Description
Den foreliggende oppfinnelse vedrører en ny farmasøytisk blanding for behandling av astma og sykdommer i respirasjonsveiene og anvendelse av taurin eller hypotaurin for fremstilling av en farmasøytisk blanding for inhalasjonsadministrering ved behandling av astma og sykdommer i åndedrettsveiene. Slike forskjellige luftveisplager kan være forårsaket av atmosfærisk forurensning hos bestemte hyperreaktive pasienter.
Taurin eller 2-aminoetansulfonsyre, er en aminosyre med lav toksisitet hvis egenskaper er beskrevet i en rekke publikasjoner. Taurin brukes ikke ofte som et legemiddel, og dets anvendelse ved terapi er i litteraturen bare blitt beskrevet for behandling av hjertesvikt.
Noen taurinderivater er også beskrevet samt de farmakologiske egenskapene derav, og f.eks. vedrører europeisk patent EP 307.788 et- taurinamid med slimløsning, hepatobeskyttende, detoksifiserende og normolipemiserende egenskaper. Noen andre taurinderivater anvendelige som leucotrienantagonister er beskrevet i japansk patentsøknad JP-A-03.068549.
Europeisk patent 307.788 beskriver taurinderivater som er antatt å gi de samme eller lignende virkninger som taurin. Videre er aerosolblandinger for inhalasjon av forskjellige farmasøytisk aktive produkter velkjente.
Dette betyr imidlertid ikke at enhver farmakologisk aktiv forbindelse kan administreres ved inhalasjon. Spesielt er det velkjent at inhalasjon av aminosyrer som påføres direkte i luftveiskanalen ofte forårsaker en hyperreaktivitet når de blir brukt til å oppløse slim. Derimot viser resultatene av eksperimentene som er utført med blandingen ifølge oppfinnelsen at taurin administrert som aerosol er virknings-full i forsøket med inhalasjon av destillert vann som induserer bronkospasmer, hvilket betyr at blandingen er effektiv i en forebyggende behandling mot astmakriser.
EP 307.788 beskriver bare de kjente egenskapene til taurin og noen derivater som antas å gi den samme virkning. Men disse eldre patentene foreslår ikke bruk av taurin ved inhalasjon. Videre nevnes ingen mulig virkning ved behanding 2
av astma (se spalte 1) .
CA 105:146178 (1986) vedrører behandling av marsvin ved intravenøs administrering, men foreslår ikke inhalasjon.
Det bør fremheves at ingen gunstige resultater oppnås ved administrering av en drikke-løsning eller intravenøs injeksjon som nevnt i den foreliggende beskrivelse (se side 3).
CA 109:10195 (1988) vedrører den samme intravenøse behandling av marsvin som ovenfor, og beskriver bare noen komplementerende data.
CA 112:93414 (1990) vedrører behandling av hamstere, hvor taurin ble administrert drikkbart i vann.
Det som er nevnt ovenfor gjelder også for disse publika-sjonene .
En rekke studier er foretatt i flere år på virkningene av atmosfærisk forurensning, nemlig ved svoveldioksyd, nitrogendioksyd, karbonmonoksyd, organiske damper, oson osv., som delvis er ansvarlig for sykdommer så som kronisk bronkitt, amfysema eller astma. Nærmere bestemt er det funnet at utviklingen av slike sykdommer er viktigere i industrialiserte land som lider av sterk atmosfærisk forurensing, nemlig i urbaniserte områder hvor forurensningen som stammer fra eksosgasser fra kjøretøyer kombineres med sådanne fra industrianlegg. Det er også blitt vist at en økning i innholdet av nitrogendioksyd i luften induserer en økning i antallet av personer som lider av astmakriser i den befolkning som innånder slik luft.
Det er kjent at lang eksponering for nitrogenoksyd induserer en betennelse i bronkiene, alveolærkanalene og de peribronkiale alveoli hos hamstere, samt endring og formering av epitelialceller. Dette skyldes sannsynligvis at nitrogendioksyd er et oksydasjonsmiddel i likhet med karbondioksyd og svoveldioksyd, som påvirker cellemembrane i følge en mekanisme som det ennå ikke foreligger klarhet i.
