Description
Oppfinnelsen vedrører fremstilling av terapeutisk aktive oktapeptider.
Et antall av somatostatin-analoger som oppviser GH-frigjø-ringsinhiberende aktivitet er beskrevet i litteraturen, inklusive analoger som inneholder færre enn de naturlig forekommende fjorten aminosyrer. For eksempel beskriver US-patent nr. 4 485 101, som herved inkorporeres ved henvisning, dodekapeptidér som har en N-terminal-acetylgruppe, en C-terminal-NH2, D-Trp ved posisjon 6, samt p-Cl-Phe ved posisjon 4. (Hvis det i denne beskrivelse ikke er noen angivelse av konfigurasjon, så menes L-isomeren).
Generelt dreier oppfinnelsen seg om fremstilling av et oktapeptid med følgende formel:
hvor Ai er H eller C^g-alkyl;
A3 er CH2-A6 (hvori A6 er pentafluorfenyl, naftyl eller fenyl);
A4 er Tyr eller pentafluor-Phe; A5 er Val eller a-amino-smørsyre, og A7 er Thr eller /3-Nal; forutsatt at hvis A3 er fenyl,
så kan A4 ikke være Tyr, A5 ikke være Val; og A7 ikke være Thr.
Fremgangsmåten er karakterisert ved følgende trinn:
1) å knytte en første aminosyre, betegnet ved formel A7 til en harpiks for å danne en aminosyreharpiks; 2) sekvensielt å koble syv ytterligere aminosyrer til nevnte aminosyreharpiks for å danne et oktapeptid koblet til nevnte
harpiks; og
3) eluere oktapeptidet fra harpiksen slik at man får et renset preparat av oktapeptidet.
I den formel som er gitt ovenfor, er konfigurasjonen av molekylet ved det karbonatom som A3 er bundet til, ikke gitt, for å indikere at aminosyreresten som A3 er en substituent i, kan ha D- eller L-konfigurasjon.
Foretrukne forbindelser som fremstilles i henhold til oppfinnelsen inkluderer:
D-)3-Nal-Cys-Tyr-D-Trp-Lys-Val-Cys-Thr-NH2; D-Phe-Cys-Tyr-D-Trp-Lys-a-aminosmørsyre-Cys-Thr-NH2; pentafluor-D-Phe-Cys-Tyr-D-Trp-Lys-Val-Cys-Thr-NH2; N-Ac-D-/3-Nal-Cys-Tyr-D-Trp-Lys-Val-Cys-Thr-NH2; D-/?-Nal-Cys-pentaf luor-Phe-D-Trp-Lys-Val-Cys-Thr-NH2; D-/?-Nal-Cys-Tyr-D-Trp-Lys-Val-Cys-/?-Nal-NH2; D-Phe-Cys-Tyr-D-Trp-Lys-Val-Cys-/3-Nal-NH2; og
D-/3-Nal-Cys-Tyr-D-Trp-Lys-a-aminosmøresyre-Cys-Thr-NH2.
I andre foretrukne utførelsesformer kan en terapeutisk effektiv mengde av den terapeutiske forbindelse og en farmasøy-tisk akseptabel bærersubstans, f.eks. magnesiumkarbonat, laktose, eller et fosfolipid med hvilket den terapeutiske forbindelse kan danne en micelle, sammen danne et terapeutisk preparat, f.eks. en pille, tablett, kapsel eller væske for oral administrering til en human-pasient, en smørbar krem, gel, lotion, eller smørelse for påføring på huden til en human pasient som har behov for forbindelsen, en væske med evne til å bli administrert nasalt som dråper eller dusj, eller en væske som kan administreres intravenøst, parenteralt, subkutant eller intraperitonealt. Pillen, tabletten eller kapselen kan være belagt med en substans med evne til å beskytte preparatet mot mavesyren i pasientens mave i et tidsrom som er tilstrekkelig til å tillate preparatet å passere udisintegrert inn i pasientens tynntarm. Det terapeutiske preparat kan også være i form av en bionedbrytbar blanding med forsinket frigivelse for intramuskulær administrering. For maksimal effektivitet er det ønsket med frigivelse av null orden. Frigivelse av null orden kan oppnås ved anvendelse av en implan-terbar eller ekstern pumpe, f.eks. en Infusoid<®->pumpe, for å administrere det terapeutiske preparat.
