Abstract
고형 지지체가 로딩된 아페레시스 컬럼은, 하나 이상의 케모카인, 특히 지지체 상에, 특히 스트렙타비딘을 가지고 있는 지지체 상에 직접 또는 간접적으로 고정된, 바이오틴화된 케모카인을 포함한다. 또한, 컬럼 및 지지체의 용도, 및 염증성 장 질환(IBD)와 같은 염증 증상을 앓고 있는 환자의 말초 혈액으로부터 세포, 특히 백혈구를 제거하는 방법에 관한 것이다.
Description
본 발명은 염증성 장 질환, 특히 궤양성 대장염(UC) 및 크론 질환(CD), 보다 구체적으로는 급성 궤양성 대장염과 크론 질환과 같은 염증 증상을 치료하기 위한 제품, 방법, 및 치료 수단에 관한 것이다
급성 궤양성 대장염은 백혈구 수치 상승과 심한 복부 통증이 특징인 궤양성 대장염이 악화된 것이다. 현재, 급성 궤양성 대장염을 앓고 있는 환자들은 고용량의 스테로이드로 치료하고 있다. 항-TNFα를 이용한 치료는 III상 단계를 테스트하고 있다. 이들 2가지 약물 모두 일반적인 염증 저해제이다. 이 약물들은 사례의 약 50%에는 효과가 있지만, 심각한 부작용을 가지고 있다. 또한, 급성 궤양성 대장염이 성공적으로 치료되더라도 재발되기도 한다.
의학적 치료에 반응을 보이지 않는 급성 궤양성 대장염 환자들의 경우, 신속한 외과적 개입이 필수적이다. 궤양성 대장염은 늘 대장(결장)으로 국한되기 때문에, 체외 복식회장절개술이 적용된다. 적어도 6개월의 회복 기간을 지난 후, 때로는 직장의 기부 염증에 대한 추가적인 의학적 조치를 거친 후, 장의 연속성을 복원하기 위해, 대부분의 환자들에게는 회장 직장 문합술이나 복부 주머니(pelvic pouch) 재건술이 수행될 것이다. 이러한 시술들로 인해 매일 약 6회 묽은 변이 배출되고 수분과 미네랄의 균형이 교란된다. 또한, 크론 질환도 급성인 경우(급성 크론성 대장염)가 있을 수 있는데, 이 역시 즉각적인 의학적 및/또는 의과적 개입이 필수적인 심각한 상태이다.
염증은 크론 질환자의 위장관의 모든 부분에서 발생될 수도 있지만, 통상적으로는 소장의 최말단과 대장의 처음 부분 (회맹부)으로 국한된다. 스테로이드나 아자-티오프린과 같은 항염증제들은 증상을 완화하지만, 의약적 치료로는 질환을 치유할 수 없다. 협착성의 누공이 형성된 장 부분을 절제하는 수술이 환자의 약 50%에게 처방되고 있으며, 이들 중 절반에서는 재발하여 추가적인 수술이 필요하게 될 것이다. 따라서, 개별 환자들에서 IBD 염증을 특이적으로 잠재우고 질병 재발을 예방할 수 있는 방법이 특히 필요하다.

염증성 장 질환은 병에 걸린 장의 염증 발생과 백혈구의 침윤이 특징적이다.
케모카인은 염증에서 세포의 동원 및 활성화에 관여하는 사이토카인 분자의 일종이다. 케모카인은 면역계에서 다양한 세포 서브집단의 화학주성과 활성화를 야기한다. 케모카인의 활성은 백혈구의 표면 상에 있는 이의 수용체와의 견고한 결합에 의해 주로 매개된다. 본 발명은, 염증 증상, 특히 케모카인 수용체-발현 세포의 염증 부위로의 동원 증가가 특징적인, 크론 질환의 염증 증상을 치료하기 위해, 케모카인과 이의 수용체를 발현하는 세포 사이의 상호작용을 이용할 수 있다는 인식을 기초로 한다. 즉, 본 발명은, 염증성 케모카인의 높은 발현 수준 유도로 인해 야기되는, 염증 부위로의 염증성 백혈구의 동원을 낮추는 것이다. 이는, 상기한 염증성 케모카인을 이용하여, 환자로부터 염증성 백혈구를 포획함으로써 달성된다. 보다 구체적으로는, 백혈구, 특히 (활성화된) T 림프구, (활성화된) 단핵구, (활성화된) 호중성 과립구, (활성화된) 호산성 과립구 중 1종 이상이, IBD에서의 염증 발병과 유지에 관여하므로, 따라서, 이들을 순환계로부터 제거하여 이러한 염증을 경감시키고, 심지어 없앤다. 본 발명자들은, 활성형의 IBD 환자로부터 수득한 장 생검 샘플을 유세포 측정함으로써, 염증 부위에 집중되지만 또한 순환성 말초 혈액에도 존재하는 (활성화된) T 림프구, 단핵구, 호중성 과립구 및 호산성 과립구를 동정하였다.
따라서, 본 발명은, 환자의 말초 혈액으로부터, 본원에 기술된 (단핵구 또는 림프구와 같이) 케모카인 또는 케모카인들의 동족 수용체를 발현하는 세포, 특히 (활성화된) 백혈구를 제거할 수 있도록, 지지체에 직접 또는 간접적으로 고정된 하나 이상의 케모카인을 포함하는, 고형 지지체를 일 측면으로 제공한다. 케모카인은 염증성 케모카인이며, 즉 상해나 감염에 반응하여 세포 또는 조직에 의해 높은 수준으로 유도되(며, 염증성 백혈구를 동원하)는 것이다.
본원에서, 용어 "케모카인"은 바이오틴화된 또는 표지된 케모카인을 포함한다. 용어 "케모카인"은 또한 변형형 또는 절단형이 이의 동족 수용체에 결합하는 능력을 유지(하고 따라서 본 발명에서 기능성을 유지)하는 한, 케모카인의 변형된 버전 및 절단된 버전들도 포함한다. 변형은 단백질 합성을 개선시키기 위해, 예컨대 생성물의 균일성 및 수율을 향상시키기 위해 행해질 수 있다. 변형은 케모카인내 하나 이상의 아미노산에 대한, 아미노산 부가, 치환, 결손 또는 그외 변형을 포함할 수 있다. 변형은 천연형 아미노산을, 노르루신(NLeu)과 같은 비천연형 아미노산 및 피로글루탐산(pyroGlu)과 같은 유도체화된 아미노산으로의 치환을 포함할 수 있다. 이러한 변형은 본 발명의 컬럼을 보관 및 이용하는 동안에 부산물이 생성되는 것을 최소화하기 위해 행해질 수 있다. 변형은 표지를 향상시키기 위해, 예컨대 바이오틴화를 용이하게 할 수 있도록, 폴리에틸렌 글리콜(PEG) 스페이서의 삽입 등과 같이, 행해질 수 있다. 바이오틴화 및/또는 형광발색단 또는 그외 표지기와의 케모카인의 접합은, 수용체의 결합력에 실질적으로 영향을 미치지 않는 방식으로 수행된다. 부위 특이적인 바이오틴화 또는 그외 표지는 바람직하게는 케모카인에 대한 비선택적인 표지이며, 수용체 결합 활성을 약화시킬 수 있다. 바이오틴화 또는 기타 표지는 일반적으로 단백질의 C-말단이나 C-말단 쪽인 것이 바람직한데, 그 이유는 본 발명자들이 이 부위에서의 변형이 일반적으로 (수용체 결합력에 대한 최소 효과 측면에서) 잘 허용된다는 것을 확인하였기 때문이다. 절단은 적절하게 N 또는 C 말단 아미노산, 또는 이 둘다의 결손을 포함할 수 있다. 전형적으로, 절단 버전은 케모카인이 올바르게 폴딩하는데, 예컨대 케모카인 폴딩 구조를 유지하는데 필요한 잔기들을 유지하는 것이며, 절단 버전은 (백혈구(의 표면 상에서)에 의해 발현된) 해당 수용체에 결합하는 능력을 유지하여야 한다는 요건을 충족시켜야 한다. 절단 버전은, 특정 구현예들에서, 천연형 아미노산 서열과 비교하여 임의 위치에서 아미노산이 1 내지 100개, 예컨대, 1, 2, 3, 4, 5개가 적을 수 있다. 물론, 절단 버전은 본원에 상세히 기술된 바와 같이 추가적인 변형을 포함할 수도 있다. 상기 변형 또는 절단 버전은, 특정 구현예에서, 전장 길이의 천연형 케모카인과 40%, 45%, 50%, 55%, 60%, 65%, 70%, 75%, 80%, 85%, 90%, 95% 또는 그 이상의 전체 아미노산 서열 동일성을 가질 수 있다(결손은 아미노산 서열 차이로 카운팅됨). 분자들 간의 공통되는 서열(즉, 결손되지 않은 아미노산)에서, 특정 구현예에서, 아미노산 서열 동일성은 80%, 85%, 90%, 91%, 92%, 93%, 94%, 95%, 96%, 97%, 98% 또는 99%일 수 있다. 