Description

【発明の詳細な説明】 ［発明の背景］ この発明は、脊椎セグメントを安定させるため
の茎ねじに関するものであり、特に、ねじを切ら
れた軸部と、軸部の頭部に配置された受取部とを
有する茎ねじに関するものである。
そのような茎ねじは、脊椎の単一セグメントま
たは複数セグメントを固定するために用いられ
る。この種の茎ねじは、ドイツ特許明細書第
2649042号から公知である。茎ねじは、ねじを切
られた軸部と、この軸部の頭にしつかりと接続さ
れる受取部とを含む。
そのような茎ねじが、たとえば４本用意され
る。各茎ねじの受取部は、その上端から切込まれ
た受けスリツトを有している。受けスリツトは、
ねじ切されたロツドを受入れる。２本の茎ねじと
１本のねじ切りされたロツドとで、１組の固定ユ
ニツトを構成する。
まず、固定しようのする脊椎のセグメントを間
に挟むような位置関係で、２本の茎ねじが脊椎に
捩込まれる。残りの２本の茎ねじも、同様に、固
定しようとする脊椎のセグメントを間に挟むよう
な位置関係で脊椎に捩込まれる。最初の２本の茎
ねじは脊椎の左側部分に捩込まれており、後の２
本の茎ねじは脊椎の右側部分に捩込まれている。
次に、左側に位置している２本の茎ねじの受取
部の受けスリツト内に、ねじ切されたロツドを嵌
め入れる。同様に、右側に位置している２本の茎
ねじの受取部の受けスリツト内にも、ねじ切され
たロツドを嵌め入れる。
最後に、固定ナツトを用いて、各ロツドと各茎
ねじの受取部とを固定する。
［発明が解決しようとする課題］ 上述の茎ねじには、次のような問題点があつ
た。上下に位置する２本の茎ねじと、これらの茎
ねじの受取部の受けスリツト内に嵌め入れられる
ロツドとで、１組の固定ユニツトを構成してい
る。ここで重要なことは、ロツドを受入れるため
には、上下の２本の茎ねじの受取部の受けスリツ
トを完全に整列させることである。ここでいう整
列とは、２方向の整列関係を意味する。すなわ
ち、まず第１に、２本の茎ねじの受取部の受けス
リツトは、上面側から見たとき、受入れるロツド
の軸線と平行に整列する必要がある。次に、２本
の茎ねじの受取部の受けスリツトは、側面側から
見たときにおいても、受入れるロツドの軸線と平
行に整列する必要がある。この整列関係が維持さ
れないと、ロツドと茎ねじの受取部とをナツトを
介して固定したとき、ロツドや茎ねじに歪みやね
じれを引起す。しかし、実際問題として、茎ねじ
を脊椎にしつかりと捩込むとき、同時に上述の整
列関係を維持するのは非常に困難である。あるい
は、整列関係を維持するように茎ねじを脊椎に捩
込むには、長時間が必要であり、これは脊椎の手
術に非常に不利である。
上述の整列関係が維持されていない状態でロツ
ドと茎ねじとを固定すれば、ロツドにせん断力が
作用する。その結果、手術後の使用に際しては、
ロツドが破壊したり、または、固定が緩んだりし
てしまう。
［発明の目的］ この発明の目的は、上述の欠点を避ける改良さ
れた茎ねじを提供することである。さらに別の目
的は、取付動作を容易にし、かつ同時に後の使用
における危険を減じるか、または避ける茎ねじを
提供することである。
［発明の要約］ この発明に従つた脊椎セグメントを安定させる
ための茎ねじは、ねじを切られた軸部と、ねじ切
りされた軸部の一端に設けられた球状の頭部と、
球状の頭部を受入れるための２個のハーフを有す
る受取部と、２個のハーフの回りに配置され、２
個のハーフを球状頭部に係合させるように保持す
る保持リングと、を備えている。
使用時には、まず始めに、ねじ切された軸部が
