Abstract
심박출량을 증가시키도록 심장질환으로 고통을 겪는 환자들을 치료하는 자극발생기(25)와 신경자극방법이 개시된다. 이식된 자극발생기(25)를 이용해 환자의 미주신경을 등간격 펄스로 전기자극하거나 조절하고, 환자의 심장이 통상의 휴식중의 심박수 범위의 하한값보다 낮은 비교적 안정된 목표 심박수 범위에 도달할 때까지 자극펄스 주파수를 조절한다. 안정된 목표 심박수 범위를 생성하여 환자가 휴식을 취하면서 그 목표값을 유지하는 주파수로 자극펄스 주파수를 유지한다. 이식된 자극발생기(25)와 연결된 활동센서(44)는 환자의 신체활동을 검출하고 이에 따라 자극펄스 주파수를 조절하여 환자의 신체활동 주기동안 심박수를 상승시킨다. 환자가 활동을 중단하면, 미주신경 자극이 소정의 강하단계를 따라 목표 심박수를 재생하는 주파수까지 조절된다.
Description
심장질환이란 비정상적인 심장기능, 특히 혈액순환이나 인체에 필요한 산소공급을 심장이 충족시킬 수 없을 정도의 낮은 심박출량을 특징으로 하는 심장상태나 장애이다. 미국에서는 대략 이백만명 이상의 사람들이 심장질환을 앓고 있으며, 이 질환으로 건강문제를 겪고 있다. 이들 질환자의 일부는 비교적 증상이 가볍지만, 심장질환을 겪는 사람들은 신체적 지구력이 거의 없고 누워만 있을 수 있다. 심박수는 심근 산소소모의 주요 결정인자이다.
심장질환을 치료하는 방법으로는 환자의 심박수를 낮추는 것이 일반적이다. 심박수를 낮추면 긴 확장 기간중에 더 좋은 관상동맥, 특히 심내막을 통한 공급을 증가시키면서 산소소모량을 줄여 심장의 산소균형을 개선할 수 있다. 이 방식은 사람의 활동능력을 무리하게 낮은 레벨까지 감소시키지만 않으면 허용가능한 방식이다. 혈압과 주변 관류의 조절을 위한 심박출량을 통한 복잡한 조절시스템 연계 심박수 때문에, 장기간의 서맥(徐脈)을 가져오는 대부분의 치료는 직간접 신경계 및/또는 호르몬 체계를 통한 주변에 대한 추가 활동을 수반한다. 운동으로 발생하는 생리적인 서맥과, A-V 차단과 같은 병적인 서맥은 교감신경과 레닌안지오텐신계의 활동 및 개선된 카테콜라민(catecholamine) 방출과 관련이 있다. 이런 상황에 공통적인 장기간의 서맥의 한가지 영향으로 심근비대증이 생긴다.
예컨대, 베타 차단제, 캄슘통로 차단제 또는 선택적 서맥 약품의 섭취에 의해 서맥을 유발하는 약리적 방법으로는, 특히 이들 약품을 만성적으로 사용할 경우 원치않는 이온수축감소와 약품의 전신성 영향을 피하기가 어렵다. 그러나, 이런 부작용에도 불구하고, 베타차단제 요법은 부작용을 견딜 수 있는 환자들에게는 아주 유리하다. Carson의 최근 공개된 연구(Progress in Cardiovascular Diseases, Vol31, No4, 1999:pp301-322)의 보고에 의하면, 베타차단제로 치료받은 환자들은 비록 심박출량이 전혀 또는 거의 개선되지 않았음에도 그 사망율이 20% 감소되었다.
Cardiovascular Research 1994;28:pp.1774-1779의 "Long term bradycardia by electrical pacing: a new method for studying heart rate reduction"이란 브라운 일행의 기사에서는, 돼지의 심박수를 낮추는 조율을 이용한 이점에 대해 기재하고 있다. 이 연구에서는 심장의 심방과 심실 모두를 자극하는 방법을 이용하여 심박수를 약 130 bpm(beats per minute)에서 약 85 bpm까지 낮추었다. 실험에 사용된 동물들은 이런 낮은 심박수에서 약 6주간 견디었다. 부검에 의하면 심근모세혈관이 약 20%까지 증가했다. 심장중량이나 심근세포 크기 어느쪽을 기준으로도 서맥을 감소시키는 약리적 방법과 관련된 어떤 심근비대증의 증거도 발견되지 않았다.
