悪性腫瘍の検出のための免疫学的方法およびその方法を実施するためのキット
技術分野
本発明は、悪性腫瘍を検出しそして患者の血清−免疫グロブリンG(IgG)の
特徴によって腫瘍塊を測定する方法に関する。
背景技術
悪性腫瘍を持つ患者には、血清−IgGの範囲内で(ジチオニトロベンゾエート
反応によって測定された)スルフヒドリルまたはジスルフィド基の量に重大な変
化を検出できることが数年間にわたって知られてきた。スルフヒドリル対ジスル
ヒド基の比率に対する実験測定値はΣS値を意味する。それによってΣS値の変
化の理由は最近まで不明であった。
この領域のさらに進んだ研究の間に、ΣS値の変化は他のサブクラス(例えばI
gG2)に対するサブクラスIgG1の比率の変位を追跡することができること
が示された。
この相関関係は新しく、基本的な科学的知識に加えて、診断学にその現象を利用
する可能性を当業者に提供する。
それらと無関係に、この分野における研究は、新鮮な血液または血清試料の測定
値は短期間内に比較的強く変化することを繰り返して示した。それゆえ、未処理
血液または血清試料についてIgGに対するさまざまな分析手順を実施すること
は、再現可能で計画可能な結果を与えるものではない。
発明の開示
それゆえ、本発明の目的は、患者の悪性腫瘍の起りうる存在および大きさおよび
発育に関して、少なくとも1日という目的にかなった時間の間に、再現性のある
結果を提供することである。
これは、患者の新鮮な血液または血清試料はラジカル抑制剤、好ましくはベルオ
キシドジムターゼ(SOD)によって安定化されること、および変位は標準の比
率と比べた、IgGサブクラスの和に対するIgGサブクラスの少なくとも1種
類の比率について試料の血清にて決定されることで達成される。
好ましくは、サブクラスIgG1および/またはサブクラスIgG2の比率の変
位は、標準の比率に比べた、IgGサブクラスの和に対する安定化された試料の
血液で決定される。
血清試料が空気にさらされて耐えているとき、血清のIgG1濃度の強い損失を
示すマンシーニ(Man c i n i)による放射免疫拡散法(RI D)
の使用によってIgG1値の安定性に関する実験を未処理試料について行った。
この効果は血液回収の実質的に直後に続いて起こり、それは平均して7時間後2
0%であり、36時間後少なくとも25%である。なお、測定は両性の健康な供
給者の血液試料を使用して行った。
効果の程度は、とりわけ、最初の値の高さによって決まる。
高いIgG1値を持つ試料は普通はその値の強い減少を示すが、予備的結果によ
って、あたかも他の因子もまた含まれるように見える。
前記のことは、血液の血清中のIgG1濃度の正確な定量は効果的な安定化なし
には臨床的環境においては不可能であることは既に明瞭であるが、そのような安
定化は、IgGサブクラス間の比率の変位が本発明による方法において腫瘍のマ
ーカーとして癌患者の血液中で評価されるとき絶対的に必要なことになる。
ラジカル抑制剤による安定化は非常にうまく行きそうに思われるが、明らかに最
善の結果はいままでのところSODの添加によって達成されてきた。SODは新
鮮な完全血液試料または血球物質が分離された後の血清試料に添加してもよい。
非常に有利な安定化結果が、SODによる血清の安定化が100〜l、000、
好ましくは300〜400国際単位SOD/ml血清の質量比で行なわれるとき
に得られる。
したがって、5.000〜io、ooo国際単位SOD/mlの濃度を有する溶
液にてSODを血清試料に添加することが好都合である。このやり方で安定化さ
れる試料は、続いて普通の比率と比較して、IgGサブクラスの和に対するサブ
クラスIgG1の比率の変位について試験される。
血液の血清中の互いに対するIgGサブクラスの比率の変化は、非常に驚くべき
ことに、悪性腫瘍の出現および身体中に存在する腫瘍塊の数および広がりにさえ
結び付き、その結果悪性腫瘍に関する定性的のみならず定量的な情報をも提供す
ることができる。
それゆえ、本発明による方法は、患者の腫瘍発育の外部(体外の)監視に好まし
く利用される。そのような方法は、主として治療的処理の間にまたは続いて世話
をする(監視する)医師に特に興味を持たれる。本発明による方法はまた予防医
学に非常に首尾良く適用でき、それによって起こりつる腫瘍の発育に対する最初
の前兆を単純なやり方で得ることができる。
その方法は患者に少しも不便ではなくて、今までのところ得られた経験によれば
、現在利用できる他の方法よりも更に信頼できる。
それらに関して、現在量もしばしば使用されている方法は腫瘍マーカー法である
。
実際の状況に本発明による方法を適用することにおいて、上記に言及した比率の
計算はIgGに対するこれまで知られた分析方法、特にクロマトグラフ法または
免疫学的方法に好都合に基づいていることが明らかになる。免疫−拡散−検定の
適用は、この状況において特に成功したことを証明した。単純な免疫−吸着カラ
ム法もまた適用できる。
その上、液体吸着クロマトグラフィもまた利用され、それによって個々の成分間
の異常な選択性(分離識別)が達成できる。得られたフラクトグラムは定性的お
よび定量的に説明することができる。
クロマトグラフ法の代わりに、測定値、すなわち免疫グロブリンG中のスルフヒ
ドリル−ジスルフィド比の原因である値としてΣS値を測定することを望んでも
よい。この比率はIgGサブクラス間の比率に関係があるという認識に基づいて
いるので、この測定値もまた本発明による方法を実施するために有用な手段であ
る。
本発明による方法の状況においてΣS値を測定するための条件の試験において、
観測値は悪性腫瘍が高度の精度を持って良性腫瘍から区別できることを確認した
。
乳房の癌腫について、それはまた個々の段階に対し相関関係を確立することがで
きる。例えば、T4段階においてほぼ65%の陽性反応が生じ、T4段階で91
%に上昇する。他の腫瘍マーカー(例えばCEA)と比較したとき、それらはT
8段階において約5%の検出率を有するので、これは異常な増加である。これに

igGサブクラス間の比率を測定する別の可能性は、放射免疫拡散(RI D)
の方法にある。
しかしながら、すべての方法について、結果は試料が予め安定化されていたとき
だけ価値を有することが普通である。
例えばRIDによって検出できない、非共有結合の凝集体の生成は、IgG11
度の不安定性の理由であると推定される。この凝集体の生成が酸素含有ラジカル
により開始されることを示した先の結果および、事実、SODを使用した安定化
の試みは、特に好結果であることを証明した。
これらの実験を行うために、完全な凝結(凝固)まで同一の放置をさせておくこ
とにより健康な供給者の完全血液から血清を得て、次いで遠心分離を行ない、そ
の血清を直ちにまたは36時間後にS −ODを使用してまたは使用せずにIg
G1について試験した。
これらの試験を第1の研究室の実験においてRIDによって行なった。等初期濃
度のIgG1に関して、SODを組合せた試料は36時間後はぼ同じ濃度を保持
したが、一方非安定化試料は最初の値のほぼ半分に減った。
タンパク質−A−セファロースを使用する最適化実験によりIgGをその両分に
分離する結果は、96〜loo%の免疫グロブリン成分の純度を得ることは、流
通速度を減少することにより主として可能であることを示した。
これらの研究結果に基づいて、本発明はまた、本発明による方法を実施するため
の、安定化反応物SODを有し、そしてサブクラスIgG1対他のIgGサブク
ラスの比率を測定するための系を有することを特徴とするキットを提供する。
次の実施例は、如何なるやり方においても本発明を限定することなく、本発明に
よる方法の説明に役立つものである。
10m1の完全血液を健康な供給者の腕の静脈から抜き取った。
5〜10分後凝結は完了した。遠心分離を440xgで1n分間行なった。血清
をピペットて抜き取り各0.1mlの4個のアリコート:
2個の0時間の試料「0−」および「0+」並びに2個の36時間後に試験する
ための試料r36−Jおよびr36+jに分けた。
試料「0+」およびr36+Jは5μlのSOD溶液(35国際単位)(Bio
trade社を通じてSigma社のロット番号10OH9311の5OD)と
混合した。2種類の0試料をRID緩衝液を使用してl 50に希釈した(0.
