Abstract
본 발명은 약물에 의한 간장 및 그 밖의 장기에 대한 장해 (DILI 등) 의 발증 가능성 등에 대해서 상세한 분석을 가능하게 하는, 약물 독성 평가 플랫폼 (평가 방법 및 그것을 위한 키트 등) 을 제공한다. 본 발명의 약물 독성 평가 방법은, 오르가노이드와 혈액 세포의 공배양계에, 약물을 첨가하는 공정, 및 당해 오르가노이드에 대한 당해 약물의 독성을 평가하는 공정을 포함한다.
Description
본 발명은 약물에 의한 간장 및 그 밖의 장기에 대한 장해 (예를 들어 약물성 간 장해) 의 발증 가능성의 예측 등을 가능하게 하는 평가계에 관한 것이다.
약물성 간 장해 (drug-induced liver injury : DILI) 는, 약물 요법을 계속하는 과정에서 발현할 수 있는 부작용이다. 약물성 간 장해는, 발생 기전에 따라서, 중독성형, 알레르기성 특이 체질형, 및 대사성 특이 체질형으로 분류된다. 중독성형은, 약물 자체 또는 약물의 대사 산물의 독성이 원인이 되는, 용량 의존적인 간 장해로서, 아세트아미노펜, 메토트렉사트 등에 의해서 야기된다. 알레르기성 특이 체질형은, 약물 자체 또는 약물의 대사 산물의 항원성 획득에 의한 자기 면역이 원인이 되는, 용량 비의존적인 간 장해로서, 티클로피딘 HCl, 록소프로펜 Na, 페니토인, 카르바마제핀, 리팜피신, 테르비나핀 HCl 등에 의해서 야기된다. 대사성 특이 체질형은, 대사 효소 등의 유전적 소인에 의한 간 독성의 대사물의 증가가 원인이 되는, 복용 기간 의존적인 간 장해로서, 디클로페낙 Na, 이소니아지드, 아카르보스 등에 의해서 야기된다. 약물 투여 전에 환자마다 DILI 의 발증 가능성을 예측하고, 복수의 약물 중에서 적절한 것을 선택할 수 있도록 하는 것은 중요하고, 그것을 위한 평가계의 확립이 요망되고 있다.
특허문헌 1 에는,「검사 대상 약물을 함유하는 배지 중에서, 피검사자 유래의 면역 세포를, 세포는 투과하지 않지만 약물 및 그 대사물은 투과하는 막을 사이에 둔 상태에서, 약물 대사 효소를 발현한 간 세포와 공배양하는 공정」 및「상기 공정 후의 면역 세포를 해석하는 공정」을 포함하는, 알레르기형 약물성 간 장해의 평가 방법이 기재되어 있다.
또, 비특허문헌 1 에도, 특허문헌 1 과 유사한, 간장 세포주 (HepG2) 와 단구/매크로파지 세포주 (THP-1) 를 공배양시키는 DILI 의 평가계가 기재되어 있고, DILI 약물 (트로글리타존, 트로바플록사신, 디클로페낙, 케토코나졸) 과 비 DILI 약물 (로시글리타존, 레보플록사신, 아세틸살리실산, 플루코나졸) 사이에서, 또 LPS 나 TNF 등의 염증 유발 인자의 유무에 의해서, 간 세포의 반응성에 차이가 보이는 것 등이 기재되어 있다.

DILI 에 관한 평가계로서, 상기 배경 기술에 든 것이 제안되어 있는데, 예를 들어, 환자 한사람 한사람에게, DILI 의 발증 가능성이 있는 약물을 피하고 대체약을 제안할 수 있는 등, 상세한 분석을 행할 수 있도록 하는 데 있어서, 여전히 개선의 여지가 있었다.
본 발명은 약물에 의한 간장 및 그 밖의 장기에 대한 장해 (DILI 등) 의 발증 가능성 등에 대해서 상세한 분석을 가능하게 하는, 약물 독성 평가 플랫폼 (평가 방법 및 그것을 위한 키트 등) 을 제공하는 것을 과제로 한다.
본 발명자들은 간장 오르가노이드를 혈액 세포 (예를 들어 단구/매크로파지와 같은 면역 담당 세포) 와 공배양하여 DILI 평가계로서 사용했을 경우, 약물에 의한 DILI 의 발증 가능성이 적확하게 반영되는 것을 알아내었다. 구체적으로는, 그 간장 오르가노이드가 DILI 환자 유래의 세포를 사용하여 제작된 것인 경우에는, 소량의 DILI 약물 (예를 들어 항균약인 암피실린) 의 첨가로도 간장 오르가노이드에 대한 독성이 인정되고, 약물에 의한 DILI 의 발증 가능성을 양호한 감도로 평가할 수 있는 것을 알 수 있었다. 또한, 상기 DILI 약물과 동일한 목적에서 사용되는 다른 약물 (예를 들어 아목시실린, 세팔렉신) 을 첨가했을 경우에, 간장 오르가노이드가 DILI 환자 유래의 세포를 사용하여 제작된 것이어도 DILI 가 인정되지 않는 경우가 있고, 그와 같은 약물은 DILI 환자에 대한 대체약이 될 수 있다고 판별할 수 있는 것도 알 수 있었다.
즉, 본 발명은 상기 과제 해결을 위해서, 이하의 [1] ∼ [7] 을 제공한다.

본 발명에 의한 약물 독성 평가 플랫폼 (평가 방법 및 그것을 위한 키트 등) 을 사용함으로써, 환자의 장기에 대한 독성을 약물마다 정확하게 판별하여 독성이 보다 낮은 약물을 선택하거나, 약물의 독성을 완화하기 위한 작용 기전 (바이오 마커 등) 이나, 종래보다 독성이 보다 낮은 새로운 약물을 알아내거나 할 수 있게 된다.
또한, 오르가노이드를 혈액 세포와 함께 사용하는 본 발명의 약물 독성 평가 방법이 우수한 이유로는, 예를 들어, 혈관 내피 세포를 포함하는 입체적인 장기 구조가 생체 내에 보다 가깝고, 약물의 독성과 혈액 세포의 상호 작용을 높은 정밀도로 재현할 수 있는 것이나, 세포간 상호 작용에서 기인하는 세포의 성숙의 요인 등으로부터 평가에 있어서 중요한 대사 효소의 활성이 높은 것 등이 추측된다. 또, 본 발명의 약물 독성 평가 방법에서는 혈액 세포, 바람직하게는 면역 담당 세포를 사용하기 때문에, 특히 그와 같은 세포 (면역계) 가 약물 독성에 관여하고 있는 경우에, 종래의 평가 방법에서는 곤란했던, 인 비트로 (in vitro) 에서의 독성의 재현성을 향상시켜, 보다 정확하게 (높은 S/N 비로) 독성을 재현하는 것이 가능해진다.
-약물 독성 평가 방법-
본 발명의 약물 독성 평가 방법은, 적어도 하기의 공정을 포함하고 있고, 필요에 따라서 추가로 다른 공정을 포함하고 있어도 된다.
공정 1 (약물 첨가 공정) : 오르가노이드와 혈액 세포의 공배양계에, 약물을 첨가하는 공정.
공정 2 (독성 평가 공정) : 당해 오르가노이드에 대한 당해 약물의 독성을 평가하는 공정.
<오르가노이드>
본 발명에서 사용하는「오르가노이드」에는, 장기 혹은 조직 오르가노이드 (본 명세서에 있어서「장기ㆍ조직 오르가노이드」라고 총칭한다.), 암 오르가노이드 등이 포함된다. 또한, 오르가노이드에는, 장기, 조직, 암 등의 오르가노이드로서 구조가 성숙한 단계에 있는 것뿐만 아니라, 단순한 세포 응집체 (스페로이드) 도 포함된다.