J Cardiovas. Surg.56(1):71-82의 Bilgutay 일행의 "Vagal Tuning"이란 기사에서는 상실성부정맥, 협심증 및 심장질환의 치료를 위한 우측 미주신경 자극에 대한 개를 대상으로 한 실험이 기재되어 있다. 이 실험은 우측 미주신경 자극에 관련된 것이고 진폭 6-10 V, 주파수는 초당 10펄스, 펄스 기간은 0.2msec를 선택했다. 모든 실험에서, 관상혈류는 일정하게 유지되었다. 그러나, 심박수는 35-50% 감소되었다. 심박당 관상혈류의 증가율은 75-100%임이 발견되었다. 다른 실험에서, Bilgutay는 빈맥(頻脈)을 유도하기 위해 Isuprel을 투여하여 심박수를 170 bpm에서 240 bpm으로 올렸다. 다음, 미주신경을 자극하고 심박수를 240 bpm에서 120 bpm으로 낮췄다. 미주신경 자극으로 심실압과 심방압은 변하지 않았지만, 수축과 이완은 늘어났다. 이 결과는, 심장의 수축성이 빈맥 모델에서는 증가했지만, 심박수는 느리게 유지되어 박출량 증가로 인한 심장의 영향이 증가되었음을 보여준다. Bilgutay는 장애심장을 치료하는 잠재적 장점에 대해서는 언급했지만, 심장질환 관련 모델들에 대해서는 어떤 실험도 하지 않았고, 또한 모세혈관 성장의 증가에 대해서도 전혀 예상하지 않았다.
Journal of the Autonomic Nervous System, 68(1998), pp.78-88의 Feliciano 일행의 "Vagal nerve stimulation during muscarinic and beta-adrenergic blockage causes significant artery dilation"이란 기사는, 개를 대상으로 한 미주신경 자극실험에서 관상동맥이 상당히 확장되었고 관상동맥혈류 역시 상당히 증가되었음을 보여준다. 10, 15, 20, 30 ㎐의 주파수에서 자극이 이루어졌다. 이 연구에 의하면, 아트로핀과 프로프라놀올로 무스카린성 수용체와 베타-아드레날린성수용체를 차단했다. 심박수는 조율에 의해 정상치로 조절되었다. Feliciano는 아트로핀, 프로파라놀올 없는 정상상태에서의 동맥확장과 조율에 의한 심박수 안정을 보여주지 못했음은 물론 모세혈관 성장의 증가도 보여주지 못했다.
미주신경의 좌측가지의 자극에 의해 심방세동의 존재하에 심실박동수를 낮추는 것이 Geddes일행의 미국특허 5,690,681과 5,916,239에 기재되어 있다. '681특허에 기재된 바와 같이, 심방세동중에 심실박동수를 조절하는데 폐루프의 다양한 주파수의 미주신경 자극장치를 이용했다. 이 장치는, 감지상태에 반응해 자동으로 변하는 자극주파수로 미주신경을 자극하는 수단, 및 이 자극수단에 출력부가 연결되어 있는 제어기를 포함한다. 제어기는, 실제 심실박동수와 원하는 심실박동수 사이의 차이의 함수로서 미주신경 자극주파수를 자동으로 연속조절하는 수단을 포함한다.
'681 특허의 장치는, 미주신경 자극에 의해 심실박동수를 자동으로 조절하여 심방세동중의 맥박결손을 최소화한다고 할 수 있다. 동맥맥박수를 검출하고, 심실박동수와 동맥맥박수를 비교했다. 심실박동수와 동맥맥박수의 차이의 함수로서 미주신경 자극주파수를 자동으로 조절했다.
많은 특허에서 심실성 부정맥의 조절을 위한 여러가지 미주신경 자극법에 대해 기재하고 있다. Collins의 미국특허 5,203,326에는, 미주신경 자극과 조율의 조합에 반응하고 심장이상-약리학적인 높은 심박수-을 검출하여 심박수를 빈맥에서 정상치로 감소시키는 심박조율기가 기재되어 있다. Schwartz의 미국특허 5,330,507에는, 소정 임계특성의 빈맥을 넘는 심실박동수에 반응하여 좌측이나 우측 미주신경을 자극하는 것에 대해 기재되어 있다. Holmstrom의 유럽특허 688577A1에는, 상실성 부정맥의 검출에 반응해 목 부위의 부교감신경계를 자극하는 것에 대해 기재되어 있다. 미주신경은 부교감신경이다. Zabara의 미국특허 5,700,282에는, 미주신경 및 심근 교감신경의 동시자극과 부정맥 검출을 위해 심장을 감시하고 심장박동을 안정시키는 방법에 대해 기재하고 있다. Stroetmann의 미국특허 5,658,318에는 신경활동신호로부터 즉각적인 심장부정맥 상태를 검출하고 부정맥 요법을 시행하는 것에 대해 기재되어 있다. Ideker의 미국특허 5,522,854에는 부교감신경 활동에 대한 교감신경 활동의 비율을 검출하고 아주 위험한 부정맥을 검출했을 때 구심신경에 자극을 전달하는 것에 대해 기재되어 있다. 그러나, 이들 특허 어느것도 심박수를 심장잘환자의 정상적인 생리학적 범위 밑으로 강하시켜 모세혈관의 성장을 촉진시키고 관상혈류를 증가시키는 것에 대해서는 언급하고 있지 않다.
