Description

【発明の詳細な説明】 本発明は、セメントなしで骨盤内に錨止されるプラスチ
ック製、とくにポリエチレン類の股関節臼、それも骨と
の接触面、すなわち、組織のゆ着と生え込みを促進させ
る構造をそなえている接触面から、少なくとも１本のピ
ン状突起が突出している形式のものに関している。
イ、従来の技術と発明が解決しようとする問題点プラス
ナック製の内移植体、たとえばポリエヂレン製股関節臼
をセメントを用いずに錨止する場合、長い間にはブラス
ッチクが体液により侵食され、事情によっては分解して
しまうことが分かつている。このため、プラスチックの
股関節臼が、リングまたは金属製ないしセラミック製外
皮のなかに埋め込まれるようになった。これらのリング
や外皮は、その場合、組織のゆ着および（または）生え
込みにより骨と固定結合するのである（ドイツ特許第２
４１１６１７号、米国特許対３，８４０．９０４号）。
しかしながら、前記の問題の解決策は、骨に対する内移
植体の弾性が失われるという欠点がある。
更に、多くの公知の股関節日用構造物の場合、骨に向い
た白衣面からピン状の突起が突出しており、これらの突
起が臼を固定するために相応の骨盤内式に押し込まれる
。これらの突起は、臼体と単体構成の臼構成部分である
から、プラスチック製臼の場合には、同じくプラスチッ
クで造られている（たとえば、スイス特゛許第６４４９
１１Ｍ、英国特許出願公告第００５８７５３号、ドイツ
特許１３３４１７２４号参照）。
口１問題点を解決するための手段 本発明の課題は、骨とプラスナックとの直接接触を出来
るかぎり防止し、しかも、良好な滑り特性のゆえに依然
として有利なプラスチックで造られた股関節臼の弾性が
許容限度以上には損われることのない股関節臼を提供す
ることにある。この課題は、本発明により次のようにし
て解決された。
すなわち、股関節臼の接触面を少なくとも２層の格子状
金属構造物で覆い、更に、突起を同様に金属製にし、股
関節臼体のプラスチック内へ溶着させておくようにした
のである。
金属製の格子は−たとえばチタンもしくはチタン合金か
ら成り一有利にはワイヤ織り物として構成し、内移植体
の弾性がほとんど損われることがないようにする。ウェ
ア、すなわちワイヤの横断面を、内移植体が許容限度以
上に剛性的になることがないように選ぶことができるか
らである。
金Ｉｎの格子の外装によりプラスチックと骨との直接接
触は少なくとも著しく低減せしめられるか、もしくは事
実上防止されざえする。同じくチタンもしくはチタン合
金で造るのが有利である単数もしくは複数の突起は、最
も簡単にはｓｔｙ圧接により臼体内で固定することがで
きる。これらの突起は、中空円筒体状スリーブとして構
成するのが有利である。
スリーブは、その自由端部の中空スペースのところをラ
ッパ状に拡大しておき、縁部は鋭角に終るようにしてお
くと、骨内へのスリーブの進入が改善され、進入のさい
に排除される骨の実質が、成長を促進されるように圧縮
されうる。
股関節臼体のプラスチック内に単数もしくは複数の突起
の進入深さを制限するには、突起の基部を拡大して支持
プレートとして構成し、ここから中空の錨止用円筒体が
臼体のプラスチック内へ突入するようにするのがよい。
プラスチック内での突起のゆるみを防１トするには、支
持プレートおよび（または）防止用円筒体にトラフ状お
よび（または）みぞ状の凹所を形成しておき、かつまた
（もしくは）中空の錨止用円筒の外とうに穴を形成して
おく。これらの凹所およびくまたは）穴により、たとえ
ば、突起の回動やせん断力による動きが生じにくくなり
、また、他方では、みぞ状凹所の間に残っているリブが
、場合によっては、錨止用円筒体の穴と協働して突起の
軸線方向の運動に対する抵抗を高めることになる。
有利には内移植技術で公知の、しばしば用いられるポリ
エチレン変体の１つであるプラスチックの接触面に格子
を取り付けるさいには、簡単に次のようにする。すなわ
ち、高めた温度で軟化させたプラスチックに格子を、圧
力を高めて圧入するのである。その場合、温度は結晶子
溶融温度（１３５〜１４０℃）以下に制限される。また
、必要な圧力は格子の構造と選択された温度に応じて決
まる。たとえば４　　ｋ（ＩＷ／　１ｍ２までの圧力を
用いることができる。
組織の生え込みやゆ着を促進させるためには、格子は２
層以上から成るようにする。その場合、格子の目の゛′
穴寸法”は外方へ向って層ごとに大きくすることができ
る。
次に図面につき本発明の１実施例を説明する。
ハ、実施例 大腿骨頭の人工補整器の関節骨頭（図示せず）を受容す
る股関節臼の林状部１（第１図）は、公知の形式で、大
体において半球形のプラスチック製臼体２内に形成され
ている。大腿骨頭人工補整器の可動域を拡大するために
、臼体２は赤道３の区域で一方の側が斜めにカットされ
ており、この結果、゛完全な″半球形と呼ぶにはオレン
ジ片はどの部分４が欠如している。本発明によれば、骨
盤骨との臼接触面を形成している臼体２球表面は、少な
くとも２層の金ａ製格子５によって覆われている。この
２層の格子の一方の膚がプラスチック表面内に圧入され
、固定されている。
面取り部を有する球欠形部分の、骨との接触面からは、
球表面に対しほぼ半径方向に３本の中空円筒形金属スリ
ーブ６がピン状の突起として突出している。これらの金
属スリーブ６は、３角形状に配置されており、臼のほぼ
主要荷重方向に向いている。
スリーブ６の中空スペース７はラッパ状の拡大部８で終
っている。このため、スリーブ６の中空円筒体壁は鋭角
の縁９で終っている。この縁９は、骨盤骨内へのスリー
ブの進入のさい、゛切断エツジ″として役立ち、そのさ
い排除された骨が中空スペース７へ“流入する”のを助
成し、この骨組織を圧縮させる。スリーブ６の中空円筒
体壁には、組織が生え込むことのできる穴１０が形成さ
れている。
スリーブ６は、臼体側では支持プレート１１で柊ってい
る。この支持プレート１１は、スリーブ６の軸線方向に
作用する力に抗して、臼体２の接触面上の支持力を増大
させる。また、プレート１１の支持面内のトラフ状もし
くはこぶ状断面形状の凹所またはみぞ１２により、回転
力ないし臼体表面に沿って作用するぜん断力に対するプ
レート１１の抵抗性能が高められる。
支持プレート１１は、中空の錨止円筒体１３のかたちで
臼体２の内部まで延びている。この円筒体３には、同様
に外側に凹所もしくはみぞ１４が形成されており、さら
に穴１５が形成されている。
格子５と結合していす、加えて支持区域に遊隙を有して
いるスリーブ６は、臼体２内にＦｊ擦圧接によって組み
付けられる。この摩擦圧接のさいには、臼体２のプラス
チックは、著しく″゛軟化パせしめられるので、錨止用
円筒体１３の進入が１能になり、この円筒体１３と穴１
５とに完全に充満せしめられる。
錨止用円筒体１３は、とりわけ、臼体２内のスリーブ６
を傾倒荷重に抗して固定するのに役立つている。その場
合、穴１５と凹所１４とは、回転荷重や引張り荷重に対
する抵抗を強める機能をもっている。
格子５同様、スリーブ６も、特に組織親和性をもつこと
が公知の金属、たとえばチタン、タンタル、ニオブ、ジ
ルコン、これらの金属との合金のいずれかをベース材料
として造られている。したがって、スリーブ６も格子５
も、組織親和的な表面構造を形成し、時が経つにつれて
、このなかへ骨組織が生え込み、それによって臼がしっ
かりと、骨用セメントなしで骨内に錨止される。格子５
の目幅は、その場合、組織が生え込んでも通りぬけるこ
とのない程度の狭さであるのが有利である。
したがって、格子は骨とプラスチックとの間を完全に遮
蔽し、分離するものとなる。

