Abstract

본 발명은 글리세롤 트리니트레이트 또는 화학적으로는 1,2,3-프로판트리올트리니트레이트로 표현되는 니트로글리세린을 함유하여 임신한 포유동물에서 원치 않는 진통작용을 억제하기 위한 경피투여형 의약품에 관한 것이다. 본 발명의 의약품을 사용함으로써 원치않는 진통작용을 억제하고, 조산을 억제하고, 진통작용을 조절할 수 있다.

Description

원치않는 진통작용을 억제하고, 조산을 억제하고, 진통작용을 조절하기 위한 경피투여용 의약품
글리세롤 트리니트레이트 (GTN)로도 알려져 있는 니트로글리세린은 다양한 적용분야를 갖는, 오랫동안 공지되어온 화학물질이다. 화학적으로 GTN은 1,2,3-프로판트리올 트리니트레이트로 표기된다. 특히, 니트로글리세린은 관상심장질환을 치료하는데 유용한 물질이다. 이러한 중요성으로 인하여, 다수의 약제학적 제제들을 이용할 수 있게 되었다. 현재까지 이 물질의 약제학적 제제에는 정제, 캅셀제, 연고제, 액제, IV용 액제, 분무제 및 경피투여용 패취제가 포함된다.
본 발명은 니트로글리세린을 사용하여 포유동물에서 원치않는 진통작용을 억제하기 위한 경피투여용 의약품에 관한 것이다.
놀랍게도, 본 발명에 이르러 니트로글리세린을 경피투여형으로 사용하면 포유동물에서 원치않는 진통작용을 억제할 수 있다는 것이 발견되었다.
동물 및 인간을 사용한 연구로, 주어진 피검자에서 니트로글리세린의 이러한 약제학적 제제를 니트로글리세린의 사용목적에 따라 사용함으로써 진통을 중지시킬수 있다는 것이 밝혀졌다.
이하에서는 인간에게서 수행된 연구를 참고로하여 본 발명을 더욱 상세하게 기술한다.
본 연구에서는 각각 조기 진통작용의 징후를 나타내는 18 명의 임부가 포함되었다. 그들은 이미 임신이 진전된 상태에 있었는데, 즉 그들은 이미 임신 25 주 내지 34 주 사이에 있었다. 사용된 GTN 패취제는 각각 공지의 진통-억제물질인 리토트린 대신에 적용하였다. 임신한 모든 피검자들은 다음과 같은 특징을 나타내었다:
- 이들의 태포는 아직 파열되지 않았다;
- 이들은 조기 출혈을 일으키지 않았다;
- 태아에 이상이 없었다.
환자들에게는 각각 1,2,3-프로판트리올 트리니트레이트 10 또는 10.8 mg을 함유하는 GTN 패취제를 1 일에 동시에 2개 까지 적용하였다. 패취제의 표면적은 데포니트 (DEPONITR) 10을 사용한 경우에는 32 ㎠였고, 슈바르쯔 파르마사 (Schwarz Pharma A.G.)에 의해서 제조된 신규한 GTN 패취제를 사용한 경우에는 16 ㎠였다.
후자의 패취제는 교차결합된 아크릴레이트 비닐아세테이트 코폴리머를 기본으로하는 접착제 조성물 중에 니트로글리세린을 함유한다.
어떤 경우든지 사용시의 결과에 있어서는 뚜렷한 차이가 없다. 두가지 패취제는 모두 조기 진통작용을 억제하거나, 조기 진통작용을 중지시키거나, 불규칙적인 진통작용을 종결시킨다.
표 1 및 2는 경피용 약제학적 제제 중의 니트로글리세린이 임신한 포유동물에서 원치않는 조기 진통작용을 억제하는 것을 보여주는 것이다.
표에서 볼수 있는 바와 같이, 진통작용은 패취제의 적용 후에 중지되었다.
환자중의 어느 누구도 원치않는 조산을 겪지 않았다. 출산예정일 까지 니트로글리세린을 함유하는 패취제를 용량-의존적 방식으로 투여함으로써, 강력한 진통작용을 중지시키거나 불규칙적인 진통작용을 조절할 수 있었다.
실시예 1
유럽특허공보 EP 0 144 486 B1의 실시예 1에 상세히 기술된 방법에 따라 패취제를 제조하였다.
본 발명에 따르는, 3층 저장구조 (three-layer reservoir structure)를 갖는 약제학적 생성물은 다음과 같이 생성되었다:
니트로글리세린을 함유하는 압력-민감성 접착제 조성물을 다음의 성분들로부터 제조하였다:
폴리이소부틸렌 (평균분자량 900,000 내지 1,400,000; 시판제품 오파놀 (OPPANOLR) B 100) 0.175 kg;
고체 지방족 탄화수소수지 (시판제품 피코택 (PICCOTAC) CBHT) 0 157 kg;
수화된 로진 (시판제품 아비틀 (ABITOL)) 0.157 kg;
중쇄 트리클리세라이드 (시판제품 미글리올 (MIGLYOLR) 812) 중의 니트로글리세린 5% 용액 0.0105 kg; 및
용매로서 석유스피리트 (petroleum spirit) 80-110 1.174 kg.
