Abstract
본 발명은 골 질환 예방 또는 치료용 약제학적 조성물에 관한 것으로, 본원발명의 약제학적 조성물을 이용하면, 세스트린2 (Sestrin2) 또는 이의 작용제 (agonist)가 파골세포의 분화 및 활성을 억제하여 골 질환을 예방, 개선 또는 치료할 수 있다.
Description
본 발명은 세스트린2 (Sestrin2) 또는 이의 작용제 (agonist)로 이루어진 군으로부터 선택되는 1종 이상을 유효성분으로 포함하는 골 질환 예방 또는 치료용 약제학적 조성물에 관한 것이다.
뼈(bone)는 인체의 연조직과 체중을 지탱해주고 내부기관을 둘러싸서 내부 장기를 외부의 충격으로부터 보호한다. 또한 근육이나 장기를 구조적으로 지탱할 뿐만 아니라 체내의 칼슘이나 다른 필수 무기질 즉 인이나 마그네슘과 같은 물질을 저장하는 인체의 중요한 부분 중 하나이다. 따라서 성장이 끝난 성인의 뼈는 멈추지 않고 죽는 날 까지 오래된 뼈는 제거하고 새로운 뼈로 대체하는 생성과 흡수 과정을 매우 역동적, 지속적으로 반복 재생 하면서 균형을 유지하게 된다. 이를 골재형성 (bone remodeling)이라고 한다. 오래된 뼈는 제거하고 새로운 뼈로 대체하는 뼈의 순환 (turnover)은 성장과 스트레스에 의해서 일어나는 뼈의 미세한 손상을 회복시키고 적절히 뼈의 기능을 유지하는데 필수적이다.
골 재형성에는 크게 두 종류의 세포가 관여하는 것으로 알려져 있다. 두 세포 중 하나는 뼈를 생성하는 조골세포 (osteoblast)이고, 다른 하나는 뼈를 파괴하는 파골세포 (osteoclast)이다. 조골세포는 RANKL (receptor activator of nuclear factor-κB ligand)와 이것의 유도 수용체 (decoy receptor)인 OPG (osteoprotegerin)를 생성한다. RANKL이 파골전구세포 (osteoclast progenitor cells) 표면에 있는 수용체인 RANK에 결합하면 파골전구세포가 파골세포로 성숙화 (maturation)되어 골흡수 (bone resorption)가 일어난다. 그러나 OPG가 RANKL과 결합하면 RANKL과 RANK간 결합이 차단되어 파골세포의 형성이 억제되고 필요 이상의 골 흡수가 일어나지 않게 된다.
파골속도와 조골속도가 동일해야만 전과 같은 골 밀도를 유지할 수 있으며, 골 재형성은 호르몬, 외부 영양, 유전적 측면 등에서 복합적으로 이루어진다. 이와 같이 중요한 뼈에 균형이 깨질 경우 많은 질병이 야기될 수 있는데 특히 골다공증, 골화석증 및 암세포의 골전이 (bone metastasis)로 인한 뼈의 손상과 관련된 질환이 대표적이다.
특히, 골다공증은 골 강도의 약화로 인해 골절의 위험성이 증가하는 가장 흔한 골격계 질환으로서, 최근 급속한 경제 성장 및 의학 발전으로 인해 고령화 사회로의 진입이 촉진됨으로써 나타나는 대표적인 질환 중 하나이다. 골다공증은 골 형성의 감소 및 골 흡수의 증가로 골 양의 전반적인 감소를 일으키는 질환으로, 구체적으로, 골 피질이 얇아지고 골소주 (Trabecula of bone)의 수량과 크기가 감소돼 골의 약화를 일으켜 골절의 위험이 증가되는 질환이다.
전세계적으로, 골다공증을 치료하기 위한 관련 연구 개발이 추진 중이며 골다공증 치료를 위한 효과적인 치료제 개발이 절실한 실정이다. 현재 대부분의 골다공증 치료 방법은 파골세포의 흡수능을 억제함으로써 골대사의 균형을 맞추려고 시도하고 있으나, 임상 효과가 미미하여 새로운 개념의 약물 개발 필요성이 대두되고 있다. 따라서, 골다공증과 같은 골 질환 치료를 위해 약제의 부작용을 낮추고 효능이 향상된 치료제를 개발하기 위해 안정적인 생물학적 제제 등과 같이 치료 효과가 높은 새로운 골 질환 치료제의 개발이 시급하다.
이에, 본 발명자들은 파골세포의 분화 및 활성을 억제하여, 골다공증과 같은 골 질환을 예방 및 치료할 수 있는 조성물을 개발하기 위해 노력하였고, 그 결과, 세스트린2 (Sestrin2) 또는 이의 작용제 (agonist)를 유효 성분으로 포함하는 약제학적 조성물은 우수한 파골세포의 분화 및 활성 억제 효과를 나타냄을 확인하였다.
이에, 본 발명의 목적은 Sestrin2 또는 이의 작용제를 유효 성분으로 포함하는 골 질환 예방 또는 치료용 약제학적 조성물을 제공하는 것이다.
본 발명은 세스트린2 (Sestrin2) 또는 이의 작용제 (agonist)를 유효 성분으로 포함하는 약제학적 조성물에 관한 것으로, 본 발명의 약제학적 조성물은 골 질환의 예방 또는 치료 용도로 사용될 수 있다.
이하, 본 발명을 더욱 자세히 설명하고자 한다.
본 발명의 일 예는 Sestrin2 및 이의 작용제로 이루어진 군으로부터 선택되는 1종 이상을 유효 성분으로 포함하는 골 질환 예방 또는 치료용 약제학적 조성물에 관한 것이다.
본 발명에 있어서 Sestrin2는 스트레스 유도성 대사 단백질로서, 다양한 유해 자극으로부터 세포를 보호하는 것일 수 있다.
본 발명에 있어서 Sestrin2는 파골세포의 분화를 억제하는 것일 수 있다.

