본 출원은 2021년 4월 6일 출원된 미국 가출원 번호 63/171,476에 대한 우선권을 주장하며, 이 출원의 내용은 모든 목적을 위해 그 전체가 참조로 본원에 포함된다.
ExThera Medical제인 Seraph® 100은 혈류 감염 중에 병원균을 감소시키기 위한 CE 인증 장치이다. Seraph® 100을 사용한 치료는 체외 필터를 통과할 때마다 들어오는 환자 혈액에서 병원균을 34 내지 99% 이상 제거할 수 있다. 만성 투석과 지속적인 신장 대체 요법에 사용되는 전형적인 혈류량에서는 단 몇 시간의 치료만으로도 환자의 혈류량이 크게 감소한다.
최근, Seraph® 100이 기계적 환기 및 혈관수축제 지원이 필요한 사례의 2019 코로나바이러스 질환에서 혈역학적 안정성을 개선하는 것으로 입증되었다. COVID-19 승압제 용량 치료에서, Seraph® 100 치료 후 체온, 인터루킨-6 및 C 반응성 단백질 수치가 감소하였다. 중증 급성 호흡기 증후군 코로나바이러스 2(SARS-CoV-2) 바이러스혈증이 검사를 받은 환자에게서 확인되었고 치료가 완료되면서 사라졌다. (SW Olson et al., Crit Care Explor, (8); 2020 Aug 참조). COVID-19 환자에 대한 치료 장려로 인해 Seraph® 100은 호흡기 영향을 받은 COVID-19 환자 치료용으로 FDA로부터 긴급 사용 승인(EUA)을 받았다.
현재까지 이루어진 진전에도 불구하고, 급성 신장 손상(AKI) 환자 또는 혈류 감염이나 기타 염증 상태로 고통받는 환자를 치료하기 위한 새로운 여과 매체가 필요하다. 본 개시 내용은 이러한 요구를 충족하고 다른 이점도 제공한다.

상세한 설명
체외 순환을 위한 혈액 회로는 일반적으로 환자로부터 혈액을 채취하기 위한 이중 또는 다중 루멘 바늘 또는 동맥측 천자 바늘을 갖는 동맥혈 회로, 혈액을 환자에게 돌려보내기 위해 끝에 정맥측 천자 바늘을 갖는 정맥 혈액 회로, 및 동맥혈 회로와 정맥혈 회로 사이 필터를 포함한다.
일 실시양태에서, 본 개시 내용은 (i) 중공사 막(들) 및 (ii) 흡착 매체의 조합을 포함하는 여과 매체를 제공한다. 본 발명의 여과 매체, 방법 및 장치는 혈액 투석 및 혈액 여과에 사용되는 것과 같은 혈액 정화 기술에 사용된다. 예를 들어, 가요성 튜브와 여과 장치로 구성된 혈액 회로가 본원에 개시된 여과 매체를 통해 대상체의 혈액을 체외 순환시키기 위해 사용된다.