변형 및 절단 2가지를 모두 포함하며, 본 발명에 사용되도록 특이적으로 개변된, 본 발명의 케모카인의 예는 본원에 상세하게 기술되어 있다. 절단형 TECK는 전장 길이의 성숙형 단백질의 1에서 74번의 잔기들에 해당되므로(즉, 75번 - 127번의 아미노산과, N-말단의 23개의 아미노산으로 구성된 신호 펩타이드가 없음), 케모카인 폴딩을 유지한다. 또한, 메티오닌을 노르루신으로 치환하므로써, 체인이 조립되는 동안에 잔기가 산화되는 것을 방지한다. N 말단의 글루타민 잔기를 피로글루타민으로 치환하여, 합성 중의 생성물의 균일성을 유지시킨다. 바이오틴화는 72번 위치에서 확인되는 라이신 잔기의 ε-작용기에서, PEG 스페이서를 통해 이루어진다. 선형 분자(즉, 72번 위치에 PEG 스페이서와 바이오틴 분자가 없는 분자)의 아미노산 서열은, 서열번호 1로 제시된 아미노산 서열을 포함하거나, 필수적으로 이들로 구성되거나, 이들로 구성된다. 본 발명의 케모카인은 임의의 적합한 수단을 통해 합성할 수 있다. 바람직하게는, 케모카인은 변형 및 표지 등이 용이하게 화학 합성된다. 그러나, 재조합 DNA 기반의 방법도 필요에 따라 적합한 표지 및 변형 기법과 조합하여 사용할 수도 있다. 따라서, 본 발명은, CCL25(의 성숙형의) 1 - 74번의 아미노산(즉, 처음 23개의 아미노산으로 구성되는 신호 펩타이드가 결핍된 형태)을 포함하거나, 이들로 필수적으로 구성되거나, 또는 이들로 구성된, 절단형 CCL25 단백질을 코딩하는 핵산 분자를 제공한다. 즉, 상기 단백질은 성숙형 CCL25 아미노산 서열에서 75 - 127번의 아미노산이 결핍되어 있다. 또한, 본 발명은 이러한 핵산 분자를 포함하는 벡터, 및 상기 벡터를 포함하는 숙주 세포에 관한 것이다. 상기 벡터는 상기 핵산 분자에 작동가능하게 연결된 적합한 프로모터를 부가적으로 포함하여, 해당 mRNA 분자의 전사를 촉진할 수 있다. 상기 숙주 세포는 CCL25의 1 - 74번 아미노산을 포함하는 (즉, 75 - 127번 아미노산이 없으며, 23개의 아미노산으로 구성된 N-말단 신호 펩타이드가 없는) 절단형 CCL25 단백질을 코딩하는 핵산 분자의 전사 및 번역에 의해 단백질을 발현할 수 있다.
케모카인 수용체는 림프구, 과립구 및 항원 제시 세포와 같이, 이동성 세포 범주들에서 발현되지만, 또한 상피 세포 또는 섬유모세포와 같이 특정한 비-이동성 세포에서도 발현된다. 전술한 바와 같이, 본 발명의 케모카인은 환자의 말초 혈액으로부터 한가지 이상의 케모카인에 대한 동족 수용체를 발현하는 세포의 제거를 허용하는 방식으로, 고형 지지체에 고정된다. 본 발명에 따른 바람직한 케모카인 각각에 대한 동족 수용체는 표 1에 나열되어 있다. 특정 예로, 케모카인에 결합할 수 있는 수용체는 2개 이상일 수 있다. 케모카인의 이러한 특성은, 염증 증상에 기여하는 수용체-발현 세포 범주들을 포획함으로써, 유익하게는 보다 효과적인 치료를 가능하게 할 수 있다. 특정 구현예에서, 상기 지지체는 전염증성 세포 범주에 대한 포획성을 높인다는 측면에서 복수의 케모카인을 함유한다.
IBD-환자에게서 나타나는 염증은 항원 제시 세포(APC)와 T-세포가 혈액 순환계로부터 장 점막으로 계속적으로 공급되어 지속된다. 이러한 세포의 계속적인 축적은 케모카인과 이의 수용체에 의해 조절된다. 즉, 케모카인은 염증기에 다양한 면역세포 서브-집단을 동원하고 활성화하는 분자이다. 염증이 발생된 장 점막에서 생산되는 한가지 국소 케모카인은 TECK(흉선에서 발현되는 케모카인, 또한 CCL25라고도 함)이다. 순환성 혈액에서 단핵구와 림프구는 TECK 수용체; 케모카인 수용체 9(CCR9)를 통해 케모카인을 인지한다. 세포는 CCR9 수용체를 통해 TECK에 결합하며, 장 염증 부위로 이동하기 시작한다(1). 염증이 발생된 점막에 도달하면, 단핵구는 APC가 된다. 또한, 순환성 CCR9를 발현하는 T-세포 역시 이 세포를 활성화시키는 장의 염증 부위로 찾아오게 된다. 마우스 모델에서, CCR9 또는 이것의 리간드 TECK가 유전자 삭제 또는 항체 저해에 의해 제거되면, T-세포의 장으로의 이동은 약화된다(2). CCR9-TECK의 상호작용은 단핵구 및 T-세포의 전체 장관; 소장 뿐만 아니라 결장으로 이동하는데 중요한 역할을 하는 것으로 보인다. 최근 연구를 통해, 건강한 사람과 IBD 환자의 대장에 CCR9와 TECK 모두 존재하는 것으로 확인되었다. CD 또는 UC 환자로부터 채취한 결장 조직을 검사한 결과, 면역조직화학적 염색을 통해, CCR9를 발현하는 T-세포가 존재한다는 것이 확인되었다. 또한, 결장에서 수용체 작용제인 TECK의 존재도 관찰되었다. IBD 환자의 경우, 이론적인 설명은 순환성 CCR9를 발현하는 단핵구 및 T-세포를, 이들이 장의 염증 부위에 도달하기 전에 제거하여, 염증을 완화한다는 것이다. 순환계로부터 CCR9를 발현하는 단핵구와 T-세포를 포획하는, 고형 지지체 상에 고정된 바이오틴화된 TECK(bTECK)를 가지고 있는 컬럼을 사용함으로써, 지속적인 염증 발화를 억제하여, 점막을 치유할 수 있다. 동일한 이론적 설명은, 염증이 발생된 장 부위로 백혈구를 동원하는데 관여하는 다수의 케모카인에도 적용할 수 있다. 예를 들어, IL-8은 장 점막에 의해 분비되어, IL-8 수용체와의 상호작용을 통해 호중구를 공격한다. CCR6은 염증이 발생된 조직에서 Th17 세포의 장으로의 이동과 작용자 T 세포의 균형/분포를 조절한다(14). 따라서, 고형 지지체 상에 고정된 CCL20은 IBD와 같은 염증 증상을 치료하는데 있어 본 발명에 유용하다.
본 발명의 케모카인은 이의 발현이 염증 장애에서 과조절되는 것으로 확인된 사실에 입각하여 선별하였다. 또한, 적절한 세포 타입 상에 존재하는 케모카인 수용체가, 수용체를 발현하는 세포를 염증 부위로 동원함으로써, 이러한 염증 장애의 발병과 연계되어 있다는 것을 확인하였다. 즉, 본 발명의 케모카인은, MIP-1a, MIP-1b, MCP-1, MCP-2, MCP-3, MCP-4, TARC, MDC, MIP-3, MIP-3a, MIP3b, MIP-4, I-309, HCC-1, HCC-2, SLC, IL-8, GROa, GROb, GROg, RANTES, NAP-2, ENA78, GCP-2, IP-10, MIG, I-TAC, SDF, 프랙탈킨(fractalkine), 림포탁틴(lymphotactin), 에오탁신(eotaxin), 에오탁신-2, I-309, BLC, CCL25 중 임의의 한가지 이상을 포함할 수 있다. 특히 바람직한 본 발명의 케모카인은 MIP-1a, MIP-1b, MIP-3a, MIP-3b, MIP-4, SLC, MCP-1, MCP-2, MCP-3, MCP-4, TARC, MDC, IL-8, IP-10, MIG, I-TAC, 프랙탈킨, CCL-25, RANTES를 포함한다. 본 발명의 가장 바람직한 케모카인은 활성화된 T 림프구에 바람직하게는 결합하는 케모카인, 특히, MIP-1a, MCP, IP-10, MIG, ITAC, CCL25이다. "바람직하게 결합하는"은, 케모카인이 비활성화된 T 림프구 및/또는 다른 혈액 세포에 비해 활성화된 T 림프구에 결합하는 경향이 더 높은 것을 의미한다. 특정 구현예에서, 케모카인은 CCL25이다. CCL25는 바람직하게는 CCR9를 발현하는 세포, 특히 (활성화된) 림프구(CD4 및 CD8 림프구) 및 단핵구(예, CD14-양성 단핵구)에 바람직하게 결합한다.