骨に捩込まれる。その際、受取部の位置を考慮す
る必要がない。ねじ切された軸部を骨に捩込んだ
後、受取部の位置を調整する。受取部が所定の整
列関係を維持したとき、１対の茎ねじを接続する
ためのロツドと受取部とを固定する。こうして、
非常に短時間で茎ねじを装着することができ、な
おかつ、１対の茎ねじを接続するロツドに対して
無理な力を与えない。
［実施例の説明］ この発明の一実施例は、第４図〜第１１図に示
されている。この実施例を説明するのに先立ち、
第１図〜第３図に示された茎ねじを説明する。第
１図〜第３図に示された茎ねじは、前述した従来
の茎ねじを改良するものであり、本願の発明者が
発明したものである。
第１図〜第３図に示される茎ねじ１は、ねじを
切られた軸部２と受取部３とを含む。
ねじを切られた軸部は背骨に捩込まれるように
適合された従来通りに設計されたねじを切られた
部分４を含む。六角ヘツド５はねじを切られた部
分の頭側端部に設けられる。外部にねじ７を有す
る突起６は、ねじを切られた部分４と反対の側に
延びる。突起６は、ねじを切られた部分４と反対
の側に設けられる球面セグメントの形の凹所８を
有する。ねじを切られた部分４、六角ヘツド５、
突起６および凹所８は第１図に示される態様で互
いに同軸に芯合せされる。
ロツクナツト９は、ねじを切られた軸部２に関
連して設けられる。ロツクナツト９は、内部にね
じ１０を有し、ねじを切られた軸部２に向かう側
に設けられている凹所１１を含む。内部のねじの
寸法は、内部のねじ１０が、突起６とねじ７と嵌
合するように選択される。球状セグメントとして
型取られた部分１２は凹所１１の底に続く。球状
セグメント１２の半径は凹所８の半径に実質的に
対応する。凹所に型取られた球状セグメントの開
放側部は凹所８と反対側にある。穴１３は突起６
の反対側で凹所１２に続く。内部のねじ１０を有
する凹所１１と、球状のセグメントの型の凹所１
２と、穴１３は互いに同軸に芯合せされる。
接続部材１４が提供され、これは、球状部分１
５と、球状部分に続きかつそれと１つの単体部分
を形成する円筒形部分１６とを含む。ロツキング
ピン１８を受取るように適合された穴１７は、円
筒部分の軸に垂直方向に円筒部分に設けられる。
球状部分１５の半径は２つの凹所８および１２の
半径と非常に近く、わずかにそれよりも小さいだ
けである。円筒形部分１６の直径は穴１３の直径
より小さく、そのため前方側部で円筒形部分を有
する接続部材１４は、凹所１１および穴１３を突
抜け、穴１３から外側に突出する。ねじ７と１０
は、凹所８および１２と球状部分１５の直径の関
数で選択され、そのため接続部材１４が装着さ
れ、かつロツクナツト９が捩込まれるとき、球状
部分１５を中心として円筒形部分１６が一方向に
動くことを可能にする。動きの程度は穴１３の大
きさおよび円筒形部分１６の外部直径との差によ
つて予め定められる。
ロツクナツト９は、接続部材１４と、ねじを切
られた軸部２との間に、固い接続が生じるように
締付けられてもよい。
受取部３は、その対称軸がねじを切られた軸部
２の対称軸と芯合せをされ、円筒形の形態で設計
される。同軸に延在している内部穴１９は、ねじ
を切られた軸部２と向い合う面に設けられる。内
部穴１９の直径は、円筒形部分１６がちようどそ
こに嵌合されるように選択される。交差穴２０
は、内部穴１９の方向に垂直に設けられ、かつロ
ツキングピンがきちんとそこに嵌合するように選
択された寸法を有する。内部穴１９の深さと穴２
０の位置は、第３図に示すように、ロツクナツト
９を締付けたときに、接続部材１４の円筒形部分
１６が内部穴１９にしつかりと嵌り込み、かつ、