Taylor의 미국특허 5,913,876에는 환자의 심장 부근의 미주신경을 자극하여 심장을 잠시 정지시킨 뒤 관상동맥을 우회한 이식수술을 행하는 방법에 대해 기재되어 있다.
본 발명은 심장질환자의 신경자극에 관한 것으로, 구체적으로는 심근 모세혈관의 성장과 심박출량을 자극하거나 촉진하기 위한 뇌신경 자극에 관한 것이다.

본 발명은 심장질환으로 고통을 겪는 환자의 심박수를 예컨대 정상적인 심박수 범위의 하한보다 낮은 값까지 낮춰 관상 모세혈관 성장 및 관상혈류를 촉진 및 향상시키는 것에 관한 것이다. 본 발명의 방법은 '681 특허에서 설명한 방법이나 목적과 대조적으로, 병리학적인 급박한 심박수, 예컨대 심방세동 상태에서의 급박한 심실박동수를 미주신경 자극을 이용해 정상범위의 심박수로 줄이는 것에 주로관련되어 있다.
본 발명에 따르면, 이식된 신경자극장치나 신경자극기(본 명세서에서는 간단히 "장치"나 "자극기"라 한다)를 이용하고 소정의 제1 충격자극 주파수로 미주신경을 자극하여 심박수, 특히 심실박동수를 원하는 범위내의 심박수로 낮추도록 프래그램되어 있다. 미주신경 자극주파수는 심실박동수와 원하는 심실박동수 사이의 차이의 함수로서 자동으로 조절된다. 통상적인 초기 시퀀스에서, 이식된 장치는 초당 1회의 펄스(1 pps)로 미주신경을 자극하기 시작한다. 이 자극을 1분동안 이 주파수로 지속하여 미주신경 입력의 존재하에 심실박동수를 안정시킨다. 다음, 미주신경 자극수를 약 2 pps까지 증가시키고, 다른 안정주기동안 이 과정을 계속한다. 심실박동수를 더 감소시켜 궁극적으로 원하는 값에 도달할 때까지 미주신경 자극주파수를 계속 변화시킨다.
본 발명에 따르면, 목표 범위를 소정 목표값의 ±5% 내지 ±10%로 설정하고, 심실박동수가 목표값 범위에 도달함에 따라 미주신경 자극주파수의 증가량을 0.5 pps까지 또는 비례적으로 낮추는 것이 바람직하다. 심실박동수가 목표 범위 밑으로 떨어지면, 이 상태를 검출하고, 이때 결핍 정도에 따라 미주신경 자극주파수를 자동으로 낮추거나 미주신경 자극을 완전히 억제(중단)한다. 히스테리시스를 갖는 완충된 피드백 루프를 이용해 목표값이나 목표범위내로 심실박동수를 유지하기에 충분한 값으로 미주신경 자극주파수를 유지할 수 있다. 한편, 다른 형태의 조절기나 조절 메커니즘을 이용할 수도 있다.
다른 방법으로는, 사전설정된 감소된 심실박동수로 1시간동안 미주신경 자극주파수를 비례적으로 증가시킨 다음 환자의 정상적인 휴식 박동범위로 1시간 동안 미주신경 자극주파수를 비례적으로 증가시켜, 주기적으로 심실박동수를 낮추도록 자극기를 프로그램할 수도 있다. 이런 형태의 교호적인 낮은 심박수/정상 심박수 요법은 하강된 심박수를 견디기 어려운 환자들에게 유리할 수 있다. 또, ON/OFF 횟수(낮은 심박수/정상 심박수)를 수분에서 수시간이나 수일까지의 범위로 선택할 수도 있다.
본 발명의 다른 특징에 의하면, 자극기에 가속도계와 같은 활동센서를 연결하거나 내장하여 환자의 신체활동, 단순한 위치변동이나 산책 등을 검출하여, 미주신경 자극을 억제하거나 미주신경 자극주파수를 조절함으로써, 더 높은 심실 목표박동수를 생성할 수도 있다. 이런 방식으로, 환자는 신체활동이나 운동 기간중에 생리적으로 적절한 더 높은 심박수를 얻을 수 있는 이점이 있다. 환자가 활동센서에 의해 검출된 신체활동을 중단하면, 자극 파라미터들이 휴식중의 심박수 범위 밑의 낮은 심박수 수치로 되돌아간다. 심장질환을 겪는 환자를 위해, 이 값은 본 발명에 따른 낮은 심실박동수이다.
주치의가 허가하면, 환자의 신체활동상태에 따라, 즉 환자가 휴식중인지 또는 약간이라도 신체적 활동사태에 있는지의 여부에 따라 환자는 심실박동수와 심박수를 조절할 약간의 치료권한을 받을 수도 있다. 이 목적으로, 활동센서(예; 가속도계) 대신이나 그에 더하여, 환자가 조작하는 외부 자석에 의해 동작되는 리드스위치를 자극장치에 장착할 수 있다. 환자가 자극기의 이식부에 자석을 위치시키면, 스위치의 동작에 반응해 이식장치가 작동되는 소정 방식에 따라, 스위치는 (환자가약간의 신체활동을 하고자 하여 심박수를 증가시키기 위해) 미주신경 자극을 억제하거나, 또는 (환자가 휴식상태로 진입하고자 심박수를 낮추기 위해) 미주신경 자극을 개시하도록 동작한다.