오르가노이드는, 인간에서 유래하는 것이어도 되고, 인간 이외의 동물, 예를 들어, 마우스, 래트, 개, 돼지, 원숭이 등의 포유 동물에서 유래하는 것이어도 된다. 즉, 오르가노이드를 제작하기 위해서 사용되는 소정의 세포 (상세한 것은 이하의 기재를 참조) 는, 인간에서 유래하는 세포여도 되고, 인간 이외의 동물에서 유래하는 세포여도 된다. 인간의 의약 개발에 있어서, 종래의 동물 실험이나 인간 세포 시험 등에서는 발견되기 어려운 약물 중독을 야기하는 약물을 검출하는 관점에서는, 오르가노이드는 인간에서 유래하는 것인 것이 바람직하다. 또, 오르가노이드를 제작하기 위해서 사용되는 소정의 세포는, 초대 배양 세포여도 되고, 계대 배양 세포 (주화 세포) 이어도 된다.
본 발명의 바람직한 일 실시형태에 있어서, 오르가노이드는 약물 독성을 발증한 환자 (인간) 에게 유래하는 것이다. 이 실시형태에 있어서, 오르가노이드를 제작하기 위해서 사용되는 소정의 세포는, 예를 들어, 약물 독성을 발증한 환자에게서 채취된 세포 (초대 배양 세포) 또는 당해 세포를 사용하여 제작된 iPS 세포, 혹은 그것들의 계대 배양 세포 (주화 세포) 이다. 예를 들어, 약물 독성을 발증한 환자에게서 채취된 세포를 사용하여 제작된 iPS 세포주는, 본 발명의 약물 독성 평가 방법을 표준적인 것으로 하는 데 있어서, 오르가노이드를 제작하기 위해서, 즉 오르가노이드의 원료가 되는 소정의 각종 세포로 분화 유도하기 위해서 바람직하다.
「장기ㆍ조직 오르가노이드」는, 인위적으로 창출된 장기 또는 조직에 유사한 조직체 (삼차원 구조체) 이다. 이미 다양한 종류의 장기 오르가노이드, 예를 들어, 간장, 췌장, 신장, 심장, 폐장, 비장, 식도, 위, 갑상선, 부갑상선, 흉선, 생식선, 뇌, 척수, 피부, 내이 등의 오르가노이드가 공지되어 있다 (예, https://www.nejm.org/doi/pdf/10.1056/NEJMra1806175, https://www.nature.com/articles/s41568-018-0007-6, http://www.amsbio.com/brochures/organoid-culture-handbook.pdf 참조). 또한,「장기ㆍ조직 오르가노이드」에는, 최종적으로 복잡한 구성을 갖는 장기에 이르는 초기 단계의 구조체인「기관아 (器官芽)」(예를 들어 간아 (肝芽), 췌아 (膵芽)) 도 포함된다.
장기ㆍ조직 오르가노이드 (기관아) 의 제작 방법은 공지되어 있고, 본 발명에서 사용하는 장기ㆍ조직 오르가노이드가 어떠한 제작 방법에 의해서 얻어진 것인지는 특별히 한정되지 않는다. 장기ㆍ조직 오르가노이드 (기관아) 의 제작 방법의 바람직한 일례로는, WO2015/129822 에 기재되어 있는, 장기ㆍ조직을 구성하는 세포, 간엽계 세포 및 혈관 내피 세포를 공배양함으로써 간아를 제작할 수 있다.
어떠한 장기ㆍ조직 오르가노이드 (기관아) 를 사용할지는, 약물 독성 평가 방법의 목적에 따라서 선택할 수 있다. 예를 들어, 간장 독성에 대한 약물 독성 평가 방법을 실시하는 경우에는, 실제로 장해 (간 세포 상해 등) 가 발생되는 장기인 간장의 오르가노이드를 사용해도 되고, 또 알레르기성 특이 체질형의 약물성 간 장해인 경우에 알레르기 반응이 발증하기 쉬운 기관인 피부, 내이 등의 오르가노이드를 사용하면 된다.
본 발명에 있어서의 장기 오르가노이드의 대표예로서「간장 오르가노이드」를 들 수 있다. 간장 오르가노이드는, 바람직하게는「간아」이다. 간장 오르가노이드 (간아) 의 제작 방법은 공지되어 있고, 바람직한 일례로는, WO2015/129822 에 기재되어 있는 장기 오르가노이드 (기관아) 의 제작 방법에 준하여, 간장 내배엽 세포, 간엽계 세포 및 혈관 내피 세포를 공배양하는 방법을 들 수 있다.
「암 오르가노이드」는, 암 세포와 그 밖의 세포로 구성되는 세포 응집체로서, 암 미소 환경을 재현하는 것이다. 암 오르가노이드의 제작 방법은 공지되어 있고, 예를 들어, 일본 공개특허공보 2018-110575호에 기재되어 있는 바와 같이, 암 세포, 간엽계 세포 및 혈관 내피 세포를 공배양함으로써 암 오르가노이드를 제작할 수 있다.
암 세포는, 기존의 암 세포주여도 되고, 인간 암원발소 (癌原發巢) 에서 분리된 암 조직을 사용하여 수립한 프라이머리 암 세포주여도 된다. 암의 종류는 특별히 한정되는 것은 아니고, 예를 들어, 간암, 신장암, 악성 뇌종양, 췌장암, 위암, 폐암 등이어도 된다.
또한, 암 오르가노이드를 사용하는 경우에는, 본 발명의 전형적인 실시형태에 따라서, 예를 들어, 약물이 투여된 암 세포 (암화된 조직) 에 있어서, 통상과는 상이한 대사 산물이 만들어짐으로써, 암의 증상이 악화되거나 다른 장기에 악영향을 미치거나 하는 양식의 약물 독성을 평가하는 것, 나아가서는 그와 같은 약물 독성에 관여하는 바이오 마커를 추정할 수 있다. 한편으로, 암 오르가노이드를 사용하는 경우에는, 본 발명의 약물 독성 평가 방법을, 독성 이외의 관점에서 약물을 평가하는 방법, 예를 들어「약물 저항성의 평가 방법」으로 변경할 수도 있다. 암 (췌암 등) 의 치료 저항성 (약물 저항성, 방사선 감수성, 면역 요법 감수성, 영양 요법 감수성 등) 에는, 암 세포와, 암 세포 주위에 존재하는 다양한 세포 (종양 관련 섬유아 세포나 혈관 내피 세포 등의 간엽계 세포, 매크로파지 등의 염증 세포 등) 의 상호 작용에 의해서 구축되는 종양 미소 환경이 중요한 역할을 담당하고 있다고 한다. 따라서, 암 오르가노이드와 혈액 세포 (면역 담당 세포 등) 의 공배양계에, 약물을 첨가하는 공정, 및 당해 암 오르가노이드의 당해 약물에 대한 약물 감수성을 평가하는 공정을 포함하는 약물 감수성 평가 방법을 실시할 수도 있다.
오르가노이드 (장기ㆍ조직 오르가노이드) 의 제작에 사용되는「장기ㆍ조직을 구성하는 세포」에는, (I) 장기ㆍ조직을 구성하는 실질 세포 및 (II) 장기ㆍ조직을 구성하는 비실질 세포가 포함된다. 또, (I) 실질 세포 및 (II) 비실질 세포에는, 각각, (i) 분화되고 성숙된, 또는 종말 분화에 이른, 실질 세포 또는 비실질 세포로서의 소정의 기능성을 갖는 세포 (본 명세서에 있어서 간단히「분화 세포」라고 부른다.), 및 (ii) 실질 세포 또는 비실질 세포에 대한 분화능을 갖거나, 또는 분화가 운명적이지만 (커미트먼트되어 있지만), 미분화이거나, 또는 줄기 세포 혹은 전구 세포의 단계이며, 실질 세포 또는 비실질 세포로서의 소정의 기능성을 아직 충분히 갖지 못한 세포 (본 명세서에 있어서 간단히「미분화 세포」라고 부른다.) 가 포함된다.
