Description

【発明の詳細な説明】 本発明は蛋白分解酵素及び／又は多糖類・ムコ多糖類分
解酵素を有効成分とする肝臓及び腎臓疾患防活療剤に関
する。
肝臓及び腎臓の疾患については既によく知られているが
、殊に近年、ウィルス　性肝炎、アルコール性肝炎及び
同種の腎炎は増加傾向を示し、社会的問題となっている
。最近の新学説にもうたわれているように、急性肝炎か
ら慢性肝炎へ、慢性肝炎から肝硬変に、肝硬変から肝＃
癌へ移行する例が増加しており、また同様に腎炎から慢
性腎炎、腎ネフローゼを経て腎不全へと悪化推移する例
も増加している。
従って、これらの臓器の障害を炎症の時期に治癒し、癌
化や腎機能不全を予防するのが目下の急務であるし、人
命を救う上に極めて大切である。
本発明者らは蛋白分解酵素の研究を永年、重ねて来た結
果、蛋白分解酵素および多糖類・ムコ多糖類分解酵素が
肝臓及び胃酸の各種疾患の予防及び治療に有用であるこ
とを見出した。
本発明は、この知見に基づくもので蛋白分解酵素及び／
又は多糖類・ムコ多糖類分解酵素を有効成分とする肝臓
及び腎臓疾患予防治療剤である。
本発明の有効成分である蛋白分解酵素としては、例えば
、トリプシン、α−キモトリズシン、プロメライン、パ
パイン、セラチオベグチダーゼ、セアプローゼ、ゾロテ
アーゼ、ストレプトキナーゼ。
ゾロクターゼ、プロナーゼ、プロザイム、ウロキナーゼ
、パンクレアチン、フィブリツリシバ１゜多糖類０ムコ
多糖類分解ｌ！素としてはα−アミラーゼ、塩化リゾチ
ーム、ヒアルロニダーゼ等カアけられ、これらを一種又
は二種以上ｔ−適宜組合せて用いる。
本発明の予防治療剤は通常の製剤技術により、例えば錠
剤、カプセル剤、散剤、拳粒剤等として経口的に、注射
剤、坐剤、軟膏剤等として非経口的に投与できる。更に
末剤は他の薬剤、例えば他の肝＊＊、腎臓薬、抗生物質
、免疫賦活剤、抗腫瘍剤などと併用できる。投与量は酵
素の種類、疾り、また胃液にて不安定な酵素の場合は腸
溶製剤として、／θ〜／θｏｏｔＲｆにより、目的を達
成することができる。また投与量を各酵素の酵素単位で
表わした場合には、腸溶製剤にてセラチオペプチダーゼ
は２万〜４１ｏ万セラチオペプチタ一ゼ単位、プロメラ
インはダ万〜１０万プロメライン単位、ストレプトキナ
ーゼは％　２万〜１０万単位、プロ廿−ゼは／万〜ｊＯ
万チロシン単位の経口投与により目的を達成することが
できる。
酵素の種類により、腸溶製剤とするのが好しい。
本肝臓及び腎臓疾患予防治療剤の特色は、吸収された蛋
白分解酵素及び多糖類・ムコ多糖類分解ＩＩ素が障害部
位の肝臓及び腎臓に極めて多量に移行し、かつ障害部位
の組織には浮謙の消褪、円形細胞の浸潤、血管の新生、
線維素の溶解、多糖類殊に酸性多糖類の増加などが認め
られ、肝シ及び腎臓の慢性炎症像即ち退行変成や異常増
殖醇に治癒反応が惹起、促進されていることが認められ
るＯ製剤例１ セラチオペプチダーゼ　／θ７及び乳糖／りＯｆ馬鈴薯
殿粉７０グを均一に混合したのち３％ヒドロキシプロピ
ルセルロース水溶液な注加練合する。
