Claims (1)
- 【特許請求の範囲】 1. カテーテルと、スタイレットとを備えたカテーテル組立体であって、該カ テーテルは、カテーテルの基部領域から末端領域まで伸長する誘導ワイヤ管腔を 有したシャフトを備えており、前記カテーテルは、迅速交換モード及びオーバー ・ザ・ワイヤモードのどちらの使用にも適しており、前記カテーテルは、該カテ ーテルの末端より近くにあるその末端領域の任意の位置で誘導ワイヤ管腔内へ進 入する側口を有していて前記迅速交換モードにおいて使用可能にしており、前記 カテーテルは、該側口の基端部分に隣接して固定され且つ第1位置及び第2位置 を有した誘導要素を有しており、該第1位置では、前記誘導要素が誘導ワイヤ管 腔を横断して下方へ伸長して進路遮断位置を取り、カテーテルの末端を介して誘 導ワイヤ管腔内へ挿入された誘導ワイヤを側口の外へ片寄らせ、該第2位置では 、誘導要素が管腔内進路遮断位置から外れてカテーテル本体に略沿って横たわり 、且つ、前記スタイレットは、迅速交換モードで前記カテーテルを使用中該カテ ーテルの誘導ワイヤ管腔の基端部分内に位置し、該スタイレットの末端が前記誘 導要素より近くにあって、前記カテーテルの対応する部分を容易に押せるような サイズにされているカテーテル組立体において、 該カテーテル組立体は、解放自在のスタイレット停止構造体を含み、該停止構 造体は、前記スタイレットの誘導ワイヤ管腔内への挿入距離を、迅速交換モード 中に使用するために前記スタイレットの末端が前記誘導要素より近くに位置する 第1スタイレット位置までに限定するように構成されており、前記停止構造体は 、解放自在となって前記スタイレットが前記誘導ワイヤ管腔内を更に前進して前 記誘導要素と係合することが可能となり、前記スタイレットは、前記停止構造体 が解放されると、前進して前記誘導要素をその前記第1位置から前記第2位置へ 変位させ、その後に前記誘導ワイヤ管腔から取り除かれるようなサイズ及び構造 にされており、これにより、前記カテーテルを迅速交換モード使用から変換しよ うとした場合には、前記スタイレットを使用して前記誘導ワイヤ管腔を開けるこ とが可能となることを特徴とするカテーテル組立体。 2. 前記スタイレットは、末梢球根状端部を有し、該末梢球根状端部は前記ス タイレットの該末梢球根状端部に直に近接する部分より大きくされており、前記 末梢球根状端部は、前記誘導要素に係合して前記第1位置から前記第2位置へ変 位させる一方で、前記スタイレットを迅速交換モードでの使用中に所定の位置に 配置した時に前記カテーテルの可撓性に悪影響を及ぼすことがないように構成さ れている請求項1に記載のカテーテル装置。 3. 前記スタイレットは、前記カテーテルに十分な剛性を与えて該カテーテル が所定位置へ押しやられるのを可能にする選択された第1の径を有する長い近位 の第1部分と、該近位の第1部分より遠くにある比較的短い第2部分とを有して おり、該第2部分は末端方向に小径となるまでテーパが施されており、比較的柔 らかな先導部として機能する請求項1または請求項2に記載のカテーテル装置。 4. 前記誘導要素は、前記カテーテルの一体部分であり、前記第1及び前記第 2位置のどちらに配置されても安定しているように二安定特性を有している請求 項1に記載のカテーテル装置。 5. 前記誘導要素は、マンドレルを前記カテーテルの誘導ワイヤ管腔内へ挿入 し、該マンドレルを前記誘導要素が配置される位置まで前進させ、前記カテーテ ルを前記誘導要素位置近傍で熱形成温度まで加熱し、及び前記カテーテルの壁材 から該壁の対応する部分に形成器具を前記マンドレルに向けて押圧し、これによ り前記カテーテル壁の係合部分が可塑変形して前記誘導要素を形成する段階から 成る方法により形成される請求項1または請求項4に記載のカテーテル装置。 6. 前記解放自在の停止構造体は、前記スタイレットの基端部分と関係したハ ンドルからの前方延長部を備えており、該延長部は、有効位置にあると前記カテ ーテルと関係した一定の表面に前向きに当接して前記スタイレットの前記誘導ワ イヤ管腔内への挿入量を制限するように構成されており、該延長部が有効位置か ら取り除かれると、前記スタイレットは、前記誘導ワイヤ管腔内を前進可能とな る請求項1に記載のカテーテル装置。 