본 발명에 있어서 작용제 (agonist)는 세포내 수용체와 결합하여 수용체를 활성화시켜 생물학적인 반응을 유도하는 물질로 신경전달 물질이나 호르몬 등과 같은 기능을 보여주는 작용물질 또는 작용약제일 수 있다. 작용제는 생체 내에서 작동하는 생체물질인 리간드(ligand)와 유사하며 분자 선택적이며 표적분자에 결합하는 성질을 보이는 것일 수 있다.
본 발명에 있어서 작용제는 Sestrin2의 발현을 증가시키는 것일 수 있다.
본 발명에 있어서 작용제는 파골세포의 활성을 억제하는 것일 수 있다.
본 발명에 있어서 작용제는 마크로라이드계 항생제일 수 있다.
마크로라이드계 항생제는 인간의 리보솜과 구조적으로 다른 세균의 리보솜에 선택적으로 작용함으로써, 세균의 단백질 합성을 저해하고 증식을 억제하여 항균작용을 나타내는 항생제를 의미할 수 있다.
본 발명에 있어서 마크로라이드계 항생제는 아지트로마이신 (Azithromycin), 에리스로마이신 (erythromycin), 올렌도마이신 (oleandomycin), 키타사마이신 (kitasamycin), 요사마이신 (josamycin), 미데카마이신 (midecamycin), 스피라마이신 (spiramycin), 카보마이신 (carbomycin), 암포마이신 (amphomycin), 블레운소마이신 (bleunsomycin), 뉴트라마이신 (neutramycin), 렐로마이신 (relomycin), 리프아미드 (rifamide) 및 클라리스로마이신 (clarithromycin)으로 이루어진 군으로부터 선택되는 1종 이상일 수 있으며, 예를 들어, 아지트로마이신일 수 있으나, 이에 한정되는 것은 아니다.

