Abstract

Description
본 발명은 일반적으로 항-에이즈(AIDS) 제제로서, 다중 고리 화합물, 즉, 인게놀 및 라노스타-8,24-디엔-3-올 화합물 및 이들의 혼합물의 제약학적 용도와 관련이 있다.
후천성 면역 결핍 증후군(또한 에이즈라고 알려진)은 인간 면역결핍 바이러스(HIV)로 야기된 인간 면역계의 질병이다.
이 증후군은 면역계의 유효를 점진적으로 감소시키고 그리고 기회감염 및 종양에 취약한 개체로 만든다. HIV는 혈액, 정액, 질 분비물, 쿠퍼액, 및 모유와 같은, 바이러스를 포함한 체액을 갖는 점막 또는 혈류의 직접적인 접촉을 통해서 전염된다.
에이즈는 현재 전세계적인 유행병이다. 보건 기구에 따르면, 2007년에는 3320만명이 전세계적인 상기 질병을 가지고 살았고, 그리고 에이즈로 33만명의 어린이를 포함하여, 약 210만명이 목숨을 잃었다고 추정되었다.
모든 증후군에 흔히 있듯이, 에이즈의 병리 생리학은 복잡하다. HIV는 CD4+ T 세포(T 세포의 서브세트), 대식세포 및 수지상 세포와 같은 인간 면역계의 중요 기관을 주로 감염시키는 레트로바이러스(retrovirus)이다. HIV는 직접적으로 그리고 간접적으로 CD4+ T 세포를 파괴한다. T 림프구는 면역반응에 필수적이며, 이들 없이 신체는 감염에 맞서 싸우거나 암세포를 죽일 수 없다. 이는 면역계를 약화시키고 기회감염을 가능케 한다.

HIV 감염에 대한 현재 치료는 일반적으로, 항레트로바이러스 제제의 최소한 2가지 타입에 속하는 최소한 3가지 약물로 구성된 조합물 (또는 "칵테일")을 포함하는 매우 활성의 항레트로바이러스 요법으로 구성된다.

임의의 다른 것을 제외함이 없이, 적합한 인게놀 화합물은 3-(2,4,6-도데카트리에노일)-인게놀, 3-(2,4,6,8-테트라데카테트라노일)-인게놀, 이의 제약학적으로 허용되는 염, 이성체, 다형체, 용매 화합물 또는 수화물, 전구약물 또는 이의 대사산물 중 하나 이상이다.
인게놀 화합물은 예로서 대극과(Euphorbiaceae) 식물에서 또는 본 발명과 무관한 경로의 화학적 합성에 의해 얻어질 수 있다. 적합한 인게놀 화합물은, 예로서, 국제 특허 공보 WO 2007000618에 따라, 대극과 식물 라텍스의 극성 용매 추출물의 분획물로서 제공될 수도 있다.

바이러스 및 세포
말초 혈액의 단핵세포 (MCPB)가 Ficol-Hipaque 밀도의 기울기 방법 (Hystopaque, Chem Sigma. 세포 생존능력의 시험 (XTT 및 블루 트리판)
세포 생존능력에 대한 단리체의 효과가 XTT 방법(2,3-비스 [2-메톡시-4-니트로-5-설포페닐] 2H-테트라졸륨-카르복사닐라이드, 공급원: Sigma)에 의해 평가되었다. 미토콘드리아 활성도에 대한 화합물의 효과를 시험하기 위하여, 96 개 웰의 플레이트 (37°C, 5% C02)에서 정상 개체의 MCPB (2x105 개 세포/200μl/웰)는 3 내지 7일 동안 물질의 상이한 농도(0.5 μg/mL, 5 μg/mL, 20 μg/mL, 40 μg/mL, 80 μg/mL, 160 μg/mL, 320 μg/mL 및 640 μg/mL)의 영향을 받았다. 이 기간의 종결 시점에서, 세포 활성도는 50ml의 XTT 용액의 첨가에 의한 비색법(colorimetry)에 의해 결정되었고 그리고 상기 결과는 450nm에서 흡광도의 판독에 의해 분석되었다(Scudiero et al, 1988, Cancer Res. 48:4827-4833에 따라서).

MCPB

실시예 5 및 6에서 이용된 유포르비아 티루칼리(Euphorbia tirucalli) 추출물로부터 만든 추출물에 관한 설명: 응고물을 얻기 위해 물에 1:1로 희석된 신선한 라텍스에, 헥산(hexane)이 첨가된다. 불용성 물질은 물로 여러 번 세척되고 여과된다. 얻어진 물질은 부탄올 및 헥산의 혼합물에 제시되고 부탄올 내에 남아있는 더욱 극성의 물질은 이후 헥산으로부터 분리되고, 여과되고 그리고 증발에 의해 최종적으로 회수된다.

