Abstract
Description
[1] 悪性腫瘍を罹患している被験体において、PD-1/PD-L1阻害剤が治療に有効であるか否かを予測する方法であって、前記被験体から採取した腫瘍細胞においてPD-1/PD-L1阻害剤の有効性に関連するゲノムの異常を検出し、異常が存在する場合に、PD-1/PD-L1阻害剤が前記被験体の治療に有効であると評価する方法。
[2] PD-1/PD-L1阻害剤の有効性に関連するゲノムの異常が、PD-L1(CD274)遺伝子の発現亢進に関連するPD-L1遺伝子の異常である、[1]の方法。
[3] PD-L1遺伝子の異常が、PD-L1遺伝子の3'UTR領域の完全なまたは部分的な欠失である、[2]の方法。
[4] PD-L1遺伝子の異常が、PD-L1遺伝子の3'UTR領域の欠失を引き起こすコピー数変化である、[2]の方法。
[5] PD-L1遺伝子の異常が、PD-L1転写物からのエクソン5、エクソン6およびエクソン7の全部または部分的な切断である、[2]の方法。
[6] PD-L1遺伝子の異常が、PD-L1転写物からのエクソン5、エクソン6およびエクソン7の全部または部分的な切断である、[2]の方法。
[7] PD-L1遺伝子のエクソン1〜エクソン4のいずれかの転写産物とPD-L1遺伝子の3'UTR領域の転写産物を定量し、エクソン転写産物の量と3'UTR領域の転写産物の量の比を算出し、該比が所定の値以上の場合にPD-1/PD-L1阻害剤ががん治療に有効であると評価する、[2]の方法。
[8] PD-L1遺伝子の異常が、PD-L1 mRNAの発現の亢進である、[2]の方法。
[9] PD-L1遺伝子の異常が、ウイルスの挿入による異常である、[2]の方法。
[10] ウイルスがHuman papilloma virus (HPV)又はEBV (Epstein-barr virus)である、[9]の方法。
[11] 被験体から採取した腫瘍細胞中のPD-L1タンパク質を、PD-L1に対する抗C末端抗体およびPD-L1に対する抗N末端抗体を使用した免疫組織染色により染色し、PD-L1に対する抗N末端抗体で染色が認められるが、PD-L1に対する抗C末端抗体では染色が認められない場合、PD-1/PD-L1阻害剤が前記被験体の治療に有効であると評価する[2]の方法。
[12] 悪性腫瘍が、成人T細胞白血病/リンパ腫、胃がん、大腸がん、膀胱がん、子宮頸がん、腎がん、肺腺がん、皮膚悪性黒色腫およびB細胞リンパ腫からなる群から選択される、[1]〜[11]のいずれかの方法。
[13] 悪性腫瘍が、食道がん、頭頸部がんおよび子宮体がんからなる群から選択される、[1]〜[11]のいずれかの方法。
[14] PD-1/PD-L1阻害剤が抗PD-1抗体または抗PD-L1抗体である、[1]〜[13]のいずれかの方法。
[15] 悪性腫瘍を罹患している被験体から採取した腫瘍細胞においてPD-1/PD-L1阻害剤の有効性に関連するゲノムの異常を検出し、異常が存在する場合に、PD-1/PD-L1阻害剤を用いて治療する方法。
PD-L1遺伝子は9p24.1領域に存在し、その構造異常として、増幅(tandem duplication)、逆位(inversion)、転座(translocation)、欠失(deletion)が挙げられる。PD-L1遺伝子の構造異常により3'UTRに欠失がある場合、PD-L1の高発現が引き起こされる。これは、3'UTRがmRNAの安定性や翻訳調節に重要な役割を果たす領域であり、この領域の欠失によりこれらの機能が失われるからである。PD-L1遺伝子の3'UTRの欠失とは、完全な欠失も部分的な欠失も含まれる。また、PD-L1遺伝子を含む9p24.1領域のコピー数の異常(CNV)として増幅や減少・片親性ダイソミーが挙げられる。コピー数の異常は構造異常に伴って起きるが、3’UTRが欠損する場合にはPD-L1の高発現が引き起こされる。
