Abstract

(57)【要約】 【構成】 通気系及び発射前に機構が閉止するのを防ぐ
クリップ適用支持系を含む内視鏡クリップ適用系。 【効果】 系はクリップの正しい装填を保証し、装填の
間に落ちるのを防ぐ。さらに系は製造が容易で、はっき
り開く系であり、必要な寸法が最小となる。さらにロッ
クアウト機構を備え、機構は不適当に発射されない。軸

【発明の詳細な説明】
【０００１】
【発明の分野】一般的に本発明は結さくクリップアプラ
イヤーに関する。特に本発明は、内視鏡結さくクリップ
アプライヤーに関する。最も特定すると本発明は、アプ
ライヤーが多重クリップ適用カートリッジから連続した
多数のクリップを置くことができる、内視鏡結さくクリ
ップアプライヤーに関する。
【０００２】
【発明の背景】近年、内視鏡の分野に多くの進歩があっ
た。これらの進歩の多くは、内視鏡ステープル及びクリ
ップ適用機構の多様性の増加による。このような機構は
内視鏡套管針のカニュールを通って置かれ、組織を切断
し、止める又は結さくすることができる。これらの内視
鏡止め及び結さく機構を用いて、内視鏡結さく法を急増
させる手段が得られた。
【０００３】しかしこれらの装置の出現に関し、ある不
十分な点に気付いた。このような不十分な点の多くは単
に内視鏡法の新規性によるとわかった。従って各外科医
個人が新しい必要及び要求を発見し、多くの機会にこの
ような要求を満たす必要性を探求することができた。
【０００４】例えば、器具の中の圧力を均等にして器具
内の液体の流れに対する抵抗の最も小さい経路を与える
ために、そのような内視鏡アプライヤーの通気をする必
要が認められた。圧力を均等化することにより、器具の
破損の可能性を最小にすることができる。
【０００５】さらに、そのような外科手術の間、クリッ
プを閉じる機構及びクリップの両方を縦方向（器具の長
軸にそって）及び横方向の両方に支えるために、適した
クリップ前進機構の必要が認められた。
【０００６】又、機構内で閉じた時に結さくクリップの
ギャップを変える機構の必要が認められた。
【０００７】さらに器具のあご部にクリップが供給され
た時にあご部が開くことを保証する機構の必要がある。
【０００８】さらにクリップのカートリッジ内の最後の
結さくクリップが発射された後、発射機構具を締め出す
機構が必要であることが認められた。
【０００９】さらに認められた必要性は、容器中に適用
する間のクリップの中心の動きを最小にするための制限
手段である。この動きの制限より、結さくの間あご部に
ある正確な形成領域に結さくクリップを保持することが
できる。
【００１０】もうひとつの必要は、内視鏡アプライヤー
軸にかかるトルクに対する抵抗を作ることである。その
ようなトルクに対する抵抗により使用中の内視鏡アプラ
イヤーチツプの横方向の歪みを最小にすることができ
る。そのようなトルク抵抗がクリップの配置の正確性を
増進する。
【００１１】さらに別に認められた必要はより小さなあ
ご部の空間であり、それにより内視鏡法の時により小さ
なカニュールで使用することができる。この問題の解決
で出会った別の問題のひとつは、あご部を閉じる時に生
ずるトルクを補いながらあご部の空間を最小にしなけれ
ばならないことである。
【００１２】さらに別の要求は、クリップアプライヤー
のあご部にクリップを二重に供給する可能性を最小にす
る方式である。そのような方式は、どのような時も一度
に唯ひとつのクリップを器具のあご部に置くための１連
のバルブ及びバネを使用することであることがわかる。
【００１３】別の要求のひとつは、操作中の腹膜性鼓腸
が大きく失われることを防ぐために内視鏡クリップアプ
ライヤーを封止する機構である。そのような密閉は、過
程を通じて腹膜性鼓腸を保つために必要である。
【００１４】さらに認められた必要のひとつは、アプラ
イヤーのあご部へのクリップの同時性供給が、使用中こ
れらのあご部の開閉と協調する方法である。
【００１５】最後のもうひとつの要求は、発射されるク
リップの、そのような内視鏡クリップアプライヤー内に