본 발명의 케모카인은 WO 00/50088 A2에 기술된 바와 같은 당해 기술 분야에 공지된 방법을 이용하여 바이오틴화할 수 있으며, 상기 문헌은 원용에 의해 본 명세서에 포함된다. 케모카인의 수용체-결합력에 현저한 영향을 미치지 않는 임의의 표지 기법을 적용할 수 있으며, 전술한 바와 같이, 본 발명의 케모카인의 부위-특이적인 표지가 바람직하다. 다양한 부위-특이적인 바이오틴화된 케모카인 및 천연 케모카인들은, 예컨대 영국 크레이가본의 알마크로부터 상업적으로 구입할 수 있다. 구체적인 구현예에서, 하나 이상의 케모카인은 스페이서 그룹을 통해 바이오틴화된다. 상기 스페이서는 케모카인의 활성, 특히 케모카인이 이의 동족 수용체와 결합하는데에 바이오틴기가 영향을 미치지 않도록 하기 위해, 적용할 수 있다. 케모카인의 수용체 결합 특성을 유지할 수 있게 하는, 임의의 적합한 스페이서를 본 발명에서 사용할 수 있다. 특정 구현예에서, 상기 스페이서는 폴리에틸렌 글리콜(PEG) 스페이서이다. 본 발명의 케모카인을 고정하기 위한 고형 지지체 물질은 당해 기술 분야에 공지되어 있다. 유용한 지지체 물질은, 혈액 세포, 특히 림프구가 지지체에 응고 또는 부착되지 않을 정도로 활성화시키지 않는 것이다. 항-응고성을 제공하기 위한 제제로 처리된 지지체, 특히 헤파린 처리된 지지체를 사용하는 것이 유용하다. 다른 예로, 환자의 혈액은 지지체에 적용하기 전에, 헤파린과 같은 항-응고제로 처리할 수 있다. 유용한 지지체 물질들로는 고분자량의 탄수화물, 특히 선택적으로 가교된 특정 형태의, 아가로스 및 셀룰로스와 같이 분자량 100 kDa 이상의 탄수화물을 포함한다. 다른 바람직한 지지체 물질로는, 카르복시화된 폴리스티렌 및 글라스와 같은 폴리머가 있다. 본 발명의 지지체는 바람직하게는 입자 또는 섬유 형태이다. 지지체 입자는 구형 또는 비드와 같이 규칙적인 형태를 취하거나, 또는 불규칙적인 형태를 취할 수 있다. 이것은 유공성 또는 무공성일 수 있다. 지지체의 바람직한 평균 입자 크기는 50 ㎛ 내지 2 mm이다. 고형 지지체 상에 케모카인을 고정시키는 방법은 당해 기술 분야에 공지되어 있다. 케모카인은 직접 또는 간접적인 방식으로 지지체에 고정시킬 수 있다. 직접 고정화는 적합한 링커를 이용한 것일 수 있다. 케모카인의 바람직한 간접적인 고정화 방법은 바이오틴과 아비딘 분자 간의 상호작용에 의지한다. 즉, 케모카인의 바이오틴화와, 고형 지지체 상에 고정되는 스트렙타비딘의 사용을 통해, 고형 지지체에 케모카인을 확실하게 부착시킬 수 있다. 구체적으로, 방법은 바이오틴화된 형태로 케모카인을 제공하는 단계, 표면에 스트렙타비딘이 고정된 고형 지지체를 제공하는 단계, 상기 지지체를 상기 바이오틴화된 케모카인의 수용액과 접촉시키는 단계, 및 상기 지지체를 수용액으로 헹구는 단계를 포함할 수 있다. 아울러, 항체-항원 상호작용도 케모카인을 지지체에 간접 고정시키는데 사용할 수 있다. 이러한 구현예에서, 지지체는, 특정 펩타이드 서열이나 소형 분자 헵텐에 대해 공지된 친화성을 가진, 항체 또는 그것의 단편이나 유도체로 유도체화될 수 있다. 케모카인 상에 또는 내부에 이러한 펩타이드 서열이나 헵텐을 병합시키면, 해당 항체 또는 이의 단편이나 유도체로 코팅된 고형 지지체 상에 고정하기 쉬워진다. 따라서, 케모카인은 상기 펩타이드 서열이나 헵텐을 포함하도록 변형되거나, 또는 측쇄나 표지로서 첨가될 수 있다. 임의의 적합한 항체-항원 쌍도 이용할 수 있다. 항체 단편 또는 유도체는 적절한 항원에 대한 특이적인 결합 친화성을 보유한 임의의 단편 또는 유도체일 수 있다. 그 예로는, Fab, scFV, VH 도메인, 나노바디(nanobodies), 중쇄 항체 및 비-인간 항체의 인간화된 버전 등이 있다. 그외 고친화성 상호작용은, 결합 활성(즉, 케모카인과 이의 동족 수용체의 결합)의 소실 없이, 케모카인을 상호작용 파트너들에서 어느 한가지의 파트너로 유도체화할 수 있고, 고형 지지체를 상호작용 파트너들의 다른쪽 파트너로 유도체화할 수 있다면, 케모카인의 고정화에 이용할 수 있다.
다른 예로, 케모카인은 당해 분야에서 확립된 바이오접합 기법을 이용하여 고형 지지체 상에 직접 고정시킬 수 있다. 예컨대, 시아노젠 브로마이드로 활성화된 고형 지지체 상에, 단백질의 일차 서열의 아미노 작용기를 통한 단백질의 직접 고정. 다른 예로, 단백질의 설피드릴(sulphydryl) 작용기를 이용하여, 단백질을 알킬 할라이드 유도체화된 지지체나 또는 유리(free) 티올 작용기들을 포함하는 지지체에 직접 고정시킬 수 있다. 다른 구현예에서, α-티오에스테르 작용기를 포함하고 있는 단백질은, 천연적인 화학 라이게이션 반응을 이용하여 1,2 아미노티올 모이어티(예, N-말단 시스테인)가 포함된 지지체 상에 직접 고정시킬 수 있다. 다른 예로, 케톤 및 알데하이드로 변형된 단백질은 하이드라존/옥심 결합 형성 라이게이션 반응을 이용하여 하이드라지닐, 하이드라지드 및 아미녹시 작용기로 유도체화된 고형 지지체상에 고정시킬 수 있다(그 반대도 가능). 다른 예로, 단백질과 지지체를 상호 반응하는 적절한 화학 작용기(아지드 및 알킨류)로 유도체화함으로써, '클릭' 화학법을 이용하여 고형 지지체 상에 단백질을 고정시킬 수 있다. 다른 구현예에서, 슈타우딩거 라이게이션 화학법을 이용하여, 적절하게 유도체화된 단백질을 적절하게 유도체화된 고형 지지체 상에 고정시킬 수 있다.
본 발명은, 지지체 상에 고정된 하나 이상의 케모카인을 포함하는, 전술한 유형의 고형 지지체가 로딩(load)된, 아페레시스(apheresis) 컬럼을 개시한다. 상기 컬럼에는, 지지체 상에 직접 또는 간접 고정된 한가지 이상의 케모카인을 포함하는 고형 지지체가 로딩되어 있어, 케모카인 또는 케모카인들의 동족 수용체를 발현하는 세포, 특히 전술한 바와 같이 (활성화된) 백혈구를 환자의 말초 혈액으로부터 제거할 수 있게 해준다. 본 발명의 바람직한 구현예에서, 상기 아페레시스 컬럼에는, 지지체 상에 고정된 스트렙타비딘과 지지체 상에서 상기 스트렙타비딘과 결합되는 하나 이상의 바이오틴화된 케모카인을 포함하는 지지체가, 로딩된다. 지지체로는 고분자량의 선택적으로 가교된 탄수화물, 예컨대 아가로스가 바람직하다. 상기 "로딩"는 (말초) 혈액이 상기 고형 지지체와 접촉하면서 컬럼을 통과하여 흐르도록 하는 방식으로 컬럼이 고형 지지체를 가지고 있거나 포함하고 있는 것을 의미한다. 즉, 고형 지지체는, 특정 구현예들에서는, 지속적인 방식으로 혈액이 흐르도록 되어 있는 컬럼내 매트릭스를 제공한다.
