Abstract
The invention relates to medicine, particularly to oncologic surgery and dentistry and can be used for treating maxillary sinus cancer simultaneously with defectoplasty.Summary of the invention consists in that it is performed the general anesthesia, then on the upper lip midline is performed an incision, which is extended under the wing of the nose, on the nasal clivus to the inner corner of the eye. The skin flap of the cheek is separated to the outside, then with an electric scalpel are dotted in the oral cavity the borders of the affected tissues, are coagulated and removed gradually small affected segments from the alveolar ridge of the jaw, from the anterior wall of the maxillary sinus and hard palate, afterwards it is completely removed the tumor from the maxillary sinus. In the medial corner of the wound is perforated the nasal cavity and is removed its outer wall together with the concha nasalis, are removed all the cells of the ethmoidal sinus and the back wall of the maxillary sinus. After removal of the tumor it is determined by palpation the most voluminous area of the zygomatic bone and from the end of the posteromedial corner of the bone is drilled with graduated milling cutters a bed for the integration of the dental implant of a length of at least 20 mm and a diameter of 3…4 mm depending on the state of the lateral wall of the orbit, at the same time the cutter is introduced 5 mm laterally. In the implant is screwed a bolt with the head of spherical shape, afterwards according to the defect size is installed a temporary prosthesis of acrylic material filled with silicone, which includes a base with artificial teeth, an obturator and a locking element, which is a socket for the bolt head, then after 3…4 months is installed a permanent prosthesis corresponding to the defect size.