Det er kjent at administrering av visse forbindelser med antioksyderende virkning eller en fri-radikal-oppfangende virkning muliggjør en viss beskyttelse mot slik skade. F.eks. er noen gunstige resultater observert under eksperimenter på 3
laboratoriemodeller med vitamin E (alfatocoferol), glutation, dimetylsulfoksyd og N-acetylcystein. Imidlertid er noen problemer som stammer fra toksisitet ennå ikke løst, og nærmere bestemt, selvom vitamin E har en fri-radikal-oppfangende virkning, gir den ikke noen effektiv beskyttelse mot oksydasjonsmidler så som nitrogendioksyd eller svovel, og forbedrer ikke epitelialbarrieren til bronkier. Således kan verken vitamin E eller andre fri-radikale-oppfangere så som DMSO modifisere hyperaktiviteten til visse pasienter, og ingen tilfredstillende beskyttelse oppnås.
Bruken av kromoglycinsalter så som dinatriumsaltet har også vært foreslått i behandlingen av astma med allergisk opprinnelse. F.eks. fører administrering av natriumkromglycat til en stabilisering av det bronkiale epitelium, men gir ikke noen beskyttelse mot oksyderende forurensninger og fri-radikal-initiatorer.
En rekke undersøkelser har vært utført for å forsøke å utvikle noen forbindelser eller blandinger som er både fri-radikal-oppfangere og antioksydanter, og som stabiliserer og beskytter det alveolære og bronkiale epitelium, slik at man får en tilfredstillende beskyttelse av åndedrettsveiene mot sykdommer som skyldes forurensning.
Eksperimenter utført av søkeren har nå avslørt at administrering av taurin eller hypotaurin ved inhalering, nemlig i form av en aerosol, kan være en utmerket forebyggende behandling mot sykdommer i åndedrettsveiene, nærmere bestemt en behandling av astma, og også visse sykdommer indusert ved atmosfærisk forurensning som er angitt ovenfor, så som kronisk bronkitt og amfysema, mens andre administreringsformer ikke er effektive.
Foreliggende oppfinnelse muliggjør en ny anvendelse av taurin og hypotaurin som aerosol for behandling av sykdommer i respirasjonsveiene, nærmere bestemt for forebyggende behandling av astmasykdommer indusert ved atmosfærisk forurensning. Følgelig omfatter den farmasøytiske blandingen ifølge oppfinnelsen taurin eller hypotaurin i kombinasjon med midler for administrering derav ved inhalasjon.

De utførte kliniske eksperimenter viser at administrer-ingen av taurin ved inhalering i form av en aerosol gir en signifikant reduksjon i bronkokramper indusert ved forstøvningsforsøket med destillert vann, og gir nærmere bestemt en beskyttende virkning ved de induserte asmatiske reaksj oner.
Resultatene av slike eksperimenter viser at den nye blandingen omfattende taurin eller hypotaurin ifølge oppfinnelsen gitt som aerosol kan brukes ved ny uventet indikasjon, dvs. den grunnleggende og forebyggende behandling av sykdommer i respirasjonsveiene, nærmere bestemt astma, med tilfredstillende effektivitet uten noen uheldig sekundær virkning.
Slike resultater kan oppnås i følge den foreliggende oppfinnelse ved anvendelse av taurin (2-aminoetansulfonsyre) samt hypotaurin (2-aminetansulfinsyre) ved inhalering, fortrinnsvis som aerosol, mens ikke noe gunstig resultat kan oppnås ved administrering i form av en drikkbar løsning eller intravenøs injeksjon. Ifølge den foreliggende oppfinnelse oppløses taurin eller hypotaurin i et passende løsningsmiddel, og fortrinnsvis i form av aerosol i en passende beholder som gjør det mulig å produsere en forstøvning som går inn i åndedrettsveiene dvs. munnen og luftveiene til pasienten som skal behandles.