混合物を整粒し、この粒状物に対しｏ３％のステアリン
酸マグネシウムを混合して打錠し、錠剤とする。
製剤例２ プロナーゼλ０７．乳糖ゲ０りを混合し、ヒドロキシプ
ロピルメチルセルロース１ＯｆＩを加え顆粒に成形した
のち酢酸フタル酸セルロースを用い均等に被膜し、腸溶
性顆粒とする。
製剤例３ 実施例２で得たプロナーゼ腸溶性顆粒をカプセルに充填
し、カプセル剤とする。
臨床例１ 男性、太正／３年生 診断名：肝硬変、慢性肝炎 主　訴二食欲不振、右季肋部やや圧迫感、全身倦怠 現病歴＝−〜３年前から上記症状あり、ｓ月２ｙ日、他
の病院にて肝硬変と診断された。
現　症：肝臓やや肥大する他異常を認めず、崩清検査及
び超音波診断を行う。
超音波診断：両腎鯵は異常なし、牌臓やや肥大。
胆嚢は大きな嚢胞状で、胆石は認めず。
総胆管は拡張せず。膵臓はやや腫張気味、嵌面表清。膵
内には着倒な変化は認められない。
肝　ｌｌ：表面はぼ平滑だが組織はやや委縮気味、肝管
腔が細くなり、屈折はあるが、肝内に限局性の異常像は
ない。肝細胞のバイオプシーにて軽度の肝硬変と慢性肝
炎の像を認む。
肝臓機能検査： 第１表に示すように５月、２４日から７月１０日までに
５回検査し、観察した。初診時は／３項中黄痕指数、Ｇ
ＯＴ、ＧＰＴ、ＬＡＰ、γ−ＧＴＰのｊ項目に異常１に
認めた。
プロナーゼによる治療： エンピナースＰ（科研化学製・腸溶剤・７錠中プロナー
ゼＶＯＯθチロシン単位含有）を７日を錠。
毎食後２錠ずつ、５月３日より内服。６月５日。
検査。’Ｉ”ＴＴが異常値になった他、各項目ともやや
増悪。
４月ｌ＃日には、急激に改善、γ−ＧＴＰのみ異常を示
す。７月ｉｏ日、γ−ＧＴＰも低下。７月３ｏ日、全て
正常値を示した。
自覚症状も快善、種々検査成績も正常になった。
その後、再発予防のため、更に３ケ月内服を続けた。
第１嵌　肝機能検査成績経過 ＊：異常値 女性、大正り年生 診断名：慢性肝炎 主　訴：全身倦怠１食欲不振 現病歴：約ｔケ月前より全身倦怠及び食欲不振を訴え、
仕事をする気力がなくなった。
現　症：ｊｌＩｉｉ色やや蒼白く、少し黄色を帯ぶ。
腹部はやや肥大するが、腹水貯留など異常を認めず。
超音波診断、し線検査：両側腎籐、牌臓異常なし。胆嚢
や＼肥大、胆石は認めず。膵臓、大腸。
小腸、胃、十二指腸には著変なし。
肝臓二表面平滑、肝管腔はや＼細くなっているが、萎縮
や限局性の異常健は肝組織に認められない。
肝機能検査： 第２表に示すように７７月７日の検査にて、／Ｊ項目中
、を項目に異常を認めず。
第２表　肝機能検査成績経過 ＊：異常値 プロナーゼによる治療： エンピナースＰを７日を錠、毎食後２錠ずつ、内服し、
経過を観察した。翌年７月３０日にはγ−ＧＴＰ　　は
正常となったが他はやや軽減したが正常には戻らカかっ
た。３月ｊ日にはＴＴＴとＺＴＴ以外は正常となり、ｊ
月ＩＩ日にはＴＴＴとアルブミン、　Ａ／Ｇ　　に異常
ｔ−認めたが、アルブミンとＡ／Ｇ　　は低かった。を
月ｊ日には、全て正常となった。治療開始よりＪ／Ｊ日
目であった。
臨床例３〜７：プロナーゼによる肝疾患治療各側ともエ
ンピナースＰ１日６錠の内服。