따라서, 본 발명의 컬럼은 혈액 샘플로부터 동족 케모카인 수용체를 발현하는 세포를 제거할 수 있게 하는 지지체를 가지고(carry) 있도록 한다. 보다 구체적으로, 상기 컬럼은, (IBD) 환자의 말초 혈액으로부터, 하나 이상의 (활성화된) 백혈구, 특히, 하나 이상의 (활성화된) T 림프구, (활성화된) 단핵구, (활성화된) 호중성 과립구, (활성화된) 호산성 과립구를 제거하는데 사용할 수 있다. 개별 환자의 세포 프로파일에 따라, T 림프구, 단핵구, 호중성 및 호산성 과립구 또는 장 염증에 관여하는 다른 종류의 활성화된 세포를 특이적으로 제거하기 위한, 지지체의 제조에, 특이적인 케모카인이 선택된다.

말초 혈액은 환자로부터 계속적으로 채혈할 수 있다. 마찬가지로, 제거 처리된 혈액 역시 본원에 기술된 적절한 회로를 이용하여 계속적으로 환자에게 주입할 수 있다. 즉, 지지체는 혈액이 흐르도록 되어 있는 컬럼 안에 배치될 수 있다. 이는, 예컨대 적합한 펌프를 이용하여 달성할 수 있다. 혈액이 컬럼을 통과하여 흐르면, 고형 지지체 상에 고정된 케모카인이 수용체를 발현하는 백혈구를 포착할 수 있게 하여, 그 결과 혈액으로부터 이들을 제거하고 염증 증상에 기여하지 못하게 할 수 있다.

본 발명의 보다 포괄적인 실현으로, 본 발명의 컬럼 또는 본 발명의 지지체는 세포, 특히 해당 케모카인 수용체를 발현하는 특정 백혈구, 예컨대 (활성화된) T 림프구, (활성화된) 단핵구, (활성화된) 호중성 과립구, (활성화된) 호산성 과립구 중 하나 이상이, 질병에 걸린 개체의 말초 혈액에 존재하는 것이 특징적인, 질환의 치료에 사용된다.

재료 및 방법
말초 혈액 백혈구의 분리. 건강한 혈액 공여자나 IBD 환자로부터 수득한 헤파린 처리한 말초 혈액을 4분간 4% 파라포름알데하이드로 고정하고, 15분간 0.83% 암모늄 클로라이드 용액으로 용혈한 다음, FACS 완충액으로 2번 헹구어, 혈액 백혈구 현탁물을 수득하였다.
케모카인 . 백혈구를 암 조건에서 4℃에서 30분간 하기 바이오틴화되고 Alexa647 Fluor®로 표지된 케모카인들과 함께 인큐베이션하였다: CCL25 (농도: 0.1 ng/㎕, 0.5 ng/㎕ 및 5 ng/㎕), MIP-1α 또는 MCP-1 (농도: 10 ng/㎕ 및 50 ng/㎕). 그런 다음, 세포를 FACS-완충액으로 헹군 후 유세포 측정으로 분석하였다. 실시예에 사용되는 모든 케모카인들은 스코틀랜드 에든버러에 위치한 Almac Sciences Scotland Ltd로부터 제공받았다.
유세포 분석. 유세포 분석은 2가지 레이저 FACS Calibur cytometer (BD Immunocytometry systems, San Jose, Ca, USA)로 수행하였다. 10000개의 세포를 계수하여, 각 샘플에서 분석하였다. 데이타 분석을 위해, 벡턴 딕킨슨 사의 Cell Quest Pro 소프트웨어를 사용하였다.
실시예 1 - MIP -1α에 단핵구 결합. 바이오틴화된 MIP-1α를 이용한 실험에서, 건강한 공여체의 말초 혈액으로부터 수득한 단핵구는 인큐베이션 30분 후에서 약 90%가 사이토카인에 결합되지만(도 1c), CD4+ 및 CD8+ 림프구는 결합하지 않는다(도 1a 및 1b)는 것을 확인하였다.
실시예 2 - MCP -1에 단핵구의 결합. 바이오틴화된 MIP-1을 이용한 실험에서, 건강한 공여체의 말초 혈액으로부터 수득한 단핵구는 인큐베이션 30분 후에서 약 90%가 사이토카인에 결합하지만(도 1f), CD4+ 및 CD8+ 림프구는 결합하지 않는다(도 1d 및 1e)는 것을 확인하였다.
실시예 3 - CCL25 에 대한 혈액 세포의 친화성. 바이오틴화된 CCL25를 이용한 실험에서, 건강한 공여체의 말초 혈액으로부터 유래된 T-세포(CD4+ 림프구; CD8+ 림프구)나 단핵구(CD14+ 단핵구)는 바이오틴화된 케모카인에 결합하지 않는 것으로 확인되었다(도 2a, 2b 및 2c). 반면에, 크론 질환자로부터 유래한 CD8+ 림프구의 약 80%, CD4+ 림프구의 약 90% 및 단핵구는 CCL25에 결합하였다(도 2d, 2e 및 2f).
실시예 4 - 바이오틴화된 IL -8에 대한 혈액 세포의 친화성. 도 3에서, 건강한 공여체로부터 수득한, CD4+ 림프구 (도 3a), CD8+ 림프구 (도 3b) 및 CD16+ 호중구 (도 3c)가 바이오틴화된 IL-8 (CXCL8)에 결합된다는 것이 확인되었다. 30분간 인큐베이션 후, CD16+ 호중구는 모두 IL-8에 결합하였다. 반면, CD4+ 림프구 및 CD8+ 림프구와의 결합은 관찰되지 않았다.
실시예 5 - 혈액 세포 아페레시스를 위한 케모카인 컬럼의 제조. 25 mM 소듐 포스페이트(pH 7.0) 및 150 mM NaCl의 수용액 중에 현탁된, 75 ㎛ - 300 μ의 범위의 스트렙타비딘 가교된 아가로스 (ProZyme, San Leandro, CA, U.S.A.) 비드에, 동일한 완충액 중의 바이오틴화된 MIP-1α (Almac Sciences) 75 ㎍의 용액을 22℃에서 첨가하고, 3분간 손으로 천천히 흔들었다. 다시 20분간 세워둔 다음, 지지체를 여과 분리하고, 중성의 소듐 포스페이트/소듐 클로라이드 수용액으로 3번 헹군 다음 유리 컬럼(직경 25 mm, 길이 12 cm)에 충진하였다.
실시예 6 - 실시예 6의 케모카인 컬럼을 이용한 건강한 공여자의 말초 혈액으로부터 단핵구의 분리. 건강한 남성 공여자로부터 수득한 헤파린 처리한 말초 혈액을 CD4+ 림프구, CD8+ 림프구 및 CD14 단핵구에 대해 유세포 측정으로 분석하였다. 혈액 100 ml을 컬럼을 통해 유속 약 8 ml/분으로 통과시키고, FACS 완충액으로 헹구었다. 여과한 혈액을 동일 세포에 대해 분석하였다. 단핵구의 약 95%는 컬럼에 체류되었지만, CD4+ 및 CD8+ 림프구는 각각 90% 이상이 회수되는 것으로 확인되었다.
실시예 7 - 바이오틴화된 MIP - 1α와 스트렙타비딘 접합된 자기 비드의 복합체의 제조. 스트렙타비딘이 접합된 자기 비드(MagCellect Streptavidin Ferrofluid, 1 ml; R&D Systems, Minneapolis, MN, U.S.A.)의 수성 현탁액을 25 mM 소듐 포스페이트(pH 7.0) 및 150 mM NaCl의 50 ml 중의 30 ㎍의 MIP-1α (Almac Sciences)와 혼합하고, 1시간 동안 서서히 교반하였다. 입자를 동일 용매 20 ml로 3번 헹구고, 4℃에서 현탁 상태로 보관하였다.
실시예 8 - 실시예 8의 스트렙타비딘 자기 비드를 이용하여 건강한 공여자의 말초 혈액으로부터 CD14 + 단핵구의 분리. 실시예 7의 건강한 공여자로부터 수득한 헤파린 처리한 혈액 100 ml을 스트렙타비딘이 접합된 자기 비드 - 바이오틴화된 MIP-1α 복합체와 혼합하고, 40분간 서서히 교반하였다. 자기 분리기에 의해 혈액으로부터 입자들을 분리하고, CD14+ 단핵구와 CD4+ 및 CD8+ 림프구에 대해 혈액을 분석하였다. 기본적으로 CD14+ 단핵구는 검출할 수 없었지만, CD4+ 및 CD8+ 림프구는 대략 오리지널 함량으로 존재하였다.