자석 가동을 위해 자극장치에 다른 심박수 목표 및/또는 범위를 프로그램할 수도 있는데, 이 목표값이나 범위는 자극장치의 진행이나 예방 동작을 위해 선택된 심박수 목표값이나 범위와는 다르고, 활동센서 작동목표값과도 다르다. 또, 활동순서에 따라 환자가 시작한 다음 활동시 환자가 활동을 중단했을 때 생리적으로 더 적절한 심박수 감소를 위해 처음 환자가 시작한 활동에 의해 유도된 높아진 심박수로부터 강하된 심박수를 개시하도록 프로그램할 수도 있다.
본 발명의 특징에 따르면, 낮이나 밤 시간동안, 또는 환자의 24시간 주기의 리듬에 따라, 취침중이거나 졸고 있을 때는 깨어있을 때의 심박수보다 낮은 심박수를 인식하도록 다른 심박수 목표값이나 범위로 자극장치를 프로그램할 수도 있다.
따라서, 본 발명의 주목적은, 심장질환으로 고통을 받는 환자의 심박수를 생명을 유지하면서도 환자의 정상적인 심박수 범위의 하한보다 낮은 심박수까지 미주신경 자극에 의해 낮출 수 있는 간단한 방법과 장치를 제공하는데 있다. 이와 관련하여, 심근 모세혈관 성장을 증가시킨다는 관점에서, 관상혈류를 증가시키고 지속적인 시간주기에 걸쳐 심박출량을 증가시키도록 미주신경 자극을 통해 심박수를 감소시키는데 목적이 있다.
본 발명의 다른 목적은 정상적인 심박수 범위의 하한 밑의 소정 목표 심박수 범위내의 심박수까지 미주신경 자극에 의해 심장질환자의 심박수를 서서히 선택적으로 낮추고, 환자의 휴식이나 신체적인 비활동 기간중에 목표 심박수 범위내로 심박수를 유지하여 관상혈류를 증가시키는데 있다.
본 발명의 또다른 목적은, 환자의 신체활동을 감지하고 이에 따라 미주신경 자극을 억제하거나 변화시켜, 또는 환자 스스로의 억제에 의해 환자가 신체적으로 활동할 때 필요한 낮은 심박수의 조절을 위해 미주신경 자극을 통해 심장질환자의 심박수를 낮추는데 있다.
본 발명의 또다른 목적은, 심장질환자에게 안전하면서도 적절한 심박수로 신체활동 기간중의 심박수를 제한하는데 있다.
이하, 첨부 도면들을 참조하여 본 발명에 대해 자세히 설명하면 다음과 같다.
본 출원의 양수인에게 양도된 R.S.Terry 일행의 미국특허 5,154,172(본 명세서에 "'172 특허"로 인용)에 기재된 이식형 신경자극기(10)의 단순화된 버전이 도 1, 2에 도시되어 있는데, 본 발명에 따라 제공되는 일정 개선점들에 대해서는 후술한다.
신경자극기(10)의 자극발생기(25)는 대개 얇은 원형, 타원형 또는 장방형으로서 이식하기에 적당한 크기를 갖는다. 신경자극기는 피부 바로 밑에 외과적으로 형성된 포켓에 이식되지만, 반드시 환자(12)의 좌측쇄골부에 이식할 필요는 없다. 자극발생기(25)의 뒷면(또는 신경자극기(10)의 전기전도성 절연 리이드(16)가 미주신경에서 리이드의 전극 어레이(15)의 이식을 위해 연장되는 방향에 대해 이식환자의 선호도에 따라 앞면)이 본 실시예에서 쇄골근육에 닿는다. (당업계에서 캔이나 케이스로 불리우는) 발생기 하우징(14)은 티타늄이나 의료용 스테인리스강과 같은 생체적합물질로 이루어지고, 그 안에 있는 전자부품과 배터리(이하, "전자패키지"라 함)에 액체가 침투하지 않도록 밀폐되어 있다.
리이드(16) 기단부의 숫커넥터를 케이스(14)의 암커넥터에 삽입하여, 리이드(16) 말단부의 신경자극 전극어레이(15)를 자극발생기내 전자 패키지의 전기회로의 적절한 노드에 전기적으로 연결한다. 이 전극어레이는 Bullara의 미국특허 4,573,481에 기재된 형태의 쌍극 자극 전극조립체인 것이 바람직하다. 자극발생기(25)의 전기적 출력펄스 파형을 리이드-전극시스템을 통해, 도 1에 도시된 경부 등의 소정 위치에서 미주신경에 인가한다.