SI (선택성 지표) = CC50/EC50는 약물의 용도에서 안정성의 정도를 나타내고, 따라서 값이 높을수록, 물질의 유효성이 더 나은 것이다.
결과의 분석
상기 실시예의 결과는 본 발명의 화합물이 ELIZA P24 방법에 의해 평가된 말초혈액의 단핵세포 (MCPB)에서 HIV-1의 생성을 억제할 수 있다는 것을 보여준다.
유포르비아 라텍스 추출물의 약 30%의 잔여 화합물로부터 단리되지 않았기 때문에, 인게놀 및 라노스타-8,24-디엔-3-올의 혼합물에서 보이는 더 높은 독성은 예상하지 못한 것은 아니다.
하지만 본 발명의 화합물 및 이들의 혼합물이 제한된 독성으로, HIV의 생성을 저해하였다는 것을 명확하게 보일 수 있다.

- HIV 감염의 예방 또는 치료를 위한 제약학적 조성물의 생산을 위한 인게놀, 라노스타-8,24-디엔-3-올 및 이들의 혼합물에서 선택된 다중고리 화합물의 용도를 특징으로 하는 방법.
- 청구항 1에 있어서, 상기 인게놀 화합물은 3-(2,4,6-도데카트리에노일)-인게놀, 3-(2,4,6,8-테트라데카테트라노일)-인게놀, 이의 제약학적으로 허용되는 염, 이성체, 다형체, 용매 화합물 또는 수화물, 전구약물 또는 이의 대사산물 중 하나 이상인 것을 특징으로 하는 용도.
- 청구항 1에 있어서, 상기 라노스타-8,24-디엔-3-올은 유폴, 티루카롤 및 라노스테롤, 이의 제약학적으로 허용되는 염, 이성체, 결정체 및 다형체, 용매 화합물 및 수화물, 전구약물 또는 이의 대사산물 중 하나 이상인 것을 특징으로 하는 용도.
- 청구항 1에 있어서, 인게놀 및 라노스타-8,24-디엔-3-올의 혼합물은 1:100 내지 100:1, 특히 1:50 내지 50:1, 더욱 특히 1:10 내지 10:1, 더욱 특히 1:4 내지 4:1의 범위에 있는 것을 특징으로 하는 용도.
- HIV 감염의 치료를 위한 방법에 있어서, 상기 방법은 제약학적으로 허용되는 담체 내의 인게놀 화합물의 제약학적 유효량을 환자에게 투여하는 단계를 포함하는 것을 특징으로 하는 방법.

- 청구항 7에 있어서, 상기 라노스타-8,24-디엔-3-올은 유폴, 티루카롤 및 라노스테롤, 이의 제약학적으로 허용되는 염, 이성체, 결정체 및 다형체, 용매 화합물 및 수화물, 전구약물 또는 이의 대사산물 중 하나 이상인 것을 특징으로 하는 방법.
- HIV 감염의 치료를 위한 방법에 있어서, 상기 방법은 제약학적으로 허용된 담체 내의 인게놀 및 라노스타-8,24-디엔-3-올 화합물의 혼합물의 제약학적 유효량을 환자에게 투여하는 단계를 포함하는 것을 특징으로 하는 방법.
- 청구항 9에 있어서, 인게놀 및 라노스타-8,24-디엔-3-올 화합물의 한가지 상기 혼합물은 다음을 포함하는 것을 특징으로 하는 방법:
(A) 3-(2,4,6-도데카트리에노일)-인게놀, 3-(2,4,6,8-테트라데카테트라노일)-인게놀, 이의 제약학적으로 허용되는 염, 이성체, 결정체 및 다형체, 용매 화합물 및 수화물, 전구약물 또는 이의 대사산물 중 하나 이상;
(B) 유폴, 티루카롤 및 라노스테롤, 이의 제약학적으로 허용되는 염, 이성체, 결정체 및 다형체, 용매 화합물 및 수화물, 전구약물 또는 이의 대사산물 중 하나 이상; 및
A와 B 사이의 비율은 1:100 내지 100:1, 특히 1:50 내지 50:1, 더욱 특히 1:10 내지 10:1, 더욱 특히 1:4 내지 4:1의 범위에 있다. - 청구항 9에 있어서, 상기 혼합물은 무게에서 약 70% 유폴, 10% 티루카롤, 10% 3-(2,4,6-도데카트리에노일)-인게놀 및 10% 3-(2,4,6,8-테트라데카테트라노일)-인게놀을 포함하는 것을 특징으로 하는 방법.
- 청구항 5 내지 11 중 어느 한 항에 있어서, 하루에 한 번 이상 병든 환자에게 투여되는, 하나 이상의 인게놀, 라노스타-8,24-디엔-3-올 또는 이들의 혼합물의 상기 유효량은 환자 체중의 kg 당 0.001 내지 2000 mg인 것을 특징으로 하는 방법.