おけるバックアップを防ぐ機構である。
【００１６】従って、現在ある種の内視鏡クリップアプ
ライヤーが存在するが、欠点又は不十分性が認められる

クリップを１対の開いたあご部に供給するという特徴の
提供が、本発明の別の目的である。これにより機構の確
実性及び性能が改良される。

具を閉鎖するロックアウト機構の提供が、本発明の別の
目的である。
【００２２】閉止の間、クリップをあごの部位に保つこ
とが本発明の他の目的である。これにより、器具のあご
部内のクリップの動きを固定し、使用者がクリップの足
の間隔をより正確に確認することができ、器具内におけ
る閉止を改良することができる。
【００２３】器具のチップの横方向の歪みを最小にする
ことができる、多重クリップアプライヤーのためのトル
ク抵抗軸の提供が、本発明のさらに別の目的である。そ
のような歪みの最小化により、発射の間のクリップの配
置の改良及び正確性を促進することができる。
【００２４】多重クリップ発射器具内で、この装置のあ
ご部に抗トルク性を与えることが、本発明の他の目的で
ある。この特徴は、適したクリップ閉止性を保持し、与
えながら、あご部の寸法を最小にすることができるよう
に設計するべきである。
【００２５】クリップをあご部に二重に供給する可能性
を最小にすることによりクリップ供給の確実性を増す、
クリップ供給方式の提供が、本発明の別の目的である。
【００２６】内視鏡多重クリップアプライヤー内に封止
系を与え、系の使用中腹膜鼓腸を保つのが本発明の他の
目的である。
【００２７】本発明の他の目的のひとつは、あご部がク
リップを受け取るために十分に開かれている間、及び器
具をその開かれた位置に放出する前の正確な時に、クリ
ップを機構のあご部に確実に供給する、内視鏡多重クリ
ップアプライヤーのタイミング機構の提供である。
【００２８】クリップアプライヤー内に保たれた組織又
は管が器具上で不正確に位置するのを防ぐための組織止
めの提供が、本発明のさらに別の目的である。そのよう
な機構は、多重発射結さくクリップ機構という意味にお
いて、クリップの確実な閉止及び配置を促進するために
有利である。
【００２９】本発明のこれらの、及び他の目的は、長く
て堅いカニュールにより内視鏡クリップ適用位置から隔
てられた発射機構を含む内視鏡結さくクリップ装置にお
いて与えられる。装置は、クリップがある方向で適用さ
れるように作られるが、クリップアプライヤーを回転さ
せて系を有利な方向に向けることができると理解され
る。クリップ適用機構において、装置に通気することが
できる溝を含む通気系について述べられている。これ
は、系内において適した発射が確実に行われるのを助け
る。又、クリップアプライヤーのあご部をカニューラに
挿入する間のその歪みを防ぐ１対の突起を含む、供給棒
支持機構も記載されている。さらにあご部内に保持され
た結さくクリップに均一な閉止力を与えるカムチャネル
機構が記載されている。カムチャネル内の突起があご部
の閉じるのを防ぎ、器具が発射されてクリップを組織の
回りに置かれた後、これらのあご部が確実に新しいクリ
ップを受けることができる。
【００３０】本系には、他の新規特徴が与えられてい
る。最初に、装置の発射路内に落ち込み、機構から最後
のクリップが発射させた後に器具が発射されないように
するレバーを含む機構がある。第２に、クリップ適用機
構上に位置するフィンガーが記載されており、これは、
器具のあご上の適所に発射されるようにクリップを確実
に固定する。第３に、発射の間の器具のチップの横方向
の歪みを防ぐために、トルク−抵抗軸が器具上に置かれ
ている。
【００３１】器具の他の１組の独特の特徴は、あご部の
アームの歪みを最小にしてクリップを正しく閉止できる
ようにするために、１対のあご部が発射の間の多重接触
を与えるようなものである。この歪みの最小化は一層満
足できる改良されたクリップ閉止を与える。クリップ供
給がんぎ方式は、クリップが発射された後、実際に唯一
個のクリップが開いたあご部に適用されるようにする。