    【特許請求の範囲】
  1. （１）セメントなしで骨盤内に錨止されるプラスチック
    製、とくにポリエチレン製の股関節臼、それも、組織の
    ゆ着と生え込みを促進させる構造をもつ、骨との股関節
    臼接触面から、少なくとも１本のピン状突起が突出して
    いる形式のものにおいて、股関節球の接触面が少なくと
    も２層の格子状金属構造物（５）により覆われており、
    更にまた、突起（６）が同じく金属製であつて、股関節
    臼体（２）のプラスチック内へ溶着されていることを特
    徴とするセメントなしで骨盤内に錨止される股関節臼。

    のところがラッパ状に拡大されており、縁部（９）が鋭
    角をなしていることを特徴とする特許請求の範囲第２項
    記載のプラスチック製股関節臼。
  4. （４）突起（６）は、基部が拡大されて支持プレート（
    １１）となつており、この支持プレートから中空の錨止
    円筒体（１３）が股関節臼体（２）のプラスチック内に
    突入していることを特徴とする特許請求の範囲第１項記
    載の股関節臼。
  5. （５）突起（６）の固定が摩擦圧接により行なわれてい
    ることを特徴とする特許請求の範囲第１項記載の股関節
    臼。
  6. （６）突起（６）と格子（５）とがチタンもしくはチタ
    ン合金製であることを特徴とする特許請求の範囲第１項
    記載の股関節臼。
  7. （７）支持プレート（１１）および（または）錨止円筒
    体（１３）には、みぞ状の凹所（１２、１４）が形成さ
    れていることを特徴とする特許請求の範囲第４項記載の
    股関節臼。
  8. （８）中空の錨止円筒体（１３）は、その外とう部に穴
    （１５）が形成されていることを特徴とする特許請求の
    範囲第４項記載の股関節臼。
