Claims (1)
- 【特許請求の範囲】 1.とられた身体体位における地球の重力場に対する患者の身体軸の照合により 、上下方向軸と前後方向軸と中心側方方向軸とを含む前記身体軸を有する患者の 身体的姿勢を決定する装置において、 前記上下方向、前後方向および中心側方方向の身体軸に一致する第1と第2と 第3の体位軸の一般的に予め定めた整合関係において、患者の身体に植込まれる 前記第1と第2と第3の体位軸を有する植込み可能なハウジングと、 前記植込み可能なハウジングの前記第1と第2と第3の体位軸の1つと整合さ れる第1の感知軸を有する前記植込み可能ハウジングの内部に取付けられ、患者 によりとられる身体体位における前記第1の感知軸に対して、地球の重力場の整 合度の関数として大きさおける極性において変動する第1のDC加速度計信号を 生じる第1のDC加速度計と、 前記植込み可能なハウジングの前記第1と第2と第3の体位軸の他の1つと整 合される第2の感知軸を有する前記植込み可能ハウジングの内部に取付けられ、 患者によりとられる身体体位における前記第2の感知軸に対して、地球の重力場 の整合度の関数として大きさおよび極性において変動する第2のDC加速度計信 号を生じる第2のDC加速度計と、 前記第1および第2のDC加速度計信号の大きさおよび極性の比較により、患 者の身体的姿勢を決定する手段と を備える、患者の身体的姿勢を決定する装置。 2.単位時間にわたり反復する身体の運動の周波数から患者の活動レベルを決定 する手段と、 患者の決定された身体的姿勢と活動レベルとを格納する手段とを更に備える請 求項1記載の装置。 3.患者の前記身体姿勢と活動レベルとに依存する処置パラメータを有する患者 に対して処置を提供する手段を更に備える請求項1記載の装置。 4.前記DC加速度計が更に、 前記上下方向の身体軸における患者の身体に対する地球の重力場の一定加速度 を測定して、患者が地球の重力場との略々整合関係になるように前記第1あるい は第2あるいは第3の感知軸を移動する種々の身体体位をとる時、前記加速度か ら前記第1のDC加速度計信号を取得する第1のソリッドステートDC加速度計 と、 前記前後方向および中心側方方向の身体軸の1つにおける患者の身体に対する 地球の重力場の一定加速度を測定して、患者が地球の重力場と略々整合関係にな るように前記第1または第2または第3の感知軸を移動する種々の身体体位をと る時、前記第2のDC加速度計の信号を前記加速度から取得する第2のソリッド ステートDC加速度計と、を含む請求項1記載の装置。 5.前記DC加速度計が更に、 前記中心側方方向の身体軸における患者の身体に対する地球の重力場の一定加 速度を測定して、患者が地球の重力場と略々整合関係になるように前記第1また は第2または第3の感知軸を移動する種々の身体体位をとる時、第3のDC加速 度計信号を前記加速度から取得する第3のソリッドステートDC加速度計を含み 、 前記患者の身体的姿勢を決定する前記手段が、前記第1と第2と第3のDC加 速度計信号のパラメータの比較に応答する、請求項4記載の装置。 6.地球の重力場に対する前記第1と第2と第3のDC加速度計の感知軸の整合 における前記第1と第2と第3のDC加速度計信号の第1の特徴的な大きさと極 性と、地球の重力場に反する整合における前記第1と第2と第3のDC加速度計 信号の第2の特徴的な大きさと極性と、地球の重力場に対して直角をなす整合に おける前記第1と第2と第3のDC加速度計信号の第3の特徴的な大きさと極性 とを規定する手段を更に備え、 前記患者の身体的姿勢を決定する前記手段が、前記取得された第1と第2と第 3のDC加速度計信号の大きさと極性と、前記第1と第2と第3の特徴的な大き さと極性との比較に応答する、請求項5記載の装置。 7.患者の閾値活動レベルを示す予め定めた周波数レンジ内で生じる身体の運動 によって生じる前記第1と第2と第3のDC加速度計信号の特徴的な活動の大き さを規定する手段と、 予め定めた期間にわたり前記特徴的な活動の大きさを越える前記第1または第 2または第3ののDC加速度計信号から活動レベル信号を取得する手段と、を更 に備える請求項6記載の装置。 8.患者の決定された身体的姿勢と活動レベル信号に依存する処置パラメータを 有する患者に対して処置療法を提供する手段を更に備える請求項7記載の装置。 9.患者の前記決定された身体的姿勢と活動レベルを格納する手段を更に備える 請求項7記載の装置。 10.患者の活動、および地球の重力場に関する上下方向の身体軸と前後方向の 身体軸と中心側方方向の身体軸とを有する患者の身体的姿勢に依存するペーシン グ拍数で患者の心臓をペーシングする装置において、 前記上下方向と前後方向と中心側方方向との身体軸の少なくとも2つにおける 患者の身体に対する重力の一定の加速度を測定して、該加速度から地球の重力場 に対する整合における特徴的な大きさと極性を有する第1および第2のDC加速 度計信号を提供し、地球の重力場に対する第1および第2のソリッドステートD C加速度計の誤整合度に依存する大きさと極性を変化させる第1および第2のソ リッドステートDC加速度計手段と、 前記第1および第2のDC加速度計信号の大きさおよび極性と前記特徴的な大 きさおよび極性との比較により、患者の姿勢と関連する姿勢体位信号を決定する 手段と、 単位時間にわたり反復する身体の運動の周波数から患者の活動信号を決定する 手段と、 決定された身体の姿勢および活動レベルにおける患者の心臓に対する生理的要 求と相関する身体体位および患者の活動信号から拍数制御信号を取得する手段と 、 前記拍数制御信号の関数として生理的逸脱間隔を規定して生理的ペーシング拍 数を確立する手段と、 前記生理的ペーシング拍数でペーシング・パルスを生成する手段と、 前記ペーシング・パルスを患者の心臓の一つの室へ印加する手段と、を更に備 える患者の心臓をペーシングする装置。 