혈관동력학적인 활성을 갖고 있는 화합물이 활성 성분으로 포함되어 있는 것을 특징으로 하는, 상처를 아물게 하는 활성을 갖는 약물이 개시되어 있다.

Description

흉터를 아물게 하는 활성을 가진 국소 약물
본 발명은 흉터를 아물게 하는 활성을 갖고 있는 국소 약물에 관한 것으로서, 상세하기로는 상처 및 흉터 치료에 유용한 국소 약물을 제조하기 위해 혈관동력학적 활성을 가진 화합물을 이용하는 것에 관한 것이다.
상처 치료 과정에는 손상된 조직이 신속하게 재생되는 것이 포함된다. 이러한 현상은 손상된 부분을 가능한 빠르게 복구하기 위해 신속하고 무질서한 세포 증식과 관련이 있다. 상처 치료과정은 흉터가 아무는 것(반흔화)으로 종결된다. 즉, 섬유성 ·연결 조직이 형성되어 피부의 손상된 부분을 복구한다. 반흔(瘢痕)성 조직의 빠르고 무질서한 성장으로 인해 일반적으로 보기흉한 켈로이드(Keloid: 상처가 아문 뒤에 돋는 군살)가 생기게 된다. 이 문제점은 특히 성형외과에서 당면하게 되는데, 수술이 아무리 정교하다고 할 지라도 보통 신체 부분에 눈에 띄는 흉터를 종종 남기게 된다.
현재까지 알려진 흉터를 아물게 하는 약물, 예를 들면 콜라겐 또는 항생제 화합물을 각각 포함하는 국소 약물은 회복과정의 자극을 그 기능을 통해 발휘하거나 또는 단지 이 과정에 유리한 환경을 유지시킨다.
본 출원인이 알고 있는 한, 보기흉한 켈로이드 없이 상처를 균일하게 치료하기 위하여 반흔화 과정을 조절하는 문제점은 아직까지 미해결로 남아 있다.
현재 놀랍게도 혈관동력학적인 활성을 갖고 있는 화합물을 봉합후의 외과적 기원의 상처 또는 이미 치료된 흉터에 국부적으로 적용하면 치료시간면에서 현저하게 개선될 뿐만 아니라 흉터의 질이 개선되는 것이 밝혀졌다. 상기 화합물은 환자의 욕창을 방지하거나 치료한다.
혈관동력학적인 활성을 갖고 있는 화합물은 한동안 순환 이상 특히 말초계 순환 이상의 치료제로 알려져 왔다.
본 출원인의 이탈리아 특허 제1,223,290호에는 다불포화성 지방산 유도체가 레이노병(괴저병), 탈모증, 발기불능증같은 기능적 또는 기관 말초성 맥관(脈管)장애의 치료와 미용상 응용에 이용된다는 것이 개시되어 있다.
본 출원인의 이탈리아 특허 제1,233,753호에는 암니 비스나가(Amni visnaga)와 암니 마저스(Amni majus)의 추출 물질 및 반합성 유도체가 미소혈관동력학적 또는 순환활성항진성 활성을 가지고 있다는 것에 대해 개시되어 있다.
본 발명에 포함되어 있는, 혈관동력학적 활성을 갖고 있는 것으로 알려진 다른 물질로는 라우바신(raubasine), 테트라하이드로알스토닌(tetrahydroalstonine)과 그 유도체, 빈카민과 그 유도체 및 에부르나모닌(eburnamonine)과 그 유도체와 같은 빈카알칼로이드(Vinca alkaloids): 에르고트 알칼로이드(Ergot alkaloids)와 그 유도체: 징코 빌로바(Ginkgo biloba)로부터 얻어진 것, 유럽 특허 제0360556호 B1에 따라 크라타저스 에스피.(Cratageus sp. )로부터 제조된 것과 같은 바이오플라보노이드(bioflavonoids)와, 부플로메틸 (buflomedil)이 있다.
일반적으로 혈관동력학적인 활성을 가지고 있는 물질은 본 발명의 목적에 유용한 것으로 믿어진다.
본 발명의 목적은 상처 및 흉터 치료를 위한 국소 약물 제조시에 혈관동력학적인 활성을 갖고 있는 물질을 이용하는 것에 관한 것이다.
혈관동력학적인 활성을 가지고 있는 화합물은 그자제나 천연 또는 반합성 유도체의 형태로 사용될 수 있다.
본 발명의 다른 목적은 상처와 흉터 치료에 유용한 국소 약물의 제조에 혈관동력학적 활성을 가지고 있는 물질을 포함하고 있는 추출물을 사용하는 것에 관한 것이다.
첫번째 바람직한 실시예에서, 본 발명은 암니 비스나가 유도체, 특히 쿠마린 켈린(coumarins khellin)과 비스나딘(visnadine) 보다 바람직하게는 켈린(khellin)의 용도를 제공한다.