さらに、適当に腹膜鼓腸を封止する機構となる封止系が
ある。又、器具のシュラウド上にＶ−型切り込みがあ
り、これは組織を器具のあご部に保持するのを助ける。
この切り込みは、器具の発射及びクリップの閉止のため
に組織の正しい位置を確保するのを助ける。
【００３２】この装置を、その具体化に含まれる多数の
特徴と関連して記載した。これらの特徴は添付した図面
の詳細な説明と関連させた時、及び以下の本発明の詳細
な説明と関連して記載した時さらに良く理解されるであ
ろう。
【００３３】図１−２２に内視鏡多重結さくクリップ適
用機構を示す。図１，２，３及び４でより良くわかる通
り、この多重結さくクリップ機構１は、外科手術用套管
針を通してクリップを適用するのに有用である。図４で
わかる通り、結さくクリップ機構１は、套管針のカニュ
ールを通して適用され、血管を閉じ又は組織を止めるこ
とができる。機構はあご部１１を用いて一連のクリップ
を適用する。あごはトリガー２５により遠隔的に発射さ
れる。図２及び３に示す通り、トリガー２５が圧縮さ
れ、あご部１１が閉じ、その中の結さくクリップ８を押
しつぶす。クリップ８が閉じた後、あご部１１は弛緩
し、もうひとつのクリップ８がその場所に装填される。
【００３４】図５，６及び７でより良くわかる通り、装
置を発射するためのトリガー機構２５わ包むハンドルの
両方１６，２８が記載されている。ハンドルの片方１
６，２８は発射機構を囲み、転心１１０を有し、その回
りにトリガー２５が回転する。トリガー２５にはフォー
マープレート２２及びフィーダーリンク２３がついてい
る。図５の右側に示す通り、フィーダーリンク２３はト
リガー２５に接続し、フィーダープレート２４を適所に
保つ。フィダープレート２４は、支柱２４ａで復帰バネ
２７に接続し、支柱２２ａでフォーマープレート２２に
接続する。従ってトリガーを発射すると、フォーマープ
レート２２は前方に押される。トリガー２５上の軸ピン
１１３はハンドルの片方１６の溝１１５内を滑り、スロ
ット１１１を器具の後部に向かわせる。フィーダーリン
ク２３上のスロット１１８は、フィーダープレート２４
を器具の後部に向かわせる。同時にトリガー２５が回転
し、スロット１１４が合わせピン２６をプレート２４の
スロット１１２中で滑らせる。又、合わせピン２６は、
ハンドルの片方１６のスロット１１７内を滑る時にフォ
ーマープレート２２の点１１６を前方に向かわせる。ト
リガー２５が弛緩すると、復帰バネ２７上の張力が支柱
２４ａを通してフィーダープレート２４に、及び支柱２
２ａを通してフォーマープレート２２に作用することに
よりトリガー２５をその位置に戻し、トリガー２５、合
わせピン２６及びフォーマープレート２２がその非−応
力位置に戻る。合わせピン２６は、フィーダープレート
２４を右ハンドル１６上に保つ。抗バックアップレバー
１９、から動きレバー１８、ラッチ１７、捩りバネ２０
及びプレ−コックトリガー２１については、本発明の一
部としてさらに記載する。
【００３５】トリガー２５が活性化されると、フォーマ
ープレート２２が前方に動かされ、支持管１５内でフォ
ーマープレート２２に接続しているカムチャネル１２が
前方に動かされる。これが内視鏡クリップ適用機構を作
動させる。図８でわかるように、供給棒９及びカムチャ
ネル１２は、床１０を囲んでいる。これらの機構は、支
持管１５内で低部シュラウド１３及びシールキャップ１
４の間に密閉されている。あご部１１は、カムチャネル
１２に接続している。このあご部が結さくクリップ８を
囲み、組織又は血管を封じる。
【００３６】フィーダーバネ５、締め付けレバー６、フ
ィードシュー７及び一連のクリップ８は、トラック軌道
４内に保持されている。クリップ軌道４は、一連のクリ
ップを前方に進め、それをあご部１１のアーム内に装填
することができる。リフターバネ３は、上部シュラウド
２によりクリップ軌道４上の適所に保持されており、ク
リップ８の堆積の第１クリップ８ａを供給棒９の平面内
に置いてあご部１１内に位置させる。シールキャップ１