장기ㆍ조직을 구성하는 세포로는, 실질 세포의 분화 세포, 실질 세포의 미분화 세포, 비실질 세포의 분화 세포 및 비실질 세포의 미분화 세포로 이루어지는 군에서 선택되는 적어도 1 종의 세포, 바람직하게는「장기ㆍ조직 오르가노이드」(예, 기관아) 의 형성이 가능한 2 종 이상의 세포의 조합을 사용할 수 있다.
장기ㆍ조직을 구성하는「실질 세포」로는, 예를 들어, 간장의 간 세포, 췌장의 내분비 세포 (예, α 세포, β 세포, δ 세포, ε 세포, PP 세포) 및 췌관 상피 세포, 신장의 요세관 상피 세포 및 사구체 상피 세포, 폐의 폐포 상피 세포, 심장의 심근 세포, 장관의 상피 세포, 뇌의 신경 세포 및 글리아 세포, 척수의 신경 세포 및 슈반 세포 등을 들 수 있다.
장기ㆍ조직을 구성하는「비실질 세포」로는, 예를 들어, 간장의 유동 (類洞) 내피 세포, 간성 세포 및 쿠퍼 세포, 췌장의 췌성 세포 및 췌 미소 혈관 내피 세포, 신장의 신사구체 내피 세포, 폐의 폐동맥 내피 세포 및 폐 선유아 세포, 심장의 심 미소 혈관 내피 세포, 대동맥 내피 세포, 관동맥 내피 세포 및 심 선유아 세포, 장관의 장관 미소 혈관 내피 세포, 뇌의 뇌 미소 혈관 내피 세포, 혈관 주피 세포, 맥락총 내피 세포 및 뇌혈관 외막 선유아 세포 등을 들 수 있다.
장기ㆍ조직을 구성하는 실질 세포 또는 비실질 세포에 대한 분화능을 갖는「미분화 장기 세포」로는, 예를 들어, 뇌, 척수, 부신 수질, 표피, 모발ㆍ조 (爪)ㆍ피부선, 감각기, 말초 신경, 수정체 등의 외배엽성 기관으로 분화 가능한 세포 ; 신장, 요관, 심장, 혈액, 생식선, 부신 피질, 근육, 골격, 진피, 결합 조직, 중피 등의 중배엽성 기관으로 분화 가능한 세포 ; 간장, 췌장, 장관, 폐, 갑상선, 부갑상선, 요로 등의 내배엽성 기관으로 분화 가능한 세포 ; 등을 들 수 있다.
간장 오르가노이드 (간아) 의 제작에 사용되는「간 세포」는, 간장의 실질 세포로서, 분화된 간 세포 (분화 간 세포) 와, 간 세포로의 분화 운명이 결정되어 있지만, 아직 간 세포로 분화되어 있지 않은 세포 (미분화 간 세포), 이른바 간 전구 세포 (예, 간장 내배엽 세포) 의 양방의 개념을 포함하는 용어이다. 분화 간 세포는, 생체로부터 채취된 (생체 내의 간장으로부터 단리된) 세포여도 되고, ES 세포나 iPS 세포 등의 다능성 줄기 세포, 간 전구 세포, 그 밖의 줄기 세포로 분화되는 능력을 갖는 세포를 분화시켜 얻어진 세포여도 된다. 미분화 간 세포는, 생체로부터 채취된 것이어도 되고, ES 세포나 iPS 세포 등의 다능성 간 세포, 그 밖의 간 세포 또는 전구 세포를 분화시켜 얻어진 것이어도 된다. 간 세포로 분화 가능한 세포는, 예를 들어, K. Si-Taiyeb, et al. Hepatology, 51 (1) : 297-305 (2010), T. Touboul, et al. Hepatology. 51 (5) : 1754-65. (2010) 에 따라서 제작할 수 있다. ES 세포나 iPS 세포 등의 다능성 줄기 세포, 간 전구 세포, 그 밖의 간장 세포로 분화되는 능력을 갖는 세포를, 간장 세포로 분화시키는 방법은 공지되어 있고, 예를 들어, iPS 세포에서는, Hepatology, 2010 ; 51 (1) : 297-305, Cell Rep. 2017 ; 21 (10) : 2661-2670 등에 기재된 방법에 준하여 행할 수 있다. 생체로부터 채취된 세포 집단, ES 세포나 iPS 세포 등의 분화 유도에 의해서 제작한 세포 집단, 어느 것에 대해서도 (특히 후자에 대해서는), 분화 간 세포의 순도가 높은 세포 집단 또는 미분화 간 세포의 순도가 높은 세포 집단을 사용해도 되고, 분화 간 세포 및 미분화 간 세포를 임의의 비율로 포함하는 세포 혼합물을 사용하면 된다.
어느 세포가 분화 간장 세포인지의 여부는, 성숙 간 세포 마커, 예를 들어, 아시알로글리코프로테인 리셉터 1 (ASGR1), 미성숙 간 세포 마커 (초기 간 분화 마커) 인 α 페트프로테인 (AFP), 초기 간 분화 마커인 알부민 (ALB), 레티놀 결합 프로테인 (RBP4), 트랜스티레틴 (TTR), 글루코오스-6-포스파타아제 (G6PC) 등의 1 종 또는 2 종 이상의 발현이 양성인지의 여부에 의해서 판별할 수 있다. 한편, 어느 세포가 미분화 간장 세포인지의 여부는, HHEX, SOX2, HNF4α, AFP, ALB 등의 1 종 또는 2 종 이상의 세포 마커의 발현이 양성인지 (ALB 에 대해서는 약양성인지) 의 여부에 의해서 판별할 수 있다.
오르가노이드 (장기ㆍ조직 오르가노이드, 암 오르가노이드 등) 의 제작에 사용되는「혈관 내피 세포」는, 조혈성 혈관 내피 세포 (hemogenic endothelial cell ; HEC) 및 비조혈성 혈관 내피 세포 (non-hemogenic endothelial cell ; non-HEC) 의 양방의 개념을 포함하는 용어이다. HEC 는, 조혈 줄기 세포를 산생할 수 있는 (조혈능을 갖는) 혈관 내피 세포로서, 혈구 산생형 혈관 내피 세포라고도 불린다. 한편으로, non-HEC 는, 그와 같은 조혈능을 갖지 않는 혈관 내피 세포이다.
혈관 내피 세포는, 생체로부터 채취된 혈관 내피 세포 (예를 들어, 미소 혈관 내피 세포 (microvessel endothelial cells : MVEC), 간 유동벽 내피 세포 (liver sinusoidal endothelial cells : LSEC), 제대 정맥 내피 세포 (umbilical-vein endothelia lcells : UVEC) 등) 의 순도가 높은 세포 집단이어도 되고, ES 세포나 iPS 세포 등의 다능성 줄기 세포, 그 밖의 혈관 내피 세포로 분화되는 능력을 갖는 세포를 분화시켜 얻어진 혈관 내피 세포의 순도가 높은 세포 집단이어도 된다.
오르가노이드 (장기ㆍ조직 오르가노이드, 암 오르가노이드 등) 의 제작에 사용되는「간엽계 세포」는, 주로 중배엽에게 유래하는 결합 조직에 존재하고, 조직에서 기능하는 세포의 지지 구조를 형성하는 결합 조직 세포를 가리키며, 분화된 세포 (분화 간엽계 세포) 와, 간엽계 세포에 대한 분화 운명이 결정되어 있지만, 아직 간엽계 세포로 분화되어 있지 않은 세포 (미분화 간엽계 세포), 이른바 간엽계 줄기 세포의 양방의 개념을 포함하는 용어이다. 단,「혈관 내피 세포」는 미분화 간엽계 세포로부터 분화되는 세포의 1 종이지만, 본 명세서에 있어서의「간엽계 세포」의 정의에서 제외되는 것으로 한다.