실시예 9 - 맞춤형 백혈구 분리 반출술
컬럼 설계 및 특성
개요
아페레시스는 항체, 저밀도 지단백질(LDL) 및 혈액 세포와 같은 혈액 성분을 제거하는데 사용하는 확립된 처리법이다. 백혈구 분리 반출술은 백색 혈액 세포, 백혈구의 제거에 사용되는 아페레시스 처리이다. 환자를 체외 혈액 순환 시스템에 연결시키고, 혈액을 한쪽 팔의 정맥으로부터 나와 컬럼 디바이스를 통과하여 다시 환자의 다른 쪽 팔로 주입되게 한다. 백혈구 분리 반출술 처리의 부작용은 두통, 현기증, 저혈압, 가슴 두근거림 및 홍조와 같이 약한 증상들로 다양하며, 시술받은 환자의 0.1 내지 5%에서 발생한다.
컬럼
컬럼은 IBD의 백혈구 분리 반출술 처리로 사용하고자 한다. CCR9-TECK 상호작용을 이용하는, bTECK 함유 수지를 사용하여 CCR9-발현성 장-회귀 백혈구를 특이적으로 제거한다. 컬럼은 플라스틱 하우스, 스트렙타비딘(SA) Sepharose™ BigBeads 매트릭스 및 상기 매트릭스에 결합된 bTECK인 3가지 조합된 구성으로 이루어진다. 처리는 표준적인 아페레시스 공정과 동일한 기법을 이용하여 수행한다.
플라스틱 하우스(도 4)
매트릭스를 통해 지속적인 혈액 흐름을 유지하도록 고안된, 플라스틱 하우스는, 투명한 본체와 붉은 색의 탑으로 구성된다. 탑에는 혈액이 균등하게 전체 매트릭스 구역으로 퍼지도록 유입부(1)에 위치된 분배판(2)이 있다. 이 판은 큰 입자가 컬럼을 통과하여 환자에게로 흘러가지 못하게 하는 제1 안전 장벽이다. 안전 필터 유닛(3 및 4)은 플라스틱 하우징의 유입부(1)와 유출부(5)에 위치한다. 안전 필터 유닛에는 견고한 경계이면서, 컬럼을 통과하여 흐르는 혈액 세포 보다 크기가 더 큰 입자들은 모두 저지하도록 고안된, 3개의 필터가 포함되어 있다. 플라스틱 하우징 디자인은 도 4에 나타낸다. 컬럼 디바이스의 양 단부에 위치한 안전 필터(3 및 4)가 있는 디자인은, 디바이스가 혈액 흐름을 실행되는 방향의 반대 방향으로 거꾸로 장착되는 경우 등의, 입자가 환자에게 누출될 위험성을 최소화시킬 것이다.
스트렙타비딘 Sepharose™ BigBeads
디바이스에서 제2 성분은 스트렙타비딘 Sepharose™ BigBeads (Sepharose™ GE Healthcare, Sweden)라 통칭되는 친화성 매트릭스이다. Sepharose™는, 해초에서 추출된 다당류인 아가로스가 가교된 비드 형태이다. Sepharose™와 아가로스는 생물의학의 친화성 기법들에 컬럼 매트릭스로서 통상 사용된다. 최적의 배분력을 고려하여 선택된 것이며, 친화성 결합에 이용가능한 넓은 면적을 제공할 수 있다.
bTECK
매트릭스에 결합된 bTECK가 디바이스의 제3 구성 요소이다. 이 bTECK 펩타이드는, 절단된 형태이고 바이오틴화된, 인간 케모카인 TECK의 합성 조작된 버전으로, TECK 수용체 CCR9에 대한 결합 활성을 보유하고 있다. 조작된 TECK를 바이오틴화함으로써, Sepharose™ 매트릭스 상에서 스트렙타비딘 분자에 결합시킬 수 있다. 바이오틴-스트렙타비딘 결합은 Kd 4 x 10-14 M로, 가장 강한 생물학적 상호작용 중 한가지인 것으로 알려져 있다. 컬럼에서 스트렙타비딘 : 바이오틴 결합 부위에 대한 계산된 비율은 10:1이다. 따라서, 매트릭스와 bTECK 간의 커플링은 즉각적이며, 매트릭스에 bTECK가 커플링되지 않을 가능성은 최소화될 것이다.
아페레시스 시스템
백혈구 분리 반출술을 수행하는데에는 다음과 같은 구성이 필요하다: 컬럼, 배관 시스템 및 4008 ADS 펌프(Fresenius Medical Care).
회로
시스템은 도 5에 예시한다. 환자(1)는 무균 벤플론 바늘을 통해 우측 및 좌측 팔의 정맥이 체외 회로와 연결된다. 식염수 백(3)도 연결되며, 식염수는 ACD 펌프(2)에 의해 펌핑된다. 혈액은 혈액 펌프(4)에 의해 무균 배관 시스템을 통해 환자의 한쪽 팔에서 나와, 컬럼(6)을 통과하여 다시 환자에게로 주입된다. 배관 시스템은 표준 투석 루어락(luer-lock) 커플링으로 컬럼에 연결된다. 컬럼의 커플링은 올바른 조립을 위해 색으로 식별되며, 붉은색 컬럼 상부에 유입되는 배관은 붉은 색이고, 환자에게 다시 유출되는 배관은 파란색이다. 공기 검출기(8)도 있다. 유압(5) 및 Pven 센서(7)를 이용하여 회로의 압력을 모니터링한다.
4008 ADS 펌프
Fresenius Medical Care 사의 아페레시스 펌프는 환자의 유입 및 유출, 체외 순환 압력을 모니터링하고, 버블 포획기 및 공기 검출기에 의해 공기를 식별할 수 있다. 혈전 포획기 필터도 상기 버블 포획기 내부에 위치되어 있다. 또한, 펌프는 밝음, 예컨대 배관 시스템에 식염수 또는 공기 존재와, 어두움, 예컨대 배관 시스템에 혈액 존재를 구별하기 위한 광학 검출기를 구비하고 있다.
공기 검출기와 광학 필터가 표시된 펌프의 개략적인 다이아그램은 도 6에 나타낸다. 펌프 시스템이 공기 버블 및 광학적 변동을 검출하거나, 또는 체외 압력 수치가 설정 범위를 벗어나면, 펌프는 즉시 중지되고, 시각적/청각적으로 경고를 나타낸다.

환자 준비
각 처치 실험 전에 환자에게 항응고제를 투여한다. 5000 IE 헤파린 처리된 무균 식염수를 이용하여 체외 시스템을 준비하고, 각 처치 실험 개시시에 회로에 4000 IE 헤파린의 볼루스 주사액을 넣는다.
백혈구 분리 반출술 시기 및 유속
아페레시스 시스템은 유속 30-60 ml/분으로 운영되어야 한다. 혈액 1800 mL을 순환시킨 후, 처치를 끝낸다.
보관 조건
컬럼 디바이스는 1 내지 25℃에서 냉동 및 상기 온도 보다 높게 승온되지 않는 조건으로 보관하여야 한다. 안정성 데이타에서, 3달 이상 시간에 따라 또는 온도(실온 및 냉장)에 따른 기능성 차이는 없는 것으로 나타난다. 컬럼은 사용전까지 냉장 조건에서 유지시킨다. 격렬한 진동 및 외상으로 인한 손상과 같은 기계적 손상은 피해야 한다. 이러한 요건을 벗어난 조건에서 보관한 컬럼은 사용하면 안 된다.
운반 조건
컬럼 디바이스는 냉장 조건 하에 냉동과 상기 조건 보다 더 높은 온도를 피하는 조건 하에서 운반한다. 격렬한 진동 및 외상으로 인한 손상과 같은 기계적 손상은 방지해야 한다.