ロツキングピン１８が穴２０と接続部材１４の穴
１７に挿入され得るように選択される。第２図に
示すように、受取部３は、穴１９の反対の端部に
設けられる受けスリツト２１を有する。受けスリ
ツト２１は、受取部の対称軸に垂直に延在し、か
つその両方の外側に凹所２２および２３が設けら
れる。受けスリツト２１の幅はそこに嵌合される
べきねじを切られたロツドがゆつたりとそこに通
過するように選択される。シリコンのような緩衝
特性を有する材料からなる緩衝ワツシヤ２４は、
ロツクナツト９と受取部３との間に設けられる。
１つの例として、凹所８および１２および／ま
たは球状部分１５の表面は、テフロンのような摩
擦防止手段でコーテイングされ、そのため、ロツ
クナツト９が締付けられている場合でも、接続部
材１４のわずかな動きが可能である。さらに別の
例では、表面は平らでないようにされ、したがつ
て、ボールは、ロツクナツト９を締付けたとき、
その位置で動かなくなり、接続部材１４とねじを
切られた軸部２との間に実質的に固い接続が得ら
れる。
ロツクナツト９は、茎ねじを背骨に挿入するた
めに、取除かれるか、または緩められる。固定し
ようとする脊椎のセグメントを間に挟むように離
れた位置で背骨に挿入される１対の茎ねじを連結
するねじ切されたロツドが用意される。茎ねじの
挿入後、受取部３の方向は、ねじ切されたロツド
に対してせん断力を作用させないような位置とな
るように位置決めをされる。それと同時にねじ切
されたロツドは、１対の茎ねじの受取部３の受け
スリツト２１に嵌め入れられ、それぞれの凹所２
２および２３に配置された締付けナツトで固定さ
れる。その後、所望されるようにそれぞれのロツ
クナツト９は接続部材１４がほとんどまたは全然
それ以上動かないように締付けられ、こうして受
取部３とねじを切られた軸部２との間の実質的に
固い接続が得られるか、またはそれはそれが２つ
の部分の間の所望された緩衝された動きを可能に
する程度にだけ締付けられる。
次に、本発明の実施例を説明する。
第４図〜第１１図に示される実施例において、
茎ねじ２５は、ねじを切られた軸部２６と受取部
２７とを含む。
ねじを切られた軸部２６は、背骨に捩込まれる
ように適合されたねじを切られた部分２８を含
む。球状の頭部２９は、ねじを切られた部分２８
の頭側端部に設けられる。球状頭部２９は、ねじ
を切られた部分２８の軸線に対して垂直に延在す
る平坦な表面３０を有する。表面３０は球状の部
分を切取ることによつて形成される。切取られた
部分は、球状本体の半径の約半分に対応する。
第６図から分かるように、球状頭部２９の平坦
表面３０には、ねじを切られた部分２８と同軸に
六角穴３１が設けられる。六角穴３１は、ねじを
切られた軸部２６を背骨に捩込むために、六角ソ
ケツトねじキーを受入れる。もちろん、六角穴以
外の他の型が、ねじドライバによつて係合するた
めに設けられてもよい。
受取部２７は、２個のヘツドハーフ３２および
３３と、これらのヘツドハーフを一緒に保持する
保持リング３４とを含む。
各ヘツドハーフは、他方のヘツドハーフに向い
合うその内部の側面に、球状凹部３５を有する。
球状凹部３５の内部半径は、球状頭部２９の半径
に対応する。切頭円錐のセグメントの形状を有
し、球状凹部３５から外方向に逸れるように設計
されるカラー部分３６が、球状凹部３５に連なつ
ている。カラー部分３６の中心軸線は、球状凹部
３５の中心を通過する。各ヘツドハーフ３２，３
３は、ねじ切されたロツドを受入れるための受け
スリツト３７を有している。受けスリツト３７、