이식형 신경자극기는 환자의 신체 외부에 있는 프로그래머 및/또는 모니터(본 명세서에서는 때로 "프로그램 컨솔"이라고도 함)에 의한 원격측정에 의해 비동기 직렬통신 방식으로 환자의 신체와 통신하여, 신경자극기의 동작상태를 선택적으로 제어하고 검출한다. 이 목적으로 이용되는 종래의 외부 장치에는, 변수 변화를신경자극기(10)로 전송하고 모니터될 장치의 변수와 신호정보를 수신하는 프로그래밍 봉(18)이 있을 수 있다. 컴퓨터에 인스톨된 기존의 소프트웨어를 통해 의사는 이식된 장치와의 통신 및 선택된 변수의 제어조정이 가능하다.
도 2에는 이식된 신경자극장치(10)의 자극발생기(25)의 단순화된 블록도가 도시되어 있다. 자극발생기(25)는 리튬카본 모노플루오르화물 전지와 같은 배터리(32)를 포함하고, 이 배터리는 전압조절기(33)의 입력단에 연결되어 신경자극장치를 가동시킨다. 조절된 출력전압은 로직/컨트롤 부(35)와, 장치의 프로그램기능을 구현하고 제어하는 마이크로프로세서(36)를 포함한 기타 전자부로 공급된다. 프로그램 기능들은 전류나 전압의 크기, 주파수, 펄스폭, 및 자극발생기에 의해 생긴 출력펄스의 온/오프 시간을 포함하여 리이드 조립체는 물론 이식될 말단전극배열과 신경에 적용할 수 있다.
이 장치의 프로그램 기능으로 인해, 주치의가 출력펄스파형을 선택적으로 조절하여 미주신경의 전기적 활동을 조절함으로써, 전술한 치료요법을 제공할 수 있다. 자극발생기의 로직/제어 기능과 기타 기능의 타이밍은 수정발진기(37)의 정밀한 출력주파수 신호에 의해 제어된다. 의사는 환자가 수동으로 자극발생기를 작동시키고 그 출력펄스를 외부 자석(도시 안됨)에 의해 신경으로 전달하기를 원할 경우 자기작동식 리드스위치(39)를 설치한다. 이 스위치의 목적은, 전술한 바와 같이, 자극 주파수를 조절하여 환자가 물리적으로 활동할 때는 심장박동수를 증가시키거나, 그 활동이 충분한 기간동안 중단되었을 때는 심장박동수를 좀더 낮은 범위로 낮추는데 있다. 리드스위치는, 프로그램된 치료에 환자가 불편함을 느끼거나 기능장애를 느낄 경우 자극을 억제하는데 이용될 수도 있다.
출력펄스 파라미터를 설정하거나 변경하고 장치의 동작상태를 검출하는데 필요한 프로그래밍 신호들을 봉(18)을 통해 공급하기 위해(도 1 참조), 이식된 자극발생기와 외부 프로그램 컨솔 사이에서 양방향으로 원격측정 데이터를 통신하는데 사용하도록 빌트인 안테나(40)를 설치한다. 일단 자극발생기가 프로그램되면, 외부 프로그램 컨솔을 이용해 (주치의가) 프로그램된 세팅들을 다시 프로그램할 때까지 자극발생기는 미리 프로그램된 세팅으로 계속 동작한다.
로직/컨트롤 부(35)는 출력회로(42)를 제어하여 처방된 치료법에 맞는 출력펄스 파형을 생성한다. 자극발생기는 치료상 검출된 환자의 심실박동수를 목표치까지 올릴 필요성에 따라 지속적으로 작동될 수도, 산발적으로 작동될 수도 있고, 또는 최소한 어느한계까지는 외부 자석을 이용해 환자가 수동으로 작동하여 조절될 수도 있다. 프로그램된 출력파형은 자극발생기 케이스의 헤더에 있는 전기커넥터를 통해 리이드(16)와 신경자극 전극어레이(15)로 전달된다(도 1 참조). 이런 전기자극으로 인해, 전극들이 이식된 신경의 전기적 활동을 조절하여 환자의 심실박동수를 조절할 수 있다.
이식형 의료장치의 수동 및 자동작동에 관한 기술이 베이커 주니어 일행의 미국특허 5,304,206(이하 "'206 특허"라 한다)에 기재되어 있는바, 이 특허는 본 출원인의 양수인에게 양도되었다. '206 특허는 환자가 이상 징후를 느껴 장치를 수동으로 작동시키고자 할 때 문제가 있을 수 있음이 밝혀졌다. 이 특허에 따르면, 자극발생기 케이스 안쪽면에 장착되어 환자가 이식부를 가볍게 두드리는 것을 감지하기에 적합한 가속도계나 압전소자 등의 센서가 수동으로 자극발생기를 작동시키거나 중단시키는 수단에 구비되어 있다. 이렇게 하면, 환자는 일정한 제한은 있지만, 주치의가 결정한 한도내에서 장치의 작동을 조절하기에 편리하기는 하다. 그러나, 본 발명의 장치와 방법에서는, 환자의 심실박동수를 감지하여 미주신경의 자극 주파수를 자동으로 조절할 수 있다.