05M1−リス、O,]、00MNaCl; IgNaNi /I ;HCI希
釈にてpH8,0に調整)。
試料r36+Jおよびr36−Jは、日光を中へ入れない状態で室温にて36時
間非希釈貯蔵した。試料rO−Jおよび「0+」は直ちにRID板上に置き、そ
れによってくぼみ当り3μ】挿入した。
RID板に対するアガロースゲル=5mlの1%アガロース溶液、0.5mlの
RID緩衝液、0.5抗1gG1血清(Binding 5ite社、PE0O
6)。標準血清(Binding 5ite社、PBO62)の希釈のために同
じ板上に適用した:希釈1:1O51:15、l:25およびl:50゜RID
緩衝液で1=50に希釈後、試料r36−Jおよびr36−Nもまた板上に置い
た。「0−」および「O+」並びに希釈標準血清の沈殿物の環の表面を測定し、
そして2日後に36時間試料のそれを測定した。次いて標準血清の希釈液につい
て測定した沈殿物の環の表面の値を既知IgG1濃度の希釈液に対してグラフ表
示した。最後に、4種類の試料から得た沈殿物の環の表面をこのようにして得た
検定図中にグラフ表示し、IgG1濃度を内挿によってそれらから決定した。
実施例 2
実施例1の実験と類似した7時間の実験で次の結果を得た・表2
今まで行なわれた全ての実験は、記述された条件下のSODの阻害効果は36〜
40時間に限定される、すなわちIgG測定はいかなる場合においてもその時間
間隔の間に行なわれるべきであること時間線はIgG1とIgG2の濃度が5O
D(350国際単位を有する5μm)の添加なしおよび添加後にRIDによって
測定される各血清試料(0,1m1)について認められた。評価は、いろいろな
時間の後で得られた測定値が最初の値のパーセントで表わされるようなやり方で
達成された。結果は、それぞれ添付の図1および2に示されるように時間にわた
る挙動であった。
添付図面の図1〜4に関する考察
1.48時間の待ち時間(密閉された、好気性の、室温)後のRIDによって測
定できるIgG1の減少は平均30%であった。
2.1gG1の減少の効果は明らかに直ちに続いて起こり、僅か7時間後に約2
0%の減少に達する。動力学的挙動は指数因子をさし示し、実験はそれぞれの反
応量のIgG1の比較的一時的の減少はこのサブクラスに直接比例することを示
した。したがって、ミハ工り/メンテン関数に従う動力学的挙動は除外すること
ができ、二次反応、多分それからより高い凝集体がそのあと生成する二量体の最
初の生成をここで生じることを推論することができる。また、熱凝集体がRID
で検出できる沈殿物の環を生成しないことを示すように、そのような凝集体の生
成はRIDで検出できないことを説明する。
3,12〜24時間の間に、さらなる反応が系の中間の自己安定化に到達するか
、それが測定点のバラツキであるかどうか安全に決定することができない。ただ
反応過程行程が後で連続的に継続することだけが明白である。
4、SODによる凝集反応の阻害は、それが多分Oラジカルにより引き起こされ
た自己凝集であることを示唆する。Wichenset al、(Biochi
rn、Biophis、Acta、Vol、742:607−616.1983
)によれば、少線量の紫外線による照射は自己凝集を引き起こし、ところで、活
性白血球により引き起こされるそのような凝集体は、沈殿物の線に実質的な減少
を示す。
図1から、IgG1の濃度は平均して24時間以内に約15%、36時間以内に
約22%減少することが続いて来る。この時間範囲内で、減少はSODの添加に
よって実質的に完全に抑制できた。この結果の重要性は、図1に平均減少パーセ
ントがグラフで示されていることおよび別個の試料について測定した個々の減少
がある特定の場合に、次の表3に示されるように実質的に大きくなりうることを
考えるならば明白になる。
実施例 4
この実施例において、健康な供給者の16種類の血清試料を試験した。最初にI
gG1と1gG2の濃度をRIDによって測定した。
このようにして得た値を次の表4に示す。
その上、本発明者等は表5に示される6個の別の値を再参照することができた。
このようにして得た平均値を平均するとき、22の健康な供給者の血清に対する
結果は、8.33±0.48mgIgG1および3゜04±0.25mg1gG
2/ml血清であった。
癌血清のIgG1濃度は平均して正常な血清よりも23〜24%低く、その結果
有効な阻害の無条件の必要性が明白である。SODはこの必要条件を満たすが、
しかしながら約40時間という血清の保存時間までだけである。しかしながら、
これは大部分の場合に十分である。
図1に示されるように、明確に限定された時間効果がIgG1に対してだけ起こ
り、一方策2の主成分、1gG2は不規則な変動を示した。しかしながら、これ