어느 세포가 미분화 간엽계 세포인지 분화 간엽계 세포인지는, 예를 들어, 미분화 간엽계 세포의 마커인, Stro-1, CD29, CD44, CD73, CD90, CD105, CD133, CD271, 네스틴 (Nestin) 등의 1 종 또는 2 종 이상이 양성인지의 여부에 의해서 (양성이면 미분화 간엽계 세포, 음성이면 분화 간엽계 세포로) 판별할 수 있다.
간엽계 세포는 또한, 본 발명에 있어서 목적으로 하는 장기 오르가노이드나, 조합하여 사용되는「장기ㆍ조직을 구성하는 세포」에 따라서, 특정한 장기 (조직) 에 특이적인 세포 마커를 발현하는 것이어도 된다. 그와 같은 세포 마커로는, 예를 들어, 횡중격 간엽 (septum transversum mesenchyme : STM) 의 세포 마커인 FOXF1, COL4A 및 ALCAM 을 들 수 있다.
<혈액 세포>
본 발명에서 사용하는「혈액 세포」에는, 적혈구, 백혈구 (단핵구 및 과립구) 및 혈소판이 포함된다. 「단핵구」에는 임파구 및 단구가 포함된다. 「임파구」에는, NK 세포, T 세포 (αβ T 세포, γδ T 세포, CD8+ T 세포, CD4+ T 세포, 종양 침윤 T 세포, 메모리 T 세포, 나이브 T 세포, NK T 세포) 및 B 세포가 포함된다. 「과립구」에는, 호중구, 호산구 및 호염기구가 포함된다.
본 발명에 있어서의 혈액 세포는, 바람직하게는 면역 담당 세포이다. 「면역 담당 세포」는, 상기「단핵구」에 거의 상당하고, 획득 면역을 담당하는 T 세포 및 B 세포, 그리고 자연 면역을 담당하는 호중구, NK 세포, 단구, 매크로파지, 수상 세포 등이 포함된다. 예를 들어, T 세포, 호중구, NK 세포, 매크로파지 등은, 약물 첨가 공정에서 오르가노이드와 공배양하는 혈액 세포 (면역 담당 세포) 로서 바람직하다.
혈액 세포, 바람직하게는 면역 담당 세포는, 생체로부터 채취된 세포 (예를 들어 말초혈로부터 단리된 단핵구) 여도 되고, ES 세포나 iPS 세포 등의 다능성 줄기 세포, 그 밖의 혈액 세포로 분화되는 능력을 갖는 세포를 분화시켜 얻어진 세포여도 된다. 또, 혈액 세포는, 초대 배양 세포여도 되고, 계대 배양 세포 (주화 세포) 여도 된다.
혈구 세포는, 인간에서 유래하는 것이어도 되고, 인간 이외의 동물, 예를 들어, 마우스, 래트, 개, 돼지, 원숭이 등의 포유 동물에서 유래하는 것이어도 된다. 인간의 의약 개발에 있어서, 종래의 동물 실험이나 인간 세포 시험 등에서는 발견되기 어려운 약물 중독을 야기하는 약물을 검출하는 관점에서는, 혈구 세포는 인간에서 유래하는 것인 것이 바람직하다. 또한, 혈구 세포의 동물종은 통상적으로 오르가노이드 (그 제작을 위해서 사용되는 소정의 세포) 의 동물종과 동일하다.
본 발명의 바람직한 일 실시형태에 있어서, 혈구 세포는 약물 독성을 발증한 환자 (인간) 에게 유래하는 것이다. 즉, 본 발명의 바람직한 일 실시형태에 있어서, 오르가노이드를 제작하기 위해서 사용되는 소정의 세포는, 예를 들어, 약물 독성을 발증한 환자에게서 채취된 세포 (초대 배양 세포) 또는 당해 세포를 사용하여 제작된 iPS 세포, 혹은 그것들의 계대 배양 세포 (주화 세포) 이다.
<약물>
본 발명에서 사용하는「약물」은, 오르가노이드의 제작 대상으로 하고 있는 장기, 조직 등 (암화된 것을 포함한다.) 에 대해서 적용되는 약물이면 특별히 한정되는 것이 아니고, 독성을 평가하는 목적에 따라서 다양한 약물을 사용할 수 있다.
또한, 약물에 의한「독성」에는, 이하에 예시하는 간장 등의 장기에 대한 독성뿐만 아니라,「약진」(특히 알레르기성 약진) 도 포함된다. 약진 중에는, 중독성 표피 괴사증, 스티븐스ㆍ존슨 증후군, 그리고 바이러스가 관여하는 약제성 과민증 증후군 등도 포함된다.
본 발명에 있어서의 약물 독성은, 간장 독성에 한정되는 것이 아니고, 예를 들어, 심장 독성, 혈액ㆍ골수 독성 등, 간장 이외의 장기 또는 조직에 대한 독성이어도 된다. 본 발명의 약물 독성 평가 방법은, 약물 독성이 발증할 가능성이 있는 (또는 가능성이 있는지의 여부를 검증해야 하는) 장기 또는 조직의 오르가노이드를 사용하여 실시할 수 있다.
본 발명의 일 실시형태에 있어서, 약물은, 투여 대상에 있어서, 간장 독성을 갖는지의 여부를 평가해야 할 약물이다. 간장 독성에 의한 간 장해는, 일반적으로, 간 세포 장해형, 담즙 정체형 및 이것들의 혼합형, 혹은 급성 간염으로 분류된다.
간장 독성 (약물성 간 장해) 의 발생 기전은, 중독성형 및 특이 체질형으로 분류되고, 특이 체질형은 다시 대사성 특이 체질형 및 알레르기성 특이 체질형으로 분류되기도 한다. 각각의 작용 기전에 관계되는 약물로는 다수의 보고가 있고, 예를 들어, FDA (Food and Drug Administration) 의「Drug Induced Liver Injury Rank (DILIrank) Dataset」에는, 약물성 간 장해에 관계되는 약물이 중증도 (Severity Class) 와 함께 공개되어 있다. 본 발명의 약물 독성 평가 방법에 있어서도, 약물 독성이 보고되어 있는 각종 약물, 혹은 그것들의 대체 (의 후보) 가 되는 각종 약물을 사용할 수 있다. 또한, 동일한 약물이어도, 복수의 작용 기전 (예를 들어 알레르기성 특이 체질형과 대사성 특이 체질형의 양방) 에 관계된다고 인정되는 경우도 있다.
중독성형의 약물성 간 장해를 발증할 가능성이 있는 약물은, 약물 자체 또는 그 대사 산물이 간 독성을 갖고, 거의 모든 대상 (인간) 에 용량 의존적으로 간 장해가 발생된다. 중독성형의 약물은, 동물 실험 등에 있어서도 비교적 재현하기 쉽지만, 본 발명의 약물 독성 평가 방법에 있어서의 약물로도 사용할 수 있다. 중독성형의 약물로는, 예를 들어, 아세트아미노펜 (아스피린), 메토트렉사트를 들 수 있다.
한편, 특이 체질형의 약물성 간 장해를 발증할 가능성이 있는 약물은, 독물 실험 등에 있어서의 재현이 비교적 곤란하여, 본 발명의 약물 독성 평가 방법에 있어서의 약물로서 바람직하다고 말할 수 있다. 알레르기성 특이 체질형의 경우, 약물 자체 또는 그 반응성 중간 대사물이 합텐이 되어, 간 세포의 다양한 구성 성분과 결합하여 항원성을 획득함으로써 알레르기 반응이 일어난다고 생각되고, 많게는 약물 복용 후 1 ∼ 8 주에 발병한다. 알레르기성 특이 체질형의 약물로는, 예를 들어, 티클로피딘 HCl, 록소프로펜 Na, 페니토인, 카르바마제핀, 리팜피신, 테르비나핀 HCl 을 들 수 있다. 또, 대사성 특이 체질형의 경우에는, 간장에 있어서의 대사 효소 활성의 개인차 등에서 기인하여 발병한다고 생각되고, 1 주 (특히 8 주 이후) ∼ 1 년 내지 그 이상의 장기의 약물 복용에 의해서 발증한다. 대사성 특이 체질형의 약물로는, 예를 들어, 아카르보스, 아미오다론, 이소니아지드, 이트라코나졸, 경구 피임약, 자필루카스트, 디클로페낙나트륨, 디술피람, 타목시펜, 단백 동화 스테로이드, 단트롤렌나트륨, 테가푸르ㆍ우라실, 염산테르비나핀, 트로글리타존 (판매 중지), 발프로산나트륨, 염산하이드라라진, 플루코나졸, 플루타미드, 페모린, 염산라베탈롤을 들 수 있다.
또, 최근에는, 약물 유발성 간 장해 (drug-induced liver injury) 를, 직접 간 독성 (direct hepatotoxicity), 특이적 간 독성 (idiosyncratic hepatotoxicity) 및 간접 간 독성 (indirect hepatotoxicity) 으로 분류하기도 한다 (N Engl J Med 2019 ; 381 : 264-273). 「직접 간 독성」형의 약제로는, 예를 들어, 고투여량의 아세트아미노펜, 니아신, 아스피린 (아세틸살리실산), 코카인, IV 아미오다론, IV 메토트렉사트, 암 화학 요법에서 사용하는 약제 등을 들 수 있다. 「특이적 간 독성」형의 약제로는, 예를 들어, 아목시실린ㆍ클라불란산, 세팔로스포린, 이소니아지드, 니트로푸란토인, 미노사이클린리, 플루오로퀴놀론류, 매크로라이드류, 항체 등을 들 수 있다. 「간접 간 독성」형의 약제로는, 예를 들어, 항종양제, 글루코코르티코이드류, 모노클로날 항체 (TNF, CD20, 체크 포인트 단백질 등에 대한 것 (항 CTLA-4 항체, 항 PD-1 항체, 항 PD-L1 항체 등)), 단백 키나아제 저해제 등을 들 수 있다.
본 발명의 보다 구체적인 실시형태의 일례로서, 화학 요법약 (항진균제를 포함함) 또는 항균약에 의한 간장 독성의 평가 방법을 들 수 있다. 화학 요법약으로는, 예를 들어, 리팜피신, 이소니아지드 (이소니코틴산하이드라지드 : INH), 살라조술파피리딘 (술파살라진), 오플록사신, 레보플록사신, 노르플록사신, 염산시프로플록사신, 술파메톡사졸ㆍ트리메토프림, 그리세오풀빈 등에 대해서, 간 세포 장해형, 혼합형, 담즙 정체형, 급성 간염 등의 간 장해가 보고되어 있다. 또, 항균약으로는, 예를 들어, 세펨계 (세포티암, 세파크롤, 세파졸린나트륨, 세프메타졸나트륨, 세팔렉신 등), 카르바페넴계 (이미페넴-실라스타틴나트륨 등), 페니실린계 (피페라실린나트륨, 암피실린, 아목시실린, 술박탐나트륨ㆍ암피실린나트륨 배합약, 옥시페니실린, 클라불란산칼륨ㆍ아목시실린 배합제, 타조박탐나트륨ㆍ피페라실린나트륨 배합제 등), 매크로라이드계 (에리트로마이신에스톨레이트, 록시트로마이신, 클라리트로마이신, 아지트로마이신 등), 테트라사이클린계 (염산테트라사이클린 등), 염산미노사이클린, 포스포마이신, 세프테람피복실, 세포독자임프록세틸, 플로목세프나트륨, 염산테르비나핀, 피라지나미드, 플루코나졸, 이트라코나졸 등에 대해서, 간염형, 혼합형, 담즙 정체형, 급성 간염 등의 간 장해가 보고되어 있다.
화학 요법약 및 항균약 이외의, 약물성 간 장해를 발증할 가능성이 있는 약물로는, 예를 들어, 다음과 같은 것을 들 수 있다 :

본 발명의 일 실시형태에 있어서, 약물은, 투여 대상에 있어서 심장 독성을 갖는지의 여부를 평가해야 하는 것이다. 심장 독성은, 예를 들어, 심장 돌연사의 원인이 되는, 치사성 부정맥인 토르사드 드 포인트 (Torsa de de Pointes : TdP) 로서 나타나는 것이 알려져 있다. 종래에는, 약물에 의한 TdP 의 발생 가능성 (TdP 리스크) 은, 그 전단층인 심근의 QT 연장 (활동 전위 지속 시간의 연장) 의 발생 유무나, hERG 채널 저해 작용의 유무 등에 의해서 평가되었다. 본 발명의 약물 독성 평가 방법에 있어서는, 예를 들어, 심장 오르가노이드에 약물을 작용시켰을 때에, 발생되는 세포외 전위의 파형이 소정의 기준에 적합한 것인지의 여부에 의해서, 혹은 그 밖의 수법에 의해서, 그 약물이 심장 독성 (TdP 리스크) 을 갖는지의 여부를 평가할 수 있다. 심장 독성을 발증할 가능성이 있는 약물은, 다양한 것이 공지되어 있어, 본 발명에 사용할 수 있다. 예를 들어, 안트라사이클린계, 시클로포스파미드, 5-플루오로우라실, 탁산 등의 항종양약제 ; 트라스트주맙, 베바시주맙, 니볼루맙 등의 모노클로날 항체 ; 수니티닙, 닐로티닙 등의 티로신 키나아제 저해제 ; 지도부딘 등의 항레트로바이러스약 ; 로시글리타존 등의 항당뇨병약 등을, 심장 독성을 갖는 약물로서 들 수 있다.
본 발명의 일 실시형태에 있어서, 약물은, 투여 대상에 있어서 혈액ㆍ골수 독성을 갖는지의 여부를 평가해야 하는 것이다. 약물에 의해서 유발되는 혈액ㆍ골수 독성으로는, 예를 들어, 적혈구 감소증, 백혈구 (과립구) 감소증, 혈소판 감소증, 응고 이상, 골수 이상 증식, 백혈병, 3 계통 (적혈구계, 과립구계 및 거핵구계) 의 조혈 부전을 수반하는 범혈구 감소증 등을 들 수 있다. 혈액ㆍ골수 독성은, 조혈기 (주로 골수) 에 있어서의 조혈 장해와, 말초에 있어서의 혈구 파괴에 의해서도 초래된다. 조혈 장해는, 약물이 골수계 줄기 세포나 조혈 전구 세포에 작용하여 분화 증식을 억제함으로써 발생된다. 혈구 파괴는, 면역학적 기전에 의한 경우가 많고, 약제 흡착형, 면역 복합형 및 자기 면역형으로 분류되어 있다. 혈액ㆍ골수 독성을 발증할 가능성이 있는 약물은, 다양한 것이 공지되어 있어, 본 발명에 사용할 수 있다. 이리노테칸, 메토트렉사트 등은, 골수 (및 그것에 부수되는 혈액 세포) 독성을 야기하는 대표적인 약물이다. 또, 호중구 등의 감소를 초래하는 과립구 감소증 (약물 기인성 무과립구증) 의 원인이 되는 약물로는, 예를 들어, 아스피린, 술피린, 디클로페낙, 인도메타신 등의 해열 진통약 (비스테로이드계 소염약) ; 클로르프로마진, 레보메프로마진, 클로르디아제폭시드, 메프로바메이트, 클로자핀 등의 항정신병약 (항울약) ; 메틸티오우라실, 프로필티오우라실, 티아마졸 등의 항갑상선 호르몬약 ; 클로르탈리돈, 클로로티아지드, 에타크린산 등의 이뇨약 ; 페니토인, 트리메타디온, 카르바마제핀 등의 항경련약 (항간질약), 클로르프로파미드, 톨부타미드 등의 경구 혈당 강하약 ; 클로람페니콜, 페니실린, 티암페니콜, 스트렙토마이신, 트리메트프림ㆍ술파메톡사졸 등의 항생 물질ㆍ항균약 ; 파라아미노살리실산 (PAS), 이소니아지드 (INH) 등의 항결핵약 ; 시프로헵타딘, 클로르페니라민 등의 항히스타민약 ; 페닐부타존, 인도메타신, 금 (金) 제제 등의 항류머티즘약 ; 프로카인 아미드, 아지말린, 퀴니딘 등의 항부정맥약 ; 티클로피딘의 항혈소판약 ; 파모티딘, 란소프라졸 등의 소화성 궤양 치료약 ; 항암제, 페니실라민, 술폰아미드 (술파제) 등을 들 수 있다.
「약물」은, 위에 예시한 저분자 의약에 한정되지 않고, 항체 의약, 펩티드 의약, 핵산 의약 등, 의약품의 유효 성분으로서 사용할 수 있는 다양한 저분자 의약 이외의 약물이어도 된다.
「약물」은, 실제로 의약품으로서 시판되고 있는 약물에 한정되지 않고, 임상 시험 또는 비임상 시험에 있어서 사용되고 있는 약물이나, 그 전단층의 개발 도중의 약물 (의약품의 유효 성분의 후보 화합물 등) 이어도 된다.
<약물 첨가 공정>
본 발명의 약물 독성 평가 방법에 있어서의 제 1 공정인 약물 첨가 공정은, 오르가노이드와 혈액 세포의 공배양계에 약물을 첨가하는 공정이다.
약물 첨가 공정에 있어서, 오르가노이드를 배양하기 위한 배지는, 오르가노이드의 종류에 따른 적절한 배지를 선택하면 된다. 오르가노이드용의 배지는 공지되어 있고, 일반적으로는, 오르가노이드 작성용의 소정의 세포의 각각을 배양하기 위해서 사용한 배지를 혼합한 배지, 예를 들어, 장기ㆍ조직을 구성하는 세포용 배지, 간엽계 세포용 배지 및 혈관 내피 세포용 배지 (배지가 공통되어 있는 경우도 있다.) 의 혼합 배지를, 오르가노이드용 배지로 할 수 있다.
약물은, 상기와 같은 오르가노이드용 배지에, 약물의 독성을 평가하기에 적합한 원하는 농도로 첨가할 수 있다. 약물의 첨가 농도는, 공배양계 (오르가노이드, 혈액 세포 및 약물 각각의 종류나 비율, 배지의 조성 등), 독성 평가 공정에서 채용하는 평가 수법, 혹은 오르가노이드를 제작하기 위해서 사용한 세포가 유래하는 환자의 속성 (예를 들어 약물 독성에 관계되는 유전자) 등에 따라서 적절히 조절할 수 있다. 또, 종래의 각종 세포, 오르가노이드 등의 배양계를 사용한 평가 방법에 있어서의 약물의 첨가량을 참조하여, 본 발명에 있어서의「오르가노이드와 혈액 세포의 공배양계」를 사용한 평가 방법에 있어서의 약물의 첨가량을 조절 (예를 들어, 본 발명에 의한 평가 정밀도의 향상 등을 감안하여, 필요에 따라서 수치 범위 (상한치 및 하한치) 를 변경) 할 수도 있다. 예를 들어, 간장 오르가노이드를 사용하여 간장 독성을 평가하는 경우, 암피실린은 20 ㎎/mL 이하 (예를 들어 0 ∼ 20 ㎎/mL 의 범위에 포함되는 몇 점의 농도, 이하의 다른 약물의 수치 범위에 대해서도 동일), 아목시실린은 1.0 ㎎/mL 이하, 세팔렉신은 2.0 ㎎/mL 이하, 레보플록사신은 4.0 ㎎/mL 이하의 농도에서, 간장 오르가노이드용 배지에 첨가할 수 있다.
<독성 평가 공정>
본 발명의 약물 독성 평가 방법에 있어서의 제 2 공정인 독성 평가 공정은, 오르가노이드에 대한 약물의 독성을 평가하는 공정이다.
독성 평가 공정에 있어서의 평가의 수법이나 기준은, 사용한 오르가노이드, 혈액 세포 및 약물의 종류나, 약물 독성의 종류 등에 따라서 적절한 것을 채용할 수 있다. 약물 독성의 종류란, 예를 들어, 간장 독성이면, 간 세포 장해형, 혼합형, 담즙 정체형, 급성 간염 등을 가리킨다.
본 발명의 일 실시형태에 있어서, 독성 평가는, 약물 첨가 공정을 거친 세포 중, 시약 (예를 들어 요오드화 프로피듐 ; PI) 을 사용하여 사세포를 검출하고, 세포 중의 사세포의 비율 (세포사율) 을 구하여, 대조와 비교함으로써, 약물 독성의 유무나, 약물 독성의 세기를 평가할 수 있다. 약물 첨가 공정에 의한 세포사는, 간 세포 장해형의 간장 독성을 반영한 것이라고 할 수 있다. 예를 들어, 세포사율이 소정의 기준 이하이면, 시험한 약물은 환자에 대해서 약물 독성을 가질 가능성이 낮다 (저독성이다) 고 평가할 수 있다. 혹은, 예를 들어, 시험하는 약물에 대해서, 약물 독성을 발증한 환자에게 유래하는 오르가노이드를 사용했을 경우와, 약물 독성을 발증하지 않는 것이 확인되어 있는 대상 (정상인) 에서 유래하는 오르가노이드를 사용한 경우에서 세포사율의 결과를 비교하여, 통계학적인 유의차가 없을 경우에, 바람직하게는 어느 쪽의 세포사율도 소정의 기준치 이하인 경우에, 시험한 약물은 환자에 대해서 약물 독성을 가질 가능성이 낮다 (저독성이다) 고 평가할 수도 있다. 또한, 상기한 세포사율은, 예를 들어, 약물을 배지에 첨가하지 않고 행한 컨트롤에 있어서의 세포사율을 규준으로 하고, 그것에 대한 비율로서 환산한「세포사 증가율」로 치환해도 된다.
-바이오 마커의 추정 방법-
본 발명의 바이오 마커의 추정 방법은, 본 발명의 약물 독성 평가 방법 (약물 첨가 공정, 독성 평가 공정 등) 을, 복수의 오르가노이드의 검체에 대해서 행하고, 약물 독성이 일어난 (약물 독성을 나타내는 지표가 소정의 기준보다 높아, 약물 독성이 있다고 인정되는) 검체와, 일어나지 않은 (약물 독성을 나타내는 지표가 소정의 기준보다 낮아, 약물 독성이 있다고는 인정되지 않는) 검체의 배양 상청 중의 물질 (예, 사이토카인) 을 대비하여, 바이오 마커가 되는 물질 (예, 사이토카인) 을 추정하는 공정을 포함한다.
본 발명의 일 실시형태에 있어서, 대비되는 배양 상청 중의 물질, 즉 바이오 마커가 되는 물질은 사이토카인이다. 약물 독성 (예를 들어 간 장해) 의 작용 기전에 있어서, 면역계의 관여 (예를 들어 T 세포로부터의 염증성 사이토카인의 방출) 가 1 요인으로 되어 있을 가능성이 있다. 그러나, 약물 모두에 대해서 면역계가 관여하고 있는지의 여부가 해명되어 있는 것은 아니고, 면역계가 관여하고 있는 것은 알려져 있지 않지만 정말로 관여하고 있지 않다고 말할 수 있을지 불명확한 약물도 있다. 또, 면역계 이외의 원인이 작용 기전에 관여하고 있을 가능성도 있다. 본 발명의 약물 독성 평가 방법을 응용함으로써, 약물 독성에 있어서의 면역계의 관여나, 그 밖의 원인을 분석할 수 있게 된다.
예를 들어, 환자 유래의 오르가노이드처럼 시험된 약물에 의해서 세포 독성이 일어난 검체의 배양 상청 중에는, 오르가노이드로부터 방출된 사이토카인 (염증성 사이토카인 등) 이 어느 정도의 농도로 포함되어 있는 것을 생각할 수 있다. 따라서, 다양한 사이토카인에 대해서, 배양 상청 중의 농도를, 세포 독성이 일어난 검체 (환자 유래의 오르가노이드 등) 와, 세포 독성이 일어나지 않은 검체 (정상인 유래의 오르가노이드 등) 로 비교함으로써, 전자의 검체에 있어서, 후자보다 유의하게 농도가 높은 사이토카인의 유무를 검출할 수 있다. 세포 독성이 일어난 검체의 배양 상청 중의 농도가 유의하게 높은 사이토카인은, 약물 독성의 바이오 마커라고 추정할 수 있다.
이와 같은 바이오 마커의 추정 방법은, 배양 상청 중에 포함될 가능성이 있는 바이오 마커가 될 수 있는 것이면, 사이토카인 이외의 물질, 예를 들어, 사이토카인 이외의 단백질, miRNA 등의 핵산 분자나 그것을 포함하는 엑소솜, 대사 물질 (지질 메디에이터, 특히 아라키돈산 유래의 리폭신, ω3 지방산 (DHAㆍEPA) 유래의 레졸빈ㆍ프로텍틴 등) 등에 대해서 적용할 수도 있다. 면역계 이외의 원인이 약물 독성의 작용 기전에 관여하고 있을 가능성도 있고, 분석 목적에 따라서, 배양 상청 중의 대비해야 할 물질을 선택할 수 있다.
-약물의 스크리닝 방법-
본 발명의 약물의 스크리닝 방법은, 본 발명의 약물 독성 평가 방법 (약물 첨가 공정, 독성 평가 공정 등) 을, 약물 독성을 발증한 환자 유래의 오르가노이드를 사용하여, 복수의 약물에 대해서 행하고, 약물 독성 효과가 낮은 약물을 선택하는 공정을 포함한다.
「약물 독성 효과가 낮은 약물」이란, 보다 구체적으로는, 약물 독성 평가 방법에 있어서의 독성 평가 공정에 있어서, 독성 (을 나타내는 지표) 이 소정의 기준 이하이거나, 또는 대조와 유의차가 없는 등, 약물 독성은 인정되지 않는다고 평가된 약물을 가리킨다.
본 발명의 약물의 스크리닝 방법은, 예를 들어, 약물 독성을 발증한 환자에게 있어서 투여할 수 있는 약물 (시판되고 있는 의약품이어도 되고, 시험 단계의 약물이어도 된다) 을 판정하고, 치료 효과의 향상과 위험성의 저하를 양립시키기 위해서 활용할 수 있다. 예를 들어, 약물 독성 (간장 독성 등) 을 발증할 가능성이 있는 약제 (항균약 등) 에 대해서, 오르가노이드가 유래하는 환자에게 투여하는 후보가 되는 복수의 약물을 선택하고, 그것들에 대해서 본 발명의 약물 독성 평가 방법을 실시함으로써, 약물 독성 효과가 낮은, 요컨대 그 환자에게 투여할 수 있는 약물을 선택할 수 있다.
또한, 본 발명의 약물의 스크리닝 방법은, 약물 독성을 발증한 환자 유래의 오르가노이드를 복수 준비하고 (예를 들어, 서로 상이한, 간 독성에 관계되는 유전자를 갖는 것), 각 오르가노이드에 대해서 약물 독성 효과가 낮은 약물을 선택한 결과를 통합하여, 다양한 속성의 환자에 대해서 세포 독성을 일으킬 가능성이 낮은, 보다 저독성의 약물을 선택하기 위해서 사용할 수도 있다.
또, 오르가노이드는, 약물 독성을 발증할지의 여부가 불명확한 대상에서 유래하는 오르가노이드이어도 되고, 그 대상에게 투여했을 때에 약물 독성 효과가 낮은 약물을 확인하고, 그 중에서 적절한 약물을 선택한다는 용도도 가능하다.
-키트-
본 발명의 키트는, 적어도 오르가노이드와, 혈액 세포를 포함한다. 이와 같은 키트는, 상기 서술한 본 발명의 약물 독성 평가 방법 등을 실시하기 위해서 사용할 수 있다. 키트에 사용되는 오르가노이드 및 혈액 세포의 기술적 사항에 대해서는, 본 명세서에 있어서, 약물 독성 평가 방법 등과의 관계에서 기재한 것과 동일하다.
본 발명의 키트는, 오르가노이드로서, 약물 독성 환자 유래의 것을 적어도 포함하고, 대조로서 정상인 (약물 독성을 발증하지 않은 대상) 유래의 것을 추가로 포함하고 있어도 된다. 또, 약물 독성 환자 유래의 오르가노이드는, 1 종류여도 되고, 2 종류 이상 (예를 들어 약물 독성에 관계되는 유전자가 서로 상이한 것) 이어도 된다.
본 발명의 키트는, 바람직하게는 사세포 검출 시약을 추가로 포함한다. 사세포 검출 시약으로는, 예를 들어, 요오드화 프로피듐 (PI) 을 들 수 있다. 키트의 용도에 따라서, 특히 본 발명의 약물 독성 평가 방법에 있어서의 독성 평가 공정에서 채용되는 평가 수법이나, 본 발명의 바이오 마커의 추정 방법 또는 약물의 스크리닝 방법에서 채용되는 수법에 따라서, 세포 검출 시약 대신에, 또는 추가로, 다른 시약을 사용할 수도 있다.
-배양물-
본 발명의 배양물은, 오르가노이드와, 혈액 세포와, 약물을 포함한다. 이와 같은 배양물은, 본 발명의 약물 독성 평가 방법에 있어서의 약물 첨가 공정에 의해서 조제되는, 및 독성 평가 공정에 제공되어 있는 배양물에 상당한다. 배양물은, 디시 (샬레) 와 같은 배양 용기에 수용된 것이어도 되고, 플레이트 상에 형성된 복수의 웰에 수용된 것 (개개의 웰 내의 수용물, 또는 그와 같은 수용물의 집합체) 이어도 된다.
실시예
[실시예 1] 항균약에 의한 간장 오르가노이드 독성 시험
[실험 방법]
(1) 인간 혈관 내피 세포 (EC) 의 제작
인간 정상인 유래 iPS 세포 (1383D2 ; 쿄토 대학 iPS 연구소) 및 약제성 간 장해 환자 유래 iPS 세포 (16-24 ; 요코하마 시립 대학) 각각을, DMEM/F-12 (Gibco) (10 ml) 에 1 % B-27 Supplements (GIBCO), BMP4 (25 ng/ml) 및 CHIR99021 (8μM) 을 첨가한 배지 중, 5 % CO2, 37 ℃ 에서 3 일간 배양함으로써 중배엽계 세포를 유도하였다. 얻어진 중배엽계 전구 세포를 추가로, Stempro-34 SFM (Gibco) (10 ml) 에 VEGF (200 ng/ml) 및 Folskolin (2μM) 을 첨가한 배지 중, 5 % CO2, 37 ℃ 에서 7 일간 배양함으로써, CD31 양성, CD73 양성 및 CD144 양성의 인간 혈관 내피 세포 집단을 얻었다.