11.それぞれ前記上下方向の身体軸と、前記前後方向および中心側方方向の身 体軸の1つに対して前記第1および第2のソリッドステートDC加速度計の第1 および第2の感知軸をそれぞれ略々整合させ、これから前記第1および第2のD C加速度計信号を取得する手段を更に備える請求項10記載の装置。 12.前記中心側方方向および前後方向の身体軸に対して、それぞれ前記第1お よび第2のソリッドステートDC加速度計の第1および第2の感知軸を略々整合 させて、これから前記第1および第2のDC加速度計信号を取得する手段を更に 備える請求項10記載の装置。 13.ペースメーカのパルス発生器ハウジングと、 直立姿勢にある間前記患者の身体軸の1つに前記第1の感知軸を略々整合させ て前記パルス発生器が植込まれるように、前記ペースメーカのパルス発生器ハウ ジングにおける第1の感知軸に沿って加えられる地球の重力場のDC作用力とA C加速度作用力とに応答して大きさが変化する第1の出力信号を生じる偏移を示 す第1の感知軸を有する第1のソリッドステートDC加速度計を取付ける手段と 、 前記第1の感知軸に対して略々直角で前記ペースメーカのパルス発生器におけ る第2の感知軸に沿って加えられる地球の重力場のDC作用力と加速度のAC作 用力とに応答して大きさが変化する第2の出力信号を生じる偏移を示す第2の感 知軸を有する第2のソリッドステートDC加速度計を取付ける手段と、を更に備 え、 前記ペースメーカのパルス発生器ハウジングが、前記第1の感知軸が患者の身 体軸の選択された1つと略々整合され、前記第2の軸が患者の身体軸の他の1つ と略々整合されるように、患者の身体に植込まれ、 患者の姿勢と関連する身体体位信号を決定する前記手段が更に、 地球の重力場の作用力に対する前記第1および第2のDC加速度計の前記第 1および第2の感知軸の整合によって生成される第1および第2の出力信号のD C成分を表わす基準DC加速度計信号の大きさを生じる手段と、 前記第1および第2のDC加速度計信号の大きさを前記基準DC加速度計信号 の大きさと比較し、該比較から地球の重力場の作用力に関する患者の身体の体位 を決定する手段と、 決定された患者の身体的姿勢を表わす身体体位信号を生じる手段と、を更に備 える請求項10記載の装置。 14.ペースメーカのパルス発生器ハウジングと、 直立姿勢にある間にパルス発生器が第1の感知軸を患者の上下方向の身体軸へ 略々整合させて植込まれるように、前記ペースメーカのパルス発生器における第 1の感知軸に沿って加えられる地球の重力場のDC作用力と加速度のAC作用力 とに応答して大きさが変化する第1の出力信号を生じる偏移を示す第1の感知軸 を有する第1のDC加速度計を取付ける手段と、 前記直立姿勢にある間に前記パルス発生器が第2の感知軸を患者の前後方向の 身体軸に対して略々整合させて植込まれるように、前記第1の感知軸に対して略 々直角に前記ペースメーカのパルス発生器における第2の感知軸に沿って加えら れる地球の重力場のDC作用力と加速度のAC作用力とに応答して大きさが変化 する第2の出力信号を生じる偏移を示す第2の感知軸を有する第2のDC加速度 計を取付ける手段と、 前記直立姿勢にある間に前記パルス発生器が第3の感知軸を患者の中心側方方 向の身体軸に対して略々整合させて植込まれるように、前記第1および第2の感 知軸に対して略々直角に前記ペースメーカのパルス発生器における第3の感知軸 に沿って加えられる地球の重力場のDC作用力と加速度のAC作用力とに応答し て大きさが変化する第3の出力信号を生じる偏移を示す第3の感知軸を有する第 3のDC加速度計を取付ける手段と、を更に備え、 前記ペースメーカのパルス発生器ハウジングが、前記直立姿勢にある間、前記 第1の感知軸が患者の上下方向の軸と略々整合され、かつ前記第2および第3の 軸がそれぞれ患者の前後方向の身体軸および中心側方方向の身体軸と略々整合さ れるように、患者の身体に植込まれるようになっており、 前記患者の姿勢と関連する身体体位信号を決定する前記手段が更に、 地球の重力場の作用力により、前記第1と第2と第3のDC加速度計の前記 第1と第2と第3の感知軸のそれぞれの整合によって生成される第1と第2と第 3の出力信号の大きさのDC成分を表わす基準DC加速度計信号の大きさを生じ る手段と、 前記第1と第2と第3のDC加速度計信号の大きさを前記基準DC加速度計 信号の大きさと比較して、該比較から地球の重力場の作用力に関する患者の身体 の体位を決定する手段と、 決定された患者の姿勢を表わす身体体位信号を生じる手段と、を更に含む請 求項10記載の装置。
