Abstract
본 발명은 α-디하이드로에르고크립틴을 사용한 신경보호 및 뇌세포 괴사 예방용제제의 제조방법에 관한 것이다. α-디하이드로에르고크립틴은 상기 용도로서의 1일 용량이 0.5 내지 100mg이 되고 메탄설폰산염의 형태로서 캅셀, 정제, 환제, 점적제, 경구용 지속적 방출제제, 비강내 투여용 제제, 비경구,정맥내 및 근육내 투여용 앰플을 포함하는 약제학적 제제로 제조된다.

본 발명은 α-디하이드로에르고크립틴(α-dihydroergocryptine: 이하 α-DHEK으로 칭한다)을 사용하는 것을 특징으로 하는 노인의 신경보호치료제로 유용한 약제에 관한 것이다.
α-디하이드로에르고크립틴, 즉 12'-히드록시기-2'-(1-메틸에틸)-5'α-(2-메틸프로필)-9, 10-다하이드로에르고트아만 -3', 6', 18-트리온은 잘 알려진 화합물로서, 천연의 알칼로이드인 α-에르고크립틴(α-ergocryptine)의 9, 10-위치에 있는 이중결합의 수소화로부터 유도된다.
α-DHEK는 특히 노인에 있어서의 뇌혈관 질환의 치료에, 대부분 디하이드로에르고크리스틴(dihydroergocristine) 및 디하이드로에르고코르닌(dihydroergocornine)과 조합하여 적용된다.
사실 디하이드로에르고크립틴 및 기타의 동족체 수소화 알칼로이드는 일반적으로 중추 및 말초신경계에 있는 α 및 D수용체와 결합한다고 알려져 있다.
이러한 약물학적 활성에 따르면 노인성 뇌혈관 부전은 그 몇가지 증상이 α-디하이드로에르고크립틴 단독 또는 조합으로서의 주된 치료적 적응증으로 된다.
본 발명자의 이탈리아 특허 1,200,603(89. 1. 27)에서는 활성성분으로서 α-디하이드로에르고크립틴을 함유하는 약제학적 제제 및, 파킨슨병, 우울증 및 두통치료에 대한 그의 용도를 개시하고 있다.
현재 α-디하이드로에르고크립틴은 놀라운 뇌신경보호활성을 갖는다는 것이 알려져 있다. 특히 이 화합물은 내인성 및 외인성 독성물질에 기인한 뇌세포의 괴사를 방지한다.
니콜레티등(Nicoletti et al.: J. Neurosc. 6. 1905, 1986)에 의해 기술된 방법에 따라, 생후 8일된 래트로부터 소뇌 뉴런의 1차배지를 제조하였다. 세포를 트립신처리후 트립신 저해제 및 데옥시리보뉴클레아제로 처리한 다음, 송아지태아혈청(10%), Kcl 25mM, 글루타민 2mM 및 겐타마이신 50㎍/㎖를 함유하는 기초이글배치(Basal Eagle's Medium: BEM)중에 현탁시켰다. 현탁된 세포(106/㎖)를 페트리 접시에 분배하고 기본기질로서 폴리-L-리진(10㎍/㎖)으로 처리하였다. 교질세포나 내막세포의 증식을 방지하기 위하여 16-18시간 후 시토신아라비노푸라노사이드(Ara-C)를 배지에 가하였다. 7-9일간의 생체외 증식(DIV)후 1차배지는 고도로 균질화된 신경집단으로 되는데, 이것은 과립층 세포(90%)로 대표되며 소수의 GABA작동성 뉴런(약5%)을 갖고 오염물질로서 약간의 교질 세포와 내막세포를 포함한다. 배양된 과립세포는 첫주에는 증식하고, 9-11일 이내에 한정된 페노형(fenotype)에 도달한다. 이어서 이들은 적절한 세포내 에너지 수준과 균형을 이루지 않는 범발성의 NMDA 수용체 활성화에 주로 기인되는 퇴화현상을 보인다.