更に第３表に示した様に１肝硬変／例と慢性肝炎ダ例に
エンピナースｐ’ｌ内服せしめ、治癒困難とされている
これらの症例に極めて有効な成績を示した。
臨床例を 男性　昭和ｊ学生 診断者：慢性糸球体腎炎 主　訴：全身倦怠 現病歴＝５〜１年前より時に全身倦怠、顔面の浮ｍ’を
訴えた。来院−退館の検査にて尿中の蛋白検出及び血圧
が高かった。
現　症：体格、栄養中等度、顔面やや蒼白、少し浮腫状
、扁桃やや肥大、血圧／／、０／。
尿検査ニア日尿蛋白　ｔ３７ 尿沈渣：派内に硝子様円柱（ト）、ａ粒円柱（ト）、白
血球円柱（ト）、赤血球円柱田、顕微鐘的血尿（ト）血
液化学：尿素窒素は正常 腎機能検査：　ＧＦＲは４／ｉｔ杉扮ｓ濃縮試験やや低
下、従って腎機能はやや低下、慢性糸球体腎炎として大
阪大学病院に紹介、そこで腎ノ（イオプシーにて、糸球
体及び腎実質にやや変成を認められた。
プロナーゼによる治療： ｒ月２ｏ日より、エンビナースＰｉ／日６錠。
毎食後２錠ずつ内服した。
デ月３０日、血圧　／ｓ２／ｒ７　　尿蛋白７日ｉ、ｏ
Ｖ。
尿沈渣　赤血球円柱（−）　　顕微鏡的血尿（−）尿蛋
白７日０７り。
１０Ａ　ｊＯＦ３　　Ｊｆ［ＬＢ：　／Ｗｌ／、。
尿沈渣　派内には硝子様円柱のみ。
７２月２６日　血圧　／　４”Ｊ／、、　　尿蛋白／日
ａ３ｆ０尿沈渣　硝子様円柱少量。
／３４ 翌年λ月２ａ日　血圧　　／７５　　尿蛋白Ｏ尿沈渣　
なし。
再び大阪大学病院で腎バイオプシー施行、糸球体及び腎
実質に異常を認めず。約ｔケ月の治療で　難治と云はれ
た慢性糸球体腎炎がほぼ治癒した。
臨床例り 男性：昭和７年生 診断名：ネフ四−ゼ症候群 主訴：食欲不振　無気力感 現病歴二半年前より食欲不振、無気力感を訴えていたが
治療せず放置。健康診断にて蛋白尿を指適 現症：体格　栄養中等度　顔面浮腫を呈す。血／４１ 圧　　　　　　／ツ、２ 尿検査：　Ｊ、Ｉ　５１７日　の多量の蛋白尿を示す。
尿沈渣：ｖＡ粒円柱、硝子様円柱、脂肪円柱が認められ
る。とくに本症例には重屈析脂肪体が認められた。
大阪府立成人病センターに紹介、種々検査の上ネフロー
ゼ症候群と診断。
プロナーゼによる治療： ／：１月！日よりエンビナースＰを７日を錠、毎食後２
錠ずつ内服した。
翌年１月７４１日、尿蛋白ユダｆ／日と減少０　尿沈渣
は変らず、全て円柱を認む。
２月２／日　尿蛋白４２７７日と減少。尿沈渣。
全般的に減少するが猶全円柱を一認む。
３月３０日尿蛋白認めず＝尿沈渣消失。
約ＩＯ日でほぼ治癒した。
動物試験例 酵素の肝臓疾患に対する効果を試験した。平均体重２３
０　ｆのラットに四塩化炭素を腹腔内にａＳ、／４投与
し、肝炎を起こさせ２ゲ時間後に血液を採取し、肝臓障
害の指標となるＧ、　Ｏ，Ｔ、　及びＧ、　Ｐ。
Ｔ、をｉｌｌ定した。なお酵素は四塩化炭素投与の６．
ｊ。
２．１日前及び当日に夫々の投与量を経口で投与した。
この結果を第寺表にまとめた。酵素投与により肝臓障害
が軽減された。

  1. 蛋白分解酵素及び／又は多糖類・ムコ多糖類分解酵素の
    一種又は二種以上を有効成分とする肝臓及び腎臓疾患予
    防治療剤。