４及び低部シュラウド１３は、カムチャネル１２、床１
０及び供給棒９を支持管１５内の適所に保持する。
【００３７】操作する場合、トリガー２５が発射され、
フォーマープレート２２がカムチャネル１２を前方に動
かす。カムチャネル１２はあご部１１を囲み、クリップ
８を組織の周りで絞める。トリガー２５が弛緩すると、
カムチャネル１２が引っ込んであご部１１が開く。クリ
ップ８の弾倉が前方に進み別のクリップ８があご部１１
に保持される。これは、バネ２７がゆるみ、フィーダー
プレート２４を進めた時に起こる。これにより今度は供
給棒９が、バネ３により供給棒９の路に位置していたク
リップ８の堆積から第１クリップ８ａをあご部１１に進
める。この操作は、図６及び７と関連付けて図９，１０
及び１１を見ることにより最も良くわかる。もちろん本
発明で開示する機構の各々をさらに本文に記載する。
【００３８】特に、クリップ８の方向が器具１のハンド
ルの部分の方向に対して横向きであることに注意しなけ
ればならない。このようにして使用者は、使用者の手を
クリップを発射する平面と平行に保つように、器具１の
ハンドル１６，２８上のトリガー２５を握ることができ
る。これは使用者にとってより典型的な参照点を与え、
器具の操作をより楽にしている。
【００３９】ここで、クリップ適用機構の種々の特徴を
それぞれ記載する。第１の特徴は、内視鏡によりこれら
のクリップを適用するための通気方式である。図１２で
わかる通り、器具の外側に通気孔１２０が位置してい
る。この通気孔１２０は支持管１５内の低部シュラウド
１３内の解放チャネル１２２に接続している。
【００４０】解放チャネル１２２は、低部シュラウド１
３の穴１２４に接続している。このため、液体が管１５
と低部シュラウド１３の間を通過することができる。こ
の低部シュラウド１３の穴は、器具の上部１２６及び低
部シュラウド１３内の内部チャネルの間の液体の通路と
なる。支持管１５内のこの解放チャネル１２２は、支持
管１５の縦軸に沿って走り、液体の流れに対して最も抵
抗の少ない路となっている。このチャネル１２２の末端
の低部シュラウド１３の穴１２４は、軸集合体の上部１
２６に通気路を与えている。従って液体は器具１内から
出ることができる。器具は１０−１５ｍｍ水銀圧下でよ
り良く機能することができる。圧力は器具全体で均等化
され、最も抵抗の小さい路が与えられる。このようにし
て相互操作的に使用した場合の器具の機能的失敗の可能
性が減少する。
【００４１】さらに図１３及び１４でわかる通り、内視
鏡多重クリップアプライヤーのための支持機構も開示さ
れている。この支持機構は、組織への適用の間の結さく
クリップのための縦方向の支持、及び内視鏡的手術の時
に套管針のカニュール１０５を通して挿入する間のあご
部１１の横方向の支持を与える。図１３及び１４でわか
る通り、器具のあご部１１が解放されている時に、結さ
くクリップ８は器具のあご部分１１ａ及び１１ｂの間に
置かれる。供給棒９は完全に前進している。器具を套管
針のカニュールを通して挿入し、組織に適用する時、あ
ご部が閉じないようにすることが重要である。図１４で
わかる通り、供給棒９上に外側への突起１３２がある。
これらの外側への突起１３２は、あご部内のクリップ８
のアングルアームに接続している。又、供給棒９の外端
１３０はそいであり、それはあご部１１の内端１３６に
接触している。
【００４２】従ってあご部１１ａ及び１１ｂは、カニュ
ール１０５を通して挿入する間に横方向に押し潰されな
いようにしてある。外側への突起１３２は、挿入の間に
クリップ８が縦方向に動くのを防ぎ、供給棒９の外端１
３０は、挿入の間にあご部が不注意に閉じるのを防ぐ。
この特徴は、クリップの閉止の改良を促進し、使用中に
傷部位に器具からクリップが落ちるのを防ぎ、有利であ
る。
【００４３】もうひとつの特徴は、この器具で使用する
クリップ閉止カムチャネル１２である。カムチャネル１
２は、結さくクリップ器具のあご部１１が閉じるのを補
助する。カムチャネル機構１２は器具の管状支持管１５