PD-L1のmRNAの異常として、mRNAの3’UTRの全部または部分的な切断およびそれに伴う高発現が挙げられる。さらに、エクソン5または6以降の切断を伴う場合がある。エクソンの切断として、エクソン6およびエクソン7全体の切断、エクソン7全体の切断、エクソン7の途中からの切断が挙げられる。本発明において、このようなエクソンの切断をエクソン6またはエクソン7の全部または部分的な切断という。さらに、エクソン4以降の切断、すなわち、エクソン5、エクソン6およびエクソン7全体の切断が生じ得る。
(方法)
ATL(成人T細胞白血病)57例の腫瘍検体を用いたRNAシーケンスとATL11例の腫瘍・正常(頬粘膜)ペアの全ゲノムシーケンスを行った。
結果を図1に示す。ATL患者57例中11例に9p24.1領域の構造異常(融合遺伝子)を認めた。
(方法)
1.のATL57例の腫瘍検体を用いたRNAシーケンスの結果を用いて、構造異常とCD274発現の関係について解析した。
結果を図2に示す。図2の棒グラフの模様(右上から左下への太い斜線、左上から右下への細い斜線、白、右上から左下への細い斜線)は図1の模様に対応している。
(方法)
2.のATL57例のRNAシーケンスデータをIGV(integrative genome viewer)(提供:Broad Institute)を用いて、シーケンスリードをCD274(NM_014143)の各エクソン毎に表示した。
結果を図3に示す。図3の下の1〜7の数字はエクソンの番号を示す。構造異常(-)例(Miya31)では他のエクソンと比較して相対的に3’UTRにおける発現が高かった。一方、構造異常(+)例(Miya26 & 30)では3’UTRにおける発現が消失していた。他の構造異常(+)例も同様であった。この結果は、構造異常(+)例では、3'UTRの欠失に伴い、同部位の発現も消失していることを示す。
(方法)
構造異常(+)例では3’UTRの発現が消失するので、3.のように遺伝子全体で発現を評価する場合、発現が過小評価されて、構造異常との関係が不明瞭になっている可能性がある。そのため、3.のATL57例のRNAシーケンスデータを用いて、CD274の各エクソンの発現および3’UTRの発現との比を計算した。その中で、構造異常との関係が最も明確であったエクソン3の発現およびエクソン3の発現/3’UTRの発現(ex3 exp/3'UTR exp)を散布図として表示した。
結果を図4に示す。図4中、白丸は構造異常(+)、黒丸は構造異常(-)の例を示す。「ex3」はエクソン3を示し、「exp」は発現(expression)を示す。CD274のエクソン3のみの発現、エクソン3の発現と3’UTRの発現の比を見ると、構造異常(+)例で、ex3 exp/3'UTR expが高く、構造異常(+)例(白丸)が明確に分離された。
(方法)
ATL57例のRNAシーケンスの結果から同定された11例のCD274構造異常(+)症例の中で、約半数ではエクソン5またはエクソン6で転写産物が切断されていた。それらの例で、RNAシーケンスで配列を確認し、転写産物の配列の同定を試みた。
結果を図5に示す。図5下部の塩基配列は構造異常(+)の症例(Miya26)における転写産物の配列である。エクソン6の3’末端までは野生型CD274が転写されるが、それ以降は3番染色体上のCBLB遺伝子のイントロン領域に繋がる(枠で囲んだ部分)。その領域では、終止コドン(イタリックで表したTGA)、polyAシグナル配列(AATAAA: polyAが結合するのに必要な配列、下線部)、polyA配列(二重下線部)を含んでいる。この結果は、CD274の構造異常により形成されたalternativeな転写産物が適切な転写産物として機能することを示している。これらの、図5に示した配列の全体を配列番号1に示し、3'UTRの配列を配列番号2に示す。
(方法)
リファレンス配列としてNP_054862を参照した。5.の転写産物の配列を元に、CD274が途中で切断される例において、そのアミノ酸配列をin silicoで同定した。