다른 실시예에서, 본 발명은 빈카민과 라우바신같은 빈카알칼로이드, 에르고트 알칼로이드, 에틸지메니네이트(ethyl xhimeninate) 및 부플로메딜의 용도를 제공한다.
혈관동력학적인 활성을 가진 화합물의 활성을 평가하기 위해서 환자와 건강한 지원자를 대상으로 한 임상 연구가 실시되었다.
켈린의 중량퍼센트 농도가 0.25, 0.5, 1%인 O/W 처방전이 만들어졌다. 장기간 치료의 경우에는 0.5% 처방이 사용된 반면, 급성평가를 위해서 1% 처방이 사용되었다.
임상학적으로 완전한 상지 피부를 가지고 있어야 하고, 테스트실시전 5일동안은 코르티코스테로이드(corticosteroid) 또는 다른 항염증제를 복용하지 않아야 한다는 판정기준에 따라 선발된 일단의 건강한 지원자들에게(26명의 피검자, 남자 7명, 여자 19명, 연령 22-45세: 평균 연령 33±11.2세) 0.5% 농도로 처방된 4㎠ 크기의 폐색패드를 피부에 착용시키고 3일동안 그 상태를 유지하였다.
3일째 마지막에 패드가 제거되었고 7일 경과후 동일한 조건에서 상기와 같은 처치가 반복되었다. 처치가 끝나면 진정제(올리브 오일)에 대해서 1내지 4의 평가 점수로 레드닝(reddening)이 존재하는 지에 대해 피부를 검사하였다.
이러한 평가에서 0은 반응이 없음을 나타내고 4는 다음날 발생하는 피부의 건조, 표피 탈락 및 소성 감각과 관련되어 레드닝의 매우 강한 반응을 의미한다.
이 화합물들은 아주 내성이 있다는 사실이 판명되었다.
성형수술(모반 등을 제거함)을 한 일단의 6명의 환자에게 1% 농도로 처방한 1g을 봉합한 상처에 바르고 부드럽게 마사지하는 치료가 실시되었다. 환자들의 외과적 상처를 치료하는 과정 동안 모세관의 신생물에 대해 조사하였다. 연구는 모세관현미경검사법(OPCV: Optical Probe Video Capillaroscopy)에 의해 실시되었다. 사용된 기기는 50배, 200배 및 400배 배율의 접안 렌즈가 장착되어 있는 모리텍스 옵티컬프로브 캐필러로스코프 스코프만 504였다(알파 스트루멘티, 이탈리아 밀란). 두가지의 독창적인 방법이 모세관 밀도를 정량적으로 측정하는데 사용되었다. 제1의 방법에서 OPCV 워크 스테이션은 볼링거와 포그렐에 의한 캐피-플로우시스템(Capi-Flow sysyem)에 연결되었다. 제2의 방법에서는 캠브리지의 상 아르키메데스의 디지탈화용 소프트웨어를 사용하여 OPCV 워크 스테이션이 리스크 영상 분석기(RISC image analyzer)와 연결되었다.
또한 레이저 도플러 플로우미터(페이플로우 2b, 스웨덴 페리메드)를 이용한 분석도 실시되었다.
켈린의 국소 투여후에 모세관의 수가 현저하게 증가한다. 혈액의 유속과 혈액량이 실질적으로 증가되어 이전에 비어 있던 모세관이 혈액으로 채워진다. 이러한 현상은 투여후 15분내지 45분까지의 시간내에 관찰될 수 있다.
상기 언급한 물질의 정확한 효과를 평가하기 위해서는 수술을 받은, 예를 들면 2cm보다 큰 상처를 가지고 있는 환자에게 봉합후 1cm는 진정제처방을 하고 1cm는 혈관작용을 갖고 있는 처방으로 조치하였다.
상처에 대한 효과는 도플러로 측정되는 바에 의하면, 고주파수 주기적 변위의 증가와 함께 모세관수가 현저하게 증가하는 것으로 나타났다. 가장 중요한 결과는 새롭게 형성된 모세관이 흉터의 전체길이와 흉터의 반대 측면 지역의 불연속적인 모세관 네트워크를 횡단하는, 길게 연속된 줄의 형태로 피부 표면에 평행하게 배열된다는 것이다.
유사한 결과는 비스나딘, 빈카민, 라우바신, 니세르고린(nicergoline), 에틸지메니네이트, 부플로메딜, 징코 빌로바 및 크라타저스 에스피.로부터 얻어진 바이오플라보노이드같은 다른 화합물을 활성 성분으로 가지고 있는, 본 발명에 의한 국소 약물에의해서도 얻어졌다.