실시예 10 - 비임상 실험
개요
일찍이 1970년대에, 공여체 양의 장간막 림프절로부터 수득한 림프구가 수여 동물에 이식된 후 장 축적되는 것으로 관찰되었다(4, 5). 이러한 최초의 동물 실험에서, 체내 여러가지 구획으로 타겟팅된 순환성 림프구들의 특이적인 회귀 능력이 제시되었다. 뮤라인 모델을 이용한 추가적인 연구에서, 여러가지 T-세포 서브세트의 장기 특이성에 관여하는 몇 가지 시그널링 경로가 확인되었다. L-셀렉틴(또한, CD65L이라 함)은 림프구가 장간막 림프절로 이동하는데 관여하는 세포 표면 단백질인 것으로 확인되었다(6). 장 혈관의 내피 세포 라이닝에서, MadCAM1과 TECK가 점막-결합성 림프구와 단핵구들의 유착 및 이동과 관련있는 것으로 밝혀졌다. 연구들로, 면역세포의 대응되는 수용체들 α4β7 및 CCR9 각각에 대해 주의를 기울이게 되었다(2). 이러한 맥락에서, 크론 질환에 대한 한가지 마우스 모델, TNFDeltaARE에서, α4β7 경로는 TECKCCR9-의존적인 경로와 독립적으로 작동되는 것으로 제안되었고, 장-회귀의 주요 기저 메카니즘인 것으로 추정되었다(7, 8). 그러나, 다른 마우스 모델에서는, TECK-CCR9 상호작용이 염증이 발생된 점막으로의 장-회귀 측면에서 동일하게 중요한 것으로 나타났다. TECK-/- 및 CCR9-/- 뮤라인 모델 뿐만 아니라 TECK-CCR9 결합의 항체-매개 저해에서, 점막 염증이 약화되는 것으로 확인되었다(9-12). 그래서, 여러가지 회귀 메카니즘의 효과는 선택된 동물 모델에 의존적인 것으로 보인다. 몇가지 뮤라인 모델 연구에서 CCR9-발현성 T-세포는 소장 선호성을 나타내었다. 그러나, 궤양성 대장염 마우스 모델인 MDR1a-/-에서 관찰되는 바와 같이, 결장에 한정된 점막 염증은 CCR9-발현성 림프구 의존성을 나타내었다. CCR9-차단 단백질 CCX282-B를 투여한 후, 결장의 염증 병소는 확실하게 해소되는데, 이는 결장 점막에서 TECK-CCR9 상호작용이 중요한 역할을 한다는 것을 시사한다(3). TECK-CCR9 회귀 메카니즘의 치료학적 의미는 인간을 대상으로 한 몇가지 연구들에서 확인되었으며, 마우스에서와 같이, CCR9-발현성 T-세포는 인간의 소장에 축적되는 것으로 밝혀졌다(2, 3, 13). CD 환자의 경우, 건강한 대조군과 비교하여, 장간막 림프절에 CCR9-발현성 림프구가 현저하게 증가되었다(13). 추가적인 연구를 통해, CD 또는 UC를 앓고 있는 환자의 결장 염증이 발생한 점막에서, TECK 및 CCR9-발현성 T-세포의 존재도 확인되었다. 건강한 대조군도 결장 점막에 CCR9를 발현하는 면역 세포를 가지고 있는 것으로 확인되어, 장-관련 면역 시스템의 정상적인 기능에도 이러한 수용체가 중요한 역할하는 것으로 확인되었다(3). 염증성 장 질환에서, 이용가능한 동물 모델들은 인간 장 염증과 구체적으로 상당히 대응되진 않는다. 따라서, 비임상적인 개념 테스트 입증을 위해, IBD 환자로부터 수득한 혈액 샘플은 시험관내 실험에 집중적으로 사용되었다. 또한, 백혈구 분리 반출술에 사용되는 bTECK 단백질은 인간 CCR9 표면 단백질에 특이적이므로, 생체내 동물 효능 검사를 실행하는데 제한 요소가 된다.
타겟 세포 집단의 시험관내 제거
CCR9를 발현하는 세포를 제거하는 능력을 평가하기 위해, bTECK 커플링된 매트릭스에 대해 시험관내 테스트를 수행하였다. 혈액 공여자와 IBD 환자들로부터 혈액을 채혈하여, bTECK 커플링된 매트릭스가 포함되어 있는 컬럼 디바이스를 통과시켰다. 컬럼 통과 전과 후에 혈액 샘플을 취하여, CCR9 발현 세포의 제거를 유세포 측정(FACS)으로 분석하였다.
그 결과, 매트릭스 관류 후 타겟 집단인 CD14-양성의 CCR9 발현 세포가 현저하게 제거되는 것으로 입증되었지만, 전체 CD14-양성 세포는 변화없이 유지된다. 제거 테스트는 건강한 공여자와 IBD 환자로부터 취한 혈액을 대상으로 수행하였고, 비슷한 효과를 검증하였다. 결과들은 도 7 및 8에 각각 나타낸다.
결론적으로, 시험관내 결과는 CCR9 발현 세포의 50-75%가 컬럼에 의해 특이적으로 줄어든다는 것을 보여준다. CCR9를 발현하지 않는 세포는 영향없이 유지되었다.
실시예 11 - 독성 평가 및 안전성 테스트
노출
컬럼 디바이스의 환자 노출은 2가지 다른 방식으로 이루어질 수 있다. 첫번째는, 국지적인 혈액 및 이의 세포가 디바이스에서 bTECK 등의 화학물질에 노출되는 것이고, 두번째는 디바이스로부터 분비되어 회귀하는 혈액을 통해 환자에게 투여되는 bTECK 등의 화학물질에 전신 노출되는 것이다. 2가지 경우 모두, 전체 노출을 평가할 수 있을 가능성은 제한적이지만, 매트릭스 안정성 실험을 통해 세파로스, 스트렙타비딘 및 bTECK에 대한 전신 노출이 밝혀지게 될 것이다. 하기를 참조한다. 그러나, 플라스틱 및 필터 물질은 조사에 의한 무균 처리를 한 후에도 FDA/ISO 10993 표준과 USP 클래스 VI의 생물 평가 요건을 충족시키고 있기 때문에, 디바이스의 이러한 부분으로부터의 독성 화합물 노출은 미미한 것으로 결론내릴 수 있다. 또한, 컬럼의 여러가지 구성 요소들 간의 어떠한 상호작용을 시사하는 데이타는 없다.
매트릭스 안정성
매트릭스의 안정성 특성을 실험하여, 컬럼의 활성 테스트시 임의의 물질 누출이 이루어지는 지를 조사하였다. 매트릭스가 충전된 컬럼을 펌프 시스템에서 2L PBS (포스페이트 완충화된 염수)로 30-100ml/분으로 헹궈, 제조 단계에서 생성된 잔류 입자들을 제거하였다. 컬럼 전과 후의 유체 샘플을 취하여, 현미경 분석하고, 생성물의 누출을 ELISA로 분석하였다.
컬럼을 2 L PBS로 헹군 후 매트릭스 물질의 가시적인 누출은 관찰되지 않았다.
결합 안정성을 ELISA로 검사하였다. 탈착된 bTECK를 검출하기 위해, 웰을 스트렙타비딘 항체와 1시간 동안 4℃에서 인큐베이션하였다. 탈착된 스트렙타비딘을 검출하기 위해, 웰을 바이오틴화한 퍼옥시다제와 1시간 동안 실온에서 인큐베이션하였다. 실험 결과, 세파로스 입자, 스트렙타비딘 또는 bTECK는 매트릭스에서 누출되지 않는 것으로 나타났다.
생물학적(독성) 데이타
원하는 생물학적 효과, 위장관을 표적으로 하는 활성화된 백혈구(장-회귀 세포)의 특이적인 제거는 강력한 수용체-리간드 친화성에 의해 세포 상의 특이적인 수용체 CCR9에 대한 bTECK 공격 및 결합에 의해 야기된다. 이 수용체를 발현하지 않는 혈액 세포는 컬럼을 통과한 다음 환자에게로 되돌아 간다. 또한, 컬럼을 의도된 용도로 노출시키면 다양한 생물학적인 부작용 (독성)을 야기할 수 있어, 이러한 카테고리의 의료 디바이스는 ISO 10993-1에 따라 조사되어야 한다. 혈액이 컬럼 구역 상에 균일하게 배분되었을 때 추정되는 국지적인 노출을 근거로, 혈액, 특히 이의 세포에 부정적으로 작용될 가능성이 있다. 디바이스에서 케모카인 bTECK 또는 임의의 화학물질이 세포독성 및 혈액부적합성(haemoincompatibility)과 같은 국지적인 효과를 야기할 가능성이 있다. 또한, 면역 세포에 대한 임의의 활성화를 조사하는 것이 가장 중요하다. 또한, 환자는, 관류하는 동안에 컬럼 디바이스나 플라스틱 배관으로부터 분리된 bTECK 또는 임의의 화학물질에 전신 노출되어, 생물학적 (독성) 효과가 발생될 수 있다. 이러한 화학물질들은 ISO 10993-1에 따라 평가되어야 하는 세포독성, 감작, 자극 및 진피내 반응성, 전신 독성(급성), 아급성 및 만성 독성 및 혈액부적합성 등의 다양한 전신 효과를 야기할 가능성이 있다. 9 kDa의 바이오틴화한 절단형 버전으로 합성된, bTECK의 생물학적 작용을 확인하기 위해, 여러가지 시험들을 수행하였다.
특이적인 세포 제거 및 FACS(형광 활성화된 세포 분류)에 의한 시험관내 분석
IBD 환자의 혈액을 대상으로 매트릭스와 bTECK를 이용한 세포 제거 테스트를 수행하기 위해, 실물 크기의 컬럼 디바이스를 이용한 과정을 시뮬레이션하기 위한 소규모의 툴을 사용하였다. 시뮬레이션은, 플라스틱 배관의 상단에 나일론 필터 세트를 구비하여 이루어졌다. 혈액을 매트릭스와 조심스럽게 혼합하고, 필터를 통과시키고 채집관으로 이동시켰다. 필터를 통과시키지 않은 혈액 샘플과 필터를 통과시킨 혈액 샘플을 용혈한 후, 항체로 염색하고, FACS로 추가적으로 분석하였다.