結果を図6に示す。CD274はExtracellular domain(細胞外ドメイン), Transmembrane domain(膜貫通ドメイン), Cytoplasmic domain(細胞内ドメイン)で構成されるが、PD1/PD-L1 機構による腫瘍細胞の免疫回避には、前2者のドメインが重要と考えられている。
(方法)
RNAシーケンスでCD274構造異常が同定された症例の腫瘍細胞を用いてフローサイトメトリーにより細胞表面におけるCD274の発現を評価した。腫瘍細胞はPE/Cy7 anti-human CD274 (B7-H1, PD-L1) Antibody (clone: 29E.2A3, BioLegend)を用いて染色し、LSR2 Fortessa(BD Biosciences)を用いて解析した。
結果を図7に示す。図7の枠で囲んだものが構造異常(+)例である。構造異常(-)例でもCD274の低発現が認められるが、構造異常(+)例ではCD274の顕著な高発現が確認され、発現量は100倍程度に増加していた。
(方法)
米国の様々な悪性腫瘍の次世代シーケンスデータのデータベースであるTCGA(https://tcga-data.nci.nih.gov/tcga/)に蓄積されているRNAシーケンスデータを用いて様々な悪性腫瘍におけるCD274の構造異常と発現の解析を行った。
結果を図8に示す。図中の略語は以下のがん腫を表す。ACC:副腎がん、BLCA:膀胱がん、CESC:子宮頸がん、COAD:大腸がん、DLBC:びまん性大細胞型B細胞リンパ腫、ESCA:食道がん、GBM:神経膠芽腫、HNSC:頭頸部がん、KICH:腎嫌色素性細胞がん、KIRC:胃淡明細胞がん、KIRP:乳頭状腎細胞がん、LAML:急性骨髄性白血病、LGG:低悪性度神経膠腫、LIHC:肝細胞がん、LUAD:肺腺がん、LUSC:肺扁平上皮がん、MESO:悪性中皮腫、OV:卵巣がん、PAAD:膵がん、PCPG:褐色細胞腫、PRAD:前立腺がん、READ:結腸がん、SARC:肉腫、SKCM:皮膚悪性黒色腫、STAD:胃がん、TGCT:精巣腫瘍、THCA:甲状腺がん、THYM:胸腺腫、UCEC:子宮体がん、UCS:子宮肉腫、UVM:ぶどう膜悪性黒色腫
図中の数字は各腫瘍におけるRNAシーケンスの例数であり、合計で10,000例以上を解析した。
(方法)
TCGAでは、RNAシーケンスデータより算出した発現データ(RSEM値を採用、胃がんのみRPKM値を採用)も公表している。そのデータを用いて、各がんにおいて症例ごとにCD274の発現(対数変換後)をヒストグラムとして表示した。
図9に4種類のがんの結果を例として示す。様々ながんにおいて一部の症例のみにCD274の高発現が認められた。そのため、発現値でカットオフ値を決めて、詳細な解析を行うこととした。カットオフ値としては、胃がん以外はRSEM値、胃がんはRPKM値をそれぞれ対数変換した値を用いて、胃がん以外の場合は8、胃がんの場合は11を用いた(カットオフ値は評価のために適宜設定した値である)。図9中、SKCMは皮膚悪性黒色腫を(図9A)、COADREADは大腸がんを(図9B)、STADは胃がんを(図9C)、DLBCはびまん性大細胞型B細胞リンパ腫(図9D)を示す。
(方法)
TCGAのカットオフ値以上の症例(CD274高発現例)について、4.と同じ方法を用いて、既にマッピング済みのRNAシーケンスデータにおける各エクソン毎の発現を評価し、IGV(integrative genome viewer)(提供:Broad Institute)で表示した。
結果を図10に示す。その中で、一部でATLと同様の発現パターン(高発現かつ3’UTRの発現が欠損)が認められた。さらに、それらの症例でRNAシーケンスデータを用いて配列を詳細に解析すると、CD274の構造異常が確認できた。例として胃がん(STAD)(図10A)、大腸がん(COAD)(図10B)を挙げている。それ以外に膀胱がん(BLCA)1例、子宮頸がん(CESC)1例、腎がん(KIRC)1例、肺腺がん(LUAD)2例、結腸がん(COAD)1例(計2例)・直腸がん(READ)1例、皮膚悪性黒色腫(SKCM)1例、胃がん(STAD)3例(計4例)にATLと同様のCD274高発現かつ3’UTRの発現が欠損が確認できた。