심부전증에 시달리는 환자를 위해, 심박수는 환자의 정상적인 휴식 박동수 범위의 하한선 밑의 박동수 범위내까지 감소된다. 이런 감소량은 환자의 정상적인 심박수보다 30-45% 낮은 심박수인 것이 바람직하다. 종래의 이식된 심박수 센서는 처방된 목표 심박수를 저장하는 비교기에 적용되어 목표 심박수를 넘는 심박수를 검출한다. 미주신경 전극에서 감지되는 심박수를 검출하는 기술이 애드킨 일행의 미국특허 5,928,272에 공개되어 있는바, 이 특허 역시 본 출원의 양수인에게 양도되었다. 이 기술은 미주신경에 자극성 전기펄스들을 전달하는 것으로 시작한다. 이식된 자극발생기는 최초 펄스 자극 주파수에서 미주신경을 자극하도록 프로그램되고, 환자의 심박수가 처방된 목표 심박수쪽으로 이동하기 시작할 때 특정 치료요법으로 들어도록 프로그램되어 있다. 이 경우, '272 특허에 기재된 바와 같이, 펄스발생기는 동일한 전극으로 감지와 자극을 동시에 할 수는 없다. 조율전극을 끄는 조치가 되었을 경우 펄스가 전달중인 동안 사건이 간과될 수도 있기 때문에 비동기 자극을 위한 다른 감지전극이 필요하다. 펄스발생기의 일부인 헤더에 별도의 감지용 전극을 설치할 수도 있다. 그렇지 않으면, 자극용 전극에서 떨어져 감지팁과 함께 별도의 리이드를 사용할 수도 있다. 필수적인 것은 아니지만, 감지팁은 심장 안이나 아주 가까이 위치할 수 있다. 또, 자극부에서 어느정도 떨어진 리이드 본체 부근에 위치한 링형 감지전극과 함께 감지팁을 하나의 리이드 본체로 병합할 수도 있다. 그러나, 자극발생기를 동기모드에만 사용할 경우, 별도의 감지전극이 불필요하고 신경자극 전극을 감지용으로도 사용할 수 있다.
게데스의 미국특허 5,916,239에 기재된 개를 대상으로 한 심박수대 좌측 미주신경 자극주파수의 그래프에 의하면 3.5㎐의 자극으로 심박수가 약 100bpm까지 제한되고, 6㎐에서는 60bpm까지, 10㎐에서는 약 35bmp까지 제한됨을 알 수 있다. '239 특허에 기재된 다른 알고리즘은 특히 심방 활동을 감지하는 동안 심실박동수를 조절한다.
본 발명의 다른 실시예에서는, 심방수축을 표시하는 P파의 존재를 감지하기 위한 증폭기가 추가된다. P파 신호진폭을 증폭하기 위해 심방내에 전극을 삽입한다. 심방 및/또는 심실 감지는 미주신경 자극률을 제어하는데 이용될 수 있다. 한편, 증폭기-전극 일체형 장치를 이용하여, P파와 R파를 식별하기 위한 신호분석기를 구비할 수도 있다.
이 경우, 미주신경의 심장가지를 자극하는 것이 중요한데, 이는 심장가지 밑의 목부위의 미주신경의 주가지의 자극은 심박수에 영향을 주지 않기 때문이다. 미주신경의 심장 목가지는 전극을 부착하기에 가장 편리한 위치이고, 이 목가지는 목부위에서 비교적 높은 미주신경의 메인 트럭에서 분기되어 목부분에서 전극을 부착하기에 충분히 긴 구간을 제공하기 때문이다. 좌측 미주신경의 자극이 바람직하지만, 우측 미주신경의 자극도 한 방법이 될 수 있다.
바람직한 요법에서는, 미주신경 자극주파수를 실제 심실박동수와 목표 박동수의 차이의 함수로서 자동으로 조절한다. 자극 초기에, 미주신경을 초당 1 펄스의 주파수로 자극하고, 이 자극주파수를 특정의 비교적 짧은 시간동안(약 1분간) 유지하여, 심실박동수를 새로운 값으로 안정시키도록 한다. 다음, 초당 2펄스의 값으로 미주신경 자극주파수를 상승시켜 심실박동수가 재차 안정될 때까지 유지한다. 심박수를 측정할 때마다 줄어들되 그 사이사이에서는 안정되도록 미주신경 자극주파수를 변화시키면서, 이런 요법을 현재 안정기간동안 내내 계속하여, 심실박동수를 궁극적으로 처방된 목표 박동수에 도달하게 한다.