の軸に沿って縦方向に伸びている。カムチャネル１２
は、あご部１１の縦軸内にある。図１５でわかる通りカ
ム機構の末端１４０は直角であり、断面を切ってある。
図１５Ａに示す通り、カムチャネル１２の前面にＺ型の
接合部１４２，１４２ａがあり、カムチャネル１２を機
械的に合わせている。このＺ型の機械的錠がクリップ形
成の間のカムチャネルの横方向（側から側）の歪みを減
少させる。このようにしてカムチャネル１２の縦方向の
動きが起こる間、カムチャネル１２の横方向の動きが妨
げられる。従ってカムチャネル１２に挿入されたＺ型の
機械的錠１４２及び１４２Ａは、カムチャネル１２の寸
法安定性を増し、結さくクリップ８の閉止の確実性を向
上させる。
【００４４】やはり図１５からわかる通り、カムチャネ
ル１２内に１対の突起１４４が記載されている。これら
の突起１４４はカムチャネル１２の末端を通気し、器具
１のあご部１１の縦軸を通って垂直に向いている。あご
部１１ａ及び１１ｂが開いている時、突起１４４の外端
はあご部１１の１１ａ及び１１ｂのアーム上にあり、機
構が閉じる時のようにそれが互いに向かって内側に動く
のを防ぐ。結さくクリップ８があご部１１内で近付く
と、カムチャネル１２は形成部位から引っ込み、突起１
４４の端が再びあご部１１の端と接触し、あご部１１を
外側に押す。これによりクリップを受け取るためにあご
部が開くのを保証する。クリップの閉止の過程を危うく
することなく、器具のクリップ供給の確実性が増す。
【００４５】図１６Ａに示す通りこの器具には、フィー
ダーシュー７に接続した回転するロックアウトレバー６
が記載されている。図１６Ａでわかる通りレバー６は上
部シュラウド２と接触すると供給棒９内に回転する。図
１６Ａで分かる通りこのレバー６が下方に回転すると、
供給棒９の前方への完全な動きを妨げる。図１６に示す
通り、これにより斜めのラッチ１７がフォーマープレー
ト２２の壁１５０とかみ合う。従ってこの多重冗長方式
は、あご部１１により器具の前端で最後のクリップ８が
発射された後に器具の後方と同様に器具を閉鎖すること
を可能にする。ロックアウトレバー６は、クリップ軌道
４にクリップ８が残っていない時にのみその場所に回転
する。クリップ８が残っている場合、供給バネ５はレバ
ー６が供給棒９の路内に回転するようにフィードシュー
７を斜めにしてロックアウトレバー６のための路をあけ
ることができない。このレバー６によるロックアウトが
起こると、供給棒は完全に前方に動くことができず、従
って斜めのラッチ１７がフォーマープレート２２の壁１
５０とかみ合う。
【００４６】又、図１６Ｂでわかる通り、管に適用する
間のアプライヤー中のクリップの後方への動きを最小に
する機構が記載されている。これは又、機構が完全に閉
止する間、クリップ８をその正しい形成領域に保つこと
ができる。この機構は支持管１５内に保持された床１０
を含む。床１０は器具のあご部１１Ａ及び１１Ｂのクリ
ップ形成領域１６０の縦平面に突き出ている。図１６Ｂ
でわかる通り、床１０上のクリップ保持フィンガー１５
６は、その末端と同様に垂直の通気突起１５４を備えて
いる。この保持フィンガーは床１０に１個であり、あご
部のクリップ形成領域１６０の縦平面と平行になるよう
にやはり斜めにしてある。フィンガー１５６の末端突起
１５４は、形成領域と垂直に位置し、あご部１１Ａ及び
１１Ｂ内のクリップ８の後方への動きを最小にする。あ
ご部が近付くと、床１０はクリップ形成領域の平面から
立ち上がる。クリップの閉止及び床１０の立ち上がりの
間ずっと、フィンガーを保持しているクリップの通気突
起１５６は、クリップ８及びあご部１１Ａならびに１１
Ｂのクリップ形成領域の面に留どまり、クリップ８の頂
点の後方に位置している。これは図１６Ｃでより良くわ
かる。従ってあご部１１に保持されたクリップ８の動き
は常に制御され、床１０のフィンガー１５６及び突起１
５４は常にクリップ８の形成の平面に留どまる。
【００４７】図８に示す通り、支持管１５はトルク抵抗
