[발명의 명칭]
골 흡수 억제용 약제학적 조성물
[발명의 상세한 설명]
본 발명은 뼈로부터 칼슘 및 골 매트릭스가 비정상적으로 감소하는 것을 포함하는 질병(골 흡수)을 예방 또는 치료하기 위한 신규한 약제학적 조성물에 관한 것이다. 비정상적 골 흡수는 골다공증 뿐만 아니라 류마티스성 관절염, 파제트병, 악성 종양의 골전이, 갑상선기능항진증 또는 난소적출술후 상태에서 나타난다. 또한, 비정상적 골 흡수는 신경 절제술, 또는 사지의 장기간에 걸친 비사용 또는 고정과 관련이 있다. 따라서, 골 취약증 및 골절 위험이 증가된다. 또한, 이러한 질병을 앓고 있는 환자, 특히 골다공증 환자는 일본인의 평균 수명이 급속히 연장됨에 따라 일본국에서 65세 이상의 노인들 사이에서 급속히 늘어나고 있다. 용어 "골다공증"은 본 명세서에서는 노인성 골다공증 및 폐경기 후의 골다공증을 나타낸다.
비타민 D3, 칼시토닌, 에스트로겐 및 비스포스포네이트 유도체를 포함하는 약제학적 조성물은 임상적으로 사용되어 왔다. 그러나, 이의 치료 효과는 완전히 만족스러운 것은 아니어서 보다 우수한 약제학적 조성물이 절실히 요망되고 있다.
골 흡수는 파골세포의 활성화시 발생하는 것으로 공지되어 있다. 본 발명자들은 높은 안정성을 제공하면서 파골 세포를 선택적으로 억제하는 약제학적 조성물을 밝혀내고자 광범위한 연구를 수행하였다. 본 발명자들은 일련의 일반식(Ⅰ)의 벤조피란 유도체가 골 흡수를 억제함을 밝혀냈다.

본 발명은 약제학적 담체 및 치료학적 유효량의 일반식(Ⅰ)의 벤조피란 유도체를 포함하는, 골 흡수 억제용 약제학적 조성물에 관한 것이다.

본 발명은 또한 골 흡수 억제용 약제학적 조성물을 제조하기 위한, 치료학적 유효량의 일반식(Ⅰ)의 벤조피란 유도체의 용도에 관한 것이다.
이들 벤조피란 유도체는 모두 공지되어 있는 화합물이다.
예를 들어, 화합물(Ⅰ)은 일본국 공개특허공보 제201182/1988호에 기술되어 있고, 화합물(Ⅱ)은 문헌[참조: Br. J. Pharmacol., vol. 88, pp. 103-111(1986), 및 일본국 공개특허공보 제67683/1983호, 제188880/1983호 및 제219183/1983호]에 기술되어 있으며, 화합물(Ⅲ)은 문헌[참조: Br. J. Pharmacol., vol. 101, pp. 605-614(1990)]에 기술되어 있고, 화합물(Ⅳ)은 문헌[참조: J. Cardiovasc. Pharmacol., vol. 15, pp. 188-197(1990)]에 기술되어 있으며, 화합물(V)은 문헌[참조 : J. Pharmacol. Exp. Ther., vol. 256, pp. 639-644(1991)]에 기술되어 있다. 화합물(Ⅰ)에 대한 급성 독성 연구 결과, 마우스 체내에서의 LD50은 470mg/kg(비경구 투여의 경우) 및 103mg/kg(정맥내 투여의 경우)이고, 래트 체내에서의 LD50은 약 2,000mg/kg(비경구 투여의 경우) 및 128mg/kg(정맥내 투여의 경우)인 것으로 나타났다. 따라서, 당해 화합물의 독성은 비교적 약한 편이다.
본 발명의 화합물은 골 흡수 억제용 약제학적 조성물에 유용하다. 당해 조성물은 피하내 주사, 정맥내 주사 또는 근육내 주사와 같은 방법으로 비경구 투여되거나 정제, 캡슐제, 입제, 산제 또는 시럽제로서 경구투여될 수 있다.
본 발명에 따르는 화합물의 투여량은 투여경로, 환자의 연령, 체중,증상 등에 의해 달라진다. 당해 화합물의 1일 투여량은 성인의 경우 1 내지 200mg, 바람직하게는 10 내지 100mg이다.
본 발명의 화합물을 통상적인 보조제, 예를 들어, 충전제, 붕괴제, 결합제, 윤활제 또는 향료를 사용하여 위에서 언급한 제형으로 만들 수 있다. 또한, 당해 화합물을 골 조직에 대해 높은 친화도를 갖는 화합물, 단백질 또는 펩타이드에 결합시켜 투여 형태에 골 조직에 대한 특이적 침투력을 제공할 수 있다.
본 발명에 따르는 화합물의 골 흡수 억제 작용은 파골 세포의 활성에 대한 당해 화합물의 직접적인 효과를 기본으로 하는 시험관내 시험, 및 실험용 과칼슘혈증을 앓고 있는 래트 및 실험용 비이동에 기인하는 골위축을 앓고 있는 래트체내에서의 이의 효과를 기본으로 하는 생체내 시험을 통해 증명된다.
파골 세포의 활성에 대한 시험관내 효과는 이른바 소와(pit) 생성 검정법에 의해 조사할 수 있다. 당해 방법에 따라, 현미경으로 관찰한 바 표면이 매끄러운 작은 아이보리색 조각(직경: 약 6mm)을 파골 세포를 포함하는 쥐과 동물의 골수 세포의 현탁액에 넣는다. 부갑상선 호르몬(PTH)을 추가로 가하여 파골 세포를 활성화시킨다. 활성화된 파골 세포는 작은 아이보리색 조각 위에서 무기 및 단백질 매트릭스를 용해시켜 소와를 형성시킨다. 본 화합물을 당해 시험 액체에 가하고, 파골 세포 유도된 골 분해에 의해 형성된, 작은 아이보리색 조각 위의 소와 수를 세어 본 발명의 화합물의 효능을 측정한다. 화합물의 골 흡수 억제효과가 크면 클수록, 생성되는 소와 수가 적어진다. 칼시토닌을 양성(positive) 대조표본으로서 사용한다.
생체내 시험에서는, 본 화합물의 전신계 효과를 고찰할 수 있다. PTH는 혈청 칼슘 이온의 양을 증가시키는 것으로 공지되어 있다. 따라서, PTH를 투여하여 실험적으로 과칼슘혈증을 유발시킬 수 있다. 이와 같이 유도된 과칼슘혈증을 앓는 래트 체내에서 혈액 칼슘 이온의 양에 대한 본 화합물의 효과는 이의 골 대사 증진 활성을 결정하는 파라미터로서 사용될 수 있다.
본 화합물의 골 흡수 억제작용을 또한 실험적 비사용에 기인하는 골위축을 앓고 있는 모델에서 검사할 수 있다. 골절 후의 단단한 드레싱 및 신경 외과 수술에 기인한 비이동은 골 흡수와 이에 따른 골 형성 사이의 균형에 손상을 입혀 골 질량을 감소시키는 것으로 공지되어 있다. 액와 부분으로부터 상완 신경을 실험적으로 절제하면, 팔의 비사용에 기인하는 척골과 요골의 뼈 질량(무수 중량을 기준으로 함) 감소에 의해 골위축이 발병한다. 당해 유형의 위축에 대한 본 화합물의 효능은 절제하지 않은 쪽의 요골 및 척골 뼈 질량에 대한 절제한 쪽의 요골 및 척골 뼈 질량의 비를 계산함으로써 측정할 수 있다.
[실시예]
본 발명의 효과를 다음 실시예를 참고로 하여 보다 상세히 기술할 것이다.
[실시예 1] 골 흡수의 시험관내 억제
골수 세포를 생후 10일이 지난 ICR 마우스의 장골로부터 수집한다. 부서진 뼈 조각을 5% 송아지 태아 혈청을 함유하는 MEM 매질중에서 원심분리하여 제거한다. 골수 세포를 동일한 매질에 107/ml의 세포 농도로 재현탁시킨다. 현탁액을 100μl/웰의 용적으로 96-웰 플레이트에 가한다. 플레이트의 각각의 웰은 당해 플레이트내에 위치한, 직경이 6mm인 작은 아이보리색 조각을 갖는다. 시험 약제를 세포 현탁액에 1 x 10-6내지 1 x 10-10mol의 농도로 가한다. 플레이트를 PTH 1 x 10-7mol의 존재하에 37 ℃에서 3일 동안 배양한다. 이어서, 아이보리색 조각을 제거하고, 쿠마시 브릴리안트 블루우로 염색한 다음, 현미경을 사용하여 생성된 소와의 수를 측정한다. 시험 약제를 투여한 그룹에서 생성된 소와의 수를 시험 약제를 투여하지 않은 그룹에서 생성된 소와의 수와 비교하고, 차이를 스튜던트 시험법(Student's t-test)에 의해 통계학적으로 분석한다. 결과는 표 1에 명시하는 바와 같다.
PTH에 의해 촉진되는 소와 수의 증가는 1 x 10-8mol 농도의 시판되는 뱀장어 칼시토닌(이후부터는 eCT라고 한다), 1 x 10-8mol 농도의 화합물(Ⅰ) 및 1 x 10-6mol 농도의 화합물(Ⅱ)에 의해 상당히 억제된다. eCT는 양성 대조표본으로서 사용된다.
[실시예 2] 과칼슘혈증을 앓고 있는 마우스체내에서의 효능(시행 1)
생후 5주된 수컷 SD 래트에게 20시간 동안 음식물을 제공하지 않는다. 이어서, 이들 래트를 3 내지 5마리의 그룹으로 나누고, 사람의 PTH(N1-34) 60U/kg을 정맥내 투여하여 과칼슘혈증을 발병시킨다. 시험 약제를 생리 식염수에 용해시키고, 당해 용액을 PTH 투여 15분 전에 정맥내 투여한다. 대조 그룹에도 유사하게 생리 식염수를 투여한다. PTH 투여 후 60분 뒤에 혈액 샘플을 취하여 혈청 칼슘 이온의 양을 측정한다. 대조 그룹과 시험 약제를 투여한 그룹의 측정 값을 비교하고, 차이를 스튜던트 시험법에 의해 통계학적으로 분석한다.
결과는 표 2에 명시하는 바와 같다. PTH 투여 결과, 혈청 칼슘 이온의 양이 상당히 증가되어 과칼슘혈증이 유발된다. 당해 이상을 억제하는 최소 투여량은 화합물(Ⅰ)의 경우에는 0.1mg/kg이고, 화합물(Ⅱ)의 경우에는 1.0mg/kg이다.
[실시예 3] 과칼슘혈증을 앓고 있는 모델에서의 효능(시행 2)
생후 5주된 숫컷 SD 래트에게 20시간 동안 음식물을 제공하지 않는다. 이어서, 이들 래트를 4 또는 5마리의 그룹으로 나누고, 사람의 PTH(N1-34) 30㎍/kg을 정맥내 투여하여 과칼슘혈증을 발병시킨다. 시험 약제를 생리식염수에 용해시키고, 당해 용액을 PTH 투여 15분 전에 정맥내 투여한다. 유사하게, 대조 그룹에도 생리식염수를 투여한다. PTH 투여 후 60분 후에 혈액 샘플을 채취하여 혈청 칼슘 이온의 양을 측정한다. 대조 그룹과 시험 약제를 투여한 그룹의 측정값을 비교하고, 차이를 스튜던트 시험법에 의해 통계학적으로 분석한다.
결과는 표 3에 명시하는 바와 같다. PTH 투여 결과, 혈청 칼슘 이온의 양이 상당히 증가되어 과칼슘혈증이 유발된다. 당해 이상을 억제하는 최소 유효 투여량은 화합물(Ⅲ)의 경우에는 0.3mg/kg이고, 화합물(Ⅳ)의 경우에는 0.1mg/kg 이하이다.