カラー部分３６の反対側に位置し、カラー部分３
６および球状凹部３５の対称軸に対して垂直に延
在する。受けスリツト３７は、他方のヘツドハー
フに向き合わない外面に、凹所３８を有する。受
けスリツト３７の幅は、そこに嵌合されるねじ切
されたロツド３９が第４図に示されるようにゆつ
たりとそこを通過し得るように選択される。
ヘツドハーフ３２，３３は、カラー部分３６の
領域に、カラー部分３６および球状凹部３５の対
称軸に対して垂直に延在している溝型凹所４０を
有する。
２つのヘツドハーフ３２および３３は同一の態
様で形成される。第８図に示すように、球状凹部
３５の中心４１は、２個のヘツドハーフが向き合
う側面４２よりも、１mmの何分の１かだけ外にあ
る。そのため、２つのヘツドハーフ３２および３
３が互いに球状頭部２９の回りに平行に組立てら
れるとき、両方のハーフの間でギヤツプ４３が得
られるようにされる。
保持リング３４は、カラー部分３６の外部表面
に一致する内部表面４４を有する。特に内部表面
４４は、突出している帯４５と、それに隣接した
端縁部分４６および４７とを含む。保持リング３
４突出帯４５は、ヘツドハーフ３２，３３のカラ
ー部分３６の溝型凹所４０に嵌合する。
保持リング３４の内部直径、特に、突出帯４５
および端縁部分４６，４７の内部直径は、以下の
動作を達成し得るように選ばれている。第４図
は、ねじ切されたロツド３９が挿入されている
が、今、ねじ切されたロツド３９が挿入されてい
ない状態を考える。２個のヘツドハーフ３２，３
３が球状頭部２９の上に取付けられている状態に
おいて、保持リング３４から２個のヘツドハーフ
３２，３３に対して力が作用するが、２個のヘツ
ドハーフ３２，３３は、カラー部分３６のテーパ
の量によつて決定される角度の範囲内で、ねじ切
された部分２６の長さ方向軸線の回りを自由に回
動することができる。２個のヘツドハーフ３２，
３３は、球状頭部２９およびねじ切された部分２
６を介して、ある程度しつかりと、互いに接続さ
れている。その状態で、ねじ切されたロツド３９
を受入れる。２個のヘツドハーフ３２，３３がね
じ切されたロツド３９を受入れる前においては、
２個のヘツドハーフ３２，３３の間隔は、保持リ
ング３４で囲まれた領域の方が小さく、受けスリ
ツト３７の形成された領域の方が大きくなつてい
る。
第４図および第９図で特に見られるように、ね
じ切された軸部２６の縦軸線と回りにおける受取
部２７の自由な動きの抑制を回避するために、受
けスリツト３７の深さは、第４図に示される組立
てられた状態において、球状頭部２９の平坦表面
３０よりも上方の位置で終端となるようにされ
る。
第４図〜第１１図に示される実施例に従つた茎
ねじを骨に取付けるために、ねじ切された軸部２
６の平坦表面３０の六角穴に六角ソケツトねじキ
ーを係合させて、ねじを切られた軸部２６を背骨
に捩込む。その後、保持リング３４によつて一緒
に保持された２個のヘツドハーフ３２および３３
を有する受取部２７を、所定の方向に整列させ
る。ここで、所定の方向とは、ねじ切されたロツ
ド３９が受けスリツト３７内に嵌め入れられたと
き、このねじ切されたロツド３９に対していかな
るせん断力も作用させない方向である。その後、
ねじ切されたロツド３９は、受けスリツト３７内
に嵌め入れられ、凹所３８内に配置された締付け
ナツト４８および４９によつて固定される。締付
けナツト４８および４９は、受取部２７が球状頭
部２９の回り回動することができず、実質的に受
取部２７とねじ切された軸部２６との間で固い接
続が得られるように締付けられるか、または２つ