(2) 인간 간장 내배엽 세포 (HE) 의 제작
인간 정상인 유래 iPS 세포 (1383D2 ; 쿄토 대학 iPS 연구소) 및 약제성 간 장해 환자 유래 iPS 세포 (16-24 ; 요코하마 시립 대학) 각각을, 기초 배지 RPMI (후지 필름) (2 ml) 에 Wnt3a (50 ng/mL), 액티빈 A (100 ng/ml) 를 첨가한 배지 중, 5 % CO2, 37 ℃ 에서 5 일간 배양함으로써, 내배엽계 전구 세포를 유도하였다. 얻어진 내배엽계 전구 세포를, 상기 기초 배지에 1 % B27 Supplements (GIBCO) 및 FGF2 (10 ng/ml) 를 첨가한 배지 중, 5 % CO2, 37 ℃ 에서 추가로 5 일간 배양함으로써, AFP, ALB 및 HNF4α 가 양성인 인간 간장 내배엽 세포 집단을 얻었다.
(3) 인간 간엽계 세포 (MC) 의 제작
인간 정상인 유래 iPS 세포 (1383D2 ; 쿄토 대학 iPS 연구소) 및 약제성 간 장해 환자 유래 iPS 세포 (16-24 ; 요코하마 시립 대학) 각각을, 기초 혼합 배지 DMEM/F-12 (Gibco) (10 ml) 에 1 % B-27 Supplements (GIBCO), BMP4 (25 ng/ml) 및 CHIR99021 (8μM) 을 첨가한 배지 중, 5 % CO2, 37 ℃ 에서 3 일간 배양함으로써 중배엽계 전구 세포를 유도하였다. 얻어진 중배엽계 전구 세포를, 동 배지에 PDGFBB 및 액티빈 A 를 첨가하여, 5 % CO2, 37 ℃ 에서 추가로 3 일간 배양하였다. 배양 후의 세포를 회수하고, 새로운 젤라틴 코트 플레이트에 재파종하고, 상기 기초 혼합 배지에, bFGF 및 BMP4 를 첨가한 배지 중, 5 % CO2, 37 ℃ 에서 추가로 4 일간 배양함으로써, FOXF1, COL4A, ALCAM 및 CD73 이 양성인 인간 간엽계 세포 집단을 얻었다.
(4) 오르가노이드 (삼차원 구조체) 의 제작
제작된 인간 간장 내배엽 세포 (HE), 인간 혈관 내피 세포 (EC) 및 인간 간엽계 세포 (MC) 를 10 : 7 : 1 의 비율의 세포수 (총수 18 × 105 개) 로 혼합하고, 삼차원 배양 용기 Elplasia (쿠라레) 상에서 1 일간, 5 % CO2, 37 ℃ 에서 공배양함으로써 응집체를 제작하였다. 이 공배양에 있어서, 배양 배지로는, HCM (Lonza) 에 FBS (5 %), HGF (10 ng/ml), OSM (20 ng/ml) 및 Dex (100 nM) 를 첨가한 간장 세포용 배지 (A) 와, Stempro-34 SFM (Gibco) 에 VEGF (50 ng/ml) 및 FGF2 (10 ng/ml) 를 첨가한 혈관 내피 세포용 배지 (A) 를, 1 : 1 의 체적 비율로 혼합한 배지 (본 명세서 중「오르가노이드용 배지」라고 한다) 2 ml 를 사용하였다.
(5) 오르가노이드와 말초혈 단핵구 (PBMC) 의 공배양 및, 항균약 첨가
제작된 오르가노이드를 오르가노이드용 배지에 의해서, 5 % CO2, 37 ℃ 에서 추가로 14 ∼ 15 일간 배양하였다. 배양 후의 오르가노이드를 회수하고, 96 웰 U 바닥면 저흡착 배양 용기 (Corning, 스미토모 베이크라이트 등) 에 1 웰당 50 ∼ 70 개 파종하였다. 액체 질소 탱크에서 동결 보존되어 있는 복수인에게서 유래하는 PBMC (HEM) 를 융해하고, 적절한 농도의 항균약 (암피실린나트륨, 아목시실린 3 수화물, 세팔렉신, 레보플록사신 0.5 수화물) 을 각각 첨가한 오르가노이드용 배지에 현탁하였다. 얻어진 PBMC 현탁액을, 1 웰당 2 × 105 개의 PBMC 가 되도록, 오르가노이드가 파종되어 있는 웰에 첨가하고, 혼합하였다.
(6) 세포사 검출 시약 (요오드화 프로피듐) 을 사용한 항균약 독성 시험
항균약을 함유하는 배지 중에서의 오르가노이드 및 PBMC 의 공배양을 5 % CO2, 37 ℃ 에서 24 시간 행한 후, 요오드화 프로피듐 (PI, 도우진 화학 연구소) 을 배지에 첨가하고, 5 % CO2, 37 ℃ 에서 30 분 반응시켰다. 반응 종료 후, 배양 상청 100 μL 를 제거하고, PBS (-) 를 100 μL 첨가 후 1 분간 정치 (靜置) 함으로써 오르가노이드만을 침전시키고, 다시 상청 100 μL 를 제거하였다. 이 공정을 2 번 반복함으로써 PBMC 의 제거와 PI 용액의 세정을 행하였다. 다시 PBS (-) 를 100 μL 첨가 후, 형광 현미경 (KEYENCE BZ-X 시리즈 등) 을 사용하여 명시야 이미지 및 형광 이미지 (여기 파장 : 530 ㎚, 형광 파장 : 620 ㎚) 를 취득하였다. 취득된 화상을, 화상 해석 소프트 Fiji 를 사용하여 형광 강도와 오르가노이드 면적을 정량하고, 오르가노이드 면적당 PI 염색률을 산출하였다 (PI 염색률 = PI 형광 강도/오르가노이드 면적). 약제 비첨가시를 컨트롤로 한 PI 염색률의 배율을 산출함으로써, 사세포 증가율을 정량하였다.
[결과]
(1) PBMC 를 포함하는 본 시험계에 있어서, 약제성 간 장해 발증 환자주 (16-24) 유래 간 오르가노이드에서는 농도 의존적으로 암피실린에 의해서 세포사가 유의하게 증가하였다 (표 1).
(2) 복수의 항균약에 의한 독성 시험을 행한 결과, 독성을 나타내는 화합물 (암피실린, 레보플록사신 0.5 수화물) 과 독성을 나타내지 않는 화합물 (아목시실린 3 수화물, 세팔렉신) 로 분류되었다 (표 2).
Claims (7)
- 오르가노이드와 혈액 세포의 공배양계에, 약물을 첨가하는 공정, 및
당해 오르가노이드에 대한 당해 약물의 독성을 평가하는 공정을 포함하는, 약물 독성 평가 방법. - 제 1 항에 있어서,
상기 혈액 세포가 면역 담당 세포인, 약물 독성 평가 방법. - 제 1 항 또는 제 2 항에 기재된 약물 독성 평가 방법을, 복수의 오르가노이드의 검체에 대해서 행하고, 약물 독성이 일어난 검체와 일어나지 않은 검체의 배양 상청 중의 물질을 대비해서, 바이오 마커가 되는 물질을 추정하는 공정을 포함하는, 바이오 마커의 추정 방법.
- 제 1 항 또는 제 2 항에 기재된 약물 독성 평가 방법을, 약물 독성을 발증한 환자 유래의 오르가노이드를 사용하여, 복수의 약물에 대해서 행하고, 약물 독성 효과가 낮은 약물을 선택하는 공정을 포함하는, 약물의 스크리닝 방법.
- 약물 독성 평가 키트로서, 오르가노이드와, 혈액 세포를 포함하는, 키트.
- 제 5 항에 있어서,
사세포 검출 시약을 추가로 포함하는, 키트. - 오르가노이드와, 혈액 세포와, 약물을 포함하는, 배양물.