생성물을 한면에는 알루미늄이 증착되어 있고, 양면은 접착방지적으로 설치되어 있는 보호층에 용매가 증발한 후에 약 20 g/㎡의 층이 수득되도록 하는 방식으로 적용시켰다. 이렇게하여 수득된 압력-민감성 접착층에 제 1 저장층을 표면적 단위 당 중량을 약 20 g/㎡으로 하여 피복물 (coating)로서 적용하였다.
이 저장층의 제조는 이하의 성분들을 함유하는 분산액을 적용함으로써 수행되었다:
니트로글리세린과 락토즈의 10% (G/G) 분쇄혼합물 0.05 kg;
폴리이소부틸렌 (평균분자량 900,000 내지 1,400,000, 시판제품 오파놀 (OPPANOLR) B 100) 0.153 kg;
고체 지방족 탄화수소수지 (시판제품 피코택 (PICCOTAC) CBHT) 0.137 kg;
수화된 로진 (시판제품 아비틀 (ABITOL)) 0.137 kg;
중쇄 트리클리세라이드 (시판제품 미글리올 (MIGLYOLR) 812) 중의 니트로글리세린 5% 용액 0.01 kg; 및
용매로서 석유스피리트 (petroleum spirit) 80-110 1.148 kg.
생성물을 비접착지 (antiadhesive paper)에 적용하고, 그후 분산제를 증발시켰다.
유사하게 다음의 성분들로부터 표면적의 단위당 중량이 약 50 g/㎡인 제 2저장층을 생성시켜 제 1 저장층 상에 피복물로서 적용하였다:
니트로글리세린과 락토즈의 10% (G/G) 분쇄혼합물 0.6 kg;
고체 지방족 탄화수소수지 (시판제품 피코택 (PICCOTAC) CBHT) 0.2 kg;
수화된 로진 (시판제품 아비톨 (ABITOL)) 0.2 kg;
용매로서 중쇄 트리클리세라이드 (시판제품 미글리올 (MIGLYOLR) 812) 0.025 kg; 및
용매로서 석유스피리트 (petroleum spirit) 80-110 1.876 kg.
유사한 방식으로, 제 3 저장층을
니트로글리세린과 락토즈의 10% (G/G) 분쇄혼합물 2.5 kg;
폴리이소부틸렌 (평균분자량 900,000 내지 1,400,000, 시판제품 오파놀(OPPANOLR) B 100) 0.657 kg;
고체 지방족 탄화수소수지 (시판제품 피코택 (PICCOTAC) CBHT) 0.77 kg;
수화된 로진 (시판제품 아비톨 (ABITOL)) 0.77 kg; 및
용매로서 석유스피리트 (petroleum spirit) 80-110 1.148 kg으로부터 제조하여 표면적의 단위당 중량이 약 200 g/㎡에 도달하도록 하였으며, 3개의 연속단계에서는 비접착지 상에 대한 분산액의 적용이 수행된다. 이렇게하여 제조된 제 3 저장층은 제 2 저장층 상에 피복물로서 적용된다.
유사한 방식으로, 표면적의 단위당 중량이 약 20 g/㎡인 압력-민감성 접착제 중간층을 다음의 성분들로부터 제조하여 제 3 저장층에 피복물로서 적용하였다.
폴리이소부틸렌 (평균분자량 900,000 내지 1,400,000, 시판제품 오파놀 (OPPANOLR) B 100) 0.179 kg;
고체 지방족 탄화수소수지 (시판제품 피코택 (PICCOTAC) CBHT) 0.16 kg;
수화된 로진 (시판제품 아비톨 (ABITOL)) 0.16 kg; 및
용매로서 석유스피리트 (petroleum spirit) 80-110 1.167 kg.
압력-민감성 접착제 중간층을 불투과성 커버층 (cover layer)으로 피복시킨 후에, 생성된 라미네이트를 치료학적 필요에 적합하도록 개개 절편으로 절단하였다.
니트로글리세린 패취제는 다음과 같은 구조를 갖는다. 이것은 폴리에스테르 필름으로 외부가 피복되어 있으며, 다층의 의약품-부하된 접촉필름, 폴리이소부틸렌, 고체 지방족 탄화수소수지 및 수화된 로진을 함유하는 매트릭스 시스템이다.
32 ㎠의 패취제는 글리세롤 트리니트레이트 32 mg을 함유한다. 피부에 대한 활성 성분의 평균방출량은 24 시간당, 10 mg이다. 경피시스템의 방출면적은 실리콘-처리되고 금속화된 폴리에스테르 필름에 의해서 보호되며, 이것은 적용하기 전에 제거된다.
실시예 2
패취제는 PCT/EP 92/01169 (= WO 92/22292 A1)의 실시예 3의 구체예에 상세히 기술된 바와 같이 제조하였다.
이 니트로글리세린 패취제는 다음의 구조를 갖는다: 이것은 폴리에스테르 필름으로 외부가 피복되어 있으며, 교차결합된 아크릴레이트 비닐아세테이트 코폴리머를 기본으로하는 니트로글리세린-함유 접착제 조성물을 함유하는 매트릭스 시스템이다.
16 ㎠의 패취제는 글리세롤 트리니트레이트 40 mg을 함유한다. 피부에 대한 활성성분의 평균방출량은 24 시간당, 10.8 mg이다. 경피시스템의 방출면적은 폴리에스테르 필름에 의해서 보호되며, 이것은 적용하기 전에 제거된다.