の部分の間の所望の緩衝された動きが依然として
可能であるようにのみ締付けられる。
上述した茎ねじの実施例を用いることによつ
て、茎ねじのねじ切された軸部を骨に捩込んだ後
においても、茎ねじの受取部を対称軸線の回りで
自由に動かすことができ、水平および垂直方向に
10°から30°までの角度領域で最適な位置を調整す
ることができる。茎ねじの受取部が最適の位置に
設定されたら、受取部とねじ切されたロツドとを
固定する。特に、この発明の実施例では、茎ねじ
の全体の長さ特に短いことが注目される。
第４図に示した実施例では、保持リング３４の
下方角部は、丸められている。第１図に示した茎
ねじにおいて、六角ヘツド５の下方角部も同様に
丸めてもよい。このように丸めておけば、それぞ
れの背骨に向かう表面はその部分でいかなる損傷
も引起し得ない。
この明細書では、特定の具体的な実施例を参照
して発明を記載したが、この発明の範囲は、その
ような具体的な実施例に制約されるものではな
く、特許請求の範囲に記載された範囲内に含まれ
るすべての修正および同等物をも含むことが理解
されるべきである。

第１図は、改良された茎ねじの分解部分断面図
である。第２図は、第１図の茎ねじを90°だけ回
転させた状態に位置する受取部を示す図である。
第３図は、第１図で示した各要素を組立てた状態
の図である。第４図は、この発明の一実施例を示
す部分側面断面図である。第５図は、第４図に示
されているねじの切りされた軸部２６を拡大して
示す図である。第６図は、第５図のねじを切られ
た軸部２６の平面図である。第７図は、第４図に
示されているヘツドハーフ３２を示す図である。
第８図は、第７図のヘツドハーフを90°だけ回転
させた状態の断面図である。第９図は、第７図に
示したヘツドハーフの後ろの部分を示す図であ
る。第１０図は、第８図に示されている部分の平
面図である。第１１図は、第４図に示されている
保持リング３４の断面図である。 図において、２５は茎ねじ、２６はねじを切ら
れた軸部、２７は受取部、２８はねじを切られた
部分、２９は球状頭部、３２および３３はヘツド
ハーフ、３４は保持リング、３５は球状凹部、３
６はカラー部分、３７は受けスリツト、３８は凹
所、３９はねじ切されたロツド、４０は溝型凹
所、４１は球状凹部の中心、４２は側面、４３は
ギヤツプ、４４は内部表面、４５は突出帯、４６
および４７は端縁部分を示す。

Claims (1)

  1. 【特許請求の範囲】 １ 脊椎セグメントを安定させるための茎ねじで
    あつて、 ねじを切られた軸部と、 前記ねじ切りされた軸部の一端に設けられた球
    状の頭部と、 前記球状の頭部を受入れるための２個のハーフ
    を有する受取部と、 前記２個のハーフの回りに配置され、前記２個
    のハーフを前記球状頭部に係合させるように保持
    する保持リングと、 を備えた、茎ねじ。 ２ 前記受取部のハーフは、前記球状頭部に対応
    した形状を有し、かつ前記球状頭部に係合する球
    状凹部を有している、特許請求の範囲第１項記載
    の茎ねじ。 ３ 前記受取部の２個のハーフは、それらの間に
    ギヤツプを生じさせるような寸法に作られてい
    る、特許請求の範囲第２項に記載の茎ねじ。 ４ 前記球状頭部は、ねじドライバに係合する手
    段を備えている、特許請求の範囲第１項に記載の
    茎ねじ。 ５ 前記受取部のハーフは、その外周上に溝を有
    しており、 前記保持リングは、その内周面に、前記溝に係
    合する突出帯を有している、特許請求の範囲第１
    項に記載の茎ねじ。