Løsningsmiddelet som skal brukes for å fremstille taurinløsningen ifølge oppfinnelsen velges fra vanlige løsningsmidler som er farmasøytisk akseptable, men fortrinnsvis brukes en vandig løsning.
Taurin eller hypotaurinkonsentrasjon i løsningen kan utgjøre mellom 1 g pr. liter og 50 g pr. liter i løsningen, og fortrinnsvis mellom 15 g pr. liter og 25 g pr. liter.
Gassdrivmiddelet kan f.eks. være en freon som ofte brukes i aerosolteknikken.
Dosene av taurin som gis med aerosol ifølge oppfinnelsen er mellom ca. 5 mg og 20 mg pr. dag og pr. kilo vekt, enhets-dosene er ca. 100 til 500 mg, og antallet inhalasjoner pr. dag er mellom 3 og 5 når pasienten er voksen.

Virkningen av taurin eller hypotaurinblandingen ifølge oppfinnelsen er bekreftet med et eksperiment utført med 9 pasienter fra 10 til 4 0 år gamle som lider av kronisk astma i de følgende beskrevne tilstander.
Astmakrisene induseres ved forsøk bestående i å inhalere ultralydforstøvet destillert vann, og pasientenes spirometiske verdier måles før og umiddelbart etter inhalasjon av destillert vann. Det samme forsøket gjentas etter en ventetid på minst 3 til 4 dager, men ca. 3 0 minutter etter inhalasjonen av 250 mg taurin som aerosol ifølge foreliggende oppfinnelse, og de spirometiske målinger utføres igjen; resultatene behandles statistisk.
Resultatene viser at i det første forsøket uten inhalasjon av taurin som aerosol, er det en signifikant reduksjon i pasientenes spirometiske verdier, som tilsvarer en bronkial ustabilitet som er karakteristisk for astma. Således reduseres FVC parameteren fra 3,32 til 2,64. Etter inhalasjon av taurin derimot blir den samme parameter ikke signifikant påvirket (3,47 før inhalasjon av vann og 3,15 etter inhalasjon).
Det samme eksperiment utføres med to grupper pasienter; den første gruppe får taurin som aerosol ifølge den foreliggende oppfinnelse (administrert dose: 125 mg) mens den andre gruppe får taurin i form av diettløsning (administrert dose: 500 mg).
Resultatene viste at bare den første gruppe motsto inhalasjonsprøven med destillert vann som induserer bronkokramper.
Resultatene ovenfor viser at blandingen omfattende taurin eller hypotaurin ifølge den foreliggende oppfinnelse gitt i form av aerosol muliggjør en effektiv virkning som forebyggende behandling mot astmakrise, og mer generelt mot åndedrettsykdommer indusert av atmosfærisk forurensning.
En sammenlignende undersøkelse utføres under de samme betingelser hvor pasientene behandles med inhalasjon av taurin i en gruppe og med en aerosol av dinatriumkromoglycat i den andre gruppe ifølge den vanlige teknikk. Det noteres at en bedre terapeutisk virkning oppnås med inhalasjon av taurin.
Claims (6)
1. Ny farmasøytisk blanding for behandling av astma og sykdommer i respirasjonsveiene,
karakterisert vedat den omfatter taurin eller hypotaurin i kombinasjon med midler for administrering derav ved inhalasjon.
2. Ny blanding ifølge krav 1,
karakterisert vedat den omfatter midler for administrering av taurin eller hypotaurin som aerosol.
3. Ny blanding ifølge hvert av de foreliggende krav,karakterisert vedat den omfatter taurin eller hypotaurin i løsning i et passende løsningsmiddel i kombinasjon med et gassformig drivmiddel.
4. Ny blanding ifølge krav 3,
karakterisert vedat taurinkonsentrasjonen utgjør mellom 5 g pr. liter og 50 g pr. liter løsningsmiddel.
5. Ny blanding ifølge hvert av kravene 3 og 4,karakterisert vedat den omfatter en vandig løsning av taurin.
6. Anvendelse av taurin eller hypotaurin for fremstilling av en farmasøytisk blanding for inhalasjonsadministrering ved behandling av astma og sykdommer i åndedrettsveiene.