Forbindelsene som fremstilles i henhold til oppfinnelsen er aktive med hensyn til å inhibere sekretering av GH, insulin og glukagon. Videre kan den aromatiske lipofile N-terminal-ende tilveiebringe langvarig aktivitet in vivo.
Andre trekk og fordeler ved oppfinnelsen vil fremgå av den følgende beskrivelse og de foretrukne utførelsesformer derav, og av kravene.
Struktur
Forbindelsene som fremstilles i henhold til oppfinnelsen har den generelle formel som er gjengitt innledningsvis. De er alle oktapeptidanaloger av somatostatin som har D-Trp ved posisjon 4; og eventuelle modifikasjoner ved posisjonene 3 (A4) 6 (A5) og 8 (A7) . Det er funnet at D-/3-naftylalanin ved posisjon 1; Tyr ved posisjon 3; og Val ved posisjon 6 er modifikasjoner som spesielt forsterker aktiviteten.
Forbindelsene kan tilveiebringes i form av farmasøytisk akseptable salter. Eksempler på foretrukne salter er slike med terapeutisk akseptable organiske syrer, f.eks. eddiksyre, melkesyre, maleinsyre, sitronsyre, eplesyre, askorbinsyre, rav-syre, benzosyre, salicylsyre, metansulfonsyre, toluensulfon-syre eller pamosyre, såvel som polymere syrer, som f.eks. garvesyre eller karboksymetylcellulose, samt salter med uorga-niske syrer som f.eks. hydrohalogensyrene, f.eks. saltsyre, svovelsyre eller fosforsyre.
Syntese
Syntese av ett oktapeptid følger. Andre oktapeptider kan fremstilles ved å gjøre passende modifikasjoner, i henhold til fagmannens kunnskaper på området, av den følgende syntetiske metode.
Første trinn ved fremstillingen av D-p-naftylalanin-Cys-Tyr-D-Trp-Lys-Val-Cys-Thr-NH2 var fremstilling av mellomproduk-tet tert-butyloksykarbonyl-D-6-naftylalanin-S-metylbenzyl-Cys-Tyr-D-Trp-N E<->benzyloksykarbonyl-Lys-Val-S-metylbenzyl-Cys-O-benzyl-Thr-benzyhydrylaminharpiks, som følger.
Benzhydrylamin-polystyrenharpiks (Vega Biochemicals, Inc.) i klorid-ion-form ble anbragt i reaksjonskaret til en Beckman 990B peptid-syntetisator programmert til å utføre følgende re-aks jonssyklus : (a) metylenklorid; (b) 33% trifluoreddiksyre i metylenklorid (2 ganger, 1 og 25 min hver gang); (c) metylenklorid; (d) etanol; (e) metylenklorid; (f) 10% trietylamin i kloroform.
Den nøytraliserte harpiks ble omrørt med Boc-O-benzyl-treonin og diisopropylkarbodiimid (1,5 mmol hver gang) i metylenklorid i 1 h, og den resulterende aminosyreharpiks ble deretter sirkulert gjennom trinnene (a) til (g) i det ovennevnte vaskeprogram. Følgende aminosyrer (1,5 mmol) ble deretter koblet suksessivt ved samme fremgangsmåte: Boc-S-metylbenzyl-Cys, Boc-Val, Boc-Ne-benzyloksykarbonyl-lysin, Boc-D-Trp, Boc-Tyr, Boc-S-metylbenzyl-Cys, Boc-D-B-naftylalanin.
Harpiksen ble vasket og tørket og deretter blandet med
4 ml anisol og 36 ml vannfritt hydrogenfluorid ved 0°C og omrørt
i 45 min. Overskudd av hydrogenfluorid ble fordampet hurtig under en strøm av tørt nitrogen, og fritt peptid ble utfelt og vasket med eter. Det rå peptid ble deretter oppløst i 800 ml av 90% eddiksyre som var tilsatt 1^ i metanol inntil en perma-nent brun farve var tilstede. Løsningen ble så omrørt i 1 h før fjerning av løsningsmiddelet i vakuum. Den resulterende olje ble oppløst i et minimalt volum av 50% eddiksyre og eluert på en kolonne (2,5 x 100 mm) av Sephadex G-25. Fraksjoner som inneholdt en hovedkomponent ved UV-absorpsjon og tynnskikts-kromatografi ble så oppsamlet, inndampet til lite volum og på-ført i en kolonne (2,5 x 50 cm) av Whatman LRP-1 oktadecylsilan (15 - 20 ^M).
