Description

이마티닙 저항성 질환 치료 또는 진단용 조성물{Composition for diagnosing or treating imatinib resistant diseases}
도 1은 발현 프로화일에 따른 이마티닙 저항성 K562 서브라인들의 계통 크러스터링을 나타낸 사진이다. 다섯 샘플에서 톱 3,000 변이 전사체들을 계통 크러스터링 방법을 사용하여 분석하였다. 수상도(dendrogram)에서, 원래(parental) K562와 함께 저용량 저항성 서브라인들(KR200nM 및 KR400nM)은 고 용량 군 (KR600nM 및 KR800nM)들의 것과 구별되었다.

도 3은 이마티닙 저항성 프라이머리 CML에서 여섯 후보 유전자들의 발현변화를 나타내는 그림이다. 본 발명은 이마티닙 저항성 질환 진단 또는 치료용 조성물에 관한 발명이다.
만성 골수종 (CML)은 상호 전위를 가지는 필라델피아 (Ph) 염색체, t(9;22)(q34;q11)의 존재에 의하여 동정된 혈액 증식 질환이다. 이 전위는 구조적으로 활성화된 타이로신 카이네이즈 활성을 가지는 단백질 산물인 p210 및 p190의 BCR-ABL 융합 유전자를 야기한다. 키메릭 BCR-ABL 타이로신 카이네이즈의 암 잠재성은 조혈세포를 트랜스포밍하여 나타나고 CML에서 원인 유전적 변이로서 암시된다(Daley GQ, 등 Science 1990; 247: 824-830;Raitano AB, 등 Biochim Biophys Acta 1997; 1333: F201-F216;Era T, 등 Proc Natl Acad Sci U S A 2000; 97: 1737-1742;Kurzrock R, 등 N Engl J Med 1988; 319: 990-998). 융합 유전자, BCR-ABL, 및 그 관련 산물들이 CML의 발병을 나타내는 주된 후보군이라는 것 때문에 그들은 여러 진단 및 치료 전략의 특정 타겟이 되어 왔다.
이마티닙(imatinib mesylate, STI-571, 글리벡)은 만성 골수종(chronic myelogenous leukemia)를 치료하는 치료제로 사용되는 선택적인 BCR-ABL 타이로신 카이네이즈 저해제이다. 이마티닙 기초 단일 치료에서 이마티닙에 대한 임상 반응 속도는 만성 CML에서 약 80%이다. 그러나 초기 혈액 및 세포유전학적 반응 후의 재발이 가속기 또는 급성기에서 종종 관찰된다(O'Dwyer ME, 등 J Intern Med 2001; 250: 3-9;Druker BJ, 등 N Engl J Med 2001; 344: 1038-1042). 게다가 비록 심지어 이마티닙 기반 치료가 BCR-ABL 전사체의 레벨을 크게 저해하여도 분자 수준에서 완전한 분자적 경감이 드물다(Holtz MS, 등 Blood 2002; 99: 3792-3800;Graham SM, 등 Blood 2002; 99: 319-325). 이것은 BCR-ABL 타겟 치료가 저항성의 습득에 의하여 방해되고 그것은 이마티닙의 임상 효과를 크게 감소한다는 것을 나타낸다.

본 발명에 따른 이마티닙 저항성의 치료는 이마티닙 저항성을 나타내는 상기의 유전자 중 업레귤레이션을 나타내는 서열번호 1에서 서열번호 16 유전자는 다운시키고, 다운레귤레이션을 나타내는 서열번호 17에서 서열번호 27의 유전자는 업시켜서 수행할 수 있다.

c) 테스트 화합물의 부존재 하에서 상기 세포에서의 발현 수준과 테스트 화합물의 존재하에서 상기 유전자들의 발현 수준이 다르다면 테스트 화합물을 이마티 닙 내성 조절자 후보군이라고 동정하는 단계를 포함하는 테스트 화합물이 이마티닙 저항성을 조절하는 후보물질인지를 결정하는 방법을 제공한다.
이하 본 발명을 설명한다.
