보툴리늄 독소 또는 이의 염을 포함하는 자궁 내막의 혈류량 증진용 조성물에 관한 것이다.

실시예
1. 실험 방법
자궁내막 요인으로 인한 착상 및 임신 실패의 문제를 해결하기 위해서 보툴리늄 톡신 A(BoTA)를 자궁강내로 주입하여 이로 인한 자궁내막 혈관 형성의 증가를 확인하고자 하였다.
BoTA를 자궁내막 내 도포하는 형식으로 처리하였고, BoTA 처리 8일 후 혈관 신생 및 자궁내막 수용성 관련된 유전자들의 발현 정도를 판단하였다. 이를 위하여 쥐의 등쪽 부분을 절개하여 쥐의 양쪽 자궁 상단부를 드러나게 한 후 1cc 주사기를 이용하여 쥐의 한쪽 자궁에는 BoTA를 주입하고 다른 한쪽에는 대조군으로 Saline을 동일한 용량으로 주입하였다. 약물 주입 후 절개한 쥐의 등을 봉합 및 회복 후 8일 후에 양쪽 자궁 조직을 적출하여 추가 실험에 사용하였다. 이때, 모든 실험에 사용된 보툴리늄 톡신 A는 휴젤에서 무상 제공한 Botulax(botulinum toxin type A)이며, 약물 조성에 표기된 유닛 농도를 실험에도 동일하게 적용하였다. 사람의 피부에 적용 시 Saline 1cc에 보톡스 1유닛을 적용하는 것을 기준으로 하여, 본 실험에 사용된 BoTA 농도는 1cc에 0.5유닛, 1유닛, 2유닛을 적용하여 쥐의 자궁 강 내에 30ul의 용량을 주입하였다.
약물 주입 후 배아 착상에 대한 효능을 확인하기 위하여 BoTA/Saline 주입 후 7일 후 PMSG, 9일 후 hCG 주사 후, 10일 후 fertile male mouse와 mating, mating 후 12일째 되는 날 자궁 적출 후 착상된 배아의 수를 비교하여 BoTA의 착상에 대한 효능을 확인하였다.
2. 실험 결과
(1) 자궁강 내 BoTA 처리를 통한 혈관 신생 및 배아 착상 관련 유전자 발현의 변화
혈관 신생 및 배아 이식과 관련되어 차별되게 발현된 유전자들을 확인하기 위해, 대조군과 비교하여 BoTA 처리군에서 차별되게 발현된 573개의 유전자들을 유전자 온톨로지 별로 구분하였다. 이 중, 혈관 신생(GO:0001525, 표 1) 및 배아 이식(GO:0007566, 표 2)으로 분류하였다.