Kolonnen ble eluert med en lineær gradient av 10 - 50% acetonitril i 0,1% trifluoreddiksyre i vann. Fraksjoner ble undersøkt ved tynnskiktkromatografi og analytisk høyytelse-væske-kromatografi (HPLC) og oppsamlet at man fikk maksimal renhet og, om ønsket, ble det fremstilt et annet slags salt, f.eks. acetat eller fosfat. Gjentatt lyofilisering av løsningen fra .vann ga 170 mg av produktet i form av et hvitt, fluffy pulver. Produktet viste seg å være homogent ved HPLC og TLC. Ami-nosyreanalyse av et syrehydrolysat bekreftet sammensetningen av oktapeptidet.
Oktapeptidene med formlene pentafluor-D-Phe-Cys-Tyr-D-Trp-Lys-Val-Cys-Thr-NH2, D-Phe-Cys-Tyr-D-Trp-Lys-a-aminosmørsyre-Cys-Thr-NH2, N-Ac-D-/3-Nal-Cys-Tyr-D-Trp-Lys-ValCys-Thr-NH2, D-/3-Nal-Cys-pentafluor-Phe-D-Trp-Lys-Val-Cys-Thr-NH2, D-/3-Nal-Cys-Tyr-D-Trp-Lys-Val-Cys-Æ-Nal-NH2, D-Phe-Cys-Tyr-D-Trp-Lys-Val-Cys-Ø-Nal-NH2; og D-/3-Nal-Cys-Tyr-D-Trp-Lys-a-aminosmørsyre-Cys-Thr-NH2 ble laget i henhold til metoder som er analoge med dem som er beskrevet ovenfor.
Anvendelse
Ved administrering til pattedyr, spesielt mennesker (f.eks. oralt, topisk, intravenøst, parenteralt i en bionedbrytbar form med forsinket frigivelse, nasalt eller ved stikkpiller), kan forbindelsene være effektive med hensyn til å inhibere GH-frigivelse såvel som å inhibere insulin-, glukagon- og pankreatisk exocrin-sekretering, og for terapeutisk å påvirke sentralnerve-systemet.
Forbindelsene kan administreres til et pattedyr, f.eks. et menneske, i de doser som anvendes for somatostatin eller, pga. sin større styrke, i mindre doser. Forbindelsene som fremstilles i henhold til oppfinnelsen kan anvendes for behandling av kreft, spesielt veksthormon-avhengig kreft (f.eks. i ben, bruskvev, pankreas (endokrin og exocrin), prostata eller bryst), akromegali og beslektede hypersekretoriske endokrine tilstander, eller av blødende mavesår hos pasienter ved nødhjelp og hos slike som lider av pankreatitt eller diarrhé. Forbindelsene kan også anvendes ved behandling av sukkersyke og for å beskytte leveren hos pasienter som lider av cirrhose eller hepatitt. Forbindelsene kan også anvendes for å behandle Alzheimers sykdom, som analgetiske midler for å behandle smerte ved å virke spesifikt på visse opiat-reseptorer, og som gastriske cytoprotektive forbindelser for mavesårterapi. Forbindelsene kan også anvendes for å behandle visse typer av soppforgiftning.
Forbindelsene kan også anvendes for å behandle diabetes-relatert retinopati. Anti-cancer-aktiviteten til forbindelsene kan relateres til deres evne til å antagonisere cancer-relaterte vekstfaktorer som f.eks. epidermal vekstfaktor.
Forbindelsene kan administreres til et pattedyr, f.eks. et menneske, i en dosering av 0,01 - 1000 mcg/kg/dag, fortrinnsvis 0,1
- 100 mcg/kg/dag.

1. Analogifremgangsmåte for fremstilling av et terapeutisk aktivt oktapeptid med følgende formel:
