Abstract

The invention relates to medicine, in particular to traumatology and plastic surgery and can be used for plastic surgery on Achilles tendon with vascular graft.According to the invention, the claimed method consists in that it is performed the dopplerography and is determined the projection of vessels, are performed two curved incisions with the concave sides pointed to the lateral and medial edge of the Achilles tendon, having the same start and end point, afterwards it is mobilized the covering fascia, the Achilles tendon, the proximal and distal vascular pedicles of the triceps muscle of calf, is prepared a graft including the skin, the covering fascia of the Achilles tendon with the vascular pedicle from the sural artery and the tendon transplant, prepared from the triceps muscle of calf, then the prepared graft is unfolded distally at 180° and placed in the region of Achilles tendon defect, and the ends of the tendon transplant are introduced into the sagittal gaps of the Achilles tendon fragments and are sutured, after which the wound is sutured in layers.

Invenţia se referă la medicină, în special la traumatologie şi chirurgia plastică şi poate fi utilizată pentru plastia tendonului ahilian cu lambou vascular. The invention relates to medicine, especially to traumatology and plastic surgery and can be used for achilles tendon plasty with a vascular flap.

Este cunoscută metoda de tratament chirurgical al leziunilor tendonului ahilian, şi anume tehnica Cernavschi prin plastie cu autogrefă tendinoasă din peretele posterior al joncţiunii tendino-musculare a muşchiului triceps sural [1]. The method of surgical treatment of Achilles tendon injuries is known, namely the Cernavschi technique by plasty with tendinous autograft from the posterior wall of the tendon-muscular junction of the triceps sural muscle [1].

Dezavantajele acestei metode constau în riscul mare de infectare a autogrefei tendinoase şi de degradare a ei cu apariţia leziunii inveterate de tendon ahilian. The disadvantages of this method consist in the high risk of infection of the tendinous autograft and its degradation with the appearance of inveterate damage to the Achilles tendon.

Problema pe care o rezolvă invenţia propusă este extinderea arsenalului de metode utilizate pentru tratamentul leziunii tendonului ahilian şi al ţesuturilor supraiacente. Avantajul metodei constă în utilizarea lamboului insular cutaneo-fascio-tendinos pe pedicul vasculonervos în restabilirea deplină anatomo-fiziologică a tendonului ahilian şi a ţesuturilor supraiacente. The advantage of the method consists in the use of the cutaneous-fascio-tendinous insular flap on the vascular pedicle in the full anatomical-physiological restoration of the Achilles tendon and the overlying tissues.

Conform invenţiei, metoda revendicată constă în aceea că se efectuează dopplerografia cu stabilirea proiecţiei vaselor, se efectuează două incizii curbate cu părţile concave îndreptate spre marginea laterală şi medială a tendonului ahilian, având acelaşi punct de pornire şi finisare, după care se mobilizează fascia de înveliş, tendonul ahilian şi pediculul vascular al muşchiului triceps sural proximal şi distal, se prepară un lambou, care include stratul cutanat, fascia de înveliş a tendonului ahilian cu pedicul vascular din artera surală şi grefa tendinoasă, preparată din tendonul muşchiului triceps sural, apoi lamboul preparat se roteşte la 180° distal şi se plasează în regiunea defectului tendonului ahilian, iar capetele grefei tendinoase se introduc în breşele sagitale ale fragmentelor tendonului ahilian şi se suturează, după care plaga se suturează pe straturi. Rezultatul constă în refacerea deplină a integrităţii anatomo-fiziologice şi funcţionale a tendonului ahilian şi a ţesuturilor supraiacente. The result consists in the full restoration of the anatomic-physiological and functional integrity of the Achilles tendon and the overlying tissues.

Metoda de plastie vascularizată a defectelor tendonului ahilian se realizează în modul următor. Bolnavul R., anul naşterii 1984, a fost spitalizat în secţia ortopedie şi traumatologie nr. 3 a IMSP Centrul Naţional Ştiinţifico-Practic de Medicină Urgentă, cu diagnosticul: defect posttraumatic de tendon ahilian al gambei stângi. Pacientul a fost operat utilizând metoda revendicată, adică s-a efectuat dopplerografia cu stabilirea proiecţiei vaselor, s-au efectuat două incizii curbate cu părţile concave îndreptate spre marginea laterală şi medială a tendonului ahilian, având acelaşi punct de pornire şi finisare, după care s-a mobilizat fascia de înveliş, tendonul ahilian şi pediculul vascular al muşchiului triceps sural proximal şi distal, s-a preparat un lambou, care include stratul cutanat, fascia de înveliş a tendonului ahilian cu pedicul vascular din artera surală şi grefa tendinoasă, preparată din tendonul muşchiului triceps sural, apoi lamboul preparat s-a rotit la 180° distal şi s-a plasat în regiunea defectului tendonului ahilian, iar capetele grefei tendinoase s-au introdus în breşele sagitale ale fragmentelor tendonului ahilian şi s-au suturat, după care plaga s-a suturat pe straturi. Intervenţia a fost efectuată la 2 pacienţi în cadrul Clinicii Chirurgie Plastică Reconstructivă şi Microchirurgia Locomotorului al IMSP Centrul Naţional Ştiinţifico-Practic de Medicină Urgentă din Cadrul Catedrei Ortopedie şi Traumatologie al USMF „Nicolae Testemiţanu”, obţinându-se restabilirea anatomo-funcţională deplină a membrului afectat. The intervention was performed on 2 patients in the Reconstructive Plastic Surgery and Locomotive Microsurgery Clinic of IMSP, the National Scientific-Practical Center for Emergency Medicine within the Department of Orthopedics and Traumatology of USMF "Nicolae Testemițanu", obtaining the full anatomical-functional restoration of the affected limb .