이들세포를 이초산플루오레세인(fluorescein diacetate) 및 요오드화 프로피디움(propidium iodide)으로 표지하여 세포활성을 측정하였다. 플루오레세인은 살아있는 세포의 에스테라제와 반응하여 형광하에서 황녹색을 발한다. 요오드화 프로피디움은 퇴화된 세포의 멤브레인을 통과하여 DNA와 결합함으로써 세포가 적색을 띠게 만든다. 생존상태는 플루오레세인 표지세포의 백분율로서 표시되며 각 배지에서 4분획을 분석하여 평가된다. α-DHEK는 극히 낮은 농도에서는 세포를 괴사로부터 보호할 수 있다. 제1도는 α-DHEK의 활성을 보여주는 것으로, 1일 1회씩 13일간 투여후 13일째 래트 신경세포의 생존상태를 나타낸 것이다.
마네브등(Manev et al.)은 최근 연구(J. Pharm, Exp. Therap. 252. 419. 1990)에서 1회의 글루탐산 펄스(pulse)에 의해 유도되는 배양뉴런의 완만한 퇴화가, 배지로부터 글루타메이트 제거후 20-50분간 지속되는 Ca++유입의 증가와 관련됨을 확증하였다. 이러한 결과에 근거하면 신경퇴화는 Ca++항상성변화에 기인하게 된다. 지연성 Ca++유입은 다음의 방법에 따라 평가된다. NaCl 154mM, KCl 5.6mM, 포도당 5.6mM, NaHCO33.6mM, CaCl22.3mM 및 HEPES(ph 7.4) 5mM을 함유하는 로크용액(Lock's solution)으로 배지를 3회 세척하였다. 원래의 로크용액에서 Mg++이온은 생략하였다. 글루탐산을 100㎛농도로 15분 동안 첨가하였다. 펄스(pulse)의 종결후 세포를 로크용액으로 3회 세척하고 37℃에서 40분간 배양하였다. 다음에 배지를 45Ca++(1μC/접시)와 함께 10분간 배양하였다. 배치를 4℃로 유지된 킬레이트화제 용액(염화콜린 154mM+EDTA 1mM+트리스-HCl 10mM, pH7.2)으로 세척하여 방사성 Ca++섭취를 중지시켰다. 세포내 방사능응 세포를 0.5N-NaOH로 용해한 후(37℃에서 30-60분)평가하였다.
α-DHEK는 극히 낮은 농도에서도 지연성 Ca++유입으로부터 세포를 잘 보호함을 보여준다. 제2도는 1일 1회씩 7일간 여러 가지 농도로 투여한 α-DHEK의 활성을 보여주는 것이다.
1-메틸-4-페닐-1, 2, 3, 6-테트라하이드로피리딘(MPTP)에 대한 전신적 투여는 사람 및 원숭이에 있어서 파킨슨병 양 증상을 유발한다. 원숭이에 있어서 저용량의 MPTP(1-5mg/kg)는 흑질치밀층(substantia nigra compacta)에 있는 신경세포의 선택적 손실, 선조체(striatum)내에서의 비가역적인 도파민 감소, 및 현저한 추체외로 운동기능 감퇴를 야기한다. 래트에 있어서 MTPT는 현저한 뇌 실질의 손상을 야기한다.
α-DHEK는 MPTP 독성 활성으로부터 마우스를 보호할 수 있다. CB57 마스우에 MPTP를 투여하면 흑질-선조체 도파민작동 신경계에 심한 퇴화를 야기한다. MPTP 투여 19일 후에 동물들을 치사시켰다. 이때 도파민과 DOPAC의 선조체 농도는 대조군의 약 50%였다. 최초의 MPTP투여 30분전에 α-디하이드로에르고크립틴을 투여하면 이러한 독성 약제의 퇴화효과로부터 보호된다. 제3도 및 4도에서 볼 수 있듯이 α-DHEK전처리 동물에 있어서 도파민과 DOPAC농도는 MPTP단독투여 동물보다 3-4배 높은 것을 알 수 있다.