본 발명에 있어서 파골세포 (osteoclast)는 뼈를 흡수하는 세포로서, 골수의 조혈모세포 (hematopoietic stem cell; HSC)에서 유래된 단핵구/대식세포 (monocyte/macrophage)가 RANKL (receptor activator of nuclear factor kappa-Βligand)을 포함한 여러 인자에 의해 분화되어 형성되는 것일 수 있다.
이와 달리, 조골세포 (osteoblast)는 뼈를 합성하는 세포로서, 그 유래는 중간엽 줄기세포 mesenchymal stem cell)에서 분화되어 형성되는 것일 수 있다. 조골세포는 파골세포와 상호작용을 통해 골의 형성 및 유지시킬 수 있다.
본 발명에 있어서 골 질환은 골다공증 (osteoporosis), 골연화증 (osteomalacia), 구루병 (rickets), 섬유성 골염 (osteitis fibrosa), 파제트 병 (Paget's disease), 무형성 골 질환 (adynamic bone disease), 신부전 환자의 신성골이영양증 (renal osteodystrophy), 류머티스성 골 질환 (rheumatoid arthritis), 뼈전이성 병소 (bone metastatic lesion) 및 대사성 골 질환 (metabolic bone disease)으로 이루어진 군으로부터 선택되는 1종 이상일 수 있으며, 예를 들어, 골다공증일 수 있으나, 이에 한정되는 것은 아니다.
본 명세서상의 용어 “골다공증 (osteoporosis)”은 뼈의 양이 감소하고 질적인 변화로 인해 뼈의 강도가 약해져서 골절이 일어날 가능성이 높은 상태에 있는 골격계 질환을 의미할 수 있다.

본 발명의 약제학적 조성물을 대상에게 투여하는 데에 다양한 기술들이 존재하는데, 통상의 방법에 따라 산제, 과립제, 정제, 캡슐제, 현탁액, 에멀젼, 시럽, 에어로졸 등의 경구형 제형, 외용제, 좌제 및 멸균 주사용액의 형태로 제형화하여 사용될 수 있으나, 이에 한정되는 것은 아니고, 만약 제제화할 경우에는 보통 사용하는 충진제, 증량제, 결합제, 습윤제, 붕해제, 계면활성제 등의 희석제 또는 부형제를 사용하여 조제될 수도 있다.
본 발명에 있어서 약제학적 조성물은 경구 투여, 정맥 내 투여, 피하 투여 또는 복강 내 투여용일 수 있으며, 예를 들어, 복강 내 투여용일 수 있으나, 이에 한정되는 것은 아니다.
본 발명은 골 질환 예방 또는 치료용 약제학적 조성물에 관한 것으로, 본원발명의 약제학적 조성물을 이용하면, 세스트린2 (Sestrin2) 또는 이의 작용제 (agonist)가 파골세포의 분화 및 활성을 억제하여 파골속도를 감소시킴으로써, 골 질환을 예방, 개선 또는 치료할 수 있다.

이하, 실시예를 통하여 본 발명을 더욱 상세히 설명하고자 한다. 이들 실시예는 오로지 본 발명을 보다 구체적으로 설명하기 위한 것으로, 본 발명의 요지에 따라 본 발명의 범위가 이들 실시예에 의해 제한되지 않는다는 것은 당 업계에서 통상의 지식을 가진 자에 있어서 자명할 것이다.
1. 실험개요
1-1. 파골세포 배양 및 TRAP assay
6주령의 ICR 마우스의 대퇴골 및 경골에서 골수성 전구세포를 분리하여 10% 소태아혈청 (Fetal Bovine Serum; FBS)을 함유한 a-MEM (Minimum Essential Medium alpha)에 M-CSF (Macrophage colony stimulating factor) 및 20 내지 120 ng/ml의 RANKL (Receptor activator of nuclear factor kappa-B ligand) 존재하에 3일 동안 37℃, 5% CO2에서 배양하였다.
파골세포의 분화를 확인하기 위해 배양된 세포를 10% 포르말린으로 고정하고 TRAP (Tartrate-Resistant acid phosphatase)으로 15분 이상 염색하였다. N plan 10X 0.25 개구수 대물렌즈 (라이카 마이크로시스템즈, Wetzlar, 독일)가 장착된 DMIRB (라이카, Leica) 현미경을 사용하여 세포를 관찰하고, ProgRes CFscan (예놉틱, Jena, 독일) 카메라와 ProgRes Capture Pro (예놉틱, Jenoptik)를 사용하여 이미지를 얻었다. 3개 이상의 핵을 포함하는 TRAP 양성 다핵 파골세포 (TRAP(+) MNCs)를 세어서 파골세포 분화 정도를 평가하였다.
TRAP activity를 측정하기 위해 세포를 1X 인산완충생리식염수 (Phosphate-buffered saline; PBS)로 세척하고 1% Triton X-100/Na-Aceate로 용해하였다. 세포 용해물에 파라-니트로페닐인산염 (p-nitrophenyl phosphate; pNPP)이 포함된 기질 (substrate) 용액을 첨가하고 37℃, 15분 후 1M 수산화나트륨 (Sodium hydroxide; NaOH)로 반응을 정지하고, 405 nm로 측정하여 파골세포 분화 활성도를 평가하였다.
1-2. 골흡수 (Resorption) 분석
파골전구세포를 osteo assay plates (코닝, Corning)에 4일 동안 M-CSF 및 RANKL을 포함하여 배양하였다. DMIRB 현미경 (라이카, Leica)을 사용하여 골흡수 자국을 세어서 골흡수 정도를 평가를 하고 ProgRes CFscan (예놉틱, Jena, 독일) 카메라와 ProgRes Capture Pro (예놉틱, Jenoptik)를 사용하여 이미지를 얻었다.
1-3. 플라스미드 제작 및 레트로바이러스 유전자 형질도입
Sestrin2 (NM_144907)는 파골전구세포의 RNA를 사용하여 RT-PCR로 생성되었다. 증폭된 Sestrin2를 레트로바이러스 벡터인 pMX-IRES-EGFP에 서브클로닝하였다. 증폭에 이용된 프라이머 서열은 표 1의 서열번호 1 및 2에 나타내었다.