'비드'는 직면한 유속 및 압력 하에서 변형/압축에 저항할 수 있을 만큼 충분히 견고한 재료(예를 들어, 중합체 비드)로 만들어질 수 있다. 변형에 대한 저항성은 패킹층 접촉기(packed bed contactor)의 틈새 치수와 전체 자유 부피 및 그에 따른 낮은 압력 강하를 유지하는 데 유리하다. 패킹층의 치수 안정성은 혈액 세포가 여과되는 것을 방지하기 위해 충분한 비드 간 분리를 유지하는 데에도 중요하다. 대부분의 기재에 접근 가능한 기공이 거의 없기 때문에 흡착/결합 전에 흡착물이 기공으로 확산될 필요가 없다. 본 개시 내용의 흡착 부위는 주로 매질의 표면 상에 있고 따라서 주로 대류 수송에 의해 그 표면으로 전달되는 혈액 내의 흡착물에 접근 가능하도록 위치된다. 거칠기(roughness)는 예를 들어 헤파린의 이온 결합 또는 바람직하게는 공유 결합에 의해 결합 부위의 부착을 위한 표면적의 바람직한 증가를 생성하기 때문에 적합한 기재는 그 표면이 완벽하게 매끄러울 필요는 없다. 반면, 분자 차원의 접근 가능한 내부 기공은 흡착물이 결합 부위에 부착되기 전에 기공으로 확산될 필요성을 제거하기 위해 대부분 피해진다.
다양한 종류의 비드가 본 개시 내용에 사용될 수 있다. 적합한 비드는 본 방법에 사용하는 동안 변형/압축을 방지할 수 있을 만큼 충분한 크기와 강성을 갖고, 본 방법에 사용하기 위해 헤파린으로 코팅될 수 있을 만큼 충분한 표면적을 가지고 있다.
비드 또는 기타 고표면적 기재는 본질적으로 침출 가능한 불순물이 없는 유리, 세라믹 및 금속을 포함하는 천연 또는 합성 중합체 또는 비중합체 물질과 같은 다양한 생체적합성 물질로 제조될 수 있다. 일부 예시적인 중합체로 폴리우레탄, 폴리메틸메타크릴레이트, 폴리에틸렌 또는 에틸렌과 다른 단량체의 공중합체, 폴리에틸렌 이민, 폴리프로필렌 및 폴리이소부틸렌이 포함된다. 유용한 기재의 예에는 비다공성 초고분자량 폴리에틸렌(UHMWPE)이 포함된다. 다른 적합한 비드는 선택적으로 가교된 폴리스티렌, 고밀도 및 저밀도 폴리에틸렌, 실리카, 폴리우레아 및 키토산이다.
이러한 비드를 제조하는 방법은 당업계에 공지되어 있다. 폴리에틸렌 비드 및 기타 폴리올레핀 비드는 중합체 합성 공정 중에 직접 생성되며 더 이상의 크기 감소 없이 사용될 수 있는 경우가 많다. 다른 중합체는 원하는 크기 분포와 모양의 비드를 만들기 위해 분쇄하거나 분무 건조되고 분류하거나 달리 가공해야 할 수도 있다.
특정 측면에서, 체외 혈액 여과를 위한 개별 비드 사이의 채널 또는 틈새 공간의 크기는 카트리지의 입구와 출구 사이의 고압 강하를 감소시키거나 제거하여 고유량 환경에서 개별 비드 사이의 혈액 세포의 안전한 통과를 허용하고 혈액 내 독소, 사이토카인 또는 병원체에 다당류 흡착제를 결합시키기 위한 적절한 간극 표면적(interstitial surface area)을 제공한다. 300 마이크론의 거의 구형 비드가 밀집된 패킹층에서, 적절한 간극 기공 크기는 직경 약 68 마이크론이다. 유용한 비드는 약 100 마이크론, 125 마이크론, 150 마이크론, 175 마이크론, 200 마이크론, 225 마이크론, 250 마이크론, 275 마이크론, 300 마이크론, 325 마이크론, 350 마이크론, 375 마이크론, 400 마이크론, 425 마이크론, 450 마이크론, 475 마이크론, 및/또는 500 마이크론과 같이 직경이 약 100 내지 500 마이크론을 초과하는 범위의 크기를 갖는다. 비드의 평균 크기는 150 내지 450 마이크론이다. 예를 들어, 평균 직경이 0.3 mm인 Polymer Technology Group(미국 버클리 소재)의 폴리에틸렌 비드가 적합하다. 간극 기공은 비드 크기의 함수이다. 사용을 위해 적합한 비드가 컬럼과 같은 용기에 수용된다.
특정 측면에서, 또 다른 적합한 형태의 기재는 망상형 발포체(reticulated foam)을 포함한다. 망상형 발포체는 개방형 셀을 가지며 예를 들어 폴리우레탄 및 폴리에틸렌으로 만들어질 수 있다. 제조 방법을 제어하여 기공 크기를 제어할 수 있다. 일반적으로, 망상형 발포체는 인치당 3 내지 100개의 기공을 가질 수 있으며 >66 c㎡의 표면적을 나타낼 수 있다.

다른 특정 예에서, 흡착 매체는 예를 들어 생체적합성 흡착 비드 기술, 예를 들어 CytoSorb™, CytoSorbents™, Inc.와 같이 순환 혈액으로부터 사이토카인을 제거하기 위한 체외 혈액흡착 필터 장치와 유사한 재료일 수 있다. CytoSorb 혈액흡착 비드는 생체적합성 폴리비닐-피롤리돈으로 코팅된 폴리스티렌-디비닐벤젠 다공성 입자(평균 입자 직경 450 μm, 기공 직경 0.8 내지 5 nm, 표면적 850 ㎡/g)이다. 예를 들어, 폴리스티렌 디비닐 벤젠 공중합체 및 폴리비닐 피롤리돈 중합체를 포함하는 조성물을 사용하는 방법을 청구하는 미국 특허 번호 8,647,666를 참조한다.

실시예
1. 비교예 1
투석 혈액 회로를 상업용 투석기에 설치하였다. 투석기의 혈액 및 투석액 측을 모두 500 ml의 인산염 완충 식염수(PBS)를 200 ml/분의 유속으로 프라이밍하였다. 그 후, 요소와 메티실린 내성 황색포도상구균(Staphylococcus aureus)(MRSA) 박테리아를 포함하는 포유류 혈액의 시험 샘플을 시스템에 통과시켰다. 상당한 양의 요소가 투석기에 의해 제거될 수 있으나; MRSA는 혈액 속에 남는다. 혈액 샘플이 혈액 회로를 여러 번 통과한 후 IL-1β, TNF-α, IL-6 및 IL-15와 같은 염증 매개체와 IL-8 및 GRO-α와 같은 케모카인의 혈액 샘플 내 농도가 증가한 것으로 나타났다.
2. 비교예 2
투석 혈액 회로를 Seraph® Microbind® Affinity Blood Filter에 설치하였다. Seraph®을 포함하는 혈액 여과 장치의 혈액 및 투석액 측을 모두 500 ml의 PBS로 200 ml/분의 유속으로 프라이밍하였다. 그 후, 요소와 메티실린 내성 황색포도상구균(Staphylococcus aureus)(MRSA) 박테리아를 포함하는 포유류 혈액의 시험 샘플을 시스템에 통과시켰다. MRSA만 샘플에서 제거되고 요소는 혈액 샘플에 남아 있었다.
3. 실험예 1
투석 혈액 회로를 상업용 투석기와 Seraph® Microbind® Affinity Blood Filter에 설치하였다. 투석기와 필터 조합의 혈액 및 투석액 측을 모두 500 ml의 PBS로 200 ml/분의 유속으로 프라이밍하였다. 상당량의 요소가 투석기에 의해 제거되고, MRSA가 Seraph® 필터에 의해 제거되었다. 혈액 샘플이 혈액 회로를 여러 번 통과한 후에는 혈액 샘플에 염증 매개체가 존재하지 않았다.
4. 비교예 3
제1 혈액 회로에는, 혈액 회로에 (i) Seraph® Microbind® Affinity Blood Filter와 (ii) 상업용 투석기 장치가 직렬로 결합하여 사용되는 두 개의 장치를 갖췄다. 제2 혈액 회로에는 Seraph® Microbind® Affinity Blood Filter와 투석기(중공사 막)가 결합된 단일 장치로 본 발명의 장치를 사용하였다. 두 컬럼(회로 1) 기술을 본 발명의 제2 혈액 회로(Seraph® Microbind® Affinity Blood Filter + 투석기 중공사 막)에 단일 컬럼으로 결합함으로써 몇 가지 이점이 실현된다. 본 발명의 단일 컬럼은 두 개의 추가적인 컬럼 단부 캡이 필요하지 않음으로써 혈액량을 감소시킨다. 각 컬럼에는 단독으로 두 개의 단부 캡이 있는데, 이는 부피를 차지하며 제1 혈액 회로에서 두 개의 컬럼을 직렬로 연결하면 4개의 앤드캡이 있음을 의미한다. 본 발명의 하이브리드 장치는 단지 두 개의 단부 캡만 필요하므로 총 혈액량이 적어진다.

Claims (43)
- 중공사 막(hollow fiber membrane)과 흡착 매체(adsorption media)의 조합을 포함하는 여과 매체(filtration media).
- 제1항에 있어서, 중공사 막은 중합체로 만들어진 여과 매체.
- 제2항에 있어서, 중합체는 폴리프로필렌, 폴리에틸렌이민 처리된 폴리아크릴로니트릴(AN69), 폴리메틸 메타크릴레이트(PMMA), 폴리설폰(PSf), 폴리에테르설폰(PES), 셀룰로오스 트리아세테이트(CA), 폴리아크릴로니트릴(PAN), 에틸렌 비닐 알코올(EVOH), 폴리비닐리덴 플루오라이드(PVDF) 및 이들의 조합으로 이루어진 군으로부터 선택된 1종인 여과 매체.
- 제1항 내지 제3항 중 어느 한 항에 있어서, 흡착 매체는 표면 코팅된 고체 기재(substrate)인 여과 매체.
- 제4항에 있어서, 표면 코팅은 헤파린, 폴리에틸렌이민(PEI), 시알산, 히알루론산, 폴리비닐피롤리돈(PVP) 및 이들의 조합으로 이루어진 군으로부터 선택된 1종인 여과 매체.
- 제1항 내지 제5항 중 어느 한 항에 있어서, 표면 코팅은 단클론 항체, 단백질, 탄수화물 및 이들의 조합으로 이루어진 군으로부터 선택된 1종인 여과 매체.
- 제1항 내지 제6항 중 어느 한 항에 있어서, 흡착 매체의 표면적은 약 5 ㎡ 내지 약 50 ㎡인 여과 매체.
- 제1항 내지 제7항 중 어느 한 항에 있어서, 흡착 매체의 표면적은 약 10 ㎡ 내지 약 20 ㎡인 여과 매체.
- 제1항 내지 제8항 중 어느 한 항에 있어서, 흡착 매체는 중공사 막의 내경보다 더 크게 치수화된 여과 매체.
- 제1항 내지 제9항 중 어느 한 항에 있어서, 흡착 매체는 비드(bead)인 여과 매체.
- (i) 중공사 막과 (ii) 흡착 매체의 조합을 포함하는 여과 매체를 포함하는 장치.
- 제11항에 있어서, 여과 매체가 사이에 배치되는 제1 단부 캡과 제2 단부 캡을 갖는 장치.
- 제11항에 있어서, 여과 매체는 제1 흡착 매체 부분, 중공사 막 부분 및 제2 흡착 매체 부분의 샌드위치 구성(sandwich configuration)이고, 상기 중공사 막은 제1 흡착 매체 부분과 제2 흡착 매체 부분 사이에 샌드위치된 장치.