심실박동수가 목표치에 접근함에 따라, 치료요법에 맞는 미주신경 자극주파수의 증가율을 프로그램하여 연속적인 안정 간격들 사이의 박동수를 초당 1펄스에서 초당 0.5펄스까지 감소시키는 것이 바람직하다. 한편, 제어기는 원하는 심실박동수와 실제 박동수 사이의 편차율을 취하여 상수로 곱한 다음 미주신경 자극율에 더하여 변형된 미주신경 자극율을 얻을 수도 있다. 심방-심실 동시성이 유지되는한, 미주신경 자극주파수를 증가시켜 심실박동수를 원하는 범위까지 낮춘다. 그러나, 심실박동수는 심방-심실 동시성을 상실하는 박동수 바로 위의 범위까지 감소된다. 심방-심실 박동수가 유지되는 최소 심실박동수는 외부 모니터링에 의해 결정될 수 있다. 자극발생기에 심방 감지전극과 증폭기가 있으면, 박동수 하락율을 자동으로 결정할 수 있다. 심실서맥(徐脈)에 대한 환자의 심박수를 기준심박수의 30-40% 범위내에서 또는 70bpm의 심박수에 대해 38-49 bpm까지 낮추면 관상혈관, 특히 심근 모세혈관의 성장이 촉진 및 향상되어 심장을 통한 관상혈류가 증가된다. 심근모세혈관의 증가와 관상혈류의 증가로 인해 여러주 또는 그 이상의 기간에 걸쳐 심박출량이 점차 개선되리라고 예상된다.
자극발생기에는 전기잡음으로 인해 환자의 심박수가 불시에 저하되는 것을 방지하는 안전장치가 있다. 첫째, 5㎐ 정도의 주파수 이상의 모든 신호는 잡음으로 보고 거부해야 한다. 둘째, 잡음이 있을 경우 환자를 심리적으로 안정시켜 환자의 심박수가 안전치 밑으로 떨어지지 않도록 하는 값을 설정하도록 최대 자극주파수를 프로그램해야 한다. 잡음검출 알고리즘을 이용하여, 잡음이 검출되면 미주신경자극을 자동으로 억제한다.
자극을 제공하는 다른 방법으로, 돌발파로 자극하는 방법이 있는데, 이 돌발파는 P파나 R파와 동기된다. 돌발파는 통상 R파 뒤로 150-200msec까지 이어진다. 이 방법의 장점은 자극 동작사이클이 낮기때문에 신경자극기의 배터리 사용빈도가 낮아진다는 것이다.
본 발명의 다른 치료법은, 이식된 신경자극기의 주파수 변동이 훨씬 작도록 프로그램하여, 환자의 심실박동수를 주기적으로 감소시키고 감소된 값으로 유지하는 것이다. 이를 위해, 미주신경 자극주파수를 1시간의 주기동안 중간 미주신경 박동수로 비례적으로 증가시킨다. 1시간 주기가 끝난 뒤 1시간 주기동안 미주신경 자극을 중단하거나 낮은 주파수로 조절하여 심박수를 정상 휴식 박동수로 되돌리고 적당한 운동을 지원하는 안전치까지 박동수를 올리되 안전치를 넘지 않도록 한다. 명목상으로(절대로 필요한 것은 아닌) 동일 또는 비슷한 시간의 충분히 늘어난 주기로 환자의 심박수를 내렸다가 정상 휴식 박동수로 되돌리는 이런 치료법은 환자가 심박수 감소로 어려움을 겪는 경우에 바람직하다.
심각한 서맥은 심부전증 환자의 심박출량을 낮춘다고 알려져 있는데, 이는 심박출량이 필연적으로 환자가 서맥 이전에 경험한 최대값 밑으로 떨어지기 때문이다. 심방운동량 역시 심부전 환자에 영향을 주고, 심박수 감소는 환자의 상태를 악화시킨다. 운동을 하면 모세혈관 성장과 관상혈류가 촉진되기 때문에 가능한한 운동을 하는 것이 좋지만, 운동을 할 경우 높은 심박수에서의 빈맥(頻脈)과 급사의 위험은 증가한다.
한편, 본 발명에 따라 실행된 치료법은 심부전증 환자에게 필요한 심근 모세혈관 성장을 촉진하고, 본 발명의 다른 특징에 따르면, 환자가 활동을 하면서 감지하는 주기동안 심실박동수의 감소-적어도 그 감소가 이식된 신경자극기로 인한 미주신경 자극에 의해 유도되는 견지에서는- 의도적으로 억제된다. 이런 감지는 가속도계(44)와 같은 통상의 활동이나 운동 감지기를 이식장치에 장착하면 가능하다(도 2 참조). 환자가 활동중이라는 것을 표시하는 가속도계의 출력은 생리적으로 적절한 심박수를 취할 수 있도록 미주신경 자극을 억제하거나 낮추는데 사용되고, 환자가 휴식중이라는 것을 표시할 때는 소정 목표범위까지 심박수를 낮추도록 자극을 증가시키는데 사용된다.