性である。すなわち支持管１５は中空の円筒状であり、
非−弾性材料で作られている。管は軸末端近辺及びクリ
ップ形成あご部１１の近くで終わっている。これは上部
シュラウド２に接続し、これらの要素の位置が共にクリ
ップ８の形成のための機構に必要な抵抗性支持を与える
ようになっている。支持管を製造した材料が非弾性なの
で、器具の安定性は増し、これがクリップ８の配置の正
確性の向上を助ける。
【００４８】図１７、１８、１８Ａ及び１８Ｂからわか
るように、器具のあご部１１Ａ及び１１Ｂは、その末端
１７８及び各あご部の閉鎖ランプに近接して位置する第
１ヒール１７６で接触していると記載されている。これ
は図１８及び１８Ａでより良くわかる。図１７でわかる
通り、あご部１１ａ及び１１ｂは一般に開位置に保たれ
ている。第１ヒール１７６の接触により、あご部１１ａ
及び１１ｂのアーム１７２の歪みが最小となり、クリッ
プ８を正しく閉止することができる。この最初の接触
は、アームの歪み及びトルクを最小にし、装填の間、系
を支える。
【００４９】図１８及び１８Ｂでわかる通り、あご部が
十分閉じるとクリップ閉止の最後の部分が装填条件の大
きさを増加させる。抗−トルク支柱と呼ばれる２本の向
き合った支柱は、あご部の形成領域近辺で互いに接触
し、あご部にかかるトルクを打ち消す。これにより一層
満足できる、改良されたクリップ閉止を行うことができ
る。各あご部のチップ１７８も接触する。従ってここに
記載する抗−トルク機構を用いると、あご部はトルク抵
抗性である必要がないので寸法を小さくすることができ
る。歪みが最小になるので、あご部１１ａ及び１１ｂは
改良させたクリップ閉止機構も与える。これにより寸法
の最小化と共に器具の信頼性が得られる。
【００５０】図１９Ａ及び１９Ｂからわかる通り、内視
鏡多重クリップアプライヤー１のクリップ供給信頼性を
改良する、供給及びがんぎ系が記載されている。これは
あご部内へのクリップの二重供給の可能性を最小にする
ことにより行う。系は２つの独立した部分を含む。クリ
ップ軌道４に１個のバルブ１８２は、リフターバネ３に
より最も前のクリップが供給棒９の平面中に偏っている
間、クリップ８の堆積を動かさずに保つ。器具が作動す
ると、供給棒９はリフターバネ３から器具の後方に引っ
込む。供給棒９が引っ込むと、供給棒９上のランプ１８
０は、クリップ軌道４に位置する第１バルブ１８２とか
み合い、クリップ軌道４は閉鎖する。
【００５１】クリップ軌道４が閉鎖すると、クリップ８
の堆積は動かずに保持される。供給棒９がリフターバネ
３からさらに引っ込むと、バルブ１８２がランプ１８０
により閉鎖され、リフターバネ３が作動して最も前のク
リップ８を供給棒９の面内に偏らせ、次のクリツプ８を
あご部内に装填することができる。これは図１９Ｂでよ
り良くわかる。しかしクリップ軌道４に位置する第１バ
ルブ１８２は、前もって点検口１８４を用いて余分のク
リップ８をクリップ軌道に戻すので、唯１個のクリップ
のみが供給棒に向かって偏る。クリップの二重供給の可
能性が減少し、アプライヤーの信頼性が増す。
【００５２】図２０及び２１に示す通り、内視鏡多重ク
リップアプライヤーの軸集合体のほとんど末端近辺に位
置する封止機構が開示されている。封止機構の断面は円
形であり、軸支持管１５内に破砕リブ１９６を通してし
っかりとまっている。封止機構内に、図２０で定義する
３つの室１９０，１９２があり、それを通って器具の供
給及び形成機構が通過する。第１室１９０は、それを通
ってシーラント液を注入することができる領域である。
シーラントは、挿入孔１９４，１９８を通って挿入する
ことができる。外部の第２室１９２は、シーラントのス
ピル−オーバー領域となっている。これは図２０でより
良くわかる。封止機構の末端は、シーラントが室１９０
から器具の機能領域に移動するのを防ぐために閉じられ
る。
【００５３】封止系は、トリガー２５の発射及び引っ込
みの間の器具の反動を最小にするための緩衝機構となっ
ている。この閉じた封止機構は、器具自身を通じて相互