(方法)
ATL426例においてSNPアレイを用いたコピー数解析(手法:GISTIC)を行った。
GISTICの結果を図12に示す。ATL患者において26個の局所的なコピー数増幅および50個の局所的なコピー数減少を同定した。その一つに9p24.1領域のCD274(PD-L1)に増幅(14%)を認めた。
(方法)
ATL13例においてRQ-PCRを用いてCD274の発現を評価した。
結果を図14に示す。ATL患者において構造異常を認める3例(棒グラフの右上から左下への太斜線のバーの2例(増幅(tandem duplication)が認められたもの)および左上から右下への細斜線のバーの1例(逆位(inversion)が認められたもの))においてCD274の高発現を認めた。

PD-L1 構造異常(SV: structural variation)がないATL059, PD-L1 SV(+)でORFが保たれているATL075, SV(+)でORFが切断されているATL012の3症例で、PD-L1に対する抗N末端抗体(E1J2J, Cell Signaling Technology)と抗C末端抗体(SP142, Spring Bioscience)を用いて免疫染色を実施した。
結果を図15に示す。図15AはPD-L1 SVがないATL059(ATL059: PD-L1 SV(-))、図15BはPD-L1 SV(+)でORFが保たれているATL075(ATL075: PD-L1 SV(+), intact ORF)、図15CはSV(+)でORFが切断されているATL012(ATL012: PD-L1 SV(+), truncated ORF)の結果を示す。また、図15の上段は、抗N末端抗体を用いた免疫染色の結果を示し、図15の下段は、抗C末端抗体を用いた免疫染色の結果を示す。図15Aに示すように、PD-L1 SVがないATL059では腫瘍細胞の染色は認められなかった(図15Aには、散発的に染色されておりモノクロ写真ではグレーに見える大型の細胞が認められるが、これはマクロファージである)。
(方法)
TCGAで公開されている33のがん腫(固形がん)、10210例のRNAシーケンスデータより算出した発現データ(RSEM値)に基づいて上位10%(最低30例)の症例を抽出し、RNAシーケンスデータをダウンロードし、CD274融合遺伝子(+)および/またはCD274領域にウイルスの挿入(+)および/またはCD274 エクソン 4 / 3’UTR比高値の症例を抽出した。DLBCおよびSTADについては頻度が高かったため、全症例を対象とした。
結果を図16に示す。

CESC: 子宮頸がん 2例
COAD: 結腸がん 2例
DLBC: B細胞性リンパ腫 4例
ESCA: 食道がん 1例
HNSC: 頭頸部がん 1例
KIRC: 腎がん 1例
LUAD: 肺腺がん 2例
READ: 直腸がん 1例
SKCM: 悪性黒色腫 1例
STAD: 胃がん 9例
UCEC: 子宮体がん 1例
(方法)
TCGAよりダウンロードした1,691例のRNAシーケンスデータで、がん関連ウイルスのゲノムへの挿入がCD274遺伝子内または周囲で認められるかを検証した。
その中で、前述の26例の中で、3例(CESC 1例、HNSC 1例、STAD 1例)で発がんウイルスのCD274領域への挿入を認めた。
(方法)
ヒト(上段)またはマウス(下段)の細胞株に対して、CD274 3’UTRの5’末端および3’末端の2ヵ所を標的とするsgRNAおよびCas9をトランスフェクションすることにより、これらの細胞株にCD274 3’UTRのほぼ全長(2.7kb)に渡る欠失または逆位をCRISPR(clusered regularly interspaced short palindoromic repeats)/Cas9システムを用いて導入した。その細胞を純化して、フローサイトメトリーによりCD274の細胞表面での発現を評価した。目的の欠失または逆位が導入されていることは、PCRおよびシーケンスにて確認した。