본 발명은 이마티닙에 저항성을 가지는 유전자 발현 형태를 동정하는 것이다. 일련의 네 저항성 K562 서브라인들을 다른 이마티닙 용량 (200, 400, 600, 및 800 nM)에 노출시켜서 얻었다. 이들 세포 주들을 마이크로어레이 기술을 이용하여 분석하였다. 저항성 서브라인을 통하여 발현 변화가 일반적이거나 용량 의존적인 유전자들의 전사체들을 생물학적으로 관련있는 후보군들로 동정하였다.이마티닙-저항성 표현형과 관련된 유전자 세트들 또한 세트 엔리치먼트 분석을 사용하여 동정하였다.
이하 비한정적인 실시예를 통하여 본 발명을 더욱 상세하게 설명한다.
실시예 1: 이마티닙 저항성 세포주의 확립
이마티닙에 원래 민감성인 적혈구성 백혈구 K562 세포주를 10% 피탈 보바인 시럼을 가지는 DMEM에서 유지하였다. 이마티닙 저항성은 매 2주 마다 50 nM 단계씩 이마티닙 농도를 증가시켜서 K562 세포에 반복노출시켜서 확립하였다. 이마티닙으로 배양 동안에 여러 이마티닙 농도들(200 nM, 400nM, 600nM, 및 800nM)에 노출된 일련의 네 저항성 서브라인들이 확립되었다. Shah NP, 등(Cancer Cell 2002; 2: 117-125)에 기재된 바와 같이 BCR-ABL 카이네이즈 도메인의 돌연변이를 스크리닝하였다. 또 돌연변이 분석에 사용된 동일 프라이머들로 실시간 RT-PCR을 사용하여 BCR-ABL 카 이네이즈의 발현 레벨을 조사하였다. 인간 GAPDH 유전자를 정량 RT-PCR (F: 5'GCGGGGCTCTCCAGAACATCAT-3'(서열번호 28) R:5'CCAGCCCCAGCGTCAAAGGTG-3'(서열번호 29)에 대한 내부 대조군으로 사용하였다.
실시예 2: RNA 제조 및 하이브리다이제이션
K562 및 이마티닙 저항성 서브라인의 전체 RNA를 제조업자의 지시에 따라 트리졸 (Invitrogen)로 분리하였다. 추출된 RNA의 양 및 질은 바이오어날라이져 2100 (Agilent Technologies)를 사용하여 분석하였다. Applied Biosystems Human Genome Survey 어레이를 다섯 세포주(원래 민감성 K562 및 4 저항성 서브라인들)의 발현 프로화일을 분석하는데 사용하였다. Applied Biosystems Human Genome Survey 마이크로어레이 V2.0는 29,098 인간 유전자들 세트를 나타내는 32,878 프로브들을 포함한다. Digoxigenin-UTP 표지된 cRNA가 생성되고 Applied Biosystems Chemiluminescent RT-IVT 레이블링 키트 V2.0를 사용하여 전체 5 ㎍ RNA로부터 선형적으로 증폭된다. 어레이 교잡, 케미루미네센스 검출, 이미지 획득 및 분석은 Applied Biosystems Chemiluminescence 검출 키트 및 Applied Biosystems 1700 Chemiluminescent 마이크로어레이 어나랄이져를 사용하여 제조업자의 지시에 따라 수행되었다. 간단하게 설명하면, 각 마이크로어레이를 블록킹 시약으로 1시간 동안 55℃에서 전-교잡하였다. 10㎍ 의 표지된 cRNA를 60℃ 에서 30분간 절편화 버퍼로 배양하여 100-400 베이스들로 절편화하고 내부 대조군 타겟(ICT, LIZ® 형광염료가 표지된 24-머 올리고)으로 혼합하고, 1.5ml 부피의 전 교잡된 마이크로어레이 상에서 55℃에서 16시간 교잡하였다. 교잡 후, 그 어레이들을 교잡 세척 버퍼와 케이무 리네센스 세척 버퍼로 세척하였다. 그 어레이들의 케미루미네센스 신호를 항-digoxigenin-알칼라인 포스페테이즈, 케미누리네센스 증진 용액 및 케미루미네센스 기질을 연속 처리하여 증폭하였다. 교잡 반응은 샘플 당 3회 실시하였다.