作用による大きな圧力の損失を防ぎ、同時にリブ１９６
を通じて、シーラントの移動による器具の機能的失敗の
可能性を最小にする。この系は患者の外部から封止さ
れ、それ自身器具の可動部分から封止される。
【００５４】図５と関連して図２２Ａ、２２Ｂ及び２２
Ｃから分かる通り、トリガー系はプレコックトリガー２
１を含み、それは予備的に圧迫され、支柱２０６上をハ
ンドル部分１６内に回転する。プレコックトリガー２１
は、ハンドル１６及び２８内に位置するプレコックトリ
ガー２１及びフォーマープレート２２の間の隙間を省略
することができるように偏っている。ハンドルの作動
中、フィーダープレート２４が形成部位（２２Ｂ）から
器具の後方に引っ込むと、フィーダープレート２４上の
カム面１９６は、プレコックトリガー２１の円筒状支柱
１９８と接触する。このため、図２２Ｂに示す通り、プ
レコックトリガー２１がそのプレコック位置に回転す
る。
【００５５】そのプレコック位置で、プレコックトリガ
ー２１の突起２００は、フォーマープレート２２から伸
びる壁２０２の後部にロックされる。フォーマープレー
ト２２から伸びるタイミング壁２０２が、フィーダープ
レート２４の路からのプレコックトリガー２１の回転を
許すまで、フィーダープレート２４は次のクリップ８を
この点からあご部に供給することができない。この時間
調節の方法により、クリップ供給を順番に操作すること
ができ、器具が部分的に発射され、再び開いた場合に器
具のあご部１１に第２のクリップが供給される可能性を
なくする。
【００５６】図５及び２２Ｃからさらにわかる通り、捩
れバネ２０はから動きレバー機構１８及び抗バックアッ
プレバー１９の両方に接続している。抗バックアップレ
バー１９の目的は、クリップが閉じられている時、カム
チャネル１２によりあご部１１の圧力を一定に保つこと
である。従ってバックアップレバー１９は、カムチャネ
ルが一定に前方に押されるようにする。これにより、ア
プライヤーが十分作動するまでクリップ８があご部１１
ａ及び１１ｂから落ちるのを防ぐ。
【００５７】系は、プレコックトリガー２１の回転によ
り作動し、プレコックトリガー２１の底からから動きレ
バー１８の穴２１０に伸び（捩れバネ２０を経て抗バッ
クアップレバー１９に接続する）突起２０８が、抗バッ
クアップレバー１９とかみ合う、又ははずれるトグル機
構として作用する。から動きレバー１８の穴の幅２１０
は、プレコックレバー２１及びから動きレバー１８の相
対的動きを決定する。これにより抗バックアップ作動機
構ａ９のタイミングがプレコックトリガー２１の作動に
関して調節される。抗バックアップレバー１９は、弾性
材料から作られ、図５に示す通りフォーマープレート２
２上の金属歯２１２とかみ合う歯２１４を持つ。抗バッ
クアップレバー１９は、抗バックアップ機構を無効にし
なければならない場合、フォーマープレート２２から歯
２１４をはずす傾向のあるカム表面を持つ。
【００５８】最後に図９、１０及び１１からわかる通
り、上部シュラウド２にＶ型切り込みとして位置する組
織止めがある。この組織止めは、あご部１１内のクリッ
プ形成領域の後端にある。図９からわかる通り、これは
クリップ８をあご部１１の形成領域内に保持し、クリッ
プの正しいギャップ寸法を保証している。これは又、ク
リップの閉止が緩すぎたり、きつすぎたりして組織を痛
める可能性を防ぐ。この考えがより確実なクリップの閉
止を促進し、組織に損傷を与える可能性を最小にする。
【００５９】従ってわかる通り、本発明の機構の操作に
おいてはすべてが類似して機能し、系は血管近辺で結さ
くクリップの閉止を操作することができる。ハンドルは
あご部がクリップを閉止するように作動する。種々のバ
ネ及びトリガー機構は、クリップを適当に支持し、閉止
に十分な力を与えるように作用する。すでに記載した通
り、多数の安全で冗長な系があり閉止が保証されてい
る。従って多くの目的及び特徴を記載したが、本発明の
目的は添付クレイム及びその同等物から誘導されるもの
と理解するべきである。