[실시예 4] 비이동에 기인한 골위축을 앓고 있는 모델에서의 효능
펜토바르비탈로 마취시킨 숫컷 SD 래트(생후 6주된 것)의 좌측 액와 부분으로부터 상완 신경을 절제하여 비이동성-관련 골위축을 앓는 모델을 만든다. 이들 래트 모델을 5 또는 6마리의 그룹으로 나누고, 절개 직후에 화합물(Ⅰ)은 1 또는 2주 동안 매일 1.0mg/kg의 투여량으로 2회에 걸쳐 정맥내 투여하고, 화합물(Ⅱ)의 경우에는 1주 동안 매일 1.0mg/kg의 투여량으로 2회에 걸쳐 투여한다. 대조 그룹에게는 1 또는 2주 동안 매일 2회에 걸쳐 정맥내 경로를 통해 생리 식염수를 유사하게 투여한다. 이어서, 요골 및 척골을 떼어내어 알콜로 탈수 및 탈지시킨다. 이어서, 이들 뼈를 160 ℃에서 6시간 동안 건조시키고 이들의 중량을 측정한다. 우측 척골에 대한 좌측 척골의 중량비 뿐만 아니라 우측 요골에 대한 좌측 요골의 중량비를 계산한다. 대조 그룹과 처리된 각 그룹의 계산 값을 비교하고, 차이를 스튜던트 시험법에 의해 통계학적으로 분석한다.
결과는 표 4에 명시한 바와 같다. 1주 동안 투여하는 경우, 화합물(Ⅱ)은 신경제거된 쪽의 척골의 중량 감소를 상당히 억제하는 반면, 화합물(Ⅰ)은 당해 감소를 억제하긴 하나 미미한 정도이다. 2주 동안 처리한 후, 화합물(Ⅰ)은 신경제거된 쪽의 척골 및 요골의 중량 감소를 상당히 억제한다.
본 발명의 제형은 다음 실시예에 의해 설명된다.
[제형 실시예]
증류된 주사용수를 화합물(Ⅰ) 0.4g 및 염화나트륨 16g에 가하여 총 2,000ml 용적의 액체를 만든다. 당해 액체를 0.22μ의 미세기공 여과지를 통해 멸균 여과시킨 다음, 용량이 5ml인 앰플 각각에 5ml의 용적으로 조제한다. 앰플을 용융 밀봉 시키고 오토클레이브-멸균시켜 주사용액을 수득한다.
상기 설명으로부터 명백히 알 수 있는 바와 같이, 본 발명에 따르는 화합물은 실험적 과칼슘혈증과 실험적 비이동에 기인한 골 위축을 앓고 있는 동물 모델에서 골 흡수의 시험관내 억제 및 비정상적인 골 대사의 생체내 교정을 제공한다. 따라서, 이들 화합물은 골다공증 및 과칼슘혈증과 같은 골 대사 관련 질병을 완화 시키는 것으로 사료된다.
Claims (3)
- 약제학적 담체와 치료적 유효량의 일반식(Ⅰ)의 벤조피란 유도체를 포함하는, 골 흡수 억제용 약제학적 조성물.상기 식에서,점선을 포함하는 결합은 이중결합일 수도 있음을 나타내고,R1은 수소원자 또는 하이드록실 그룹이며,R2는 시아노 그룹, 페닐설포닐 그룹 또는 할로겐 치환된 메톡실 그룹이고,R3는 구조식의 그룹이다.
- 제1항에 있어서, R1이 하이드록실 그룹이고 R2가 시아노 그룹이며 R1에 대해 트란스 위치에 존재하는 2-옥소-1-피롤리디닐 그룹이고 점선을 포함하는 결합이 단일결합인 약제학적 조성물.
- 제1항에 있어서, R1이 하이드록실 그룹이고 R2가 페닐설포닐 그룹이며 R3이 R1에 대해 트란스 위치에 존재하는 2-옥소-1-피롤리디닐 그룹이고 점선을 포함하는 결합이 단일결합인 약제학적 조성물.