結果を図18に示す。上段はヒト細胞株の結果を、下段はマウス細胞株の結果を示す。
(方法)
CD274 3’UTR異常に伴うCD274高発現が免疫系に与える影響をin vitroで評価するために、CRISPR-Cas9システムによりCD274 3’UTR異常に伴うCD274高発現が誘導されたPC-9細胞株またはコントロール細胞株(Mock)と、PD-1を導入したJurkat T細胞株またはPD-1を導入していないコントロール細胞株(Mock)を共培養後に、Jurkat T細胞のアポトーシスの程度をAnnexin-Vを用いたフローサイトメトリーにより評価した。
結果を図19に示す。横軸の「PC-9」の「SgPD-L1」は、CD274 3’UTR異常に伴うCD274高発現が誘導されているPC-9細胞株を示し、「(-)」はCD274 3’UTR異常に伴うCD274高発現が誘導されていないPC-9細胞株を示す。横軸の「Jurkat」のPD-1はPD-1を導入したJurkat T細胞株を示し、「Mock」はPD-1を導入していないコントロール細胞株を示す。
(方法)
CD274 3’UTR異常に伴うCD274高発現が造腫瘍能に与える影響をin vivoで評価するために、CRISPR-Cas9システムによりCD274 3’UTR異常に伴うCD274高発現が誘導されたEG7-OVA細胞株(SgPd-l1)およびコントロール細胞株(Mock)を同系マウスに皮下移植し、免疫賦活化剤であるpoly(I:C)またはコントロールとしてPBSを移植後7日目より投与し、定期的に腫瘍径を測定した。この実験では、poly(I:C)は腫瘍(特にovalbumin)に対する免疫が誘導されると考えられる。
図20−1に実験のプロトコルを示し、図20−2にMock(図20−2A)およびCD274高発現が誘導されたEG7-OVA細胞株(SgPD-L1)(図20−2B)における腫瘍径の変化を示す。
(方法)
18の実施例と同様のEG7-OVA細胞株の同系移植の実験において、移植後14または15日目にマウスから腫瘍を採取し、CD8およびDAPIの免疫染色によりCD8陽性T細胞の腫瘍内への浸潤の程度を評価した。CD8陽性細胞数は代表的な2〜3匹のマウスから得られた切片より20視野以上計測した。
図21−1に染色像を示し、図21−2に腫瘍内のCD8陽性T細胞を示す。図21−1の染色像では、poly(I:C)投与を投与したコントロール細胞株(Mock)の染色像(右上)において、緑色に染色されたCD8陽性T細胞の数の増加が認められる。
(方法)
CD274 3’UTR異常に伴うCD274高発現による造腫瘍能に対するPD-1/PD-L1阻害の効果をin vivoで検証するために、CRISPR-Cas9システムによりCD274 3’UTR異常に伴うCD274高発現が誘導されたEG7-OVA細胞株を同系マウスに皮下移植し、免疫賦活化剤であるpoly(I:C)を投与したマウスに対して抗PD-L1抗体またはアイソタイプコントロールの腹腔内注射を行い、EG7-OVA腫瘍に与える影響を評価した。これらのレシピエントマウスについては、定期的に腫瘍径を測定し、移植後14または15日目にCD8およびDAPIの免疫染色によりCD8陽性T細胞の腫瘍内への浸潤の程度を評価した。
図22−1に腫瘍径の経時的変化のグラフを、図22−2にCD8およびDAPIの免疫染色像を、図22−3に腫瘍内のCD8陽性T細胞を示す。図22−2の染色像では、抗PD-L1抗体を投与したマウスにおいて(図22−3B)、アイソタイプコントロールを投与したマウス(図22−3A)よりも、緑色に染色されたCD8陽性T細胞の数の増加が認められる。
(方法)