실시예 3: 데이타 가공
유전자 발현 분석에 대한 5초 노출 길이를 가지는 2 "short" 케미루미네센스 화상, 형태 발견 및 초점 표준화에 대한 2 형광 이미지 및 스펙트럼 크로스 토크 교정에 대한 2QC를 포함하는 고 해상도, 큰 포맷 CCD 카메라를 갖춘 1700 어날라이져를 사용하여 각 어레이 교잡의 이미지를 수집하였다. 이미지들은 자동-격자화되고 케미루미넨센스 신호들은 정량화되었다. 배경 공제 후에 스팟 강도 데이타들은 변이 안정화 및 표준화(VSN) 방법들을 사용하여 표준화하였다(Huber W, 등 Bioinformatics 2002; 18: S96-104).
실시예 4: 다르게 발현된 전사체의 결정
원래(parental) K562 세포주들을 포함한 다섯 세포주들에 대하여 29,098 유전자들의 발현 평균에 대한 원-웨이 ANOVA 분석을 수행하였다. 가장 차별적인 3000 전사체를 선택하여 통상 소프트웨어인 Cluster and Treeview (http://rana.lbl.gov/EisenSoftware.htm)를 사용하여 평균 링킹을 통하여 계통적 크러스터링을 수행하였다(Eisen MB, 등 Proc Natl Acad Sci U S A 1998; 95: 14863-14868). 저항성 서브라인들에서 전사체가 업- 또는 다운-레귤레이트되었는지를 결정하기 위하여 민감성 원래 대조군과 비교한 발현 변화를 상대적 차이 r.d.를 계산하여 다음과 같이 측정하였다:

는 각각 민감성 원래 세포주들의 평균과 표준편차를 나타낸다. 본 발명자들은 저항성 서브라인들에서 업- 또는 다운-레귤레이트된 전사체들을 선택하기 위하여 ± 4 r.d.의 설대적 컷오프를 사용하였다. 어레이 가공 및 계산은 마이크로소프트 엑셀 및 R-패키지 6.0(http://www.r-project.org/)을 사용하여 수행하였다.
실시예 5: 용량-의존적 발현 변화를 가지는 전사체들의 동정

실시예 6: 유전자 세트 강화(enrichment) 분석
기능적으로 관련된 유전자들을 공공 유전자 데이터베이스: GO (Gene Ontology), BioCarta (http://www.biocarta.com), KEGG (Kyoto Encyclopedia of Genes and Genomes), 및 GenMAPP (Gene MicroArray Pathway profiler)로부터 얻었다. ㄷ데뎅데이터세트내의 부합하는 유전자들을 확실한 유전자 주석인 RefSeq로 단일화하고 RefSeq 및 기능적 주석(예를 들어 GO 카테고리) 사이의 40493 매치를 준비하였다. 유전자-대-주석 매치(match)들에 따라 유전자들을 주석-특이적 유전자 세트들로 범주화하고,멤버가 공통 기능 주석을 가지는 3774 유전자 세트들을 처음에 수집하였다. 유전자 세트들은 일 군의 기능적으로 관련된 유전자들을 나타내고 유전자 세트들 사이의 유전자 오버랩을 허용하여 배제할 필요가 없었다. 그 후 본 발명자들은 유전자 강화의 계수적 분석(PAGE)를 사용하여 기능적으로 주석화된 유전자 세트에 대한 강화 분석을 수행하였다. 실시예 7:Real-time 정량적 PCR 어세이
이마티닙-저항성을 얻기 전 후의 Total RNA from blood samples of 6 CML 환자의 혈액 샘플로부터 전체 RNA(한국 백혈병 세포 및 유전자 은행으로부터 기증)를 6 후보 유전자들인; CASP4, TNFRF21, RAB11A, IGF1, LYN, 및 RHOA.에 대한 real-time QPCR 분석에 이용하였다. 환자 중 세 명은 BCR-ABL 돌연변이를 가지고 있었고, 다른 세 명은 돌연변이를 가지지 않았다. M-MLV Reverse Transcriptase (Invitrogen, USA)를 사용하여 첫번 가닥의 cDNA를 합성하여서 PCR에 사용하였다. Real-time QPCR은 소프트웨어 MxPro Version 3.00 (Stratagene, CA, USA)을 가지는 Mx3000P QPCR 시스템을 사용하여 수행하였다. 20 ㎕의 real-time QPCR 혼합물은 10ng의 cDNA, 1X SYBR®Green Tbr 폴리머레이즈 혼합물(FUNNZYMES, Finland), 0.5× ROX, 및 primers of 20 pmole의 프라이머를 포함한다. GAPDH를 각 과정에서 내부 대조군 으로 사용하였다. 열 주기는 다음과 같았다: 95℃에서 10 분 후에 40주기의 94℃에서 10초, 55 - 60℃에서 30초 및 72℃에서 30초를 수행하였다. 특정 증폭을 확인하기 위하여 융용 커브 분석을 수행하였다(55 - 95℃, 0.5℃/sec). 상대적 정량화를 ΔΔC T 방법(Livak KJ, 등. 상기 실시예의 결과는 다음과 같다.