もまたSODにより阻害された(図2)。SODはRIDにおいて示されたIg
G1の減少を阻害し、そしてこのサブクラスだけ分子内に反応性のジスルフィド
架橋S81を含有するから、SODはまた、85%までのSS8に対する項から
なるΣS値の測定のための安定剤として利用できる。
実施例 5
タンパク質−A−セファロースカラムでのIgGサブクラスlおよび2の分離
緩衝液の組成:
0.05Mリン酸塩・クエン酸塩緩衝液pH4,7,0,086Mリン酸塩・ク
エン酸塩緩衝液pH4,3、流速は7070−1O/hに変化した。
関係があるピークBおよびCにおいて、成分に対してそれぞれ95%までおよび
100%純度が達成される、添付のフラクトグラム(図3および4)を得た。
実施例 6
一方においてタンパク質−八−セファロースを有するアフィニティクロマトグラ
フィーによる、他方において参照のためにRIDを使用するIgG1およびIg
G2各濃度の測定2種類の方法を比較する目的のために、20人の健康な供給者
の血清および卵巣の、子宮頚管部の、および結腸癌腫を持つ40人の女性患者の
血清を使用したが、血液試料は手術に先立って採血した。
血液試料(2〜10m1)は採血直後に440xgで10分間遠心分離し、血清
をピペットで抜き取り、RID緩衝液で1:50に希釈し、RID板に適用した
。沈殿物の環を2日後に測定し、IgG1およびIgG2濃度を標準血清(マン
シー二技法)を使用して測定した。
0.3mlの血清をタンパク質−A−セファロースによってIgG1および1g
G2の単離のためにカラム(K9/15)に適用し、0.16Mリン酸塩・クエ
ン酸緩衝液pH7,0および10m1/hの流速を使用して溶離させた。それに
よりIgGはFc部分によってタンパク質−Aに結合し、他の血清タンパク質は
非結合状態で溶離された。IgGのほかに、IgMおよびIgAもまたタンパク
質−Aに結合した。基線に到達後、緩衝液交換を達成した:最初に、1gG2を
0.05Mリン酸塩・クエン酸塩緩衝液pH4,7で、次いでIgG1を0.0
86Mリン酸塩・クエン酸塩緩衝液pH4,3で溶離した。それからゲルを0.
1Mクエン酸塩緩衝液で清浄化し、次いでクエン酸塩緩衝液を出発緩衝液(0,
16Mリン酸塩・クエン酸塩緩衝液pH7,0)と交換した。IgG1およびI
gG2の溶離液をlNNaOHでpH7,0まで滴定し、タンパク質濃度を27
8nmで光度測定法で定量した。吸光係数ε′はl、58であり、さらにはまた
濃度の定量ができる。
表6に、RIDおよびタンパク質−Aを使用して測定した、血清試料(±sem
)のIgG1およびIgG2濃度の平均値を示す。
比率G1./G2を次いでこれら2つの値から決定できる。

RIDを使用して測定した、健康な供給者と腫瘍患者との間のIgG1濃度の平
均値は、非常にいちじるしく (P=0.005)相違し、それにより27.5
%の間違った負のおよび25%の間違った正の結果を得た。タンパク質−Aを使
用したIgG1の単離も同様の結果:P=0.005および30%の間違った負
のおよび35%の間違った正の結果を示した。

Claims (8)
- 1.患者の新鮮な血液または血清試料がラジカル抑制剤、好ましくはペルオキシ ドジムターゼ(SOD)によって安定化されること、および試料の血清における 変位が標準の比率と比較した、IgGサブクラスの和に対するIgGサブクラス の少なくとも1種類の比率から決定されることを特徴とする、患者の血清免疫グ ロブリンG(IgG)の特徴によって悪性腫瘍を検出またはそれらの腫瘍塊を測 定する方法。
- 2.標準の比率と比較した、IgGサブクラスの和に対するサブクラスIgG1 および/またはサブクラスIgG2の比率の変位が、安定化された試料の血清に おいて決定される請求の範囲第1項に記載の方法。
- 3.比率の前記変位がΣS値を測定することにより決定される請求の範囲第1項 または第2項に記載の方法。
- 4.比率の前記変位が本質的に知られているIgGに対する分析方法、特にクロ マトグラフ法または免疫学的方法の結果から計算される請求の範囲第1項または 第2項に記載の方法。
- 5.比率の前記変位が免疫拡散検定によって決定される、請求の範囲第4項に記 載の方法。
- 6.血清の安定化が100〜1000、好ましくは300〜400国際単位/m l血清の質量比のSODによって達成される請求の範囲第1項〜第5項のいずれ か1項記載の方法。
- 7.患者の腫瘍発育の体外測定のための請求の範囲第1項〜第6項のいずれか1 項記載の方法の使用。
- 8.安定化試薬、好ましくはSOD、およびIgGサブクラスの和に対する少な くとも1種類のIgGサブクラスの比率を決定するための組体を有することを特 徴とする請求の範囲第1項〜第7項のいずれか1項記載の方法を実施するための キット。