혈액 공여자로부터 수득한 혈액 샘플을 모우고, bTECK가 커플링된 매트릭스가 함유되어 있는 컬럼 기본형을 이용하여 세포 제거 테스트를 수행하였다. 샘플은 컬럼 통과 전과 후에 취한 다음, 용혈시키고, 항체로 염색한 다음 FACS로 분석하였다.
bTECK 수용체를 발현하는 세포의 특이적인 제거는 소규모 툴 뿐만 아니라 컬럼 디바이스 기본형 둘다에서 성공적으로 달성되었다. 또한, CCR9를 발현하는 세포 집단이 특이적으로 제거되는 것을 확인할 수 있었다. 예컨대, 도 8 및 7에서, CCR9 양성 세포인 CD14 및 림프구 세포들(CD4 및 CD8)이 현저하게 감소되어, 컬럼을 통과한 수는 1/5 미만이었지만, CD14 및 림프구 집단의 전체 카운트는 컬럼 통과 후에도 변하지 않았다.

컬럼 디바이스를 통과한 세포의 활성화 및 기능 특성을 연구하기 위한 것이다.
활성화 마커
용혈시킨 세포를 15분간 실온에서 10% HUS (인간 항체 혈청)와 인큐베이션하여, 세포의 표면 상에서의 비특이적인 Fc-수용체 결합을 예방하였다. 세포를 활성화 마커로 착색하였다: CD69 (림프구), CD66b (과립구) 및 HLA-DR (단핵구). 세포를 FACSAria에서 채집하여, FACSDiva 소프트웨어로 분석하였다. 활성화 마커를 이용하여 테스트한 세포의 수는, 컬럼 통과 후와 전이 동일하였다(도 9).
사이토카인 분비
염증성 사이토카인의 분비 검사로서, 인터페론 γ(IFN γ) 분비 분석 키트를 이용하여, 매트릭스와의 접촉 전 과 후의 세포의 활성화를 조사하였다. 분비 검사는 자극 후에 IFNγ를 생산하는 세포의 표현형과 양을 나타낼 것이다. 4명의 혈액 공여자로부터 수득한 말초 단핵구 혈액 세포(PBMC)를 피콜 분리하였다. 세포를 세포 배양 배지(1% Pest + 1% L-Glut + 5% HUS이 첨가된 RPMI)에 1x106 세포/ml로 재현탁하였다. 매트릭스를 PBS로 헹구고, 0.1 ㎍/ml의 bTECK와 혼합하였다. 세포 현탁액의 절반은 매트릭스가 구비된 소규모 툴을 통과시켰다. 필터 비통과 세포와 필터 통과 세포를 48웰 플레이트에 500 000/웰로 넣고, 16시간 동안 37℃에서 인큐베이션하였다. PMA (50 ng/ml) + 이오노마이신 (1 ㎍/ml)을 세포에 양성 대조군으로 첨가하였다. 16시간 후, 세포에서 표면 결합된 IFNγ와 유리형 IFNγ의 양을 분석하였다. FACS 분석을 위한 다른 Mab들로 CD3, CD14 및 DAPI를 사용하였다. IFNγ 분비에 대한 유의한 변화는 관찰되지 않았다(도 10).

1인의 혈액 공여자로부터 수득한 헤파린 처리한 전혈액으로부터 피콜 분리에 의해 PBMC를 분리 및 준비하였다. 세포를 계수하고, 세포 배양 배지(RPMI+ 10% BGS+ 1% pest 및 1% L-glut)에 2x106 세포/ml로 희석하였다. 세포 현탁액의 절반은 bTECK 200 nM (0,2 ㎍/ml)이 커플링된 매트릭스(SA 농도 4 mg/ml)를 구비한 소규모 툴에 통과시켰다. 2x106 세포/ml의 세포 현탁액 (100 000 개의 세포)을 50 ㎕/웰로 프로토콜에 따라 96웰 플레이트에 3 세트로 넣었다. 세포 배양 배지 50 ㎕/웰을 음성 대조군으로 사용하였고, 50 ㎕/웰의 PHA (피토헴어글루티닌) 항원 (5 ㎍/ml)을 양성 대조군으로 사용하였다. 세포는 2일, 3일 및 4일까지 37℃에서 인큐베이션하였다. 세포를 수확하기 전에, 티미딘 [3H] 25 ㎕을 웰에 첨가하고, 18시간 동안 37℃에서 추가로 인큐베이션하였다. 18시간 후, 세포를 수확하여, 신틸레이터로 병합된 [3H]을 측정하였다. 세포에서의 증식의 신호로서의 [3H] 병합은 컬럼 전과 후에 동일하였다(도 11).
아넥신 V를 이용한 세포 사멸-세포자살 분석
피콜 분리에 의해 3명의 혈액 공여자로부터 PBMC를 분리하였다. 세포를 PBS로 2번 헹구고, 배양 배지(1% L-glut, 1% pest + 10% BGS가 첨가된 RPMI)에 1x106/ml의 농도로 재현탁하였다. 세포를 5 ㎍/ml 또는 10 ㎍/ml의 bTECK로 자극시켰다. 세포를 24웰 플레이트에서 16시간 인큐베이션하였다. 양성 대조군으로서 덱사메타손(1 μM)을 사용하였다. 세포를 PBS로 2번 헹구고, 아넥신 V로 BD 아넥신 V 키트 프로토콜에 따라 염색하였다. FACS 분석하기 전에, 세포에 100 ㎕의 DAPI를 첨가하였다. 샘플을 FACS Aria에서 FACS Diva 소프트웨어로 분석하였다. 세포자살 세포는 아넥신 V 양성 및 DAPI 음성으로 정하였다. 아넥신 V 양성 세포의 수는 5 또는 10 ㎍/ml bTECK에 노출 후에 현저하게 증가되지 않았다(도 12).
요약
컬럼 디바이스 통과 전과 후, 또는 bTECK가 커플링된 스트렙타비딘 세파로스 매트릭스와 직접 접촉시킨 후, 세포 실험을 수행하였다. 그 결과, 매트릭스 및 bTECK와 접촉된 세포에는 영향이 없거나 미미하였다. 활성화 마커를 구비한 CD69(림프구), CD66b(과립구) 및 HLA-DR(단핵구) 세포의 수는, 컬럼 통과 전과 후에 동일하였다. 독성 연구
시험관내 세포독성 분석
배양한 포유류 세포(L929 마우스 섬유모세포)에 대한 시험관내 세포 독성에 대해 컬럼 매트릭스를 테스트하였다. 테스트는 ISO 10993-5 용출 테스트 가이드라인에 따라 수행하였다. 테스트 품목은 20% 에탄올 중의 코팅된 아가로스 비드 슬러리로 제공하였다. 컬럼 매트릭스를 의도한 용도로 사용하기 전에 세정하는 바와 같이, 아가로스 비드를 헹군 다음 동일 부피의 무균 등장성 식염수(0.9% NaCl)에 재현탁하여, 테스트하기 전에 에탄올을 제거하였다. 세정된 테스트 품목을 완전 세포 배양 배지(10% 소 태아 혈청 및 50 ㎍/ml의 젠타마이신이 첨가된 HAM F12 배지)에서 24시간 동안 37℃에서 천천히 혼합하면서 인큐베이션하여, 컬럼의 추출물을 준비하였다. 배지 ml 당 테스트 품목 0.2 ml의 추출 비율(ca. 0.2 g/ml)을 사용하였다. 추출물은 희석하지 않은 경우와 이를 신선한 세포 배양 배지에 1 : 3으로 희석한 경우로 테스트하였다. 음성 대조군(폴리프로필렌 추출물, 6 cm2/ml), 양성 대조군(tin-안정화된 폴리비닐 클로라이드 추출물, 0.3 cm2/ml) 및 완전 세포 배양 배지를 처리한 무처리 대조군 배양물을 포함시켰다. 3세트의 세포 배양물을 48시간 동안 각 테스트 시점에 처리하였다. 대조군 처리시 적절한 반응이 형성되었는데, 이는 테스트 시스템이 바르게 기능하며 민감성을 갖추고 있다는 것을 의미한다. 희석하지 않은 컬럼 추출물과 희석한 컬럼 추출물 모두 독성을 나타내지 않았다(독성 등급 0).