7. 前記延長部は、前記ハンドルと一体に形成され、屈曲自在となって前記有 効位置から取り外し可能となる請求項6に記載のカテーテル装置。 8. 前記延長部は、ヒンジにより前記ハンドルに結合される請求項7に記載の カテーテル装置。 9. 前記延長部は、破壊されて前記ハンドルから取り外されるように構成され ている請求項7に記載のカテーテル装置。 10. 前記解放自在の停止構造体は、枢動自在のワイヤ取っ手である請求項1 に記載のカテーテル装置。 11. 前記解放自在の停止構造体は、ハウジング内に配置され、該ハウジング は、前記スタイレットへのアクセスが望まれる時には取り外し自在となる請求項 1に記載のカテーテル装置。 12. 前記ハウジングは、取り外されて前記スタイレットに沿って加圧した噴 出液の進入を可能とする注射器と交換可能となる取り外し自在の基端キャップを 有している請求項11に記載のカテーテル装置。 13. バルーンが前記側口から遠くにあり且つ前記カテーテルの末端より近く にある部位で前記カテーテルシャフトの回りに取付られ、該シャフトは、前記カ テーテルの基端から前記バルーンの内部まで伸長する膨張管腔を含んでいる請求 項1に記載のカテーテル装置。 14. 前記バルーンは、PTCAバルーンである請求項13に記載のカテーテ ル装置。 15. カテーテルを迅速交換モードからスタイレットを使用したオーバー・ザ ・ワイヤモードへ変換する方法であって、前記カテーテルは、該カテーテルの基 部領域から末端領域まで伸長する誘導ワイヤ管腔を有するシャフトを備えており 、前記カテーテルは該カテーテルの末端より近くにある該カテーテルの末端領域 の一定の位置で前記誘導ワイヤ管腔に進入して前記迅速交換モードで使用可能と なる側口を有しており、前記カテーテルは、該側口の基部に隣接して固定され且 つ第1及び第2位置を有した誘導要素を有しており、該第1位置では、該誘導要 素は、前記誘導ワイヤ管腔を横断して下方に伸長して進路遮断位置を取り、前記 カテーテルの末端を介して前記誘導ワイヤ管腔内へ挿入された誘導ワイヤを前記 側口の外側へ進路方向を変更させ、該第2位置では、管腔進路遮断位置から外れ てカテーテル本体に略沿って横たわり、前記スタイレットは、迅速交換モードで 前記カテーテルを使用中該カテーテルの前記誘導ワイヤ管腔の基端部分内に位置 し、前記スタイレットの末端部が前記誘導要素より近くに位置して前記カテーテ ルの 対応する部分を容易に押せるようなサイズにされているカテーテルを迅速交換モ ードからスタイレットを使用したオーバー・ザ・ワイヤモードへ変換する方法に おいて、 前記カテーテル及び前記スタイレットにより画定されるカテーテル装置は、解 放自在のスタイレット停止構造体を含み、該停止構造体は、前記スタイレットの 前記誘導ワイヤ管腔内への挿入距離を前記スタイレットの末端部が前記誘導要素 より近くに位置し迅速交換モード中に使用されるようにされた第1スタイレット 位置までに限定するように構成されており、前記停止構造体は解放自在となって 前記スタイレットが前記誘導ワイヤ管腔内を更に前進して前記誘導要素と係合す ることが可能なようにされ、前記スタイレットは、前記停止構造体が解放される と前進して前記誘導要素をその前記第1位置から前記第2位置へ変位させ、その 後に前記スタイレットが前記誘導ワイヤ管腔から取り除かれるようなサイズ及び 構造にされており、これにより、前記カテーテルを迅速交換モード使用から変換 しようとした場合には、前記誘導ワイヤを取り外して、前記停止構造体を解放し 、前記スタイレットを前進させて前記誘導要素をその前記第2位置に配置し、そ の後、前記スタイレットを後退させて前記誘導ワイヤ管腔を開放することを特徴 とするカテーテルを迅速交換モードからスタイレットを使用したオーバー・ザ・ ワイヤモードへ変換する方法。