도 1(표 1 및 2)에 표기된 바와 같이, BoTA 처리 8일 후, 10개의 유의하게 차별적으로 발현된 유전자들을 혈관 신생 GO term으로 분류하였다. 상향 조절로 분류된 유전자들은 Ccl7, Cyr61, Itgb3, Foxc1, Clec14a, Hif3a, Gpx1 및 Cd34이었고, 하향 조절로 분류된 유전자들은 Ccbe1 및 Tgfbi이었다. Ccl7은 Hif-1a과의 상호작용을 통해 혈관 재형성에 필수적인 대식세포 모집에 참여하는 것으로 알려져 있다. Cyr61 및 Itgb3는 서로 상호작용을 통해 내피 세포의 접착력을 증가시킴으로써 혈관 신생을 조절한다. 뇌에서 혈관주위세포에 의해 발현되는 Foxc1는 뇌의 내피의 증식 및 혈관 재형성에 필요하다. 저산소증 유발 인자들은 저산소증-반응성 요소들의 전사 개시를 유도하는 혈관 신생에서 가장 중요한 유전자 중 하나이다. 글루타티온 과산화효소-1의 부족은 내피 전구 세포 기능 장애와 관련이 있으며, 이는 혈관 신생 조절 장애를 일으킨다. Cd34는 조혈 줄기 세포 마커로서 알려져 있고, 염증 및 혈관 형성에 필수적이다. BoTA 처리된 자궁에서 하향 조절된 Tgfbi는 FAS1 도메인과의 상호작용을 통해 항-혈관 신생 및 항-종양 효과를 갖는 것으로 알려져 있다. 배아 이식 GO term에서 4개의 유의한 유전자 리스트들이 분류되었다: Lif, Itgb3, Stc2, Stc1은 JAK/STAT 및 MAPK 신호전달의 활성과 혈관 신생의 증가를 통한 배아 이식에서 가장 중요한 요인 중 하나이다. 임신 동안 Stc1 및 Stc2 모두 RNA 수준이 자궁 상피에서 점진적으로 증가하며, 배아 이식 부위에서 Stc1 및 Stc2 유도의 증가가 유발된다고 보고된 바 있다. 그러나, 탈락막화에서는 STC-1은 전체 탈락막화 과정에서 일관되게 발현되는 반면, STC-2는 탈락막화의 말기에 발현이 감소되었다. 이러한 보고에 따르면, STC-1은 전체 탈락막화 과정에 관계되는 반면, STC-2는 1차 탈락막화에서 주로 관여될 수 있다는 것을 나타낸다.
최근 보고된 연구(참고자료: Identification of gene expression changes associated with uterine receptivity in mice, Jia-Peng He et al. Frontiers in Physiology, 2019)에서 임신 동안 인간과 쥐 모두에서 자궁 수용성과 관련된 유전자 발현의 변화를 확인하였다. 차별되게 발현된 총 115개의 유전자들이 인간과 쥐 모두에서 동시에 확인되었고, 비수용성 자궁과 비교하였을 때, 수용성 자궁에서 그 중 50개의 유전자들이 상향 조절되었고, 25개의 유전자들이 하향 조절되었다. BoTA 처리 8일 차의 우리의 DEG들 및 자궁 수용성을 나타내는 유전자들과 참고자료에서 보고된 115개의 DEG들 간의 상관 관계를 확인하기 위해, 두 연구 사이의 유전자 발현 패턴을 비교하였다. 인간 및 쥐 모두의 수용성 자궁에서 25개의 하향 조절된 유전자들 사이에서, 7개 유전자들이 우리 데이터에서 동시 하향 조절되었고, 2개의 유전자들이 반대의 패턴을 보였다(도 2). 다른 한 편으로, 인간과 쥐 모두의 수용성 자궁에서 50개의 상향 조절된 유전자들 사이에서, 대조군에 비해 BoTA 처리 군에서 9개의 유전자가 상향 조절되었고, 2개의 유전자가 하향 조절되었다(도 2). 이러한 데이터에 다르면, 자궁에 대한 BoTA 처리가 혈관 신생, 배아 이식 및 자궁 수용성에 긍정적인 조절이 있는 것으로 추론될 수 있다.
(2) 자궁강 내 BoTA 처리에 의한 혈관 형성의 증가 확인
혈관 신생은 자궁 내막 수용성 증가와 함께 자궁 내막 조직의 재생 과 강하게 관련이 있다.
도 3에 표기된 바와 같이, BoTA 처리된 파라핀 내장 자궁 내막과 대조군 부위에서 형광 면역 염색을 통한 혈관 형성을 위한 대리 마커인 CD31의 발현 정도를 비교하였을 때, BoTA 처리된 자궁 내막 상피 층에서 CD31 단백질의 발현 정도가 대조군과 비교하여 유의미하게 증가하였다.
(3) 자궁강 내 BoTA 처리에 의한 배아 착상 효능 증가 확인
도 4에 표기된 바와 같이, BoTA 처리된 자궁쪽에서 Saline 처리된 대조군과 비교하였을 때, 확연하게 다른 혈관 형성과 함께 착상된 배아의 수가 유의미하게 증가하였음을 확인하였다.

- 보툴리늄 독소(Botulinium toxin) 또는 이의 염을 포함하는 자궁 내막의 혈류량 증진용 조성물.
- 청구항 1에 있어서, 상기 보툴리늄 독소는 보툴리늄 독소 A인, 조성물.
- 청구항 1에 있어서, 상기 보툴리늄 독소 또는 이의 염의 조성물 내 농도는 0.1 내지 1 Unit인, 조성물.
- 청구항 1에 있어서, 상기 조성물은 Ccl7, Cyr61, Itgb3, Foxc1, Clec14a, Hif3a, Gpx1, Cd34, Lif, Itgb3, Stc2, Dll1, Cd160, Calca 및 Cd31으로 이루어진 군에서 선택되는 적어도 하나의 유전자의 발현을 증가시키는 것인, 조성물.
- 청구항 1에 있어서, 상기 조성물은 Ccbe1, Tgfbi, Stc1, Adra2b 및 Tbx4로 이루어진 군에서 선택되는 적어도 하나의 유전자의 발현을 감소시키는 것인, 조성물.
- 청구항 1에 있어서, 상기 조성물은 자궁 내막의 혈관 형성을 증가시켜 자궁 내막의 혈류량을 증가시키는 것인, 조성물.
- 보툴리늄 독소(Botulinium toxin) 또는 이의 염을 포함하는 착상능 증진용 조성물.
- 청구항 7에 있어서, 상기 보툴리늄 독소는 보툴리늄 독소 A인, 조성물.
- 청구항 7에 있어서, 상기 보툴리늄 독소 또는 이의 염의 조성물 내 농도는 0.1 내지 1 Unit인, 조성물.

- 청구항 10에 있어서, 상기 난임 또는 불임증은 자궁 내막의 손상, 자궁의 기능 저하, 습관성 유산, 원인 불명의 반복적인 착상 실패 및 얇은 자궁 내막으로 인한 착상 실패로 이루어진 군에서 선택되는 적어도 하나에 의한 것인, 약학적 조성물.
- 청구항 10 내지 15 중 어느 한 항의 약학적 조성물을 난임 또는 불임증을 보유한 개체에 투여하는 단계를 포함하는 불임 또는 난임증의 예방 또는 치료 방법.