평균체중 3kg의 마카카 파시쿨라리스(Macaca fascicularis)원숭이에 0.2mg/kg i.v. 용량의 MPTP를 투여하여 신경독성을 유발하고; 8마리 동물을 케타민(Ketamine)으로 마취한다. 4마리로 구성된 대조군은 생리식염수만을 투여하였다. 8마리 동물에 대하여 MPTP투여는 4일동안 매일 시행하고; 이들중 4마리에는 생리식염수(4㎖/kg)를 경구투여하고(MPTP군)나머지 4마리에는 α-다하이디로에르고크립틴을 6mg/kg(4㎖/kg)용량으로 1일 2회(매 12시간마다) 4일동안 경구투여하였다(MPTP+α-DHEK군). 투여종결시 MPTP군 원숭이에는 생리식염수(4㎖/kg)를 1일 1회씩 10일간 더 투여하였고, MPTP+α-DHEK에는 α-디하이드로에르고크립틴을 6mg/kg(4㎖/kg)의 경구용량으로 1일 1회씩 10일간 더 투여하였다. 대조군에는 동일한 방법으로 생리식염수(4㎖/kg)을 경구 투여하였다. 14일째날에 행동양상의 관점에서 동물을 관찰하고; 15일째 날에 동물들을 케타민, 넴부탈(nembutal) 및 펜토바르비탈(pentobarbital)로 마취하여 뇌를 꺼내었다. 피각(putamen)과 미상핵(caudate)을 0.32M 서당용액중에 균질화하여 도파민과 호모바닐린산(HVA)을 HPLC법으로 측정하였다.
표 1에서 볼 수 있듯이, α-디하이드로에르고크립틴은 행동양상 및 신경화학적 요소 양자모두에 있어서 MPTP신경독성작용으로부터 뇌의 보호활성을 나타낸다
MPTP의 신경독성은 두가지 작용기전을 통해 발현되는 것으로 보인다. 모노아민옥시다제(MAO)에 의해 유도되는 도파민 대사에서는 HO가 생성되어 도파민 작동성 뉴런의 노화를 야기시킨다. MPTP는 도파민을 저장소로부터 유리시키고 과산화물 유리기(radical)의 현저한 증가가 야기된다.
또 다른 작용기간은 MAO에 의해 생산된 MPTP대사 중간생성물이 신경독성유도체라도 여겨진다.
α-디하이드로에르고크립틴은 그 자체로서나 생리적으로 허용가능한 염으로서, 원하는 용도에 적절하게 제제화된 약제학적 제형의 형태로 경구, 설하, 비경구, 경피 또는 비강을 통한 경로에 의해서 투여될 수 있다.
바람직하게는 메탄설폰산염으로 되는, 신경보호활성을 얻기 위한 α-DHEK의 1일 용량은 체중 및 환자의 상태에 따라 0.5 내지 100mg의 범위에 있을 수 있다.
본 화합물의 약제학적 조성물은 약제학적으로 허용되는 부형제와 담체를 사용하여 통상적인 기술에 따라 제조되며, 이들은 보조적 또는 유용한 활성을 갖는 다른 활성성분들과의 조합을 선택적으로 함유할 수 있다.
이들 제제에는 예를들어 캅셀, 환제, 정제, 점적제, 비강내 투여용제제, 근육주사용 또는 정맥내 투여용 앰플제제, 경구용 지속적 방출제제등이 포함된다.
다음의 실시예는 점적제, 앰플제제 및 정제와 같은 약제학적 제제의 형태로 약제를 제조하는데 있어서 α-디하이드로에르고크립틴의 용도를 보여준다.
미세결정형 셀룰로스등 적량을 가해 100mg으로 한다.
Claims (6)
- α-디하이드로에르고크립틴 또는 생리적으로 허용되는 그의 염을 포함하는 것을 특징으로 하는, 환자의 신경보호 및 내인성·외인성 독성물질에 의해 야기되는 뇌세포 괴사의 예방용 약제.
- 제1항에 있어서, 상기 α-디하이드로에르고크립틴의 염이 메탄설폰산염인 것을 특징으로 하는 약제.
- 제1항에 있어서, 상기 α-디하이드로에르고크립틴이 0.5 내지 100mg/일의 양으로 투여하기에 적절한 단위용량으로 함유되는 것을 특징으로 하는 약제.
- 제2항에 있어서, 상기 α-디하이드로에르고크립틴이 0.5 내지 100mg/일의 양으로 투여하기에 적절한 단위용량으로 함유되는 것을 특징으로 하는 약제.
- 제1항 내지 제4항의 어느 한항에 있어서, 상기 약제가 경구, 설하, 비경구, 경피, 및 비강내 투여에 적합한 약제학적 제제의 형태인 것을 특징으로 하는 약제.
- 제5항에 있어서, 상기 약제학적 제제가 캅셀, 정제, 환제, 점적제, 경구용 지속적 방출제제, 비강내 투여용 제제, 비경구, 정맥내 및 근육내 투여용 앰플을 포함하는 것을 특징으로 하는 약제.