여러 이마티닙 농도들(200 nM, 400nM, 600nM, 및 800nM)에 노출하여 K562세포로부터 4 이마티닙-저항성 서브라인들을 확립하였다. 이 서브라인들을 KR200nM, KR400nM, KR600nM, 및 KR800nM로 명명하였다. 원래(parental) 민감성 세포주를 K562로 명명하였다. K562 및 그것의 4 저항성 서브라인들을 Shah NP, 등(Cancer Cell 2002; 2: 117-125) 에 기재된 것과 같이 DNA 서열분석 및 정량적 RT-PCR를 사용하여 스크리닝하였다. 4 저항성 서브라인들에서 BCR-ABL 카이네이즈 도메인에서 공지된 핫 스팟 포인트 돌연변이나 BCR-ABL 유전자의 전사적 업-레귤레이션이 관찰되지 아니하였다.
K562 및 이마티닙-저항성 서브라인들의 발현 프로화일을 ~30000 인간 유전자들을 포함하는 올리고-마이크로어레이를 사용하여 조사하였다. 확보된 발현 프로화일들을 계통 크러스터링을 사용하여 분석하였다. 가장 변화가 있는 3000 전사체의 비통제(unsupervised) 크러스터링을 해당 샘플들을 확립하기 위하여 사용된 이마티닙 용량에 따라 다섯 샘플에서 결정하였다(도 1). 크러스터링에 따라 고 용량 군들(KR600nM 및 KR800nM)의 발현 프로화일이 저 용량 군(KR200nM 및 KR400nM) 및 K562의 것과 달랐다.
이마티닙-저항성에 관여하는 생물학적으로 관련된 후보군을 동정하기 위하여 K562 대조군과 비교하여 4 저항성 서브라인들에서 공통적으로 업- 또는 다운-레귤레이션된 전사체들을 먼저 조사하였다. 상대적 차이 지수 r.d.를 사용하여 K562 대조군과 비교하여 4 저항성 서브라인들에서 ±4 r.d. 컷오프를 통과한 27 전사체들 을 동정하였다. 이들 중 16 및 11 전사체들은 각각 저항성 서브라인들에서 공통적으로 과발현 및 저발현되었다(표 1).

표 1에서 r.d는 K562 대조군에 대한 해당 저항성 서브라인들에 대한 유전자의 발현 변화를 나타내고, up- 과 down-regulated 유전자들은 네 저항성 서브라인들에서 각각 +4 r.d 및 -4 r.d 컷오프를 통과할 때 결정하였다.
저항성 서브라인들에서 CASP4, TNFRSF21TNFRSF9와 같은 아폽토시스 관련 유전자들이 일반적으로 업-레귤레이트되었다. 전사 및 신호전달 관련 유전자들인 NOTCH2, SQSTM1TGIF, 또한 업-레귤레이트되었다. 또 세 업-레귤레이트된 유전자들인 UMPS, PIGCRRS1은 여러 대사 과정에서 효소 기능을 가진다.
저항성 서브라인들에서 다운-레귤레이트된 유전자들인 IGF1PLCD1 은 신호전달과 관련이 있다.
4 저항성 서브라인들에서 이마티닙 용량이 증가함에 따라 상관관계있는 발현 수준을 가지는 두번째 세트의 후보 유전자들을 조사하였다. 22 및 20 전사체들이 각각 이마티닙 용량이 증가함에 따라 직접 및 반대의 상관관계를 보였다(표 2).
표 2는 저항성 서브라인들에서 이마티닙 용량과 상관관계있는 발현 변화를 가지는 유전자들을 나타낸다.