용혈 테스트
컬럼 매트릭스에 대해 시험관내 용혈 활성(적혈구의 용혈) 검사를 수행하였다. 용혈 테스트는 ISO 10993-4 가이드라인의 요건에 따라 수행하였다. 테스트는 미국 테네시에 위치한 재료 과학 협회(MSI)의 권고안(1979)에 따라 설계하였고, 테스트 품목을 무균 식염수 중의 토끼 혈액의 희석 혼합물과 직접 접촉시켰다. 테스트 품목은 20% 에탄올 중의 코팅된 아가로스 비드 슬러리로 제공하였다. 컬럼 매트릭스를 의도한 용도로 사용하기 전에 세정하는 바와 같이, 아가로스 비드를 헹군 다음 동일 부피의 무균 등장성 식염수(0.9% NaCl)에 재현탁하여, 테스트하기 전에 에탄올을 제거하였다. 테스트 품목을 식염수 ml에 대한 테스트 품목 0.2 ml의 비율(ca. 0.2 g/ml)로 무균 등장성 식염수에 넣었다. 이를 37℃에서 39분간 인큐베이션한 다음, 토끼 혈액(20 ㎕ 혈액/ml 식염수)을 첨가하여, 다시 60분간 계속 인큐베이션하였다. 음성 대조군(등장성 식염수) 및 양성 대조군(증류수)도 포함시켰다. 모든 처리는 3세트로 수행하였다. 인큐베이션 종료시, 혼합물을 5분간 500 x g로 원심분리하였다. 그런 다음, 상층액의 흡광도는 545 nm에서 측정하였다. 용혈율을 계산하였다. 본 실험에서 사용한 조건에서, 컬럼 매트릭스가 처리된 혈액 샘플에서 관찰되는 평균 용혈율은 -0.%였다. 즉, 컬럼 매트릭스는 MSI 용혈 테스트 요건(용혈율 < 5%)을 통과하는 것으로 결론지었다.
인간 혈중 응고 테스트
인간 혈액 샘플의 시험관내 응고율에 대한 효과를 컬럼 매트릭스를 대상으로 테스트하였다. 테스트 품목은 20% 에탄올 중의 코팅된 아가로스 비드 슬러리로 제공하였다. 컬럼 매트릭스를 의도한 용도로 사용하기 전에 세정하는 바와 같이, 아가로스 비드를 헹군 다음 동일 부피의 무균 등장성 식염수(0.9% NaCl)에 재현탁하여, 테스트하기 전에 에탄올을 제거하였다. 세정한 테스트 품목의 샘플(0.2 ml)을 시험관에 넣었다. 음성 대조군(무처리) 및 양성 대조군(훌러스 어스(Fuller's Earth)) 시험관도 준비하였다. 신선한 인간 혈액 (1 ml)을 각 시험관에 첨가하였다. 테스트 품목은, 혈액 ml 당 약 0.2 ml의 비율(ca. 0.2 g/ml)로 하였다. 시험관을 약 37℃의 수조에 넣고, 일정하게 교반하였다. 혈액 전체가 응고되는데 걸리는 시간을 기록하였다. 테스트 품목과 각 대조군을 4명으로부터 수득한 각 혈액으로 1회 테스트하였다. 대조군의 처리 결과로, 테스트 시스템의 유효성과 민감성이 검증되었다.
매트릭스로 처리한 혈액의 평균 응고 시간은 평균 음성 대조군 결과의 91%로 약간 단축되었다. 그러나, 4명의 공여자들 간의 개별 테스트 편차가 매우 커서, 이러한 단축은 유의한 것으로 간주되지 않는다. 이에, 컬럼 매크릭스는 본 테스트에서 인간 혈액의 응고 시간에 영향을 미치지 않는 것으로 결론내릴 수 있다.
요약
수행한 테스트 결과와 컬럼 평과 결과를 기초로, 독성이 매우 낮고, 타겟 장기에 대한 특이적인 타입의 독성은 없는 것으로 결론내릴 수 있었다.
실시예 12 - TECK-PEG-바이오틴 합성의 요약
타겟 분자:
Lys72의 ε-아미노 측쇄 작용기에서 PEG-바이오틴으로 유도체화된 TECK (Met이 Nleu로 치환됨)(TFA 염)
변형:
케모카인의 폴딩 서열을 포함하는, 성숙 단백질의 1-74번 잔기에 대응되는, 절단형의 인간 TECK. 전장 길이의 성숙 단백질은 127개의 아미노산(150개의 아미노산으로 이루어진 미성숙 단백질에서 신호 펩타이드는 23개의 아미노산임)이다. 이 서열내 단일 메티오닌을 노르루신으로 변형시켜, 천연 서열 유도체의 합성시에 관찰되는, 체인의 조립시 이 잔기가 산화되는 것을 경감시킨다. 단백질의 N-말단 위치의 Gln을 생리학적 조건 하에서 pyroGlu로 만들었다. 그래서, 이 서열의 Gln1을 피로글루타민으로 치환시켜, 제조 중에 N-말단의 Gln 및 proGlu 종이 혼성되는 것을 방지하였다. 이는 합성 수율을 개선시키고, 컬럼 제조 및 사용시 케모카인의 균일성을 보장한다. 72번 위치의 천연 라이신을 수지에서 바이오틴화를 통해 변형시켰다. Dde 보호기 제거:
모든 수지(2.5 g)에 DMF 중의 2% 하이드라진 용액을 1시간 동안 처리하여, Dde 보호기를 제거하므로써, 2.0 g의 수지를 수득하였다.
표지 단계:
1. Fmoc-8-아미노-3,6-디옥탄산의 커플링
수지(1.5g)를 DMF (2 ml)에서 부풀린 다음, Fmoc-8-아미노o-3,6-디옥탄산 (0.38 g, 1 mmol), DIC 용액 (2 ml, DMF 중의 0.2 M) 및 HOCt 용액 (2 ml, DMF 중의 0.2 M) 용액을 첨가하였다. 혼합물에 2시간 음파처리한 다음, DMF로 헹구었다.
2. 캡핑
5분간 0.5 M 아세트 안하이드라이드/DMF 용액(20 ml)을 처리하여 수지를 캡핑하고, DMF로 헹구었다.
3. Fmoc의 탈보호
DMF 용액(2 x 50 ml) 중의 20% 피페리딘을 각각 15분씩 처리하여, Fmoc 탈보호를 수행하였다. 수지를 DMF로 헹구었다.
4. 바이오틴-OSu의 커플링
DMF(10 ml) 중의 바이오틴-NHS 에스테르 (341 mg, 1 mmol) 및 DIPEA (348 ㎕) 용액을 수지에 첨가하고, 혼합물을 3시간 음파 처리하였다. 수지를 DMF 및 DCM으로 잘 헹구고, 진공 건조하였다. 수지를 건조하여 1.5 g을 수득하였다.
절단:
건조한 펩타이드 수지 (1.5 g)와 혼합물을 TIS, 티오아니솔, 물, EDT 및 페놀로 구성된 포착 칵테일이 포함된 TFA (30 ml)로 절단하고, 혼합물을 실온에서 6시간 교반하였다. 용액을 차가운 에테르로 여과하고, 수지를 TFA로 헹구었다. 펩타이드를 원심 분리하고, 에테르로 헹군 다음, 원심 분리한 후 동결 건조하여, 조산물 펩타이드 1.0 g을 수득하였다.
폴딩 프로토콜:
조산물 펩타이드 (100 mg)를 6 M GnHCl (233 ml)에 용해한 다음, 0.5 mM GSSG 및 5 mM GSH가 포함된 50 mM TRIS pH8 (467 ml)을 첨가하여, 2 M GnHCl로 신속하게 희석하였다. 혼합물을 실온에서 2.5일간 교반한 다음, HPLC (Jupiter C18, 250 x 4.6mm 컬럼, 30분간 10-60% B)로 분석하였다. HPLC 분석으로, 원하는 산물의 형성과 잘못 폴딩된 부산물이 확인되었다.
정제:
폴딩된 단백질을 Jupiter (18, 250 x 21mm 컬럼, 9 ml/min, 50분간 10-60% B)를 이용하여 역상 HPLC로 정제하였다. 이로써 폴딩된 순수한 Nle-TECK-바이오틴 11.1 mg을 수득하였다.
도 13은 폴딩된 순수한 바이오틴-TECK(Nleu)의 HPLC를 나타낸다. 단백질을 21.6분에 단일 피크로 용출되었다.
도 14는 정제된 폴딩된 바이오틴-TECK(Nleu)의 전자분사 이온화 및 탠덤 질량 분광분석(ES/MS) 데이타이다. 질량은 8959.4 Da로 예상되었다.
기능 분석 데이타:
hCCR9에 대한 Aequorin 분석(Euroscreen)으로 작용자의 활성을 TECK-바이오틴-Nle에서 테스트하였고, EC50 수치는 63.6 nM로 기록되었으며, 이와 비교하여 천연 TECK의 EC50은 67.87nM이었다.