상기 보고된 결과에 따르면, 본 발명의 약물은 반흔성 부위에 능동적 출혈을 유도할 수 있고 치료시간 단축 및 회복 과정에 수반되는 비유미적인 것이 제거되는 이중 효과와 함께 반흔성 자체의 조절에 유용하다.
이러한 회복 과정과 함께, 생성물 자체 또는 그것을 포함하고 있는 처방이 정맥성부전(靜脈性不全)에서 기인된 궤양과 정맥류에 국소투여되면 신속하고 정돈된 반흔화과정을 진행시킨다는 사실(본 발명의 목적이기도 함)이 놀랍게도 발견되었다. 특히 이 생성물이 실리콘 캐리어에 포함되어 있는 처방은 욕창의 예방 및 치료에 특별히 유용하다는 것이 밝혀져 있다.
누워만 있는 사람의 천골(薦骨)부 반대 측면의 한 쪽에는 본 발명의 생성물을 투약하고 또 다른 부분에는 진정제 처방을 하고, 기타의 점에서는 동일한 조건을 유지하면, 진정제 처방을 한 부분과는 달리 본 발명의 생성물로 치료한 부분에는 욕창이 생기지 않는다는 사실이 놀랍게도 발견되었다.
의수(義手) 및 의족(義足)으로 고통받는 수족 절단 환자에게 켈린이나 비스나딘을 투여하면, 즉 켈린 또는 비스나딘중의 하나를 3% 농도로 포함하고 있는 처방으로 2주일 치료를 하면 문제점이 해결되어 반흔성 안지오테토닉(angiotettonic)을 개선시킬 수 있다는 것이 놀랍게도 발견되었다.
본 출원인은 본 발명의 목적인 미소혈관동력학적인 활성을 갖는 굿소 약물이 괴사성 조직의 분해 이화물질을 신속하게 제거하여 지향성 모세관형성을 유도하는데 중요한 역할을 하는 것으로 믿는다: 그러나 본 발명은 어떠한 이론적 고찰에 의해서도 한정되지는 않는다.
본 발명에 따른 상처와 흉터를 치료하기 위한 국소 약물은 당해 기술분야에서 통상의 지식을 가진 자에게 알려진 일반적인 방법에 의해, 예를 들면 "레밍턴 약제학 핸드북" (XVII ed. 맥 출판, 미국 뉴욕)에 기재된 바와 같이 제조된다. 본 발명에 따르면 이러한 국소 약물은 진연고(slaves), 연고(ointment), 크림(cream), 로숀(lotion), 용액(solution), 현탁액(suspension), 스틱(stick), 스프레이(spray) 석고(plaster) 및 약물처리된 붕대(medicated bandage)의 형태일 수 있다. 특히 국소 약물은 포화 및 불포화 인지질 함유물이 다량 포함된 제형일 수 있다. 본 발명에 따르면 이러한 국소 약물은 또한 흉터를 아물게 하는 활성을 갖고 있는 것으로 알려진 다른 약물, 화학요법의, 항균성의, 면역자극제 및 콜라겐, 하이알루론산(hyaluronic acid) 등과 같은 약물과 결합될 수도 있다.
본 발명에 의한 국소 약물에는 혈관동력학적인 활성을 가지고 있는 하나 또는 그 이상의 화합물이 중량페센트농도가 0.01-10wt.%로 포함되어 있다.
약용량학의 1회 투여량은 의사에 의해 결정되어지지만, 어쨌든 적용실시예에서 필요량에 따라 하루 내지 한달동안 하루에 한 번 또는 그이상이다.
다음의 실시예들은 본 발명을 상세히 설명한다.

Claims (5)

  1. 암니 비스나가 쿠마린, 빈카 알칼로이드, 에르고트알칼로이드, 징코 바이오플라보노이드 및 크라터저스 바이오플라보노이드로 구성된 군으로부터 선택되는 말초 혈관동력학적인 활성을 가지고 있는 화합물이 활성성분으로서 포함되어 있는, 욕창, 정맥류, 만성 정맥성 기능부전과 관련한 흉터를 아물게 하는 활성을 가진 국소 약물.
  2. 제1항에 있어서, 상기 화합물이 켈린, 비스나딘, 빈카민, 라우바신, 테트라하이드로알스토닌, 니세르고린, 에틸 지메니네이트 및 에부르나모닌으로 구성된 군에서 선택된 것을 특징으로 하는 약물.
  3. 제1항에 있어서, 상기 화합물이 추출물의 형태로 투여될 수 있는 것을 특징으로 하는 약물.
  4. 제1항에 있어서, 상기 국소 약물이 0.01내지 10%의 활성 성분을 포함하는 것을 특징으로 하는 약물.
  5. 제1항에 있어서, 상기 국소 약물이 진연고, 연고, 크림, 로숀, 용액, 현탁액, 스틱, 스프레이, 석고 및 약물처리된 붕대의 형태인 것을 특징으로 하는 약물.