ATL患者検体(ATL049、ATL050、ATL022およびATL079の5検体)またはレトロウイルスによりCD274野生型(WT)または切断変異体を導入したPC-9細胞株を用いて、PD-1 Igに対する結合能をフローサイトメトリーにて評価した。ATL患者検体において、ATL050はORF切断がなく(Intact)、ATL022およびATL079はORF切断が認められた(Truncated)。PC-9に導入した変異体はATL020(Ex7を欠損)、ATL079(Ex6および7欠損)由来であった。
図23に結果を示す。図23AはATL患者検体の評価結果を示し、図23BはPC-9細胞株の結果を示す。
(方法)
TCGAからダウンロードしたDLBC 48検体、STAD415検体、自験例であるATL43検体で、RNAシーケンス解析により求めたCD274発現(エクソン 4 RPKM)とSNPアレイより求めたCD274領域のコピー数がCD274発現とどのように関係する検討した。統計手法としては共分散分析を用いた。
図24に結果を示す。図24AがDLBC(B細胞性リンパ腫)の結果、図24BがSTAD(胃がん)の結果、図24CがATL(成人T細胞白血病)の結果を示す。図24中、黒丸はCD274融合遺伝子(+)および/またはCD274 エクソン 4 / 3’UTR比高値の症例であり、二重丸はCD274領域にウイルスの挿入(+)の症例である。
Claims (14)
- 悪性腫瘍を罹患している被験体において、PD-1/PD-L1阻害剤が治療に有効であるか否かを予測する方法であって、前記被験体から採取した腫瘍細胞においてPD-1/PD-L1阻害剤の有効性に関連するゲノムの異常を検出し、異常が存在する場合に、PD-1/PD-L1阻害剤が前記被験体の治療に有効であると評価する方法。
- PD-1/PD-L1阻害剤の有効性に関連するゲノムの異常が、PD-L1(CD274)遺伝子の発現亢進に関連するPD-L1遺伝子の異常である、請求項1記載の方法。
- PD-L1遺伝子の異常が、PD-L1遺伝子の3'UTR領域の完全なまたは部分的な欠失である、請求項2記載の方法。
- PD-L1遺伝子の異常が、PD-L1遺伝子の3'UTR領域の欠失を引き起こすコピー数変化である、請求項2記載の方法。
- PD-L1遺伝子の異常が、PD-L1転写物からのエクソン6およびエクソン7の全部または部分的な切断である、請求項2記載の方法。
- PD-L1遺伝子の異常が、PD-L1転写物からのエクソン5、エクソン6およびエクソン7の全部または部分的な切断である、請求項2記載の方法。
- PD-L1遺伝子のエクソン1〜エクソン5のいずれかの転写産物とPD-L1遺伝子の3'UTR領域の転写産物を定量し、エクソン転写産物の量と3'UTR領域の転写産物の量の比を算出し、該比が所定の値以上の場合にPD-1/PD-L1阻害剤ががん治療に有効であると評価する、請求項2記載の方法。
- PD-L1遺伝子の異常が、PD-L1 mRNAの発現の亢進である、請求項2記載の方法。
- PD-L1遺伝子の異常が、ウイルスの挿入による異常である、請求項2記載の方法。
- ウイルスがHuman papilloma virus (HPV)又はEBV (Epstein-barr virus)である、請求項9記載の方法。
- 被験体から採取した腫瘍細胞中のPD-L1タンパク質を、PD-L1に対する抗C末端抗体およびPD-L1に対する抗N末端抗体を使用した免疫組織染色により染色し、PD-L1に対する抗N末端抗体で染色が認められるが、PD-L1に対する抗C末端抗体では染色が認められない場合、PD-1/PD-L1阻害剤が前記被験体の治療に有効であると評価する、請求項2に記載の方法。
- 悪性腫瘍が、成人T細胞白血病/リンパ腫、胃がん、大腸がん、膀胱がん、子宮頸がん、腎がん、肺腺がん、皮膚悪性黒色腫およびB細胞リンパ腫からなる群から選択される、請求項1〜11のいずれか1項に記載の方法。
- 悪性腫瘍が、食道がん、頭頸部がん、直腸がんおよび子宮体がんからなる群から選択される、請求項1〜11のいずれか1項に記載の方法。
- PD-1/PD-L1阻害剤が抗PD-1抗体または抗PD-L1抗体である、請求項1〜13のいずれか1項に記載の方法。
