjoner, emulsjoner, siruper og farmasøytisk akseptable elexirer inneholdende inerte fortynningsmidler som vann, etanol, glycerol, vegetabilske oljer eller paraffinolje. Disse preparater kan inneholde andre stoffer enn fortynningsmidlene, for eksempel fuktemidler, smaks-, fortyknings-, aroma- eller stabiliseringsmidler.
Sterile preparater for parenteral administrering kan fortrinnsvis være vandige eller ikke-vandige oppløsninger, suspensjoner eller emulsjoner. Som oppløsningsmiddel eller bærer kan man benytte vann, propylenglykol, en polyetylenglykol, vegetabilske oljer og særlig olivenolje, injiserbare organiske estere som etyl-oleat eller andre hensiktsmessige organiske oppløsningsmidler. Disse preparater kan likeledes inneholde adjuvanter og særlig fukte-, isotoniserings-, emulgerings-, dispergerings- og stabiliseringsmidler. Steriliseringen kan skje på flere måter, for eksempel ved aseptisk filtrering, ved i preparatene å innarbeide steriliseirngsmidler, ved bestråling eller ved oppvarming. De kan likeledes fremstilles i form av faste, sterile preparater som kan oppløses på brukstidspunktet i sterilt vann eller et hvilket som helst annet injiserbart sterilt medium.
Preparatene for rektal administrering er suppositorier eller rektal-kapsler som, i tillegg til den aktive bestanddel, inneholder drøyemidler som kakaosmør, semisyntetiske glycerider eller polyetylenglycoler.
Dosene avhenger av den tilsiktede virkning, behandlingsvarigheten samt den benyttede administreringsrute og dosene ligger generelt mellom 50 og 400 mg/dag oralt for en voksen person med enhetsdoser fra 25 til 200 mg aktiv bestanddel.
Rent generelt er det legen som bestemmer den egnede dosering i forhold til alder, vekt og alle andre faktorer av betydning hos individet som skal behandles.
De følgende eksempler skal illustrere medikamentene ifølge oppfinnelsen:

Oppfinnelsen angår likeledes anvendelse av riluzol eller et farmasøytisk akseptabelt salt derav for fremstilling av et medikament for prevensjon eller terapi av cerebellum dysfunksjoner som skyldes en dårlig levering av glutamat til cerebellum celler, og særlig for prevensjon eller terapi av cerebelløse ataxier. Nevnte fremstilling omfatter fortrinnsvis å blande riluzol eller farmasøytisk akseptable salter derav med et eller flere kompatible eller farmasøytisk akseptable fortynnings- eller tilsetningsmidler.
Claims (3)
1.
Anvendelse av riluzol eller et farmasøytisk akseptabelt salt derav for fremstilling av et medikament for prevensjon eller terapi av cerebellum dysfunksjoner som skyldes en dårlig levering av glutamat til cerebellum celler.
2.
Anvendelse ifølge krav 1 for prevensjon eller terapi av cerebelløse ataxier.
3.
Anvendelse ifølge hvilket som helst av kravene 1 til 2 for fremstilling av et medikament som omfatter 25 til 200 mg riluzol.