패키징 세포주인 Plat-E 세포는 10% FBS를 함유한 DMEM (Dubecco's modified Eagle's medium)에서 유지되었다. 레트로바이러스 상층액을 제조하기 위해 재조합된 유전자를 Plat-E에 Fugene6를 이용하여 형질도입 (transfection)하고, 48시간 후 배양 배지로부터 바이러스 상층액을 획득하여 파골전구세포에 10 ug/ml 폴리브렌 (polybrene)의 존재 하에서 6시간 동안 배양하였다.
1-4. siRNA 형질도입 및 정량적 PCR
si-Control (D-001810-01)과 si-Sestrin2 (L-052642-01)는 다마콘 (Dharmacon)에서 구입하였다. Lipofectamine RNAiMAX를 사용하여 파골전구세포에 형질도입하고, 넉다운 수준은 정량적 PCR (quantitative PCR)로 확인했다. 사용된 siRNA (small interfering RNA)의 염기서열은 표 2의 서열번호 3 내지 10에 나타내었다.

RNA 분리는 퀴아졸 (Qiazol)로 세포를 용해함으로써 수행하였다. 추출된 RNA는 Gostript 역전사효소 (Reverse Transciptase)를 이용하여 역전사되고, 생성된 cDNA는 SYBR에 기초하는 실시간 PCR (Realtime PCR)을 수행하여 각 유전자의 mRNA 발현 수준을 확인하였다. PCR 조건은 95℃에서 15분 후, 94℃에서 15초, 55℃에서 30초 및 72℃에서 30초로 진행되고, 총 40사이클로 진행하였으며 글리세르알데이드-3-인산탈수소효소 (Glyceraldehyde-3-Phosphate Dehydrogenase; GAPDH)를 이용하여 각 표적유전자에 대한 상대적 정량값을 표준화하였다.