단순히 위치를 바꾸거나 천천히 걷는 것도 관련 활동 센서에 의해 감지되어 미주신경 자극을 억제시키거나 미주신경 자극주파수를 조절하여 근본적으로 심실박동수를 높일 수 있지만, 심박수 증가는 안전치내에서 제한된다. 따라서, 환자는 적어도 신체활동기간동안 생리적으로 적절한 높은 심박수를 얻을 수 있으면서도, 빈맥이나 급사를 가져올 수 있는 비정상적으로 높은 심박수로부터는 보호된다. 환자가 휴식상태로 돌아가면, 활동이 없음(또는 내적으로 심박수 강하)이 활동 센서에 의해 감지된다. 휴식 박동수 밑으로 심실 박동수를 다시 낮추는 주파수에서의 미주신경 자극은 환자의 활동이 명백히 중단될 때까지는 다시 시작되지 않는다.
또, 위에서 설명된 다른 방법에서, 이식장치가 미주신경의 자극을 위해 작동되어 심박수를 낮추고 정상 휴식값으로 심박수를 되돌리도록 억제되는(또는 자극이 충분히 감소되는) 시간(즉, 간격)은 수분에서 수시간 심지어 몇일까지의 기간으로 장치의 프로그램의 일부로서 주기적으로나 산발적으로 조절될 수 있다.
마찬가지로, 본 발명에 따라 시행된 각각의 치료법에서, 이식장치는 환자의 심실박동수가 처방 박동수 밑으로 떨어지는 상태를 감지하고, 처방 박동수로의 회복이 감지될 때까지 신경자극주파수를 자동으로 낮추거나 자극을 완전히 중단한다. 감쇄된 피드백은 자극률을 낮추거나 비례적으로 감소시키는 전술한 기술을 이용해 원하는 심박수를 원하는 범위내로 증가시키거나 감소시키는데 이용된다. 실제, 이런 감쇄는 신경자극주파수를 증가시켜 심박수를 낮추는데 이용되어야 하고, 심박수가 너무 낮아 심박수를 증가시키도록 결정될 경우 신경자극주파수를 감소시키는데는 이용되지 않는다.
심장전문의가 이식장치를 적절히 프로그램하여 환자 치료를 제한된 한도까지 조절할 수 있는바, 예를 들면 환자의 신체활동 유무의 인식에 있어서 환자가 이식장치를 잠깐 끄거나 신경자극 주파수를 제한된 범위내에서 조절하여 신경자극을 조절하도록 할 수 있다. 이것은 자극기의 가속도계를 대체할 수도 있고 가속도계와함께 사용될 수도 있다. 전술한 바와 같이, 환자 자신의 제어가 허용된다면, 이는 분명히 외부 자석을 통해 이루어지거나, 신체의 이식부위나 그 부근을 두드리거나, 당분야의 다른 적절한 기술로 이루어질 수 있다.
본 발명의 다른 특징에 의하면, 환자가 일하고 있지 않은 졸거나 잠자고 있는 동안에는 심박수가 낮은 것이 적절하기 때문에, 낮이나 밤이 아닌 하루종일 또는 24시간 주기의 리듬으로 여러 다른 목표 심박수로 신경자극을 겪고 있는 환자들이 장치를 프로그램하는 것이 바람직하다. 이런 시도는 이식장치의 전자적 동작을 위해 제공된 시계가 아닌 통상의 시계를 이용해서 부분적으로 달성된다.
이상 본 발명에 따른 심부전 환자의 치료장치와 치료법에 대해 예를 들어 설명했지만, 당업자라면 이상의 설명으로부터 본 발명의 정신과 범위를 벗어나지 않고 전술한 장치와 방법을 변형 내지 변경할 수 있을 것이다. 예컨대, 신경자극 주파수를 변경하여 심장질환을 겪고 있는 환자의 심박수를 원하는대로 조정하는 것이 바람직하지만, 신경자극전압이나 자극에 이용된 전력의 크기와 같은 기타 전기적 인자들의 값을 조절하거나, 본 발명의 기본원리에서 알 수 있는 다른 어떤 기술을 이용해 원하는 심박수를 얻을 수도 있는바, 본 발명은 첨부된 특허청구범위와 관련법의 규칙과 원리에 필요한 정도까지만 제한되어야 한다.

- 심박출량을 증가시켜 심부전증으로 고통받는 환자를 치료하기 위한 장치에 있어서:소정의 제1 주파수로 환자의 미주신경에 전기펄스를 인가하여 환자의 심박수를 낮추는 이식형 신경자극기;환자의 휴식을 위해 정상 심박수 범위보다 낮은 목표치까지 환자의 심박수를 낮추기에 충분한 조절된 주파수로 미주신경에 펄스를 인가하기 위한 프로그래머블 펄스주파수 조절기; 및상기 펄스주파수 조절기를 제어하여 환자가 정상적으로 휴식하고 있는 동안 상기 조절된 주파수로 펄스를 계속 인가함으로써, 환자의 심박수를 상기 목표치로 유지하기 위한 심박수 검출기;를 포함하는 것을 특징으로 하는 장치.