샘플에서의 업- 및 다운-진행의 발현 경향은 두 유전자 세트들을 사용하여 나타내었다(도 2). 이마티닙 용량이 증가함에 따라 직접 상관관계를 보인 22 유전자들 중 4 전사-관련 유전자들 NMI, FOXO3A, ZNF226ZNF140을 동정하였다. NMI-코딩되는 단백질은 N-MYC, C-MYC와 같은 전사 인자들 및 신호 전달자 및 전사 활성 인자들과 상호작을을 한다(STAT). 또 다른 전사인자 FOXO3A는 이마티닙 유도 세포 주기 정지에서 관여하는 아폽토시스 효과를 가진다. ABCA8 또한 이마티닙 용량과 직접 상관관계를 보이는 발현 변화를 보이는 유전자들 중 하나인 것으로 동정되었다.
세포 증식-관련 유전자들은 이마티닙 용량과 반대 상관관계를 보이는 전사체들에서 동정되었다.
기능적으로 주석된(annotated) ~ 700 유전자 세트에 대한 강화의 통계적 유의성을 측정하여 이마티닙 저항성-관련 유전자 세트들을 동정하기 위하여 유전자 세트 강화 분석을 사용하였다. 활성화된 15 유전자 세트들 중, 4 유전자 세트들이 유전자 데이터베이스에서 전사, 전사의 DNA-의존 조절, 전사 코리프레서 활성 및 전사 인자 활성과 같은 전사와 기능적으로 관련이 있다. 두 독립적인 데이터베이스로부터 두 아폽토시스-관련 유전자 세트들인 GO 및 GenMAPP 또한 활성화된 것으로 범주화되었다.
생합성, 이동 및 폴딩과 같은 단백질 가공에 관여하는 유전자 세트들은 억제되고, 크렙스 회로 및 미토콘드리아 전자 전달에 관여하는 에너지 대사 관련 유전자 세트들도 또 억제된다. 또 GTP 결합, GTPase 활성 및 작은 GTPase-매개 신호-주석 유전자 세트들을 포함하는 GTPase 신호 전달-매개 세 유전자 세트들도 억제된다.
또 인 비트로 모델에서 이마티닙 저항성과 관련된 발현의 변화가 프라이머리 CML 케이스에서 상관관계가 있는지를 확인하기 위하여 이마티닙 저항성을 보이는 6 CML 환자를 분석하였다. BCR-ABL 돌연변이가 있는 환자(n = 3) 또는 없는 환자(n = 3)의 여섯 환자들에서 Log2 비율의 저항성 획득 전 후의 유전자 발현값이 계산되었다(도. 3). K562 대 KR800nM 발현 비율도 참고로 나타내었다. 여섯 환자들에서, 4 환자의 CASP4 및 세 환자의 TNFRSF21이 이마티닙 저항성 습득 후에 업-레귤레이트되었다. 그러나 발현 비율은 BCR-ABL 돌연변이 양성 및 음성 환자들 사이에 뚜렷한 차이가 없었다. 언더발현에서, RAB11AIGF1는 BCR-ABL 돌연변이 음성의 경우에 늘 다운-레귤레이트되었으나 셋 BCR-ABL 돌연변이 양성 케이스 중 둘에서는 업-레귤레이트되었다. LYNRHOA은 셋 BCR-ABL 돌연변이 음성 케이스 중 둘에서 언더발현이 관찰되었고, 셋 BCR-ABL 돌연변이 양성 케이스 중 둘에서 현저하게 업-레귤레이트되었다.
상기의 구성에서 알 수 있는 바와 같이 모든 이마티닙 저항성 서브라인들의 발현 변화에서 공통적이며, 용량 의존적인 변화의 특성을 모두 가지는 세 가지 업-레귤레이트된(VCX, TNFRSF21 및 TNFRSF9) 및 여섯 개의 다운-레귤레이트된(SSX6, ANTXR2, IGF1, ACSM3, CCNB1IP1 및 C1orf19) 유전자를 포함하는 아홉개의 유전자들은 이마티닙-저항성을 가지는 마커로 사용될 수 있다. 또 본 발명에서 얻어진 것은 이마티닙 저항성 질환에서 개선된 진단 및 치료 전략을 개발하는데 도움이 될 것이다.