1-5. 세포의 활성산소종 생성 분석
파골전구세포에 RANKL, 및 아지트로마이신 (Azithromycin; AZM) 20 ng/ml 또는 40 ng/ml을 6시간 처리한 후, 배양액을 제거하고 세척한 후 디클로로플루오르세인 디아세테이트 (Dichlorofluorescein diacetate; DCFDA)로 세포를 37℃에서 45분 동안 염색하였다. Leica DMIRB 형광 현미경을 사용하여, DCFDA 염색된 파골전구세포의 활성산소종 (Reactive oxygen species; ROS)의 생성을 형광으로 확인하고 ProgRes CFscan (예놉틱, Jena, 독일) 카메라와 ProgRes Capture Pro (예놉틱, Jenoptik)를 사용하여 이미지를 얻었다. 디클로로플루오르세인 (Dichlorofluorescein; DCF)은 Ex/Em = 485/535nm에서 형광측정기로 측정하여 평가하였다.
1-6. 골다공증 동물 모델 실험
7주령 ICR 마우스의 복강내 주사법으로 PBS 또는 RANKL 300 ug와, 다이메틸 설폭사이드 (Dimethyl Sulfoxide; DMSO) 또는 아지트로마이신 (Azithromycin) (15mg/kg/mouse)을 혼합하여 24시간 간격으로 3번 주사하였다.
4일째 대퇴골과 경골을 분리하여 골밀도 및 조직학적 분석을 수행하였다. 고해상도 Skyscan1172로 uCT 촬영하고 CT-Analyser (CTAn) 소프트웨어로 얻은 소주골 (trabecular bone)의 뼈 파라미터를 분석하여 골밀도를 평가하였고, CT-Volume (CTvol) 소프트웨어를 이용하여 소주골의 3D 이미지를 재구성하였다.
조직학적 분석을 위해 채취한 경골은 4% 파라포름알데히드 (Paraformaldehyde)에 고정하고 5.5% EDTA (Ethylene-diamine-tetraacetic acid) 완충액에서 2주 동안 탈회 (demineralization)한 후, 점진적으로 탈수하고 파라핀 (paraffin)에 심어서 5 um 두께로 잘랐다. 자일렌 (xylene)에서 탈파라핀화 (Deparaffinization)한 후, 헤마톡실린 및 에오신 (Hematoxylin& Eosin; H&E) 또는 TRAP 염색하여 각각 조골세포 및 파골세포를 정량화하였다.
2. 결과 및 고찰
2-1. 세스트린2에 의한 파골세포 분화 억제 효과 확인
RANKL을 처리한 대조군 파골세포의 c-Fos, NFATc1, OSCAR, TRAP, CathepsinK 및 Sesn2의 mRNA 발현을 확인한 결과를 도 1 및 표 4에 나타내었다.

도 1 및 표 4에서 확인할 수 있듯이, RANKL 처리 1일 후 세스트린2 (Sestrin2)의 발현이 감소되고 다시 회복되는 것을 고려하면, Sestrin2가 파골세포 분화동안 존재하여 파골세포 분화 및 형성에 영향을 줄 수 있을 것이라고 제안할 수 있다.
파골전구세포는 RANKL에 의해 성숙한 파골세포로 분화가 유도되므로, 다음으로 RANKL에 의한 파골세포 분화 동안, Sestrin2의 과발현이 어떤 영향을 주는지 확인하기 위해, 파골전구세포에 레트로바이러스를 이용하여 Sestrin2를 형질도입한 후 M-CSF와 0, 20, 50 또는 100 ng/ml의 RANKL을 처리하여 파골세포 형성을 TRAP 염색법으로 확인한 결과를 도 2a 및 2b에 나타내었고, 도 2b의 TRAP(+)MNCs/well 결과값을 표 5에 나타내었다.

도 2a 및 2b, 표 5에서 확인할 수 있듯이, Sestrin2의 발현을 증가시키면 파골세포의 형성이 억제되었다.
뿐만 아니라 골흡수 자국을 셈으로써 골흡수능을 확인한 결과를 나타낸 도 3a 및 3b와 도 3b의 결과값을 나타낸 표 6에서 확인할 수 있듯이, 골흡수능도 감소하였음을 알 수 있었다.

또한, 파골세포의 분화 및 활성에 관여하는 전사인자 및 표적유전자의 발현에 Sestrin2의 발현 및 활성화가 어떤 영향을 주는지 확인한 결과를 도 4 및 5와 도 4의 결과값을 표 7에 나타내었다.

도 4 및 표 7에서 확인할 수 있듯이, 파골전구세포에 Sestrin2의 발현을 증가시키면, 파골세포의 분화에 관련된 전사인자 및 활성 표적 유전자인 c-Fos, NFATc1, OSCAR, TRAP, CathepsinK의 mRNA 발현이 감소되고 또한, 도 5에서 확인할 수 있듯이, 파골세포 표적 단백질 발현이 감소됨을 확인하였다.
2-2. 세스트린2 타겟의 siRNA를 이용한 파골세포의 분화 촉진 효과 확인
RANKL에 의한 파골세포 분화에 Sestrin2를 타겟으로 하는 siRNA의 도입에 의하여 Sestrin2의 발현 억제가 어떤 영향을 주는지 확인하기 위해 파골전구세포에 대조군 올리고머와 Sestrin2 siRNA를 각 형질도입하고 M-CSF와 RANKL의 존재 하에서 파골세포 분화를 유도한 결과를 도 6 내지 도 9에 나타내었고, 도 6의 결과값은 표 8에, 도 7b의 결과값은 표 9에, 도 8b의 결과값은 표 10에, 도 9의 결과값은 표 11에 나타내었다.

도 9 및 표 11에서 확인할 수 있듯이, 대조군 올리고머를 도입한 세포에 비해 Sestrin2 siRNA 도입한 세포에서 파골세포 분화, 활성의 전사인자 및 표적유전자인 c-Fos, NFATc1, OSCAR, TRAP(Acp5), CathepsinK의 mRNA 발현이 증가하였다.
2-3. 세스트린2 작용제 아지트로마이신의 파골세포 분화 억제 및 골손상 감소 효과 확인
활성산소는 파골세포의 활성을 증가시키고 골다공증과 관련되어 골밀도 간에 음의 상관관계가 있다. Sestrin2는 스트레스 반응성 유전자로 파골세포 형성 과정에서 생성되는 활성산소의 작용 표적이 될 수 있다. 마크로라이드 (macrolide)계 항생제의 하나인 아지트로마이신 (Azithromycin)은 항산화 및 항염 효과가 있으므로, 파골세포에서 아지트로마이신이 Sestrin2의 작용제로써 가능성을 알아보기 위해, 아지트로마이신을 농도 의존적으로 처리한 후 Sestrin2의 발현에 어떠한 영향을 주는지 확인한 결과를 도 10 및 표 12에 나타내었다.

도 10 및 표 12에서 확인할 수 있듯이, 파골전구세포에 아지트로마이신을 농도별로 처리하면 Sestrin2의 mRNA 발현이 농도 의존적으로 증가하였고, 이를 통해 아지트로마이신을 Sestrin2 작용제인 것으로 확인하였다.
또한, RANKL에 의한 파골세포 분화에 대하여 아지트로마이신이 어떠한 영향을 주는지 확인하기 위해 파골전구세포에 아지트로마이신을 농도 의존적으로 처리한 결과를 도 11 및 표 13에 나타내었다.

도 11 및 표 13에서 확인할 수 있듯이, TRAP 염색 및 활성측정을 통해 아지트로마이신에 대하여, 농도 의존적으로 파골세포 형성이 감소하였다.
또한, 파골세포에서 생성되는 활성산소에 Sestrin2 작용제인 아지트로마이신의 처리에 의해 어떠한 영향이 있는지 DCFDA로 염색하여 형광법을 통해 확인한 결과를 도 12a, 12b 및 표 14에 나타내었다.

도 12a, 12b 및 표 14에서 확인할 수 있듯이, 파골전구세포에 RANKL를 처리하면 활성산소가 생성되는데 아지트로마이신 농도가 증가할수록 활성산소의 생성이 억제됨을 알 수 있었다.
이와 같은 결과를 통해, Sestrin2와 이의 작용제 아지트로마이신은 파골세포에서 항산화 역할을 통해 파골세포 분화 형성 및 활성을 억제할 수 있다는 것을 제안할 수 있다.
골손상 동물실험을 통해 Sestrin2 작용제인 아지트로마이신이 골손상 회복에 어떤 영향을 주는지 확인하였다. 생쥐의 복강내 RANKL를 주사하여 골손상을 유도하고 아지트로마이신을 동시에 주입하여 uCT 촬영 후 뼈 밀도를 분석한 결과를 도 13, 도 14a 내지 14c, 및 표 15에 나타내었다.

도 13, 도 14a 내지 14c, 및 표 15에서 확인할 수 있듯이, 복강내 RANKL 주입에 의해 골손상이 유도되어 골밀도가 감소되고 파골세포의 수가 늘어났다. 반면에, 아지트로마이신을 혼합하여 주입한 동물모델에서는 RANKL 주입한 생쥐에서 관찰되었던 골밀도의 감소가 억제되고 파골세포 수도 감소되었다.
이러한 결과는 Sestrin2의 작용제 아지트로마이신이 파골세포에 의한 골 분해를 억제 시킬 수 있음을 시사한다.
